Cytotoxic Compounds From Dendrobium Ochreatum Against Human Lung Cancer H460 Cells, Natthaphitcha Khunsantiphong

The chemical analysis of crude extracts from Dendrobium ochreatum ethanol revealed 12 compounds. Among them, dendroochreatene, a novel discovered compound, and 11 previously identified compounds were found: atraric acid, docosyl trans-ferulate, moscatilin, cinnamic acid, β-hydroxypropiovanillone, loddigesiinol C, dihydroconiferyl dihydro-p-coumarate, indole-3-aldehyde, vanillic acid, naringenin, and 2,4,7-trihydroxy-9,10-dihydrophenanthrene. Structural elucidation of these compounds was achieved through MS, IR, UV, and NMR spectroscopic analyses, corroborated by comparison with existing literature data. The cytotoxicity evaluation against NCI-H460 non-small cell lung cancer cells employed two methods: direct cell testing and literature review. This research aimed to study the physicochemical properties, including physical appearance, percentage of labeled amount (%LA), and release profile, of Asiatic acid (AA) encapsulated in solid lipid microparticles (SLM) composed of 10% and 15% cetyl alcohol with poloxamer 188 (F6 and F12). The results indicated that F12 demonstrated a significantly higher %LA and exhibited a release kinetic that fit the Higuchi model with a slower sustained-release profile compared to F6. Thus, F12 was selected for further clinical study. วัตถุประสงค์: เพื่อศึกษาผลลัพธ์ของการเริ่มต้นและปรับขนาดยาโดยใช้วาร์ฟาริน โนโมแกรมในผู้ป่วยที่เริ่มใช้ยาวาร์ฟาริน ณ โรงพยาบาลเจริญกรุงประชารักษ์ วิธีการดำเนินการวิจัย: การศึกษานี้เป็นการทดลองแบบสุ่มและมีกลุ่มควบคุม ในอาสาสมัครที่เริ่มใช้ยาวาร์ฟารินขณะเข้าพักรักษาตัวในโรงพยาบาลเจริญกรุงประชารักษ์ ระหว่างเดือน สิงหาคม ถึง พฤศจิกายน พ.ศ. 2566 โดยกลุ่มทดลองจะได้รับการเริ่มต้นและปรับขนาดยาวาร์ฟารินโดยใช้วาร์ฟารินโนโมแกรม และกลุ่มควบคุมจะได้รับการเริ่มต้นและปรับขนาดยาวาร์ฟารินโดยดุลยพินิจของแพทย์ ประเมินสัดส่วนของอาสาสมัครที่มีค่า INR อยู่ในช่วง 2.0 – 3.0 ภายใน 14 วัน ผลการวิจัย: จากกลุ่มตัวอย่างทั้งหมด 98 ราย แบ่งเป็นกลุ่มควบคุม 49 ราย และกลุ่มทดลอง 49 ราย ผลลัพธ์ของการวิจัยพบว่าสัดส่วนของอาสาสมัครที่มีค่า INR อยู่ในช่วง 2.0 – 3.0 ภายใน 14 วันหลังได้รับยาวาร์ฟารินของกลุ่มที่เริ่มต้นและปรับขนาดยาโดยใช้วาร์ฟารินโนโมแกรมมากกว่ากลุ่มที่เริ่มต้นและปรับขนาดยาโดยแพทย์อย่างมีนัยสำคัญทางสถิติ (44.9% และ 24.5%, p=0.034) ขนาดยาวาร์ฟารินเริ่มต้นเฉลี่ยของทั้งสองกลุ่มไม่แตกต่างกัน (2.1 มิลลิกรัมต่อวัน และ 2.2 มิลลิกรัมต่อวัน, p= 0.666) สรุป: การใช้วาร์ฟารินโนโมแกรมในการเริ่มต้นและปรับขนาดยาวาร์ฟารินส่งผลให้ค่า INR เข้าสู่ช่วงเป้าหมายของการรักษาเพิ่มขึ้น นอกจากนี้การใช้วาร์ฟารินโนโมแกรมมีความปลอดภัยเทียบเท่ากับการเริ่มต้นและปรับขนาดยาวาร์ฟารินโดยแพทย์

วัตถุประสงค์: เพื่อเปรียบเทียบการเปลี่ยนแปลงอัตราการกรองของไต (estimated glomerular filtration rate, eGFR) ในผู้ป่วยโรคเบาหวานชนิดที่ 2 ที่ได้รับการรักษาเสริมด้วยยากลุ่ม sodium-glucose cotransporter 2 inhibitors (SGLT2 inhibitors) ร่วมกับยาต้านระบบ renin-angiotensin (RAS blockers) เทียบกับกลุ่มที่ได้รับ RAS blockers วิธีการ: งานวิจัยนี้เก็บข้อมูลย้อนหลังจากเวชระเบียนอิเล็กทรอนิกส์ในโรงพยาบาลสวรรค์ประชารักษ์ การศึกษาคัดเลือกผู้ป่วยโรคเบาหวานชนิดที่ 2 อายุ 18 ปีขึ้นไป ระยะเวลาตั้งแต่มกราคม พ.ศ. 2560 ถึงธันวาคม พ.ศ. 2564 เข้างานวิจัยจำนวน 350 ราย กลุ่มศึกษา คือ กลุ่มที่ได้รับยากลุ่ม SGLT2 inhibitors ร่วมกับ RAS blockers 175 ราย กลุ่มควบคุม คือ กลุ่มที่ได้รับยากลุ่ม RAS blockers 175 ราย ผลลัพธ์ของการศึกษา คือ ค่าอัตราการกรองของไต น้ำตาลสะสมฮีโมโกลบิน ความดันโลหิต และน้ำหนักตัวซึ่งวัด ณ เดือนที่ 0, 3, 6 และ 12 ผลการวิจัย: อัตราการกรองของไตระหว่างกลุ่มศึกษาและกลุ่มควบคุมทั้งในเพศหญิงและเพศชายไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ โดยมีเพศหญิงเป็นปัจจัยที่มีผลต่อการเปลี่ยนแปลงอัตราการกรองของไตที่ P=0.024 น้ำตาลสะสมฮีโมโกลบินของทั้ง 2 กลุ่มไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ โดยมีระยะเวลาการเกิดโรคเบาหวานและค่าน้ำตาลสะสมฮีโมโกลบินพื้นฐานเป็นปัจจัยที่มีผลต่อการเปลี่ยนแปลงค่าน้ำตาลสะสมฮีโมโกลบินที่ P=0.003 และ P

การวิเคราะห์และปรับปรุงกระบวนการทำแกรนูลเปียกสำหรับการผลิตยาเม็ดเมตฟอร์มิน, นัชชา เตียสุนทรารมย์

การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่มีผลต่อการเกิดแกรนูลที่มีขนาดมากกว่า 2 มิลลิเมตร ซึ่งไม่สามารถผ่านแร่งในกระบวนการทำแกรนูลเปียกของยาเมตฟอร์มิน (Rejected granules) ที่ถูกออกแบบให้ผลิตด้วยเครื่องจักรที่มีระบบปิด มีขนาดการผลิต 600 กิโลกรัม จากการเก็บรวมรวมข้อมูลปริมาณแกรนูลของยาเมตฟอร์มินที่ไม่สามารถผ่านแร่งได้จำนวน 100 รุ่นการผลิต พบว่ามีปริมาณสูงสุดถึง 21.54 กิโลกรัม จึงออกแบบการทดลองแบบแฟคทอเรียลเต็มรูป เพื่อศึกษาปัจจัยของกระบวนการทำแกรนูลเปียกของยาเมตฟอร์มิน จำนวน 5 ตัวแปร ตัวแปรละ 2 ระดับ ได้แก่ เวลาที่ใช้ในการลดขนาดก้อนวัตถุดิบเมตฟอร์มิน (15 และ 20 นาที) อัตราการไหลของลมทางเข้าของเครื่อง Fluid Bed Dryer (1,700 และ 1,900 ลูกบาศก์เมตรต่อชั่วโมง) อุณหภูมิที่ใช้ในการถ่ายแกรนูลจากเครื่อง Fluid Bed Dryer ไปยังเครื่อง Dry mill (25 และ 30 องศาเซลเซียส) ความกว้างในการเปิด Flap ขณะถ่ายแกรนูล (80 และ 90%) และระยะห่างระหว่างแร่งและใบพัดของเครื่อง Dry mill (2.5 และ 3.5 มิลลิเมตร) และมีการทำซ้ำ 2 ครั้ง ผลการวิจัยพบว่าพารามิเตอร์ของกระบวนการที่มีผลต่อปริมาณการเกิด Rejected granule อย่างมีนัยสำคัญ ได้แก่ ระยะห่างระหว่างแร่งและใบพัดของเครื่อง Dry mill และเวลาที่ใช้ในการลดขนาดก้อนวัตถุดิบเมตฟอร์มินตามลำดับ แต่ปัจจัยที่ศึกษาไม่มีผลต่อการกระจายตัวของขนาดอนุภาคและปริมาณน้ำที่สูญเสียไปของแกรนูล นอกจากนี้ได้ระดับที่ดีที่สุดของพารามิเตอร์กระบวนการทำแกรนูลเปียกที่ปรับใช้ในระดับอุตสาหกรรม คือ เวลาที่ใช้ในการลดขนาดก้อนวัตถุดิบเมตฟอร์มิน 20 นาที อัตราการไหลของลมทางเข้าของเครื่อง Fluid Bed Dryer 1,900 ลูกบาศก์เมตร/ชั่วโมง อุณหภูมิที่ใช้ในการถ่ายแกรนูลจากเครื่อง Fluid Bed Dryer ไปยังเครื่อง Dry mill 25 องศาเซลเซียส ความกว้างในการเปิด Flap ขณะถ่ายแกรนูล 80% และระยะห่างระหว่างแร่งและใบพัดของเครื่อง …

The release of coated silicone oil was found to affect the product quality and patient safety. Cyclodextrins (CDs) are cyclic oligosaccharides composed of glucose units linked by α-1,4 glycosidic bond. CDs are commonly used as pharmaceutical excipients and they offer versatile benefits in various industries. Recently, CDs have been promising for the application of medical imaging, especially nuclear medicine imaging. Herein, we aim to radiosynthesize 68Ga-alpha-cyclodextrin (68Ga-αCD) by directly radiolabeling Ga-68 to αCD. Briefly, Ga-68 was eluted, mixed with αCD solution and heated at 100 °C for 20 min. In this study, six triterpenoids were isolated from the stem of Diospyros gracilis H.R.Fletcher (Ebenaceae), including two friedelane-type triterpenoids namely friedelin and epifriedelanol, and four lupane-type triterpenoids namely lupeol, betulin, betulinic acid and betulinaldehyde. Their chemical structures were identified by spectroscopic analysis (IR, NMR and MS) and comparison with those reported in the literature. In the investigation for their in vitro cytotoxic activity against cancer cells, lupeol, betulin and betulinic acid exhibited cytotoxic effects against glioblastoma (U87) and breast cancer (MDA-MB231) cells (IC50 = 13.16, 15.39, 8.31 µM and 34.41, 74.49, 43.88 µM, respectively). วัตถุประสงค์: เพื่อศึกษาปัจจัยที่สัมพันธ์กับการได้รับยาในกลุ่ม Angiotensin converting enzyme inhibitors (ACEIs) หรือ Angiotensin receptor-neprilysin inhibitor (ARNI) หรือ Angiotensin receptor blockers (ARBs) ขนาดต่ำกว่าเป้าหมาย ในผู้ป่วยคนไทยภาวะหัวใจล้มเหลวเรื้อรังที่เกิดร่วมกับการบีบตัวของหัวใจห้องซ้ายล่างลดลง วิธีการดำเนินการวิจัย: การวิจัยเชิงวิเคราะห์จากเหตุไปหาผลแบบย้อนหลัง ในผู้ป่วยคนไทยภาวะหัวใจล้มเหลวเรื้อรังที่เกิดร่วมกับการบีบตัวของหัวใจห้องซ้ายล่างลดลง แผนกผู้ป่วยนอก โรงพยาบาลตราด และโรงพยาบาลพระปกเกล้า ตั้งแต่เดือนมกราคม พ.ศ. 2559 ถึงเดือนธันวาคม พ.ศ. 2563 ทุกราย วิเคราะห์ปัจจัยที่สัมพันธ์กับการได้รับยาในกลุ่ม ACEIs/ARBs/ARNI ขนาดต่ำกว่าเป้าหมาย ผลการวิจัย: ผู้ป่วยทั้งหมดในการศึกษานี้มีจำนวน 396 ราย แบ่งเป็นกลุ่มที่ได้รับยา ACEIs/ARBs/ARNI ขนาดต่ำกว่าเป้าหมาย จำนวน 289 ราย กลุ่มที่ได้รับยา ACEIs/ARBs/ARNI ขนาดมากกว่าหรือเท่ากับเป้าหมาย จำนวน 107 ราย เมื่อควบคุมปัจจัยอื่นๆที่ศึกษา พบว่าปัจจัยที่มีความสัมพันธ์กับการได้รับยาขนาดต่ำกว่าเป้าหมาย คือ NYHA class III/IV (Odd ratio=1.84), 95%CI (1.11, 3.06) และการได้รับยากลุ่ม beta blockers ขนาดมากกว่าหรือเท่ากับร้อยละ 50 ของขนาดเป้าหมาย (Odd ratio=0.37), 95%CI (0.22, 0.6) สรุปผลการวิจัย: ปัจจัย NYHA class III/IV เพิ่มความเสี่ยงที่ผู้ป่วยจะได้รับยาขนาดต่ำกว่าเป้าหมาย การได้รับยากลุ่ม beta blockers ขนาดมากกว่าหรือเท่ากับร้อยละ 50 ของขนาดเป้าหมาย ช่วยลดความเสี่ยงที่จะทำให้ผู้ป่วยได้รับยา ACEIs/ARBs/ARNI ขนาดต่ำกว่าเป้าหมาย

Background and objective: Urinary tract infections (UTIs) are a common problem in pediatric urgent care medicine. There are multiple quality improvement (QI) projects related to the management of UTIs documented in the pediatric literature. We developed a project to decrease the prescribing of ultimately unneeded antibiotics for possible UTIs in a pediatric urgent care setting. A similar project has not been described in the pediatric literature.
Methods: We first reviewed the charts of patients presenting to a system of pediatric urgent care centers with a possible UTI over a 2-year period. We then launched a QI project with three plan, …

The current GAG anticoagulants such as heparin, heparin derivatives, and vitamin K antagonists, such as warfarin continue to be the backbone of anticoagulant therapy. These drugs act through an indirect mechanism to convey inhibition of several coagulation enzymes. However, xv their use leads to several serious adverse effects, such as excessive bleeding risk and unpredictability of patient response. Regardless of their clinical achievement, every individual agent is accompanied by several side effects, particularly major/minor bleeding, thrombocytopenia, drug-food or drug-drug interactions, or absence of antidote. Pharmaceutical product hops are anticompetitive maneuvers that often represent a last-ditch effort by brand manufacturers to preserve market share in the face of generic competition. An integral part of product life cycle management strategies, product hops may offer marginal benefits to patients but can substantially increase costs to payers and patients alike. Yet industry advocates maintain that this is essential follow-on research and development, resulting in the development of novel products that would otherwise never reach the market.

The pathogen Ophidiomyces ophidiicola, widely known as the primary cause of snake fungal disease (SFD), has been detected in Texas’s naïve snakes. Our team set out to characterize O. ophidiicola’s spread in eastern Texas. From December 2018 until November 2021, we sampled and screened with ultraviolet (UV) light 176 snakes across eastern Texas and detected 27. O. ophidiicola’s positive snakes using qPCR and one snake in which SFD was confirmed via additional histological examination. Pharmacogenomics (PGx) is considered an emergent field in developing countries. Research on PGx in the Latin American and the Caribbean (LAC) region remains scarce, with limited information in some populations. Thus, extrapolations are complicated, especially in mixed populations. In this paper, we reviewed and analyzed pharmacogenomic knowledge among the LAC scientific and clinical community and examined barriers to clinical application. We performed a search for publications and clinical trials in the field worldwide and evaluated the contribution of LAC. Cancer-associated fibroblasts (CAFs), one of the most abundant types of stromal cells in the tumor microenvironment, are known to provide functional and structural support for tumor progression. These cells also play a role in cancer chemoresistance, but a detailed understanding of the process, particularly in chemoresistant lung cancer, is lacking. BACKGROUND: A growing body of evidence has shown the involvement of the kynurenine pathway (KP), the primary route of tryptophan (TRP) catabolism, in the pathophysiology of neuropsychiatric disorders.
OBJECTIVE: The study aims to provide a comprehensive and critical overview of the clinical evidence on the KP involvement in the pathophysiology of Alzheimer's disease (AD) and Parkinson's disease (PD), discussing therapeutic opportunities.
METHODS: We searched for studies investigating KP metabolites in human subjects with AD and/or PD.

Antibiotics have emerged as ground-breaking medications for the treatment of infectious diseases, but due to the excessive use of antibiotics, some drugs have developed resistance to microorganisms. Because of their structural complexity, most antibiotics are excreted unchanged, polluting the water, soil, and natural resources. Acute Respiratory Distress Syndrome (ARDS) is triggered by a variety of insults, such as bacterial and viral infections, including SARS-CoV-2, leading to high mortality. In the murine model of ARDS induced by Staphylococcal enterotoxin-B (SEB), our previous studies showed that while SEB triggered 100% mortality, treatment with Resveratrol (RES) completely prevented such mortality by attenuating inflammation in the lungs. In the current study, we investigated the metabolic profile of SEB-activated immune cells in the lungs following treatment with RES. RES-treated mice had higher expression of miR-100 in the lung mononuclear cells (MNCs), which targeted mTOR, leading to its decreased expression. …

Humans operating in extreme environments often conduct their operations at the edges of the limits of human performance. Sometimes, they are required to push these limits to previously unattained levels. As a result, their margins for error in execution are much smaller than that found in the general public. These same small margins for error that impact execution may also impact risk, safety, health, and even survival. Thus, humans operating in extreme environments have a need for greater refinement in their preparation, training, fitness, and medical care. Trichomonas vaginalis is responsible for 156 million new cases per year worldwide. When present asymptomatically, the parasite can lead to serious complications, such as development of cervical and prostate cancer. As infection increases the acquisition and transmission of HIV, the control of trichomoniasis represents an important niche for the discovery and development of new antiparasitic molecules. This urogenital parasite synthesizes several molecules that allow the establishment and pathogenesis of infection.
