Class II division 1 malocclusion with deepbite and gummy smile can be treated in several ways. Adequate intrusion and retraction of maxillary incisors may improve the smiling appearance as well as the facial profile. However, anterior torque control can be challenging. Herein, we report a patient with 5 mm gummy smile, where the final esthetic improvement was achieved by combined use of posterior and anterior miniscrews. Patients with facial asymmetry usually accompany with lip canting. Most of these patients came for orthognathic surgery combine orthodontic treatment for chin deviation and asymmetric facial problem. However, the improvement of lip canting after surgery is not as predictable as correction of chin deviation. From our literature review, the correlated factors of lip canting were discussed, and the relationship between improvement of lip canting with two jaw/ one jaw orthognathic surgery were reviewed.

Purpose. To investigate etiologies associated with impacted maxillary permanent central incisors, retrospectively. In later part of this article, we also reviewed the literature of the orthodontic management of impacted maxillary permanent central incisors. Methods. Standardized periapical radiographs of upper anterior region from 2005 to 2014 in Department of Orthodontics and Pediatric Dentistry, Taichung Veterans General Hospital, Taiwan, were reviewed. From the 1025 reviewed upper anterior periapical radiographs, a total of 64 patients with 68 impacted maxillary permanent central incisors were found and were subsequently evaluated for the etiologies which cause impaction. Class III malocclusion is common in Asian population and is a major challenge in orthodontic treatment. Some patients received orthopedic treatment to correct the skeletal disharmony in an earlier stage might have to be retreated later because of differential skeletal growth of the maxilla and mandible during the pubertal growth spurt. This article reviews some literature and discusses the craniofacial growth in Class III malocclusion. The methods and formula to predict future Class III growth pattern are also reviewed. Temporary anchorage devices (TADs)are common used in orthodontic treatment for anchorage and force application. They have many advantages, including small size for insertion, potential for immediate loading, no need of patients’ compliance. The success rate of miniscrews was more than 80%. There are several factor affecting stability of TADs, including: insertion torque, soft tissue condition and bone quality of insertion area, magnitude and direction of orthodontic force, oral hygiene condition and insertion method. Recently, there are many ways to accelerate orthodontic tooth movement and relieve pain during the treatment progress. “Low level laser therapy” which is proven by many clinical studies is a non-invasive way to achieve these goals. This article reviews cellular, animal, and clinical studies about low level laser therapy used in orthodontics.

While moving teeth under orthodontic force, there are unavoidable tissue reaction due to the forces applied to the dental pulp and periodontal tissue. Under appropriate orthodontic treatment planning, most of the tissue reactions were limited to temporary pulpitis, blunting of dental root apex etc. However, a few patients will start up serious reactions such as dental pulp obliteration, tooth discoloration, dental pulp necrosis, or even severe apical root resorption and invasive cervical root resorption. This 25-year-old male was characterized with mandibular prognathism and spaced dentition. We applied bilateral sagittal split osteotomy and genioplasty to set back the mandible and the prominent chin. The excess spaces of lower dentition were consolidated and then closed surgically by anterior subapical osteotomy. After treatment, his facial esthetics and occlusion were improved significantly. In the spaced dentition, the space closure by anterior subapical osteotomy after space consolidation may provide a more efficient alternative treatment.

Orthodontic management in adult patients with advanced periodontitis is complicated and highly technique sensitive because of missing teeth and reduced alveolar attachment.This case report presents a 32-years old female who was referred from periodontal department, complained about maxillary protrusion and anterior spacing. The diagnoses were Class II division 1 malocclusion, combined with bilateral posterior crossbite and deep overbite. Three-dimensional planning of a dental-implant site is critical when the dental implants placed in the edentulous area are to be used initially as anchor for tooth movement and later as prosthetic abutments for the fixed restoration. The combined orthodontic and prosthetic implant restorative treatment of an adult patient with missing teeth, diastema of teeth, and malocclusion is described in this article. Plastic cast bases combined with customized position plates allowed the precise transfer of the proposed implant position from the diagnostic arrangement to the original cast to avoid compromising subsequent orthodontic treatment.

Correction of facial asymmetry is one of the most difficult challenges in orthognathic surgery. Bardinet classified facial asymmetries based on three clinical entities: mandibular lateral deviations, dental asymmetries without skeletal involvement, and skeletal asymmetries. Objective: Tooth agenesis is quite common in cleft lip and palate patients. This study was aimed for retrospectively investigating the prevalence and patterns of tooth agenesis among bilateral cleft lip and cleft palate (CLCP) patients. Patients: The hospital data and x-ray films of bilateral CLCP patients no younger than 7 years old in the Craniofacial Center, Kaohsiung Chang Gung Memorial Hospital from June 1974 to September 2015 were selected. The digital dental models are not subject to physical damage, and require negligible storage space. They can be transferred electronically to specialists easily and can be carried out with virtual setups. A precision 3-dimensional head model can be fabricated by integrating a digital dental model into a maxillofacial 3-D CT image. Using this 3-D image, it is possible to perform dental decompensation, reposition of osteotomized bony structures, and simulate the postoperative results on hard and soft tissue in 3-D on our computer screen. Besides, it also spares the laboratory time with CAD/CAM surgical splint manufactured.

The etiologies of skeletal Class III malocclusion include excessive growth of mandible, deficient growth of maxilla, and the combination of both. Anterior openbite may be due to skeletal, dental, habitual, or functional problems. Mini-implant can be a reliable anchorage for maxillary molar intrusion, which facilitate anterior openbite closure by counterclockwise rotation of the mandible. However, the mandibular counterclockwise rotation may increase antero-posterior jaw discrepancy in Class III malocclusion. The overall prevalence of peg-shaped maxillary permanent lateral incisors was 1.8%. The occurrence rates were higher in Mongoloid (3.1%) than in black (1.5%) and white (1.3%) patients. The presence of peg lateral incisors was also a complicated esthetic issue for orthodontist. This case is a skeletal Class III malocclusion 20-years-old male. Concurrent impaction and transposition of the maxillary anterior teeth is unusual and may present a great challenge to dental practice. Early diagnosis and elimination of impacted anterior teeth helps provide esthetic and functional outcomes. The purposes of this literature review were to evaluate and compare the correction rate of posterior crossbite of different kinds of maxillary expansion. The study selection criteria included RCTs involving subjects with posterior crossbite with maxillary expansion which searched from MEDLINE. This case report describes a 22-year-old female with skeletal Class II and dental Class II division 1 malocclusion. The chief complaint is her protrusive upper front teeth. Obstructive sleep apnea (OSA) is the most common category of sleep-disordered breathing. The common signs of OSA include loud and irregular snoring and disturbed sleep due to the multiple awakenings. OSA is frequently associated with cardiovascular and metabolic disorders. Many studies have confirmed that there are clear benefits for patients who treated OSA. Continuous positive airway pressure (CPAP) is the gold standard treatment for OSA, but there are many patients with OSA who are intolerant of CPAP therapy. Mandibular symphysis is the critical reference for facial profile and determines the lower incisor position indirectly. It also serves as the primary consideration for diagnosis and treatment plans. This essay reviews articles relevant to the embryology and growth, morphology of mandibular symphysis, and its restrain on orthodontic tooth movement of lower incisors. Discussion mainly focused on the effectiveness of genioplasty, mandibular symphysis distraction osteogenesis, periodontally accelerated osteogenic orthodontics on mandibular symphysis, which may breach its limitation on range of orthodontic tooth movement.

Idiopathic condylar resorption (ICR) is a well-documented but poorly understood progressive disease that affects the TMJ. ICR can result in malocclusion, facial disfigurement, TMJ dysfunction, and pain. Some cases occur spontaneously, whereas others appear during orthodontic therapy or as sequelae to orthognathic surgical procedures. It is essential for orthodontists to recognize the onset of ICR in patients as early as possible .This article discusses practical aspects of recognizing and managing ICR in orthodontic patients by a combination of orthodontic and orthognathic surgery treatment.
