Laparoscopic liver biopsy (LLB) provides a way to obtain liver tissue samples under direct visual inspection of the liver surface. It complements percutaneous liver biopsy (PLB) for complex or high-risk cases where PLB cannot be performed. Clinical practices for LLB vary across centers due to the lack of international standards. To improve the procedural quality and diagnostic efficiency of LLB, a multidisciplinary panel of 45 international experts from 6 continents, including hepatology, hepatobiliary surgery, gastrointestinal surgery, and metabolic and bariatric surgery, developed this consensus statement through three rounds of a modified Delphi process. This consensus covers clinical application scenarios, high-risk …

Background: Understanding how bones adapt during growth is essential for promoting lifelong skeletal health. Peripheral quantitative computed tomography (pQCT) is a widely used imaging technique for assessing volumetric bone mineral density (vBMD) in pediatric populations; however, conventional methods may obscure subtle cortical adaptations. The primary aim of this study was to evaluate the feasibility of shape-based cortical vBMD profiling, with an exploratory objective of assessing its sensitivity to inter-individual variation in lean mass index (LMI). Methods: Eighteen children (6–12 years) underwent pQCT scanning at the 38% and 66% tibial sites (distal and proximal diaphysis). Cortical vBMD was quantified across three …

Abdominal aortic calcification (AAC), a marker of subclinical cardiovascular disease, has previously shown to be associated with low BMD and fracture. However, it remains unclear whether AAC is associated with trabecular bone score (TBS), a gray-level textural measure, or whether it predicts fracture risk independent of this measure. Here, we examined the cross-sectional association of AAC scored using a validated machine learning algorithm (ML-AAC24) with TBS, and their simultaneous associations with incident fractures in 7691 individuals (93.4% women) through the Manitoba BMD Registry (mean age 75.3 yr). The association between ML-AAC24 and TBS was tested using generalized linear regression. Cox …

Globally, regional and remote communities are burdened by both an increased prevalence and worse prognosis of many infectious and chronic diseases. However, largely owing to logistical challenges, these communities are under-represented in clinical trials and research studies. As individuals from rural communities experience unique environmental exposures and risk factors for disease, immune phenotyping data collected from metropolitan populations may not be broadly generalizable. To address this, we present a workflow that enables the inclusion of resource-limited sites in high-parameter mass cytometry studies. In this approach, whole blood (WB) or peripheral blood mononuclear cells (PBMCs) are collected, stained fresh for surface …

Background: Abdominal aortic calcification (AAC) is a subclinical measure of atherosclerotic cardiovascular disease (ASCVD). AAC can be captured on lateral spine images obtained from bone density machines during routine osteoporosis screening. Identifying individuals with AAC provides a new opportunity to prevent disease progression. Objectives: The aim of the study was to externally validate a machine learning-derived AAC 24-point algorithm (ML-AAC24) with incident ASCVD. Methods: Middle-aged individuals from the UK Biobank Imaging Study with lateral spine images, obtained via dual-energy x-ray absorptiometry, were included. ML-AAC24 scores were grouped as low (<2), moderate (2 to <6), and high (≥6). Linked health records were used to identify ASCVD-associated events, including hospitalizations and death. Results: Among 53,611 participants (52% female; mean age 65 years), 78.2% had low, 16.4% had moderate, and 5.4% had high ML-AAC24. After excluding people with prevalent ASCVD or missing data, 1,163 (2.3%) of 50,923 people had an incident ASCVD event over a median follow-up of 4.1 [3.0-5.5] years. In age- and sex-adjusted analysis, compared to those with low ML-AAC24, those with moderate (HR: 1.80 [95% CI: 1.57-2.08]) and high ML-AAC24 (HR: 2.87 [95% CI: 2.39-3.44]) had a higher HR for incident ASCVD. Results remained comparable after adjustment for established ASCVD risk factors. Consistent patterns were observed when considering incident coronary artery disease, myocardial infarction, and stroke. Conclusions: Assessing ML-AAC24 on lateral spine images offers a new and promising screening method to identify people with higher risk of incident ASVD events.

Background: Assessing bone mineral density (BMD) in children and adolescents is essential for detecting skeletal deficits and fracture risk. Peripheral quantitative computed tomography (pQCT) has gained increasing attention as a valuable imaging tool due to its ability to assess bone quality. Unlike dual-energy x-ray absorptiometry, pQCT provides volumetric BMD measurements and distinguishes between cortical and trabecular compartments. Despite these advantages, its clinical adoption remains limited due to the lack of reference data and standardized protocols. Methods: A systematic search was conducted in CINAHL, Embase, PubMed, SPORTDiscus, and Web of Science from inception to November 2024. Studies reporting pQCT-derived tibial BMD …

Insight into disease detection and treatment through comprehensive immune phenotyping relies on the generation of high-quality data. However, the execution of robust immune monitoring in clinical trials by flow cytometry is complex due to logistical challenges in sample preparation and reagent stability. These challenges are exacerbated when considering remote and rural communities in Australia, which are burdened by both an increased prevalence and a worse prognosis of many chronic and infectious pathologies. This is not unique to the Australian context as globally remote and rural communities are underrepresented in biomedical research studies, contributing to persistent health inequities. To address these …

Accurately performed thermoregulation is life-important for the human body. Therefore, a relatively narrow temperature range of 36.5–37 °C, which all our biochemical reactions are adapted to, is rigorously kept by the body allowing for the most effective kinetics of all physiological processes. In contrast, feeling inappropriately cold or too hot in the environment with comfortable temperature ranges are symptoms of an altered or even disordered thermoregulation described for a number of syndromes as well as patient cohorts. The rationale of the paper is to contribute to the paradigm shift from reactive to proactive healthcare considering thermoregulation deficits as an important …

Research outputs 2022 to 2026
Aim: Deep learning (DL), a branch of artificial intelligence (AI), has been applied to diagnose developmental dysplasia of the hip (DDH) on pelvic radiographs and ultrasound (US) images. This technology can potentially assist in early screening, enable timely intervention and improve cost-effectiveness. We conducted a systematic review to evaluate the diagnostic accuracy of the DL algorithm in detecting DDH. Methods: PubMed, Medline, EMBASE, EMCARE, the clinicaltrials.gov (clinical trial registry), IEEE Xplore and Cochrane Library databases were searched in October 2024. Prospective and retrospective cohort studies that included children (< 16 years) at risk of or suspected to have DDH and reported hip ultrasonography (US) or X-ray images using AI were included. A review was conducted using the guidelines of the Cochrane Collaboration Diagnostic Test Accuracy Working Group. Risk of bias was assessed using the QUADAS-2 tool. Results: Twenty-three studies met inclusion criteria, with 15 (n = 8315) evaluating DDH on US images and eight (n = 7091) on pelvic radiographs. The area under the curve of the included studies ranged from 0.80 to 0.99 for pelvic radiographs and 0.90–0.99 for US images. Sensitivity and specificity for detecting DDH on radiographs ranged from 92.86% to 100% and 95.65% to 99.82%, respectively. For US images, sensitivity ranged from 86.54% to 100% and specificity from 62.5% to 100%. Conclusion: AI demonstrated comparable effectiveness to physicians in detecting DDH. However, limited evaluation on external datasets restricts its generalisability. Further research incorporating diverse datasets and real-world applications is needed to assess its broader clinical impact on DDH diagnosis.

Diagnostic tests are frequently applied within clinical practice to assist with disease diagnosis, differential diagnosis, disease grading and prognosis evaluation. Receiver operating characteristic (ROC) curve analysis is one common approach for analysing discriminative performance of a diagnostic test, where it can determine the optimal cut-off value with the best diagnostic performance.1 However, as a majority of clinicians are non-statisticians, several errors have been observed in clinical research when applying ROC curves. These errors may be misleading in the selection of diagnostic tests and disease diagnosis, thus adding to patient burden. To address these errors, clinicians do not need a …
Optimal Bag Valve Mask Ventilation Efficacy Of Smart Bag Ventilation Compared To Traditional Adult And Paediatric Bag Valve Ventilation During Cardiopulmonary Resuscitation, Ian Dawson
Theses: Doctorates and Masters
Background: Out-of-Hospital Cardiac Arrest (OHCA) is one of the world’s leading causes of mortality. Survival rates are low, ranging from 3.1–20.4% internationally. The most effective oxygenation method for patients during pre-hospital cardiac arrest is ventilating using a self- inflating Bag-Valve-Mask (BVM). Yet, extensive research has identified a lack of compliance with ventilator guidelines during OHCA cardiopulmonary resuscitation (CPR). As a result, clinicians often ventilate patients too aggressively using the BVM, delivering breaths excessively higher than the International Liaison Committee on Resuscitation (ILCOR) recommended rate of 10 breaths per minute (BPM). Hyperventilation can lead to complications such as aspiration, barotrauma, and …

Surgical subspecialties have successfully combined robotic technology with traditional practices. In doing so, they have benefited from increased precision in techniques, reduced incidence of surgical complications, and reduced MSK disorders. This scoping review aims to examine the extent of cobotics in obstetric ultrasound. The review was conducted using the JBI PRISMA protocol to map the topic of robotics in obstetric ultrasound. Teleoperated remote ultrasound formed the largest combined clinical cohort of 67 patients. Limitations of the 20-week scan concerned incomplete visualization of fetal structures. These included cavum septi pellucidi, cardiac outflow tracts, kidneys, lips and spine. Third trimester scans omitted …

Bacterial vaginosis (BV) is an abnormal gynecological condition caused by the overgrowth of specific bacteria in the vagina. This study aims to develop a novel method for BV detection by integrating surface-enhanced Raman scattering (SERS) with machine learning (ML) algorithms. Vaginal fluid samples were classified as BV positive or BV negative using the BVBlue Test and clinical microscopy, followed by SERS spectral acquisition to construct the data set. Preliminary SERS spectral analysis revealed notable disparities in characteristic peak features. Multiple ML models were constructed and optimized, with the convolutional neural network (CNN) model achieving the highest prediction accuracy at 99%. …

Research outputs 2022 to 2026
Polymeric vascular bioresorbable stents (BRSs) have been widely used for the treatment of coronary artery diseases. While additive manufacturing (AM) is changing the landscape of the healthcare sector by enabling the realization of patient-specific stents with highly complex structures. There are, however, challenges associated with the use of polymeric BRS, particularly in-stent restenosis (ISR), related to its poor mechanical properties. Therefore, the aim of this review is to provide an overview of recent advancements in the development of polymeric BRSs designed to meet both mechanical and biological requirements. First, biopolymers as well as shape memory polymers (SMPs) that are suitable …

An individual participant data (IPD) meta-analysis can assess the predictive value of data on outcomes at the individual level, offering a potential tool for developing personalized pain management. Pretreatment quantitative sensory testing (QST) may stratify patient groups, which are then linked to treatment outcomes. Our objective was to determine if measures of QST at baseline are related to treatment outcomes (at any time point) for pain and disability in lower-limb osteoarthritis. We performed a systematic review with an IPD meta-analysis. Searches were conducted in 9 databases until May 5, 2023 for intervention studies that measured baseline QST and longitudinal measures …

Feasibility Of Conducting A Randomized Controlled Trial To Explore The Muscular Influence Of Post-Operative Intravenous Iron Treatment For Anaemia After Major Abdominal Surgery, Beth Maclean, Robert U. Newton, Jayne Lim, Toby Richards
Research outputs 2022 to 2026
Background and Objectives: Iron deficiency anaemia is common in patients recovering from major surgery and is associated with poorer post-operative outcomes. We designed a randomized controlled trial treating post-operative anaemia with iron therapy to observe the influence on post-operative recovery. Materials and Methods: Anaemic patients (haemoglobin [Hb] < 120 g/L for women, Hb < 130 g/L for men) recovering from major abdominal surgery at Fiona Stanley Hospital were recruited. Patients were double-blind randomized 1:1 to ferric carboxymaltose (FCM) or saline administered 4 weeks post-discharge and included in a 12-week exercise programme. Iron indices, quality of life (QoL) questionnaires and muscle function tests were conducted at 4 weeks (baseline), 8-, 12- and 16-weeks post-discharge. This pilot study primarily aimed to assess the feasibility of recruiting 20 patients per intervention arm. Trial registration: ACTRN12622001447741. Results: Of 205 eligible patients screened between 5 May 2023 and 31 August 2023, only four patients were recruited, of which one completed the study. After randomization to FCM, results obtained from the sole participant that completed the trial suggested a trend towards improvement in QoL outcomes, Hb and muscle function. Conclusion: Recruitment to a randomized controlled trial exploring the influence of iron therapy on muscle function after major abdominal surgery was not feasible.
