A 11-year-old girl came with her grandmother due to her impacted maxillary incisor. She was suggested to receive orthodontic treatment while having routine oral examination at school. After orthodontic examination & evaluation, skeletal Class III jaw relation with orthodivergent facial pattern, Class III malocclusion and impacted maxillary central incisor was diagnosed. After full mouth fixed edgewise orthodontic guiding treatment and surgical closed eruption of the impacted incisor, the maxillary central incisor was moved into a proper position. Angle Class I molar relation, acceptable overbite and overjet and well-aligned dentition was also achieved. The total treatment period was 35 months.

Orthodontic treatment has become popular now in Taiwan, and motivation of orthodontic treatment is a key factor of treatment outcome; therefore, understanding the motivation of orthodontic patients is very important. There are many studies focusing on motivation of orthodontic patients; however, Taiwan is still lack of sufficient sample in related issue. Our study is to analyze motivations about Taiwanese to improve orthodontic treatment quality and outcome. This is a case of a 26 year-old female who complained of mandibular prognathism and malocclusion.Extraoral view revealed a concave profile with midface deficiency, mandibular prognathism, and chin deviation to right side by 4 mm. Class III malocclusion along with bilateral posterior crossbite and severe crowding were noted. Panoramic X-ray showed #83 retained, and #43 deeply impacted approximating to the mandibular lower border with a mass of radiopacity over it. With the advances of dental implants, temporary skeletal anchorage devices have now become established orthodontic anchorage aids and launched a new era for clinical orthodontic therapy. Nowadays, orthodontic miniscrews are commonly used in daily practice and demand great stability and high predictability for therapeutic needs. In recent years, numerous publications have investigated multiple factors affecting success rates of orthodontic mini-screws. The aims of this review were to investigate the relationships between timing of loading and implant stability, and the impact of osseointegration on orthodontic mini-screws.

The effect of oral appliances in treating mild to moderate obstructive sleep apnea syndrome (OSAS) patientis found to be similar to continuous positive airway pressure in a study in 1955. Oral appliances make tongue or mandible forward position to increase airway space. Mandibular advancement devices and tongue retaining devices are oral appliances used in clinic today. Long term effects of oral appliances are discussed in three parts in this article: airway, dentition and temporomadibular joint. Total airway volume increases after using oral appliances. Overbite and overjet are found decreasing. Oral appliances cannot worsen the temporomadibular joint problems.

Bimaxillary protrusion is characterized by protrusive and proclined upper and lower incisors. Since patients with bimaxillary protrusion generally have perfectly good occlusion, orthodontic treatment is often solely sought to correct the protrusive profile and less concern about the airway space change. Typical orthodontic treatment includes extraction of the bimaxillary premolars and anterior tooth retraction, with maximum anchorage. Rapid maxillary expansion (RME) is a common method used by orthodontists to increase the transverse dimension of maxillary arch and arch perimeter, by a tooth-borne device to stretch the median palatal suture through the mechanical force. The orthopedic effects produced by the RME device include not only opening the median palatal suture, but also rotating the maxillary alveolar shelves, which has an influence on the adjacent sutures of facial skeleton and the morphology of nasal cavities. Growth modification by using removable orthopedic appliances has been commonly used in growing patients to correct maxilla-mandibular jaw discrepancy. Previous studies reported the different effects of various orthopedic appliances on the airway dimension. Low-pull headgear therapy may induce a short-term decrease of the upper airway dimension in Class II patients. Face mask therapy seems to be associated with the increase of upper airway dimension in growing Class III patients. However, mandible protruding appliances can increase the upper airway dimension, especially in the Class II patients with mandible retrusion.

Acute lymphoblastic leukemia (ALL) makes up approximately 75% to 80% of childhood leukemia. Longterm survivors through current medical management make orthodontic treatment available. However, the risk of these patients should be carefully evaluated. After the patient has completed all cancer therapy and has at least a 2-year event-free survival, orthodontic treatment can be started. Patients with ALL sometimes have to accept a compromised treatment result and finish the treatment earlier than normal due to the concerns of recurrence. Corticotomy-assisted orthodontics (CAOT) can shorten orthodontic treatment time by increasing regional acceleratory phenomenon. The periodontal wound healing caused by corticotomy are similar to the periodontal tissue reaction induced by orthodontic force; if the two phenomena could be accumulated and both enhancing bone remodeling, orthodontic tooth movement should be accelerated theoretically. The main purpose of this study was to investigate the mechanical properties of Ni-Ti coated orthodontic archwires after surface modification. Universal test machine was used to measure the tensile strength of four groups of coated Ni-Ti wires and uncoated control group. Immersion test was applied for 21 days to discover whether the coating would peel off under friction and brushing after immersion. Student’s t-test was used to explore any difference between three groups of coated wires and uncoated conventional wires in mechanical properties. Anterior open bite often combines with longer anterior facial height, higher mandibular plane angle, mandible clockwise rotation and mandible retrognathism. The facial profiles of patients with OSA ( obstructive sleep apnea syndrome) also often reveal mandible retrognathism. Therefore, it is an interesting subject that whether treating anterior open bite has the advantage of improving symptom of sleeping problem. The cases of Angle Class II Division 2 malocclusion are rare in the population. These cases are characterized by retroclined upper anteriors and deep overbite. Facial profile have relatively retrusive lips as well as prominent chins. Extraction of premolars followed by incisor and lip retraction will further retrude the lips. So decided the extraction treatment plan of these patient requires careful diagnosis. In this case report, we will discuss the considerations of extraction or non-extraction treatment of Class II division 2 malocclusion.

The replacement of teeth by osseointegrated implants is usually restricted to patients with completed craniofacial growth. Implant insertions in children, adolescents and young adults who we refer to as young patients are circumvented due to several unfavorable potential effects including trauma to tooth germs, tooth eruption disorders, and multidimensional restrictions of dental alveolus and skeletal craniofacial growth. The common treatment protocol for skeletal Class II malocclusion during mixed dentition is a two-stage treatment, namely functional appliance followed by fixed appliance. The goal of the functional appliance is to enhance the horizontal growth of mandible. After reevaluation, the second stage of fixed appliance is implemented. This is a 2-case report of skeletal Class II malocclusion. Both clinical examinations showed mandible retrognathia, large overjets and full Class II Angle’s molar relationships. A 27-year-old female patient with secondary cleft deformity and malocclusion was initially treated with reconstructive surgery. However, localized severely gingival recession on the upper right first premolar and massive scar tissue over the buccal vestibular area were formed after the surgical orthodontic treatment. The surgical technique of choice in this patient was laterally positioned pedicle flap (LPF) combined with subepithelial connective tissue graft (SCTG) because of the area adjacent to the recession defect showed a great amount of attached gingiva and without interproximal bone loss. A harmonious face may involve the proper skeletal and dental relationship, in combination with the color, contour, and position of surrounding soft tissue. A 51-year-old female patient complained about her protrusive teeth and asked for improvement of her facial profile. The clinical examination of the patient revealed bimaxillary protrusion, gummy smile, and several ill-fitted fixed partial dentures. Objective: The aim of this study was to investigate the morphological characteristics of the craniofacial pattern in skeletal Class II adults with mandibular retrusion. Materials and methods: Lateral cephalograms of 80 adults with skeletal Class II malocclusion with mandibular retrusion (∠ANB > 5°, ∠FNB < 86°) compared with 80 adults with normal occlusion (equal sexes in both groups). Thirty five landmarks were identified and digitized. Cephalometric analysis utilizing 30 linear, 29 angular and 4 ratio measurements were undertaken. Statistical comparison of the two groups was performed with Student’s t-test and 2 way-ANOVA. Results: (1) The skeletal Class II sample with mandibular retrusion had longer anterior cranial base length but the internal cranial base angle and the saddle angle were not different from the normal occlusion subject. (2) The sample with mandibular retrusion had longer maxillary length but the maxilla was in normal position related to the cranial base. The mandible was more retrusive and smaller but the ramus width was wider than normal occlusion subject due to compensatory growth. (3) The mandibular retrusion sample had more tilting maxillary plane, mandibular plane, ramus plane and occlusal plane. The sample had longer total anterior facial height, longer upper and lower anterior facial height and shorter posterior facial height (shorter ramus height) than normal occlusion subjects. Hyperdivergent facial pattern and the mandible rotated clockwisely were noted. (4) The mandibular retrusion sample had extrusive but normal inclined upper incisors, more inclined and extrusive lower incisors, smaller interincisal angle, larger overjet and deep overbite. (5) The mandibular retrusion group had more mesial tipping and more extrusive upper 1st molars. Besides, more mesial tipping, more retrusive, and more extrusive lower 1st molars were noted. Conclusion: Multiple variations of skeletodental features were found for skeletal Class II malocclusion with mandibular retrusion.

Protraction facemask is one of the common effective methods to treat the patient with maxillary deficiency in mixed dentition and adolescent patients. In this case report, we present the treatment of a 10-year-old female patient with skeletal class III with maxillary retrognathism, anterior crossbite, left lateral incisor congenital missing, right peg lateral incisor and midline discrepancy with class III malocclusion. These patients often combine maxillary deficiency with anterior crossbite. In this report, an 8-year-old female patient with Class III malocclusion, anterior crossbite and protrusive lower lip.
