This Is a retrospective quality assurance study of all frozen sections done at The Aga Khan University Hospital during a six year period (1986 to 1991). There were 1,031 frozen sections out of a cumulative total of 42,985 surgical specimens (2.39%). Nine hundred and seventy-six (94.66%) were concordant. In 92(8.9%) fresh specimens were brought from other hospitals of Karachi, in 37 cases (3.58%) the diagnosis was deferred till the evaluation of permanent paraffin sections and 18 (1,74%) were discordant with 7 (0.67%) false positive and 11(1,06%) false negative. Trismus associated with mandibular fractures is thought to increase the risks of general anesthesia. Nasotracheal intubation is necessary since the dental arches will be occluded at the end of the procedure. To establish an airway, an awake nasal intubation or fiberoptic endoscopy is required if direct laryngoscopy cannot be performed due to trismus. A prospective analysis of fifty-one patients with various types of mandibular fractures indicated that trismus was for the most part due to two factors, pain induced guarding and infection. Tracking Plasma Lactate Concentration In Vivo With A Catheter-Tip L-Lactate Sensor, Brett T. Weinzapfel, Mark D. Ball, Lee R. Waite, Nacer E. Abrouk, Shun P. Lim

To circumvent the problems of repeated blood sampling for in vitro analysis, a catheter-tip L-lactate sensor has been developed. The sensor was tested in anesthetized pigs (n=6). The sensor in vivo tracked the lactate concentration non-linearly, seeming to obey Michaelis-Menten kinetics. Calibration time was short, typically 1.5 min per lactate standard. Furthermore, time drift was small, typically -1.3% to -3.3% per hour of in vivo use.

The validity of the Minnesota Multiphasic Personality Inventory (MMPI-2) clinical scales in differentiating schizophrenia and bipolar-manic disorders was evaluated in terms of MMPI-2 congruence with psychiatric hospital treatment team diagnoses. From a pool of 286 MMPI-2 profiles, valid profiles were obtained on 5 schizophrenia (paranoid type) inpatients and 5 bipolar-manic disordered psychiatric inpatients and were assessed by using each of four MMPI profile classification methods. Overall, the rate of agreement between treatment team diagnosis and the MMPI profile classification methods for both the schizophrenia (paranoid type) and bipolar-manic disordered groups was inconclusive. Reversed-phase high-performance liquid chromatography (RP-HPLC) is an important tool in the purification of radiolabeled peptides and proteins for immunoassay. However, for some proteins and peptides it is difficult to achieve reproducible behavior in RP-HPLC because of the low recovery of these compounds. Factors that can be varied to improve recovery include the strength or pH of the mobile phase, the chain length and spacing of groups on the reversed-phase support, and the flow rate or steepness of the elution gradient (1-5). ... The performance and clinical utility of an automated assay of intact parathyroid hormone (parathyrin, PTH) are evaluated. The method is based on the extraction of PTH from plasma by an HPLC column containing immobilized anti-(44-68 PTH) antibodies. The PTH retained is detected with a postcolumn reactor and use of anti-(1--34 PTH) chemiluminescent-labeled antibodies. The total cycle time of the assay is 6.5 mm per injection after a 1-h incubation.The lower limit of detection for PTH in a 66-pL plasma sample was 0.5 pmol/L based on peak heights and 0.2 pmol/L based on peak areas. Although a number of manufacturers are marketing disposable prophylaxis angles, no literature exists regarding the mechanical efficacy and efficiency of these products. It was the purpose of this in vitro evaluation to compare and evaluate five brands of commercially available disposable prophylaxis angles for vibration, noise, heat rise, and torque. Random samples of each brand of disposable prophylaxis angle were utilized. A wide variety of disposable infection control products is being marketed including disposable prophylaxis angles. It was the purpose of this in vivo investigation to evaluate the clinical efficacy of five different brands of commercially available disposable prophylaxis angles. Utilizing a splitmouth design. 11 dental hygienists evaluated disposable prophylaxis angles while completing a routine oral prophylaxis. Fifty samples each of Brahler. Ash/Dentsply, Denticator. and Young Dental angles were compared to a control angle (Teledyne Getz). The purpose of this study was to compare the fracture strength of ceramic brackets which are currently on the market with ceramic brackets tested in a previous study at Loma Linda. Fracture strengths of standard edgewise ceramic brackets vs. pre-torqued ceramic brackets and bicuspid ceramic brackets vs. central incisor ceramic brackets were also compared.

Radiation due to x-ray exposure is a concern, and the development of the Dolphin™ DigiGraph represents an attempt to limit this exposure. The purpose of this paper was to compare measurements (using Ricketts analysis) obtained from conventional radiographic cephalometric tracings with those obtained by computerized digitizing with the DigiGraph. Forty-six patients from the Loma Linda University School of Dentistry graduate orthodontic clinic were digitized, and the results were compared to the x-ray tracings of these patients. In this study arch length discrepancy or ALD was studied in 50 untreated patients. The measurements obtained from Hilgers' corrected occlusal x-ray using his Occlusal Paralleling Instrument or O.P.I, were compared with those derived by four conventional methods, - "Eye balling," digital caliper determination, conventional mandibular x-ray, and the Rocky Mountain Data Systems computer analysis (31 cases). When using the OPI, results give a higher negative ALD as it takes into consideration the space required to level a curve of Spee.

The pharmacological effects of suxamethonium and pancuronium on the cardiovascular system may vary and therefore alter the haemodynamic response to intubation. The arterial blood pressure, the heart rate and the rate pressure product were measured as parameters of haemodynamic change in forty adult ASA. I and II patients undergoing laryngoscopy and endotracheal intubation in a randomised controlled study. The patients were induced with either thiopentone/suxamethonium (Group A) or thiopentone/pancuronium (Group B). There was no significant difference between the groups on comparison of systolic and diastolic blood pressure changes. If nuclear binding sites for estrogens exist in bone cells, then the origin of estrogen-deficiency-induced osteopenia may be at the level of bone matrix protein synthesis. Since collagen is the most abundant protein in the bone matrix, and since hydroxyproline is an imino acid nearly exclusive to collagen, a monitoring of the metabolism of hydroxyproline in ovariectomized and estrogen treated rats may provide insight into the patterns of estrogen-deficiency-induced osteopenia. This study was performed to determine if there is a quantifiable difference between quantitative fit testing using a probed facepiece and a probed cartridge. Four subjects were tested on four different respirators. The respirators used were MSA & 3M full and half face respirator. Each subject performed each method five times per respirator. The results indicated no correlation between the two fit testing methods overall. However, a moderate correlation was found between the MSA respirators and the two methods. The rapid palatal expander is an appliance which has been used to laterally expand the maxillary dental arch, with subsequent mandibular dental arch expansion.

The purpose of this study was to evaluate and quantify long term post treatment changes in the length and width of both dental arches following rapid palatal expansion.

Plaster models from eleven patients treated with rapid palatal expansion were obtained seven or more years after treatment. The dental arches were intact anterior to the first molars and no retreatment had taken place.

The purpose of this study was to determine the relative safety of headgear safety straps. Samples from five different safety headgear straps simulated orthopedic and orthodontic use and data were collected from a series of pulls on an Instron machine. The straps were compared by the distance the face bow traveled before the safety mechanism released, the force built into the face bow at safety release and the force required to remove the inner bow out of the molar tube. This clinical study evaluated a modified Nance appliance designed by the author to distalize molars unilaterally for Class II correction. This fixed appliance did not rely upon extraoral traction or intraoral elastics. Twelve unilateral Class II patients aged 13 to 17 were treated nonextraction with the appliance. Pretreatment and posttreatment photographs were taken of the palatal soft tissue and did not reveal any untoward changes as a result of the Nance acrylic button.
