An Examination Of "Right To Treatment" Standards: Mental Health Policy Within The Context Of The State Hospital System, Kathryn Glass

The Journal of Sociology & Social Welfare

This paper discusses the use of court-imposed standards for public mental hospitals as a method of improving public mental health services. The standards set out in Wyatt v. Stickney are examined, and the author concludes that if implemented nationally such standards would transform the public hospitals. In addition, implementation would alter the power structure of mental health workers, effect the allocation of state and federal funds, and influence the larger system of mental health services. Socio-economic characteristics of public mental hospital patients, and an assessment of present care in this system are presented as central issues in mental health policy …

It is a descriptive study of requests for voluntary admission to Oregon State Hospital. An Exploration Of The Relationship Between Loneliness And Differential Diagnosis In A Rural Mental Health Center, Kathryn Dunscomb

The review of the literature discussed loneliness in general and specified three seperate forms of loneliness; social, psychic, and existential. The relationships between loneliness, social interaction, and pathology were discussed. 139 clients were given the Belcher Extended Loneliness Scale - Revised prior to therapy. They were assigned one of six diagnostic categories to determine loneliness score (LS), diagnostic, and sex interactions. An analysis of variance found a significant difference (p <.0001) between the diagnostic categories’ LS. The schizophrenic’s and the neurotic’s LS s were significantly higher (<.05) than the remaining categories. The males had significantly (p <.036) higher LS than the females. It was also hypothesized that the degree of loneliness would increase the lower the diagnostic developmental level. The hypothesized order was not consistent with the results. Further research was recommended.

Inland Counties Comprehensive Health Planning Council wished to estimate the psychiatric bed needs by type (such as state hospitals, board and care homes, etc.) in San Bernardino County from the distributions of patient arrivals and of lengths of stay. A statistical description of the system during 1973 was considered the first step toward estimating future bed needs. A computer simulation model using IBM's programming language General Purpose Simulation System (GPSS) was developed and was found to be unsatisfactory.

Theorems were then developed for a statistical model, seeking to predict future bed needs. The most convenient of these theorems states that …

Concerned that depression might be a common malady among youth who are placed in treatment centers for delinquents, we undertook an exploratory study to determine the validity of this premise. The authors of this study have all worked with youth who, for various reasons, have been placed in residential treatment centers. The authors have observed that symptoms of depression seem to be more prevalent in these youth than in non-institutionalized adolescents, and that these symptoms are often perceived as individual pathology rather than emotional responses common to institutionalized youth. The current study was designed to further examine the effects of verbal reward and punishment on task performance of schizophrenics. The relationship of need for approval of schizophrenics (as measured by the M-C SDS) to performance and to reward and punishment was also examined. The purpose of this study was to evaluate a token economy program at Oregon State Hospital in terms of patients’ behavioral improvement during the first seventy days of the program. As the study progressed, it became apparent that a study of the problems involved in initiating this program would be significant. Thus an attempt has been made to evaluate both the patients’ behavioral improvement and the problems involved in setting up an effective token economy program.

The development of the infant from birth to four years will approximately parallel the stages of development of the profoundly retarded children enrolled in the Development Centers for Handicapped Minors ( DCHM ) in California. The retarded child is a human being and may be assumed to have the same basic equipment as the normal child. If we expand and prolong the development stated of the normal child we will be able to lead the DCHM child through these states in slow motion to insure progress.
