Focused Ethnography: Storytelling For Hispanics With Low Health Literacy And Diabetes, Virginia H. Cadenhead
Nursing Theses and Dissertations
Background: Storytelling is an intervention for health communication that has many forms, addresses varied health issues, and is suitable for low health-literate populations. Storytelling can impact knowledge, behaviors, attitudes, and health outcomes. Population Focus: Fictional storytelling has not been researched as an educational adaption in diabetes self-management for Hispanics with low health literacy. Forty-one percent of Hispanics in the United States have low health literacy, a contributing factor to poor health outcomes in chronic disease management. The Hispanic population has disproportional rates of diabetes complications. Research Question: How can short, fictional, orally presented stories be developed as culturally-adapted educational tools …

A Mixed-Methods Approach To Investigating The Influence Of Education On Nurses’ Attitudes Toward Sex Trafficking Victims, Tabitha Mcdaniel
Nursing Theses and Dissertations
Significance of problem: Sex trafficking (ST) impacts victims of all ages, genders, races, and ethnicities. It is a significant public health issue which is shrouded in secrecy. Although victims are typically isolated by their trafficker, they will still seek out health care services. Nurses are frontline staff with rare access to victims in a setting where help can be given.
Purpose: The primary purpose of this mixed-methods study is to explore targeted education about sex trafficking and its influence on nursing attitudes and knowledge about sex trafficking and its victims. This study also analyzes demographic information and how those variables …

2020-Current year OA Pubs
BACKGROUND: High-intensity glucocorticoid regimens are commonly used to induce and maintain remission in Juvenile Dermatomyositis but are associated with several adverse side-effects. Evidence-based treatment guidelines from North American and European pediatric rheumatology research societies both advocate induction with intravenous pulse steroids followed by high dose oral steroids (2 mg/kg/day), which are then tapered. This study reports the time to disease control with reduced glucocorticoid dosing.
METHODS: We retrospectively reviewed the records at a single tertiary-care children's hospital of patients diagnosed with Juvenile Dermatomyositis between 2000 and 2014 who had a minimum of 2 years of follow-up. The primary outcome measure …

Glycans are fundamental cellular building blocks, involved in many organismal functions. Advances in glycomics are elucidating the essential roles of glycans. Still, it remains challenging to properly analyze large glycomics datasets, since the abundance of each glycan is dependent on many other glycans that share many intermediate biosynthetic steps. Furthermore, the overlap of measured glycans can be low across samples. We address these challenges with GlyCompare, a glycomic data analysis approach that accounts for shared biosynthetic steps for all measured glycans to correct for sparsity and non-independence in glycomics, which enables direct comparison of different glycoprofiles and increases statistical power. …

The Histories Of Hivs : The Emergence Of The Multiple Viruses That Caused The Aids Epidemics, William H. Schneider
Ohio University Press Open Access Books
This new collection of essays on HIV viruses spans disciplines to topple popular narratives about the origins of the AIDS pandemic and the impact of the disease on public health policy.
With a death toll in the tens of millions, the AIDS pandemic was one of the worst medical disasters of the past century. The disease was identified in 1981, at the height of miraculous postwar medical achievements, including effective antibiotics, breakthrough advances in heart surgery and transplantations, and cheap, safe vaccines—smallpox had been eradicated just a few years earlier. Arriving as they did during this era of confidence in …

Long-term endocrine treatment which results in estrogen deprivation causes chronic stress associated with a series of uncomfortable symptoms leading not only to a decrease in quality of life but also to cancer recurrence, which may be mediated primarily through the enhanced expression of angiogenic factors, as well as a series of inflammatory microenvironmental changes that favor tumor progression. In this study, we designed a clinical trial and aimed to explore the effects of Sanhuang Decoction (SHD) treatment on chronic stress, inflammatory factors, and breast cancer recovery. A total of 90 patients with breast cancer who met the inclusion/exclusion criteria were …

The Reel Deal: The Stacked Benefits Of A Reel Mower, John K. Hix, Simone Bailey
Advances in Clinical Medical Research and Healthcare Delivery
"Mow-lawning" is a colloquial term referred to actively cutting grass using a reel mower, typically at a faster than normal pace in order to maximize exercise benefit and minimize time, with the emphasis on the physical action of mowing and only secondarily on the resulting lawn care. While mow-lawning remains relatively uncommon in the United States, adoption of this practice by those healthy enough to engage in the level of exercise required can deliver a number of benefits that improve environmental, safety, and personal goals simultaneously. The stacking of so many benefits in one practice deserves more attention and promotion.

Omental Tuberculosis In A Patient With Alcoholic Liver Cirrhosis-The Diagnostic And Treatment Dilemma, Ekta Tirthani, Mina Said
Advances in Clinical Medical Research and Healthcare Delivery
Alcoholic cirrhosis is frequently complicated by spontaneous bacterial peritonitis (SBP) due to the translocation of gut bacteria. However, in immigrants to the USA from parts of the world with high tuberculosis burden, a high degree of clinical suspicion of tuberculous peritonitis should be maintained when a patient presents with symptoms similar to SBP. We describe a case of a 45-year-old Nepali man with a history of alcohol abuse who immigrated to the USA six years prior and presented to the hospital with abdominal pain, night sweats, fevers, and a 10 kg weight loss. A CT scan revealed abdominopelvic ascites, liver …

A Case Of Stress-Related Heart Block In Cardiac Sarcoidosis, Ekta Tirthani, Venkata Satish Pendela, Pujitha Kudaravalli, Mallory Balmer-Swain
Advances in Clinical Medical Research and Healthcare Delivery
There are often remarkably long delays in the diagnosis of cardiac sarcoidosis. We present a case of a 61-year-old man with exertional dyspnea and light-headedness. A resting ECG showed a 1st-degree AV block. A complete heart block was unmasked by an exercise stress test. The coronary angiogram was normal as were the cardiac troponins and Lyme antibody titer. He received a pacemaker on an emergent basis. Cardiac PET scan confirmed sarcoidosis. Computed tomography of the chest was significant for hilar and mediastinal adenopathy. He was treated with methotrexate with good symptom relief. This case demonstrates the importance of prompt clinical …

Advances in Clinical Medical Research and Healthcare Delivery
The SARS-CoV-2 led to a global pandemic, infecting millions of lives within only a short period of time. The symptoms of the disease vary with a subset of the population developing debilitating sequelae. Through the emergency use authorization (EUA), multiple vaccines have been released against the novel virus in record time. Long term and rare sequelae from the vaccine are poorly understood. DeQuervain’s subacute thyroiditis is a self-limiting and painful inflammation of the thyroid gland usually associated with viral infections. We report a rare case of subacute thyroiditis in a healthy 46-year-old woman presenting with a painful swelling in the …

The Upfront Cost Of Translating Graduate Medical Education Into A Virtual Platform, Ronak Mistry, Alex Glaser
Advances in Clinical Medical Research and Healthcare Delivery
Graduate medical education was drastically disrupted by the COVID-19 pandemic. With a halt on all in-person educational sessions in response to the need for social distancing, residency programs across the country scrambled to develop virtual curricula with some difficulty, given the limited guidance to creating online learning platforms prior to the onset of the pandemic. In-person Socratic discussions have always been the cornerstone of internal medicine training. Here, we discussed our experiences of translating the in-person model and to a virtual platform, highlighting the upfront financial burden that doing so can have. Time will need to pass before we can …

Advances in Clinical Medical Research and Healthcare Delivery
Objective:
The clinical internship (postgraduate year-1), which precedes many advanced residency training programs, are traditionally fulfilled by a transitional year (TY) or preliminary year of training in medicine or surgery. TY programs can take place in a variety of settings, ranging from community hospitals to large academic medical centers. In the present study, we sought to evaluate if resident physicians who completed a community hospital-based TY internship felt prepared for their subsequent specialty residency training.
Methods:
A cross-sectional survey was adapted and distributed to resident physicians who completed their TY internship at a university-affiliated community hospital in Baltimore, Maryland, between …
Evaluating A1c Reduction In Patients With Diabetes Receiving Pharmacy Intervention, Christopher R. Evans, Kirsten Held, Stephanie Thomas, Jeanne Spencer
Evaluating A1c Reduction In Patients With Diabetes Receiving Pharmacy Intervention, Christopher R. Evans, Kirsten Held, Stephanie Thomas, Jeanne Spencer
Advances in Clinical Medical Research and Healthcare Delivery
Introduction: Diabetes affects 34 million Americans and is the 7th leading cause of death in the United States. According to the American Diabetes Association (ADA) the A1c goal for most patients is less than 7%; however, in patients at risk for hypoglycemia or with complications that goal can be increased up to less than 8%. Management of diabetes with diet and exercise is essential, but insufficient for most patients and pharmacologic intervention is necessary to achieve therapeutic goals. Reducing A1c is associated with lower cardiovascular risk and a reduction in diabetes-associated complications. Previous studies have shown that pharmacist management of …

Preparing Internal Medicine Trainees For Independent Practice: A Pilot Simulation Program On Emotional Intelligence, Deborah Maier, Nagesh Jadhav, Christina Goodermote, Richard Alweis
Advances in Clinical Medical Research and Healthcare Delivery
Introduction: Emotional intelligence (EI) is the ability to monitor emotions and use this knowledge to guide one’s thinking and actions. EI shapes the physician’s bedside manner and leads to a better doctor-patient relationship. Higher physician EI is inversely proportional to feelings of burnout and increases resiliency to the stresses of the profession. Given the increasing levels of physician burnout, there has been a call to incorporate the development of emotional intelligence into medical education. However, little guidance exists on best practices in incorporating EI training into graduate medical education.
Purpose/Aim: Utilization of EI simulation as a pedagogical instrument to increase …

Novel Coronavirus (COVID-19) is a new strain of coronavirus that may cause illness in animals or humans. SARS-CoV-2 was first identified in late December 2019, in Wuhan, China. The coronavirus infected patients ‘who suffering from Immundeficient or chronic inflammation are at higher risk of getting a severe infection from COVID-19. The symptoms such as proinflammatory and hypercoagulable, non-cardiac pneumonia, multi-organ failure, and ARDS resulting in sudden death have been seen in those patients. Supportive treatment is still the main strategy in treating this disease since there is no evidence that current medicine can prevent or cure the infection resulting from …

Faculty Research 2021
BACKGROUND: Gastric cancer (GC) is one of the leading causes of cancer-related deaths worldwide. Human epidermal growth factor receptor 2 (HER2) amplification occurs in approximately 13-23% of all GC cases and patients with HER2 overexpression exhibit a poor prognosis. Lapatinib, a dual EGFR/HER2 tyrosine kinase inhibitor, is an effective agent to treat HER2-amplified breast cancer but it failed in gastric cancer (GC) clinical trials. However, the molecular mechanism of lapatinib resistance in HER2-amplified GC is not well studied.
METHODS: We employed an unbiased, genome-scale screening with pooled CRISPR library on HER2-amplified GC cell lines to identify genes that are associated …

The increased demand of sugarcane in Costa Rica and the lack of occupational opportunities in Nicaragua has resulted in an increase in migrant sugarcane workers. Because many individuals cannot afford proper documentation for immigration, a significant number cross the border undocumented looking for financial stability. The vast availability of sugarcane work in Costa Rica provides Nicaraguan migrants with an opportunity for work, however, they are often paid less than minimum wage due to their undocumented immigration status. Migrant workers are subject to harsh working conditions including extreme physical exertion, dehydration, and heat exposure. The combination of this trifecta seen in …
