Patients in rural areas are more likely to experience worse health outcomes than patients in urban or suburban areas. The reasons for this discrepancy are multi-faceted, including inequities in resources and access, as well as differences in the environment. Rural providers understand what education and resources patients are missing as they often interact with patients in multiple contexts. We interviewed rural providers and performed an inductive thematic content analysis of the interviews to shed some insight into what information rural patients would need to be better healthcare advocates for themselves. The origin of the patient-centered interdisciplinary collaborative round, which involves health care members of all levels at a time, is an effective and emerging approach to offer inpatient healthcare. Traditionally, post-partum rounds at teaching hospitals consisted of separate visits from all members of the obstetric team that resulted in patient inconvenience, care insufficiencies, and communication gaps. The main objective of this study was to assess the patient's satisfaction level from the traditional post-partum round versus the patient-centered collaborative care (PCCC) round. This study showed that in the traditional round, the patient's satisfaction with the treatment was 95.1%; whereas, in PCCC …

This poster evaluates the current understanding of Takotsubo Cardiomyopathy (TTC) and describes current treatment recommendations, long-term prognosis, and advanced practice nursing implications. TTC is a relatively rare disease and is present in about 2% of suspected ST-segment myocardial infarctions in women. (Sattar et al., 2017) TTC is not well understood and has several current hypotheses regarding cellular pathology. This poster explores the current understanding of pathophysiologicderangements from an organ system and two pathologic hypotheses at the cellular level. Over the last century, science and literature has reported neurological manifestations during various disease outbreaks as well as long-term sequelae following epidemics and pandemics. Some of the first neurological symptoms reported with Coronavirus Disease 2019 (COVID-19; caused by severe acute respiratory syndrome coronavirus 2 [SARS-Cov-2]) were anosmia and dysgeusia, suggesting SARS-CoV-2 penetrance of the central nervous system (CNS). To date, the definite clinical and pathological basis of CNS involvement of SARS-CoV-2 remains poorly understood, with the neurovirulence of SARS-CoV-2 and its neuroinvasive potential remaining largely unknown. This poster takes a glimpse at one of these factors: the role of neuroinflammation in mood disorders. Above normal levels of inflammation are found in patient’s suffering from depression and bipolar disorder. Elevated inflammation has significant negative effects on the brain that as a result cause dysregulation in mood. Mental health treatments that have been found to effectively address neuroinflammation include nutritional and exercise interventions. Those working in pharmaceuticals are in a tempting position as the nature of the product and supply is complex, making detection of fraud difficult and expensive. However, a reliable pharmaceutical supply can often be a life-or-death situation for patients. Thus, when detection of fraud occurs, a Regulator's Dilemma often emerges (recall a drug for which a supplier is the sole source, or allow a substandard product to be sold)—generally resulting in pharmaceutical companies receiving minimal penalties even for the worst acts. Background: There is a global emphasis on engaging community health volunteers (CHVs) in low- to middle-income countries (LMICs) to reach to the vast underserved populations that live in rural areas. The obesity pandemic is associated with increased consumption of restaurant food. Labeling of menus is an intervention used to provide consumers with kilocalorie (calorie) information in hopes of them making healthier food choices. This study evaluated the relationship between young adults’ calorie choices on restaurant menus and menu design, dietary behaviors, and demographic characteristics. A 3 (fast-casual restaurants) × 4 (menu-designs based on menu engineering theories) between-subjects (n = 480, 18–24-year olds) experimental design was used. This report examines trends in vaccination among older adults in Singapore to better understand why segments of older adults continue to resist vaccination against COVID-19. We find that individuals who were most likely to still be waiting to vaccinate or to not want to be vaccinated as of June 2021; are relatively older (aged 71-75), are of lower socioeconomic status (lower education levels and living in 1-3 room HDB flats), were the least likely to rely on Newspapers and Government Sources as sources of information for COVID-19 related news in November 2020, were least trusting of all sources of information, …

Objective: To evaluate cardiovascular outcomes in patients with normal nuclear myocardial perfusion imaging, but ischaemic electrocardiogram changes during pharmacological or exercise stress tests.
Methods: The retrospective study was conducted at the Aga Khan University Hospital, Karachi, and comprised data of patients who underwent either pharmacological or exercise stress myocardial perfusion scan and had a normal scan with ischaemic electrocardiogram changes between January 2013 and December 2014. All cardiac events, including angina, myocardial infarction, heart failure, coronary revascularisation and cardiac death, as well as non-cardiac deaths were noted. Data was analysed using STATA 14.2.

The medical “transition” of children with “gender dysphoria” is increasingly normalized in North America, Western Europe, Australia, New Zealand, and the United Kingdom. Although each country has specific national gender identity development services, the rationale for prescribing hormone treatment is broadly similar. The most common foot deformity in children with arthrogryposis is clubfoot, which is typically stiffer than in the idiopathic clubfoot (IC). While the use of the Ponseti method in IC has led to improved foot mobility and reduced invasive surgical procedures, there is currently limited information of the effectiveness of serial casting (SC) in arthrogryposis. The purpose of this research is to determine the effect of serial casting in recurrent clubfoot in children with arthrogryposis on brace tolerance, foot position, patient reported outcome, and the need for surgery.

T Helper 17 (Th17) cell-driven neutrophilic asthma is a rare, yet severe phenotype that accounts for over 75% of all asthma related medical costs. Neural innervation has been known play a role in the immune response and we investigated whether the addition of neurotransmitters to would affect Th17 cell differentiation. Results indicated that neural innervation upregulated Th17 cell differentiation and expression of its cytokine IL-17 that is responsible for the severe symptoms seen in neutrophilic asthma.

To explore the Th-17 activation mechanism, renal CD4+ cells were isolated from post ischemic rats and stimulated in-vitro with varying conditions. These conditions focused on the effect of adenosine on IL-17 response using RT-PCR. Additionally, inhibitors of adenosine were added to test their IL-17 response in conjunction with adenosine. Specifically, an inhibitor for the Orai1 calcium channel. Furthermore, adenosine agonists were tested with to determine their effect on IL-17 response in the in-vitro stimulation plate.

Tumor heterogeneity presents a significant barrier to glioblastoma treatment. While multiparametric imaging can improve characterization of such heterogeneity, advances have been limited by access to ground truth pathology and genomics. Stereotactic localization addresses the former issue, allowing pairing of biopsy location with corresponding voxels in MR image. Using stereotactic localization, as well as voxelwise MR image analysis, a feedforward dense network, and whole-exome sequencing, the authors sought to improve multiparametric MR characterization of tumor heterogeneity.

Importance: Women studying medicine currently equal men in number, but evidence suggests that men and women might not be evaluated equally throughout their education.
Objective: To examine whether there are differences associated with gender in either objective or subjective evaluations of medical students in an internal medicine clerkship.
Design, Setting, and Participants: This single-center retrospective cohort study evaluated data from 277 third-year medical students completing internal medicine clerkships in the 2017 to 2018 academic year at an academic hospital and its affiliates in Pennsylvania. Data were analyzed from September to November 2020.

OBJECTIVES: Since the onset of the coronavirus disease 2019 pandemic, immune modulators have been considered front-line candidates for the management of patients presenting with clinical symptoms secondary to severe acute respiratory syndrome coronavirus 2 infection. Although heavy emphasis has been placed on early clinical efficacy, we sought to evaluate the impact of pharmacologic approach to coronavirus disease 2019 within the ICU on secondary infections and clinical outcomes.

Millions of Americans live with chronic health conditions which require careful attention and specific actions for optimal management. Chronic Disease Self-Management Education (CDSME) has been effectively utilized to improve health outcomes and quality of life for individuals with chronic disease, enhancing knowledge and self-efficacy. Traditional CDSME programs are less available for rural residents. An examination of successful CDSME programs highlights common interventions that yield success and may be utilized in a variety of settings. Self-management is more complex, but also more essential, for individuals with more than one chronic condition. Obesity is associated with the leading causes of death in the United States and the world. Internationally, 39% of the world’s population was overweight or obese, in 2014, with 9% having diabetes. These causes include diabetes, stroke, certain types of cancer and heart disease. Obesity can lead to poorer mental health outcomes and cause a decrease in the quality of life. This integrative review is to determine if health and dietary smart-phone apps are effective tools to help overweight and obese people lose weight and maintain that loss.

By the year 2060 in the United States (U.S.), not only will the number of adults aged 65 and older double, non-Hispanic Whites (Whites), who currently constitute 77% of the older adult population, will constitute just slightly more than half of older adults. As the older adult population diversifies, so will their caregivers. Over 60% of informal caregivers of older adults (caregivers) are employed, and the majority of these are employed full time. Older age is associated with an increased risk for cognitively impairing diseases such as dementia. Despite significant research to find ways to cure this disease, there has been little success. However, a critical need when an intervention is discovered is a need to find ways to identify people who are at the greatest risk of developing dementia earlier in the disease process so that interventions can be implemented at that time. This could potentially lessen their risk or delay when they are diagnosed.
