End of life (EOL) issues are far too often addressed reactively in Emergency Departments when patients can no longer express their wishes. This scenario can result in unwanted futile care, increased healthcare costs, and emotional hardships for family and healthcare workers. The purpose of this quality improvement project was to implement a provider-patient communication protocol that would shift advanced directive conversations proactively back into the local rural primary care setting.

Colorectal cancer is the 2nd leading cause of cancer related deaths in the United States (USPSTF, 2016). In 2017 there were over 141, 000 cases diagnosed with over 51,000 deaths reported (Center for Disease Control, 2020). This raises great concern for primary care clinicians. One guideline recommended by USPSTF, is that patients undergo screening who are at low to moderate risk for colorectal cancer. This can be done by various studies. Osteoporosis affects over 12 million Americans, leading to fractures, chronic pain, reduced independence, and decreased quality of life (United States Preventative Services Task Force [USPSTF], 2018). Up to 30% of individuals who suffer an osteoporotic hip fracture will not survive 12 months post-fracture, and 71% of all osteoporotic fractures occur in women (USPSTF, 2018). Due to increased risk with age, the USPSFT recommends all women have initial bone densitometry at age 65 or greater (2018). Objectives: It has been hypothesised that attentional bias to environmental threats can contribute to persistent pain. It is unclear whether people with acute low back pain (LBP) have an attentional bias to environmental threats. We investigated if attentional bias of threat related words is different in people with acute LBP and pain-free controls.

Objectives: Our primary objective was to report the presence of a conditioned pain modulation (CPM) effect in people with localised mid-portion Achilles tendinopathy and whether changes occur over a 12- week period. Our secondary objectives were to quantify the proportion of participants who present for tendinopathy research with previous interventions or co-morbidities, which may impact the CPM-effect and investigate modulating factors.
Design: Prospective, observational cohort pilot study. Method: 215 participants presented for this Achilles tendinopathy research and were screened for inclusion with nine being included. วัตถุประสงค์: เพื่อศึกษาปัจจัยที่มีความสัมพันธ์กับภาวะแขนบวมในผู้ป่วยที่ได้รับการผ่าตัดมะเร็งเต้านมชนิดที่มีการตัดต่อมน้ำเหลือง
รูปแบบการวิจัย: การวิจัยเชิงพรรณนาหาความสัมพันธ์
วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ป่วยมะเร็งเต้านมภายหลังได้รับการผ่าตัดมะเร็งเต้านมชนิดที่มีการตัดต่อมน้ำเหลืองมาเป็นระยะเวลา 1 ปีขึ้นไป จำนวน 148 ราย ที่เข้ารับการตรวจรักษา ณ โรงพยาบาลระดับตติยภูมิในเขตบริการสุขภาพที่ 4 จำนวน 3 โรงพยาบาล คัดเลือกด้วยวิธีสุ่มแบบเจาะจงตามเกณฑ์คัดเข้า รวบรวมข้อมูลด้วยแบบสอบถาม 4 ส่วน ประกอบด้วย 1) ข้อมูลส่วนบุคคล 2) อาการที่สัมพันธ์กับภาวะแขนบวม 3) การได้รับข้อมูลเกี่ยวกับภาวะแขนบวม และ 4) พฤติกรรมการลดความเสี่ยงในการเกิดภาวะแขนบวม ซึ่งมีความตรงตามเนื้อหา และค่าสัมประสิทธิ์แอลฟาครอนบาคของแบบสอบถามที่ 2, 3, และ 4 เท่ากับ .88, .82, .88 ตามลำดับ ประเมินภาวะแขนบวมจากการวัดเส้น รอบวงแขนทั้ง 2 ข้าง จำนวน 6 ตำแหน่ง หากมีความแตกต่างของเส้นรอบวงแขนที่ตำแหน่งใดตำแหน่งหนึ่ง > 2 เซนติเมตร ให้ถือว่ามีภาวะแขนบวม วิเคราะห์ข้อมูลโดยสถิติพรรณนา สถิติสหสัมพันธ์พอยท์ไบซีเรียล การทดสอบไคสแควร์ และฟิชเชอร์เอกซ์แซคท์

บทความนี้มีวัตถุประสงค์เพื่อเสนอบทบาทของพยาบาลในการส่งเสริมโภชนาการและพฤติกรรมการรับประทานอาหารสำหรับเด็กวัยก่อนเรียน ภาวะโภชนาการในเด็กสามารถเป็นตัวชี้วัดถึงภาวะสุขภาพในภาพรวมและเป็นพื้นฐานที่สำคัญต่อการเจริญเติบโตด้านร่างกายและสติปัญญาของเด็ก หากเด็กไม่ได้รับการดูแลที่เหมาะสม และมีพฤติกรรมการรับประทานอาหารที่ไม่ดี ย่อมส่งผลต่อสุขภาพของเด็กได้ทั้งในปัจจุบันและอนาคตเมื่อเด็กโตขึ้น พยาบาลเด็กจึงมีบทบาทสำคัญในการให้คำแนะนำกับผู้ปกครอง รวมทั้งเป็นสื่อกลางในการให้ความรู้กับครูและพี่เลี้ยงเด็กในโรงเรียนหรือสถานรับเลี้ยงเด็กในการส่งเสริมโภชนาการและพฤติกรรมการรับประทานอาหารของเด็กให้เหมาะสม ซึ่งพยาบาลเด็กต้องมีความเข้าใจเบื้องต้นเกี่ยวกับพัฒนาการของเด็กวัยก่อนเรียนที่สอดคล้องกับการรับประทานอาหารของเด็ก ความต้องการสารอาหารของเด็ก ตลอดจนวิธีการดูแลจัดเตรียมอาหาร การส่งเสริมเด็กให้มีพฤติกรรมการรับประทานอาหารที่ดี และการจัดบรรยากาศที่น่ารื่นรมย์ในการรับประทานอาหาร ซึ่งเป็นการส่งเสริมพฤติกรรมการรับประทานอาหารที่เหมาะสมสำหรับเด็กวัยก่อนเรียน เพื่อให้เด็กวัยก่อนเรียนได้รับการดูแลที่เหมาะสม และมีสุขภาวะที่ดีต่อไป

วัตถุประสงค์: เพื่อเปรียบเทียบพฤติกรรมก้าวร้าวของเด็กออทิสติกวัยเรียนอาการรุนแรงก่อนและหลังได้รับการใช้สิ่งแวดล้อมบำบัด และเปรียบเทียบพฤติกรรมก้าวร้าวของเด็กออทิสติกวัยเรียนอาการรุนแรงระหว่างกลุ่มที่ได้รับการใช้สิ่งแวดล้อมบำบัดกับกลุ่มที่ได้รับการพยาบาลตามปกติ
รูปแบบการวิจัย: การวิจัยกึ่งทดลอง
วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ เด็กออทิสติกที่อาการรุนแรง อายุ 6-12 ปี และผู้ดูแลหลักที่มารับบริการแผนกผู้ป่วยใน สถาบันราชานุกูล ซึ่งมีคุณสมบัติตามเกณฑ์ โดยได้รับการจับคู่ (Matched pair) และการสุ่มเข้ากลุ่มทดลองและกลุ่มควบคุม จำนวนกลุ่มละ 18 คู่ กลุ่มทดลองเข้าร่วมโปรแกรมการใช้สิ่งแวดล้อมบำบัดระยะเวลา 4 สัปดาห์ ส่วนกลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการทดลอง คือ 1) แบบสอบถามข้อมูลส่วนบุคคลของเด็กออทิสติกและผู้ดูแลหลัก 2) แบบประเมินพฤติกรรมก้าวร้าวเด็กออทิสติก 3) แนวทางปฏิบัติการพยาบาลโดยใช้สิ่งแวดล้อมบำบัดสำหรับเด็กออทิสติกวัยเรียนอาการรุนแรง 4) คู่มือการใช้สิ่งแวดล้อมบำบัดสำหรับเด็กออทิสติกวัยเรียนอาการรุนแรงสำหรับผู้ดูแล และ 5) แบบประเมินความสามารถการจัดการพฤติกรรมก้าวร้าวเด็กออทิสติกวัยเรียนอาการรุนแรงที่บ้านสำหรับผู้ดูแลหลัก เครื่องมือทุกฉบับได้รับการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิจำนวน 5 ท่าน และได้ค่าความเที่ยงแบบประเมินพฤติกรรมก้าวร้าวเท่ากับ .87 วิเคราะห์ข้อมูลโดยใช้สถิติทดสอบที (t-test)

The number of opioid overdoses (ODs) has risen in recent years and has become more complex due to the co-involvement of both prescription and illicit opioid drug use. Provisional programs for take-home naloxone (a medication designed to rapidly reverse opiate OD symptoms) kits have been distributed to combat this potentially fatal epidemic. Although there is strong evidence to support the efficacy of naloxone in the reversal of opiate OD, there is limited evidence to support the efficacy of take-home naloxone (THN) kits. Alcoholic liver disease (ALD) is a significant public health problem worldwide. Numerous natural molecules, including curcumin, have been extensively studied as potential treatment agents for ALD. Tetrahydrocurcumin (THC), the reductive metabolite of curcumin, was previously reported to exert the potent antioxidant activity against chemical-induced oxidative stress suggesting the potential of THC to be used as a protective agent for ALD. However, THC has poor water solubility limiting its pharmacological activity. In the present study, tetrahydrocurcumin-diglutaric acid (TDG) was synthesized. Aging workers has been linked with high prevalence of both musculoskeletal disorders (MSDs) and non-communicable diseases (NCDs). Health status has a significant impact on work ability. This study compared work ability, using the Work Ability Index (WAI), among aging office workers with and without MSDs and NCDs and determined factors associated with WAI scores. A cross-sectional study was conducted among office workers aged between 45-60 years. An online questionnaire was adopted to collect data. Anatomical knowledge of the occipital condyle (OC) and its relationships to surrounding structures is important for avoiding injury during craniovertebral junction (CVJ) surgeries such as the far lateral approach (FLA). Therefore, this study was conducted to evaluate the morphology and morphometry of OC and its relationship to digastric point (DP), foramen magnum (FM), jugular foramen (JF), and hypoglossal canal (HC). The study was performed on 100 dry skulls. Oval-like condyle was the most common OC shape, representing for 33.0% of all samples. The OC protruded into FM in 31.5 %. การศึกษาครั้งนี้มีวัตถุประสงค์ 1) เพื่อพัฒนาเครื่องมือการรายงานผลเกี่ยวกับภาวะสุขภาพด้วยตนเองสำหรับผู้ป่วยมะเร็งระยะท้ายที่ได้รับการดูแลแบบประคับประคอง (PROMs-TCP) และทดสอบความตรงและความเที่ยงของเครื่องมือ PROMs-TCP 2) การพยากรณ์สถานะสุขภาพด้วยแบบจำลอง Markov การศึกษาประกอบด้วย 3 ขั้นตอน ได้แก่ 1) การพัฒนาเครื่องมือใช้เทคนิคการทบทวนวรรณกรรม ร่วมกับการประชุมกลุ่มย่อย ในกลุ่มผู้เชี่ยวชาญ และกลุ่มพยาบาลที่ปฏิบัติงานในคลินิกดูแลผู้ป่วยแบบประคับประคอง และทดสอบความถูกต้องของเนื้อหาโดยการประเมินของผู้เชี่ยวชาญ 2) การทดสอบความตรงและความเชื่อถือได้ของเครื่องมือ โดยเก็บข้อมูลนาน 10 วัน ในอาสาสมัคร จำนวน 169 ราย ตรวจสอบความถูกต้องตามเกณฑ์ ความตรงเชิงจำแนก ความไวต่อการเปลี่ยนแปลง และความน่าเชื่อถือภายในและภายนอก 3) การพยากรณ์สถานะสุขภาพด้วยแบบจำลอง Markov ผลการศึกษาพบว่า เครื่องมือประเมินคุณภาพชีวิตโดยผู้ป่วยหรือผู้ดูแลเพื่อการบริบาลผู้ป่วยมะเร็งระยะท้าย หรือเครื่องมือการรายงานผลเกี่ยวกับภาวะสุขภาพด้วยตนเองสำหรับผู้ป่วยมะเร็งระยะท้ายที่ได้รับการดูแลแบบประคับประคอง (PROMs-TCP) ประกอบด้วย 6 ข้อคำถาม ความถูกต้องของเนื้อหารายข้อคำถาม (I-CVI) อยู่ระหว่าง 0.8 ถึง 1 และความถูกต้องของแบบสอบถามทั้งฉบับ (S-CVI) เท่ากับ 0.97 ความสัมพันธ์ระหว่าง PROMs-TCP และ Palliative care Outcome Scale (POS) อยู่ในระดับปานกลางถึงสูง (r = -0.7 ถึง -0.8) เครื่องมือ PROMs-TCP มีจุดตัดที่ 4 คะแนน คัดกรองสถานะสุขภาพไม่ดี ภายใต้พื้นที่ใต้โค้ง Receiver Operating Characteristic (ROC) เท่ากับ 0.91 (95% CI 0.90, 0.93) ความไวร้อยละ 76.91 ความจำเพาะร้อยละ 88.67 การทดสอบความเชื่อถือได้ของเครื่องมือ พบว่ามีความสอดคล้องภายในอยู่ในระดับดี ค่า Cronbach’s alpha coefficient = 0.85 ความสอดคล้องภายนอกระหว่างผู้ป่วยกับผู้ดูแล และระหว่างผู้ป่วยกับพยาบาลที่ให้การดูแล อยู่ในระดับปานกลางถึงดี (Cohen's Weighted …

The purpose of this study was to develop Eudragit® -nanoparticles (NPs) eye drop formulations containing fenofibrate (FE) for diabetic retinopathy and age-related macular degeneration. Due to the limited aqueous solubility of FE, it may hamper the ocular bioavailability and results in low efficiency to treat such diseases. Hence, the increased solubility of FE was determined by complexation with cyclodextrins (CDs) and CD derivatives The solubility determination was evaluated by phase-solubility technique with heating method. We developed two models of chemically induced chronic lung injury and pulmonary fibrosis in mice (intratracheally administered hydrochloric acid (HCl) and intratracheally administered nitrogen mustard (NM)) and investigated male-female differences. Female mice exhibited higher 30-day survival and less weight loss than male mice. Thirty days after the instillation of either HCl or NM, bronchoalveolar lavage fluid displayed a persistent, mild inflammatory response, but with higher white blood cell numbers and total protein content in males vs. females. Furthermore, females exhibited less collagen deposition, milder pulmonary fibrosis, and lower Ashcroft scores. After instillation of either HCl or NM, all animals displayed …

Exposure to hydrochloric acid (HCl) represents a threat to public health. Children may inhale higher doses and develop greater injury because of their smaller airways and faster respiratory rate. We have developed a mouse model of pediatric exposure to HCl by intratracheally instilling p24 mice (mice 24 days old; 8–10 g) with 2 µL/g 0.1 N HCl, and compared the profile of lung injury to that in HCl-instilled adults (10 weeks old; 25–30 g) and their age-matched saline controls. Limited literature exists regarding how individuals with this degree can become part of an athletic training faculty. To identify department chairs' perceptions of the DAT degree and determine whether they viewed the degree as viable when hiring new faculty within a postbaccalaureate professional athletic training program. The prevalence of food insecurity (FI) and housing insecurity (HI) in college students has increased over the last decade along with an associated increase in mental health problems. Studies show significant increases in many markers for mental distress in this population including a higher prevalence of mood disorders, non-lethal self-harm, and attempted and completed suicides, particularly over the last decade. Compounding these challenges is the low level of resilience found in college-age young adults, potentially limiting their ability to cope with and recover from the hardship of FI and HI.
