OBJECTIVES: This study was undertaken to investigate the incidence of posttransplant recoarctation of the aorta, delineate the mode of presentation, identify risk factors that predict recoarctation and examine the results of intervention for posttransplant recoarctation.

BACKGROUND: Patients with aortic arch hypoplasia require extended arch reconstruction at transplant, with an inherent possibility of subsequent recoarctation of the aorta.

METHODS: This was a retrospective review of all children (ageyears) who underwent cardiac transplantation over a 10-year period. OBJECTIVES: The purpose of this study was to determine the feasibility, safety and diagnostic accuracy of dobutamine stress echocardiography (DSE) for evaluating posttransplant coronary artery disease (TxCAD) in children, and to determine the frequency of selected cardiac events after normal or abnormal DSE.

BACKGROUND: Posttransplant coronary artery disease is the most common cause of graft loss (late death or retransplantation) after cardiac transplantation (CTx) in children. Coronary angiography, routinely performed to screen for TxCAD, is an invasive procedure with limited sensitivity. The efficacy of DSE for detecting atherosclerotic coronary artery disease is established, but is unknown in children after CTx. …

The ability of cells to respond to mechanical stimuli has been studied through a variety of techniques in numerous cell types. The cells of the vascular wall have adapted to specific mechanical stresses through the activation of intracellular signaling pathways which result in cell-specific responses such as hypertrophy, hyperplasia, proliferation, and migration. Vascular smooth muscle of the arteries have been shown to be sensitive to mechanical stimuli such as stretch, and pressure.

The blood-brain barrier (BBB) is formed by cerebral vascular endothelial cells. Brain ion and fluid homeotasis essential for proper neural functioning is due to the BBB. Sodium-potassium and calcium-activated adenosine triphosphatase (Na+, K+-ATPase and Ca2+-ATPase) serve as one of the main mechanisms controlling brain Na+, K+, and Ca2+ concentrations. Plethysmography is a technique used for measuring alterations in the volume of organs or limbs. Japanese researchers have developed a relatively new application of plethysmography (accelerated plethysmography) that uses information from the pulse wave measured at the fingertip to make inferences about health and fitness status. The instrument used to collect data -- the Precaregraph (accelerated plethysmograph) -- provides an accelerated pulse waveform and APG index, a mathematical representation of the waveform. OBJECTIVE: The concept of a lateral tunnel for the Fontan operation is now widely accepted. Most lateral tunnels are constructed intraatrially with the use of aortic crossclamping. Construction of extracardiac lateral tunnels with the use of homografts or other nonviable tubes eliminates aortic crossclamping but lacks growth potential in length or width. The native pericardium, which is "sealed" posteriorly along the pulmonary artery, atrium, and inferior vena cava, could be turned down onto the right atrium to form a viable extracardiac lateral tunnel.

This research examined potential differences in cardiovascular disease risk (CVR) in two groups: Japanese Natives living in Saga, Japan and Japanese Americans living in or near Portland, Oregon. It explored data gathered in an effort to build a theoretical framework for future studies. Environmental, nutritional, and lifestyle differences were also examined to compare the health status of Japanese residents in the two countries. The history of Japan from World War II to present, and the history of Japanese Americans both first and second generation, are discussed in the literature review.

Ion mobility spectrometry (IMS) was used to detect cocaine and/or the metabolite ecgonine methyl ester (EME) in tissue and plasma samples from male, maternal, and fetal New Zealand White (NZW) rabbits. EME was consistently detected in all adult plasma samples 5 minutes after the injection of cocaine hydrochloride (cocaine•HCI, 2 mg/kg). EME was also in the liver, kidneys, testicles, and ovaries. Five minutes after the injection of cocaine•HCI, the male arterial pressure (MAP) increased from a mean control value of 79 ± 3.2 mmHg to 88 ± 4.1 mmHg, and the pCO2 increased from a control value of28 ± …

This study was designed to characterize structural remodeling of male Wistar rat mesenteric arteries exposed to elevated blood flow in vivo for 1, 3, or 7 days. A series of arterial ligations induced blood flow increases in ileal and second-order branch arteries compared to same animal control vessels. Neither mean carotid nor local mesenteric arterial pressures changed significantly pre- to post-ligation. The primary flow-mediated force in both vessels was shear stress with possible involvement of acute stretch-induced wall stress in the ileal artery. A Quasi-One-Dimensional Theory For Anisotropic Propagation Of Excitation In Cardiac Muscle, Jiashin Wu, Edward A. Johnson, J. Mailen Kootsey

It has been shown that propagation of excitation in cardiac muscle is anisotropic. Compared to propagation at right angles to the long axes of the fibers, propagation along the long axis is faster, the extracellular action potential (AP) is larger in amplitude, and the intracellular AP has a lower maximum rate of depolarization, a larger time constant of the foot, and a lower peak amplitude. These observations are contrary to the predictions of classical one-dimensional (1-D) cable theory and, thus far, no satisfactory theory for them has been reported. Early left ventricular (LV) remodeling following pediatric cardiac transplantation has not been described. To identify patterns and determinants of change in left ventricular mass and volume posttransplant, we studied 125 consecutive children who underwent cardiac transplantation between January 1, 1989 and July 31, 1993. Two-dimensional imaging-directed M-mode echocardiograms were studied weekly until 26 weeks post-transplant. LV mass and volume (indexed to BSA1.5) were measured. LV mass index increased until 3 weeks post-transplant, and then decreased. The mean decrement in LV mass index after 8 weeks post-transplant (relative to baseline) was significantly larger in patients with donor-recipient weight ratio > 1.5 compared …

The purpose of this research was to determine the effects of cryopreservation on the physical, biochemical and cellular aspects of heart valve tissue. The present study demonstrated that radiolabeled proline accumulation provides a quantitative estimate of total cellular metabolic viability. Radiolabeled inulin was demonstrated to be a useful marker for extracellular tissue space. Assessment of metabolic viability of the cellular population of leaflet tissue as a function of procurement and processing revealed that proline accumulation declined with increasing warm and cold ischemic times. Amphotericin B and streptomycin were toxic to the cells. Cefoxitin, lincomycin, polymyxin B, vancomycin and penicillin had …

A basic relationship between stethoscopic auscultation and background noise interference was reviewed and examined in this study. The principle experimental design of the study questioned whether hospital background noise levels are capable of masking the threshold of detection for auscultated heart sounds. The dynamics of transcapillary fluid and protein distribution and endocrine regulation of cardiovascular function were studied in chronically instrumented fetal sheep during the third trimester. Average experimental changes in vascular pressures, blood volume, plasma hormone concentrations, hematocrit and plasma protein concentration were analyzed using mathematical modeling and parameter optimization techniques in order to determine values for the major parameters which regulate fluid, protein and endocrine dynamics and to determine endocrine dose-response relationships to changes in cardiovascular function. An investigation was undertaken to synthesize and evaluate the cardiovascular activity of two imidazole ring systems. Imidazoles with hypotensive activity would provide novel potential antihypertensive drugs. These novel structures may also find use as chemical probes in the study of the cardiovascular system and as prototypes for other potential cardiovascular agents.

The histamine derivative, 7,7-dipyridyl-4,5,6,7-tetrahydroimidazo-[4,5-c]pyridine (ligand l), was demonstrated to possess significant hypotensive activity in laboratory rats. A 15 mg/Kg dose of ligand 1 produces a statistically significant decrease in blood pressure when injected IP. This hypotensive effect is also associated with marked increase in heart rate. The hematological and rheological characteristics of blood from a number of vertebrates was compared to assess possible species differences in blood viscosity that may influence cardiovascular oxygen transport. Nucleated red blood cells (RBCs) were more viscous (measured by cone-plate viscometry) in comparison with enucleate (mammalian) RBCs at hematocrits greater than 40% when measured at equivalent temperatures. The lower viscosity of enucleate RBCs is attributed to an enhanced deformability of enucleate cells in comparison to nucleated cells.

A computer model of the left ventricular (LV) epicardial surface was developed from Jang’s truncated ellipsoidal model, which has a minor to major axis ratio of 6:10. The model was applied in calculating the area of a segment of the myocardium, jeopardized by an impending occlusion of its associated artery. The endangered segment of the LV muscle was also visualized in different projections by rotations in three space and graphical displays simulating the three dimensions.

The problem of this study was to review 100 open heart surgical cases in a selected hospital to determine the changes occurring in central venous pressure in relation to blood loss.

The review of literature revealed that central venous pressure is an important factor in determining immediate postoperative care of open heart surgical patients. If central venous pressure falls below the normal range of 5 to 10 centimeters of water, hemorrhage is suspected. If central venous pressure is elevated between 15 and 20 centimeters of water, cardiac tamponade is suspected.

Literature was reviewed in the areas of patients' opinions about their hospitalization and pertinent concepts in medical and nursing literature regarding cardiac rest. A study was not found which polled cardiac patient specifically regarding their ability to rest. Most authorities in this area strongly advocate rest for the cardiac patient, but there is disagreement of how and how long the patient should rest.
