A Coaching And Team Performance Evaluation Model To Build Capacity For High-Impact Lean Improvement, Ruth Hanselman, Mark Parker, Suneela Nayak, Stephen Tyzik, Amy Sparks

Operations Transformation

There is abundant evidence that links a strong culture of safety with improved patient and staff experience. However, there has been no clear avenue identified as to how to achieve this metric.

A team in a large academic tertiary teaching hospital set about leveraging their daily managing system (DMS) to attain improvement in their institution’s safety. The goals of this quality improvement project were to use DMS to identify and report safety concerns and increase frontline team knowledge and comfort with reporting safety concerns during Gemba walks.

A root cause analysis identified 5 areas for improvement and several countermeasures were …

Implementation Of Trauma Service Guideline For The Use Of Phenobarbital In The Management Of The Non-Icu Trauma Patient At Risk Or Experiencing Severe Alcohol Withdrawal, Joseph Rappold, Julianne Ontengco, Stephen Tyzik, Suneela Nayak, Ruth Hanselman, Amy Sparks Sep 2019

Implementation Of Trauma Service Guideline For The Use Of Phenobarbital In The Management Of The Non-Icu Trauma Patient At Risk Or Experiencing Severe Alcohol Withdrawal, Joseph Rappold, Julianne Ontengco, Stephen Tyzik, Suneela Nayak, Ruth Hanselman, Amy Sparks

Operations Transformation

The trauma service in a large academic tertiary medical center admits a large proportion of patients with the secondary diagnosis of alcohol use disorder. Given the successful use of phenobarbital in the critical care unit for withdrawal prophylaxis and treatment of acute withdrawal, a quality improvement project was established to create and implement guidelines for the non ICU patient.

A root cause analysis demonstrated several issues to include inconsistent clinical decision documentation. As a result, several countermeasures were initiated to address the various issues.

Post implementation of countermeasures, a decrease in the amount of severe alcohol withdrawal as well as …

Journey Of The State Budget, Karina Carmichael

Masters Theses/Capstone Projects

In attempts to make government fair and equitable, the process of any bill but especially the budget bill can be extremely convoluted and difficult to follow. I was able to learn how to navigate the system and understand the processes by which things happen at the statehouse.

The statehouse is filled with representatives of countless interest groups all fighting for their opinion of how things should be done. As demonstrated by this budget cycle, government can be a push and pull directed by politics, personal opinions, and countless other factors, which may result in nothing getting done.

Department of Public Health Scholarship and Creative Works

Background: The prevalence of type 2 diabetes (T2D) in Mexico is one of the highest in the world, with high morbidity and mortality, and difficulty meeting glycemic targets. The purpose of this study was to identify the challenges for T2D self-management as perceived by both adults with T2D and health care providers in primary health clinics from Seguro Popular in Mexico City.

Methods: This was a qualitative descriptive study conducted in three Seguro Popular primary care clinics in Mexico City using convenience sampling. Semi-structured interviews were conducted with participants and data were analyzed using a content analysis approach.

Results: The …

The Power Of Pharmacist-To-Pharmacist Handoffs During Transitions Of Care, Jessica Wooster, Laressa Bethishou

Pharmacy Faculty Articles and Research

"Hospital readmissions are often medication-related and potentially preventable. Pharmacists can play a vital role in improving medication outcomes during transitions of care (TOC). Although numerous TOC practice models have been described, it remains unclear what practices will promote optimal continuity of care."

Data-Driven Decision-Support For Process Improvement Through Predictions Of Bed Occupancy Rates, Kar Way Tan, Qi You Ng, Francis Ngoc Hoang Long Nguyen, Sean Shao Wei Lam

Research Collection School Of Computing and Information Systems

Managing bed utilization and ensuring the supply keeps up with the demand is not an easy task in a large public hospital with many medical disciplines. The bed managers who makes decisions on reserving and allocating beds centrally require high-dimensional data from several hospital information systems supporting emergency room, specialized clinics and bed management processes. In this work, we put together an automated process for cleaning, consolidating and integrating data from several hospital information systems to several reports required by the bed managers to analyse the bed occupancy situations across more than thirty medical disciplines. To prevent bed crunch situations …

Data-Driven Surgical Duration Prediction Model For Surgery Scheduling: A Case-Study For A Practice-Feasible Model In A Public Hospital, Kar Way Tan, Francis Ngoc Hoang Long Nguyen, Boon Yew Ang, Jerald Gan, Sean Shao Wei Lam

Research Collection School Of Computing and Information Systems

Hospitals have been trying to improve the utilization of operating rooms as it affects patient satisfaction, surgery throughput, revenues and costs. Surgical prediction model which uses post-surgery data often requires high-dimensional data and contains key predictors such as surgical team factors which may not be available during the surgical listing process. Our study considers a two-step data-mining model which provides a practical, feasible and parsimonious surgical duration prediction. Our model first leverages on domain knowledge to provide estimate of the first surgeon rank (a key predicting attribute) which is unavailable during the listing process, then uses this predicted attribute and …

House Calls Are Reaching The Tipping Point — Now We Need The Workforce, Thomas Cornwell Jul 2019

House Calls Are Reaching The Tipping Point — Now We Need The Workforce, Thomas Cornwell

Journal of Patient-Centered Research and Reviews

Home-based primary care (HBPC) improves the lives of high-cost, frail, homebound patients and their caregivers while reducing costs by keeping patients at home and reducing the use of hospitals and nursing homes. Several forces are behind the resurgence of HBPC, including the rapidly aging population, advancements in portable medical technology, evidence showing the value of HBPC, and improved payments for HBPC. There are 2 million to 4 million patients who could benefit from HBPC, but only 12% are receiving it. The number of these patients is expected to double over the next two decades. This requires a larger and better …

Measuring Patients’ Perceptions Of Health Care Encounters: Examining The Factor Structure Of The Revised Patient Perception Of Patient-Centeredness (Pppc-R) Questionnaire, Bridget L. Ryan, Judith Belle Brown, Paul F. Tremblay, Moira Stewart Jul 2019

Measuring Patients’ Perceptions Of Health Care Encounters: Examining The Factor Structure Of The Revised Patient Perception Of Patient-Centeredness (Pppc-R) Questionnaire, Bridget L. Ryan, Judith Belle Brown, Paul F. Tremblay, Moira Stewart

Journal of Patient-Centered Research and Reviews

Purpose: Given the ongoing desire to make health care more patient-centered and growing evidence supporting the provision of patient-centered care, it is important to have valid tools for measuring patient-centered care. The patient-centered clinical method (PCCM) is a conceptual framework for providing patient-centered care. A revision to the PCCM framework led to a corresponding need to enhance the Patient Perception of Patient-Centeredness (PPPC) questionnaire. The original PPPC was aligned with the components of the PCCM conceptual framework and developed to measure patient-centeredness from the patient’s perspective. The purpose of this study was to examine the factor structure of a revised …
