Robotic-assisted gait training has been cited as a potential rehabilitation intervention for the recovery of ambulation in hemiplegic stroke patients. Previous reviews on robotic gait training have elicited a need for more randomized, clinical studies on robotic gait training for the development of evidence-based intervention protocols and elimination of research bias. Biomechanical analysis is widely used to assess human movement sciences, specifically using three-dimensional motion capture modelling. There are unprecedented opportunities to increase quantitative knowledge of rehabilitation and recreation for disadvantaged population groups. Specifically, 3D models and movement profiles for human gait analysis were generated with emphasis on post-stroke patients, with direct model translation to analyze equivalent measurements while horseback riding in use of the alternative form of rehabilitation, equine assisted activities and therapies (EAAT) or hippotherapy (HPOT). Significant improvements in gait symmetry and velocity were found within an inpatient rehabilitation setting for patients following a stroke, and the developed movement …

Clinical Question: In postoperative patients, how does listening to music affect perceived pain levels? Clinical Bottom Line: There is consistent evidence to suggest music decreases perceived pain in postoperative patients undergoing nasal septal surgery, open heart surgery, total knee replacement surgery, and general orthopedic surgeries requiring hospitalization. Athletic trainers can confidently implement music during patients’ post-operative care sessions to help reduce pain.

Glioblastoma is reported to be the most aggressive and common brain tumor in adults (Davis, 2016). To help alleviate pain in patients with glioblastoma, floatation-REST (Restricted Environmental Stimulation Technique) is proposed. Such therapy has been shown to benefit chronic tension headaches, muscle tension, and other stress-related pain conditions (Feinstein, 2018). The magnesium and sulfate components of Epsom salt combined with controlled warm water allow the muscles and joints to relax. During exercise, lower leg muscle activation has been shown to increase venous return, exercise cardiac output, and arterial blood pressure (BP). Here we investigate BP at rest, with an increase in lower body pressure by unweighting volunteers in a lower body positive pressure treadmill (LBPP-TM). The purpose of this study was twofold; to determine if BP changes while standing over five-minute stages in response to four conditions. Nine participants (21.3±1.8 years) stood in a LBPP-TM in the following ordered conditions: 100%BWset (no unweighting, control), 70%BWset, 35%BWset, and 90%BWset (35%BWset is the greatest unweighted condition). Accurate heart rate (HR) measurements are key to exercise prescription. Many portable HR devices are available including the Apple watch. Resting HR from Apple series 3 watch were compared to measurements using a clinal grade Suntech Tango M2 (ST) and Vitalstream Caretaker (VS) pulse monitors. College-aged participants (N = 5) stood in the Alter G treadmill under positive pressures of 70%, 35%, and 90% body weight (BWset), respectively. Lower body positive pressure treadmills (LBPP-TM) provide assistive body weight support to the user by forcing air into an inflatable chamber in which the user is secured. The result of this positive pressure can be experienced as a lift to the user. The degree of chamber air pressure (CAP) determines the amount of supportive lift provided with greater and lesser CAP producing more and less lift, respectively. Exercise studies consistently report lower effort perceptions with greater CAP which suggest a reduced physiological and mechanical strain on the body. Lower extremity peripheral artery disease (PAD) is a cardiovascular condition caused by narrowed or blocked arteries supplying blood to the legs. Leg pain and myopathy significantly impair gait from the first step. Using AFO may assist the gait and prolong the distance patients with PAD can walk. This study aimed to investigate the effect of AFO on the gait biomechanics of patients with PAD. Background: Medial elbow laxity develops in throwing athletes due to valgus forces. Medial elbow instability in professional, collegiate, and high school athletes is well documented; however, the medial elbow of young throwing athletes has received less attention. This study investigated the medial elbow and common flexor tendon during applied elbow valgus stress of youth baseball players.

Methods: The study included 15 participants. The medial elbow width and thickness of the common flexor tendon were measured on ultrasound images.

Results: No significant side differences in medial elbow width or common flexor tendon were found at rest or under applied valgus stress. …

The purpose of this study was to determine muscle-specific contributions to lower extremity net joint moments (NJMs) during squats with different external loads. Nine healthy subjects performed sets of the back squat exercise with 0, 25, 50, and 75% of body mass as an added external load. Motion capture and force plate data were used to calculate NJMs and to estimate individual muscle forces via static optimization. Individual muscle forces were multiplied by their respective moment arms to calculate the resulting muscle-specific joint moment. (EMG) is a method for measuring muscle activity by an electrical signal, and is useful in studying motor control, postural control, and in physical therapy. A current research topic is creating an algorithm that can use the EMG signal to reliably classify a muscle as active or inactive. This thesis presents a classification algorithm for leg muscles with a single activation spike while walking. Lateral ankle sprain (LAS) is among the most common lower limb musculoskeletal injuries impacting both the general and sport populations. Even more problematic is that approximately 70% of individuals who sustain an LAS develop residual deficits. Of that subgroup, roughly 40% develop chronic ankle instability (CAI). This study used principal component analysis (PCA) of waveforms to extract movement patterns from hip and knee angle time-series data; and determined if the extracted movement patterns were predictors of ball velocity during a soccer kick. Twenty-three female and nineteen male professional soccer players performed maximal effort instep kicks while motion capture and post-impact ball velocities data were recorded. Three-dimensional hip and knee joint angle time-series data were calculated from the beginning of the kicking leg’s backswing phase until the end of the follow-through phase and entered into separate PCAs for females and males. To identify the accuracy of Body Mass Index (BMI) to categorize body weight in a sample of children with spina bifida and Down syndrome as compared to typically developing peers.

METHODS:

A secondary analysis of 32 children with spina bifida, Down syndrome or no chronic illness. A calculated BMI was plotted on the Centers for Disease Control and Prevention age- and sex-specific BMI growth charts to determine each child’s weight status. Maximal oxygen uptake (VO2max) declines with advancing age and is a predictor of morbidity and mortality risk. The purpose here was to assess the utility of constant load tests performed either above or below peak work rate obtained from a graded exercise test for verification of VO2max in older adults. Twenty-two healthy older adults (9M, 13F, 67 ± 6 years, BMI: 26.3 ± 5.1 kg·m−2) participated in the study. Chronic pain is perhaps the most pervasive medical issue in the United States. The long-term impacts of chronic pain often cause individuals to reduce or eliminate physical activity. The purpose of this study was to investigate the influence of the bar position on ankle, knee, and hip net joint moments (NJMs), relative muscular effort (RME), and vertical ground reaction forces (GRFs) during isometric pulling exercises, such as the isometric midthigh pull. Eight female lacrosse athletes performed maximal effort isometric pulls at 3 different bar positions (low: above patella, mid: midthigh, and high: crease of hip) while motion capture and GRF data were recorded. There are various ways to decrease muscle and tendon stiffness, including but not limited to: static stretching, ballistic stretching, cycle stretching, passive and active stretching, and foam rolling. The purpose of this paper is to review different studies that focus on the acute effects of static stretching and foam rolling on muscles of the leg including the gastrocnemius, rectus femoris, hamstrings, soleus, etc. Results showed that static stretching and foam rolling decrease muscle and tendon stiffness in the leg while simultaneously increasing their range of motion, flexibility, and muscle fascicle length immediately after exercise.

Stroke survivors are at high risk of falling during turning. The kinematics of performing a 360° turn have not been fully analyzed among individuals after stroke. Quantitative differences in the parameters of turning between healthy older adults and those after stroke could provide detailed information on turning ability among these groups. Tensiomyography (TMG) is a non-invasive tool used to assess skeletal muscle tissue, including Displacement (Dm), Contraction Time (Tc), Sustain Time (Ts), Relaxation Time (Tr), and Delay Time (Td). This tool can be used clinically to determine muscular response to exercise.

PURPOSE: The purpose of this study was to investigate the reliability of TMG measurements.

METHODS: Studies were identified from multiple databases and evaluated for inclusion. All studies underwent a quality assessment using the Modified Downs and Black checklist for assessing quality studies, and results were extracted from qualified articles.
