การศึกษานี้มีรูปแบบการศึกษาจากเหตุไปหาผลแบบไปข้างหน้าร่วมกับย้อนหลัง วัตถุประสงค์เพื่อศึกษาลักษณะทางคลินิกและปัจจัยที่มีผลต่อการเกิดเหตุการณ์ไม่พึงประสงค์จากการใช้ยาในผู้ป่วย วัณโรคปอด ณ คลินิกควบคุมการติดเชื้อทางเดินหายใจ โรงพยาบาลจุฬาลงกรณ์ ระหว่างเดือนมิถุนายน พ.ศ. 2562 ถึงมิถุนายน พ.ศ. 2565 โดยเก็บข้อมูลจากเวชระเบียนอิเล็กทรอนิกส์ผู้ป่วยนอก วิเคราะห์ปัจจัยที่มีความสัมพันธ์ต่อการเกิดเหตุการณ์ไม่พึงประสงค์ด้วยสถิติการถดถอย (Cox regression) ในการศึกษามีผู้ป่วยทั้งหมด 371 ราย อายุเฉลี่ย 52.6 ± 19.9 ปี พบเพศชายร้อยละ 63.6 และพบผู้ป่วย วัณโรคที่ไวต่อยาร้อยละ 81.9 ผลการศึกษาจากการวิเคราะห์พหุตัวแปร (multivariate analysis) พบปัจจัยที่มีความสัมพันธ์ต่อการเกิดเหตุการณ์ไม่พึงประสงค์ ได้แก่ เพศหญิง (HR=1.64, 95%CI 1.22–2.20) วัณโรคดื้อยา (HR=1.48, 95%CI 1.01–2.15) โรคไตเรื้อรัง (HR=1.62, 95%CI 1.03–2.54) และการมีประวัติโรคตับ (HR=1.94, 95%CI 1.37-2.76) พบอุบัติการณ์การเกิดเหตุการณ์ไม่พึงประสงค์ 12.0 (95% CI: 10.4 – 13.8) รายต่อ 100 ประชากรต่อเดือน พบอัตราการเกิดเหตุการณ์ไม่พึงประสงค์ร้อยละ 55.5 เหตุการณ์ไม่พึงประสงค์ที่พบมาก 3 อันดับแรก ได้แก่ ผื่นคัน อาการคลื่นไส้-อาเจียน และภาวะ ตับอักเสบ เหตุการณ์ไม่พึงประสงค์ส่วนใหญ่มีความรุนแรงเล็กน้อยถึงปานกลาง พบร้อยละ 76.5 และผลลัพธ์ภายหลังการเกิดเหตุการณ์ไม่พึงประสงค์ส่วนใหญ่หายเป็นปกติ พบร้อยละ 70.8 อย่างไรก็ตาม ไม่พบความสัมพันธ์กันระหว่างการเกิดเหตุการณ์ไม่พึงประสงค์กับอัตราการรักษาไม่สำเร็จ อุบัติการณ์การเกิดเหตุการณ์ไม่พึงประสงค์จากการใช้ยาต้านวัณโรคในผู้ป่วยวัณโรคปอดสูง จึงมีความสำคัญในการติดตามและเฝ้าระวังการเกิดเหตุการณ์ไม่พึงประสงค์ในผู้ป่วยที่ได้รับยาต้านวัณโรคอย่างใกล้ชิด โดยเฉพาะผู้ป่วยที่มีความเสี่ยงต่อการเกิดเหตุการณ์ไม่พึงประสงค์ คือ เพศหญิง วัณโรค ดื้อยา ผู้ป่วยโรคตับและโรคไตเรื้อรัง

Checkpoint blockade immunotherapy has transformed the course of cancer treatments. The use of monoclonal antibodies (mAbs) specific against immune checkpoints has displayed satisfactory clinical benefits. However,its effectiveness is limited to just a small percentage of cancer patients. In this work, a recombinant DVD-AT2C bispecific antibody (bsAb) was developed for dual inhibition of PD-1/PD-L1 and CTLA-4 axes and produced in plants. Plant system was utilized as an alternative protein production platform. After the body is invaded by pathogens, it responds by secreting cytokines, for example, interleukin 6 (IL-6), which activates an intracellular signaling pathway involving JAKs/STAT3 through phosphorylation. However, overexpression of pSTAT-3 can lead to a positive feedback loop of IL-6, causing the release of more cytokines and potentially leading to cytokine storm. Metal nanomaterials can enhance the efficacy of current cancer therapies. Here, we show that Ti0.8O2 nanosheets cause cytotoxicity in several lung cancer cells but not in normal cells. The nanosheet-treated cells showed certain apoptosis characteristics. Protein analysis further indicated the activation of the p53-dependent death mechanism. Transmission electron microscopy (TEM) and scanning electron microscopy (SEM) analyses revealed the cellular uptake of the nanosheets and the induction of cell morphological change. Ti0.8O2 nanosheets induce apoptosis through a molecular mechanism involving peroxynitrite (ONOO−) generation. As peroxynitrite is known to be a potent inducer of S-nitrosylation, we further found that the nanosheets mediated …

การตรวจติดตามสภาวะแวดล้อมมีบทบาทสำคัญในการสร้างความมั่นใจต่อระดับความสะอาดของสภาพแวดล้อมในระหว่างกระบวนการผลิต โดยเฉพาะอย่างยิ่งในการผลิตผลิตภัณฑ์ประเภทยาฉีด โดยควรต้องกำหนดให้มีโปรแกรมการตรวจติดตามสภาวะแวดล้อมอย่างเป็นประจำ เพื่อลดความเสี่ยงของการปนเปื้อนจากเชื้อจุลินทรีย์และอนุภาคสิ่งแปลกปลอมอื่นๆ ในการศึกษาวิจัยนี้มีวัตถุประสงค์เพื่อพัฒนาโปรแกรมการตรวจติดตามสภาวะแวดล้อมตามแนวทางการประเมินความเสี่ยงในห้องสะอาดสำหรับยาฉีดของแผนกบรรจุผลิตภัณฑ์ ศูนย์ผลิตผลิตภัณฑ์จากพลาสมา สภากาชาดไทย จำนวน 13 ห้อง แนวทางการจัดการความเสี่ยงคุณภาพอ้างอิงตาม International Council for Harmonization Q9 โดยใช้เครื่องมือ Risk ranking and filtering ในการประเมินและจัดระดับความเสี่ยงเพื่อกำหนดความถี่และตำแหน่งในการตรวจติดตามสภาวะแวดล้อม สำหรับขั้นตอนการระบุความเสี่ยง มีการประเมินความเสี่ยงเพื่อค้นหาปัจจัยที่อาจทำให้เกิดการปนเปื้อนจากเชื้อจุลินทรีย์ด้วย Fishbone diagram ในการวิเคราะห์ความเสี่ยง ได้ทำการเลือกปัจจัยที่จะส่งกระทบผลต่อคุณภาพและความปลอดภัยของผลิตภัณฑ์ ในแง่ของความรุนแรงและโอกาสที่จะเกิดการปนเปื้อน ปัจจัยที่นำมาพิจารณา ได้แก่ กิจกรรมการผลิต, ระยะเวลาที่มีการปฏิบัติงาน, ขั้นตอนกระบวนการผลิต, ความยากง่ายต่อการทำความสะอาดอุปกรณ์, ความถี่ในการทำความสะอาดและกำจัดเชื้อโรค, ข้อมูลประวัติแนวโน้มของการตรวจติดตามสภาวะแวดล้อม, จำนวนบุคลากรที่ปฏิบัติงาน และการมีท่อเดรนหรือท่อน้ำในพื้นที่ ทั้งนี้เกณฑ์และคะแนนของแต่ละปัจจัยจะถูกกำหนดและประเมินโดยคณะทำงานประเมินความเสี่ยง เพื่อคำนวณค่าความเสี่ยงและจัดระดับได้เป็น 5 ระดับ แนวทางการประเมินความเสี่ยงนำไปสู่โปรแกรมการตรวจติดตามสภาวะแวดล้อมใหม่ ซึ่งมีความถี่ในการตรวจติดตามสภาวะแวดล้อมของแต่ละพื้นที่ได้เป็นสัปดาห์ละครั้ง จำนวน 5 ห้อง สองสัปดาห์ครั้ง จำนวน 1 ห้อง และสามเดือนครั้ง จำนวน 7 ห้อง สำหรับตำแหน่งและวิธีการทดสอบที่ประเมินได้ คือ Particle count 26 ตำแหน่ง, Volumetric air sampling 26 ตำแหน่ง, Settle plate 29 ตำแหน่ง และ Contact plate 33 ตำแหน่ง ในขั้นตอนการทบทวนความเสี่ยงพบว่าหลังจากเปลี่ยนโปรแกรมการตรวจติดตามสภาวะแวดล้อมใหม่ ผลการตรวจยังคงผ่านตามเกณฑ์ที่กำหนด และเมื่อพิจารณาจากการคำนวณ %Contamination recovery rate (CRR) พบว่ายังคงผ่านตามเกณฑ์ที่กำหนดเช่นเดียวกัน ดังนั้นจึงไม่ได้มีผลกระทบต่อคุณภาพของผลิตภัณฑ์หรือระดับการปนเปื้อนในห้องสะอาด โดยรวมแล้วความสำเร็จของโปรแกรมการตรวจติดตามสภาวะแวดล้อมจะขึ้นอยู่กับประสิทธิภาพของการประเมินความเสี่ยง การวิเคราะห์ข้อมูลที่เกี่ยวข้อง และวิธีในการทดสอบตรวจติดตามสภาวะแวดล้อม อย่างไรก็ตามสิ่งที่สำคัญหลังจากที่จัดทำหรือปรับปรุงโปรแกรมการตรวจติดตามสภาวะแวดล้อมแล้วจะต้องมีการประเมินและปรับปรุงโปรแกรมอย่างต่อเนื่องสม่ำเสมอ เพื่อให้มั่นใจว่าสภาวะแวดล้อมในพื้นที่สะอาดจะคงอยู่ในสถานะที่ควบคุมได้และไม่ส่งผลกระทบต่อคุณภาพของผลิตภัณฑ์

มะเร็งเต้านมเป็นปัญหาสาธารณสุขที่สำคัญ ผู้ป่วยที่เป็นมะเร็งเต้านมระยะแพร่กระจายเป็นปัจจัยพยากรณ์โรคที่ไม่ดี อย่างไรก็ตาม ยังมีข้อมูลเกี่ยวกับระยะเวลาการรอดชีวิตโดยรวม ปัจจัยที่มีผลต่อระยะเวลาการรอดชีวิต และ อัตราการรอดชีวิตที่ระยะเวลา 5 ปีของผู้ป่วยมะเร็งเต้านมระยะแพร่กระจายชาวไทยอยู่จำกัด การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่มีผลต่อระยะเวลาการรอดชีวิตของผู้ป่วยมะเร็งเต้านมระยะแพร่กระจาย โดยเก็บข้อมูลย้อนหลังในผู้ป่วยมะเร็งเต้านมระยะแพร่กระจายที่มารับการรักษาที่โรงพยาบาลศิริราชตั้งแต่วันที่ 1 มกราคม พ.ศ. 2555 ถึง 31 ธันวาคม พ.ศ. 2559 ในช่วงเวลาดังกล่าวมีผู้ป่วยที่มีข้อมูลครบถ้วนและสามารถประเมินข้อมูลระยะเวลาการรอดชีวิตได้จำนวน 790 ราย อายุเฉลี่ย 54+11.8 ปี ส่วนใหญ่ร้อยละ 50.5 เป็นมะเร็งเต้านมชนิด luminal A โรคมะเร็งมีการแพร่กระจายไปที่กระดูกมากที่สุดร้อยละ 70.8 รองลงมาคือที่ปอดและตับ ร้อยละ 55.7 และ 37.3 ตามลำดับ ค่ามัธยฐานของระยะเวลาการรอดชีวิตโดยรวมของผู้ป่วยคือ 21.9 เดือน อัตราการรอดชีวิตที่ 1, 3 และ 5 ปี คือร้อยละ 32.3, 14.4 และ 5.6 ตามลำดับ ผลการวิเคราะห์แบบหลายตัวแปรพบว่าปัจจัยที่มีผลต่อระยะเวลาการรอดชีวิตโดยรวมของผู้ป่วย ได้แก่ ประเภททางโมเลกุลของโรคมะเร็ง ระยะเวลาตั้งแต่ได้รับการวินิจฉัยมากกว่า 6 เดือนขึ้นไปและการแพร่กระจายของโรคมะเร็งไปที่ตับ และการได้รับยาฮอร์โมนหรือยามุ่งเป้า ขณะที่อายุ ดัชนีมวลกาย การได้รับยา chemotherapy หรือสูตรยาที่ได้รับ ไม่มีผลต่อระยะเวลาการรอดชีวิตโดยรวมของผู้ป่วยมะเร็งเต้านมระยะแพร่กระจาย ผู้ป่วยมะเร็งเต้านมระยะแพร่กระจายที่เป็นแบบตัวรับทั้งหมดเป็นบวกหรือแบบไม่มีตัวรับใดเป็นบวก, ได้รับการวินิจฉัยโรคตั้งแต่ 6 เดือนขึ้นไป, มีการแพร่กระจายของโรคมะเร็งไปที่ตับ และผู้ที่ไม่ได้รับยาฮอร์โมนหรือยามุ่งเป้ามีความเสี่ยงในการมีระยะเวลารอดชีวิตโดยรวมที่สั้นกว่า ควรได้รับการวางแผนการรักษาที่เข้มข้นอย่างเหมาะสมเพื่อเพิ่มระยะเวลาการรอดชีวิตโดยรวม

The objective of this study was to evaluate the effects of solid lipids: beeswax and cetyl alcohol, and surfactants: Tween 80®, soybean lecithin and poloxamer 188 on the characteristics of solid lipid microparticles containing asiatic acid (AASLM). The AASLM were prepared from 10% or 15% of beeswax or cetyl alcohol together with 3% of surfactant F1-F12) by melt dispersion cum freeze-drying technique to obtain a solid dosage form. Coronavirus disease 2019 (COVID-19) pandemic caused by severe acute respiratory syndrome coronavirus 2 made a devastating impact worldwide, and virus management is challenging owing to a lack of specific therapies or affordable vaccines, especially in low-and middle-income countries. Hence a low-cost and efficient vaccine is necessary. In the present study, the immunogenicity of the plant-produced S1 subunit protein of SARS-CoV-2 was investigated in order to utilize it as a subunit vaccine. The outcome of SARS-CoV-2 infection may vary depending on gender, lifestyle, and age. The elderly is more likely to have the severe condition than the younger population. However, several young people suffer from severe lung diseases caused by cytokine dysregulation, also known as a "cytokine storm." Cytokine storms are associated with Covid-19 severity and mortality. Identification of potential cytokine storm targets is critical for improving patient survival. Lung cancer is a prevalent cause of death, indicated by an elevated c-Myc level. We synthesized a new derivative of renieramycin T (RT), named DH_22, and tested it for anti-cancer activities in human lung cancer cells. In the cell viability and nuclear staining tests, DH_22 was discovered to be cytotoxic, with an IC50 of around 13 μM; and found to mediate apoptosis in the lung cancer cells. Meanwhile, in a mechanistic sense, DH_22 contributed to the activation of p53-dependent apoptosis and decreased the cellular level of c-Myc. Detecting immunity against SARS-CoV-2 is vital for evaluating vaccine response and natural infection, but a conventional virus neutralization test (cVNT) requires BSL3 and live viruses, and a pseudo-virus neutralization test (pVNT) needs specialized equipment and trained professionals in BSL2. The surrogate virus neutralization test (sVNT) was developed to overcome these limitations. This study explored the use of angiotensin converting enzyme (ACE2) produced from Nicotiana benthamiana for the development of an affordable neutralizing antibodies detection assay. Free radicals are unstable, highly reactive, molecules which can cause several degenerative diseases such as mutagenesis and carcinogenesis. This research aims to study the chemical constitutents of Dendrobium virgineum and their antioxidant activity in 3 different test models including DPPH radical scavenging assay, superoxide radical scavenging activity assay and oxygen radical absorbance capacity assay. Nine compounds were isolated and determined to be two new compounds named 2,8-dimethoxy-9,10-dihydro-4,5-phenanthrenediol and 2,5-dimethoxy-7-hydroxy-9,10-dihydro-1,4-phenanthrenequinone, together with seven known compounds, which are 2-methoxy-9,10-dihydro-4,5-phenanthrenediol, gigantol, 5-methoxy-7-hydroxy-9,10-dihydro-1,4-phenanthrenequinone, p-coumaric acid, tristin, 2,5,7-trihydroxy-4-methoxy-9,10-dihydrophenanthrene and 9,10-dihydro-2,4,7-phenanthrenetriol. The result obtained from the three testing models revealed that 2,5,7-trihydroxy-4-methoxy-9,10-dihydrophenanthrene showed strongest antioxidant activity. …

The Association of Southeast Asian Nations (ASEAN) countries are among the tropical regions with disproportionately high incidence of snakebite. Understanding the snakebite and antivenom market situation and burden of snakebite is crucial for developing evidence-based strategies to pursue the goal set by the World Health Organization (WHO) to halve morbidity and mortality of snakebite by 2030. Firstly, we systematically review 23 cost of illness studies and 3 economic evaluations. Economic burdens of snakebite were underestimated and not extensively studied. Transcatheter pulmonary valve replacement (TPVR) was launched since 2000. It indicated for cardiac patients who have the right ventricular outflow tract obstruction (RVOT) and a prior surgery history. TPVR have a shorter hospital stay, fewer complications, and a less-invasive method. However, TPVR is more expensive than surgical pulmonary valve replacement (SPVR). This raises access concerns for low income countries. There are no cost-effectiveness studies in Indonesia, and limited efficiency evidence due to rare disorder. This study aims to examine outpatient dispensing patterns in Thai hospitals, focusing on the current situation. The research explores the structures, processes, outputs, and environmental factors of alternative outpatient dispensing services. Surveys were conducted through phone interviews and in-depth interviews with pharmacy department heads using structured interviews with accurate content. Key findings from 197 participating hospitals revealed that 97.35% of interviewed hospitals offered at least one alternative outpatient dispensing service. The most popular method was home delivery (88.6%), followed by sub-district health promoting hospital medication pick-up service (45.1%), hospital refill service (44.7%), community pharmacy medication pick-up service (26.6%), and drive-thru …

Photoelectron Photoion Coincidence (PEPICO) Spectroscopy studies on two unsaturated hydrofluorocarbons (HFCs), also known as hydrofluoroolefines (HFOs), are presented here. Previously, the Sztáray group has studied the dissociation of trans-1,3,3,3-tetrafluoroprop-1-ene (ElixClean), which is a fourth-generation refrigerant and propellant and has lower global warming potential than its precursors. My study is an extension of the ElixClean study as it aims to explore how the different number of fluorine atoms impact the dissociation reactions of these molecules. Traditional drug discovery has rapidly accelerated thanks to development of computational molecular modeling. The crucial component that these computational studies hinge upon is having a well-defined, and energetically favorable structure. Structures of proteins and ligands that meet these criteria are important for accurately simulating models used to study drug binding. Naturally developed proteins are capable of carrying out a wide variety of molecular functions due to their highly precise three-dimensional structures, which are determined by their genetically encoded sequences of amino acids. A thorough knowledge of protein structures and interactions at the atomic level will enable researchers to get a deep foundational understanding of the molecular interactions and enzymatic processes required for cells, resulting in more effective therapeutic interventions. The goal of this research project was to develop an infant air-jet dry powder inhaler (DPI) platform for high-efficiency nose-to-lung (N2L) aerosol administration to infants through optimization of individual system components followed by expansion of the system to higher doses and different formulations using a new approach. Primary challenges associated with aerosol delivery to infants using the N2L route include the extremely narrow extrathoracic airways, short inspiratory times (~0.2 sec), low safe airway pressure (H2O), and very low volumes of air (~10 mL) available to form the aerosol. Background: Opioids are a class of drugs that bind to opioid receptors (mu, delta, and kappa), located in the central and peripheral nervous systems, to exert responses such as analgesia, respiratory depression, euphoria, and miosis. The opioid epidemic is characterized in large part by an increase in opioid overdose deaths. Community pharmacists are one of the most accessible healthcare professionals who frequently interact with patients and can implement OUD and opioid overdose prevention strategies. Treatment for opioid overdose and OUD include naloxone for overdose deaths as well as medication-assisted treatment for OUD.

Tumors may induce tolerogenesis through signaling dendritic cells to produce tolerogenic molecules, such as indoleamine 2, 3-dioxygenase 1 (IDO1). Tumor-associated immunosuppression is associated with higher mortality in patients. Small interfering RNA (siRNA) has been shown to silence specific target genes in the target cell. The siRNA associated with these genes could support a gene knockdown of these immunosuppressors and reduce mortality. Delivery of these therapeutic nucleic acids is difficult in vivo because siRNA is easily broken down inside the cell and the bloodstream through present nucleases. Oxidative stress is an imbalance between reactive oxygen species production and elimination, favoring the former. Reactive oxygen species serve as important signaling molecules for physiological homeostasis. However, when produced in excess, these once important signaling molecules become detrimental, disrupting cellular functions, and ultimately leading to cell death. Transcranial low-intensity focused ultrasound is a unique technology to modulate the integrity of tight endothelial junctions and transiently increase BBB/BTB permeability to enhance therapeutic delivery. Despite promising early studies, present literature lacks agreement on key experimental conditions, which restricts our knowledge and the technique's widespread translation. This dissertation first provides a critical review of the current gaps in knowledge regarding the universal use of LiFUS in preclinical and clinical use.
