ที่มา: ภาวะหัวใจห้องบนเต้นระริกเป็นภาวะหัวใจเต้นผิดจังหวะที่พบบ่อยในเวชปฏิบัติ ซึ่งเป็นสาเหตุของภาวะหลอดเลือดสมองทั่วโลก ยาต้านการแข็งตัวของเลือดเป็นยาที่ถูกแนะนำให้ใช้เพื่อป้องกันภาวะหลอดเลือดสมองในผู้ป่วยภาวะหัวใจต้องบนเต้นระริก แม้ว่าผู้เชี่ยวชาญและข้อแนะนำส่วนใหญ่แนะนำให้เลือกใช้ยาต้านการแข็งตัวของเลือดกลุ่มไม่ได้ออกฤทธิ์ต้านวิตามินเคเป็นยาที่ใช้ลำดับแรกและมีประสิทธิภาพในการป้องกันภาวะหลอดเลือดสมองเหนือว่ายาต้านการแข็งตัวของเลือดกลุ่มออกฤทธิ์ต้านวิตามินเคหรือยาวาร์ฟาริน แต่ในทางปฏิบัติในประเทศไทย ยาวาร์ฟารินก็ยังเป็นที่ถูกเลือกนำมาใช้เป็นลำดับแรกเพื่อป้องกันภาวะหลอดเลือดสมองเนื่องจากราคาถูกและมีใช้อย่างแพร่หลายในทุกโรงพยาบาล ทีทีอาร์ถือเป็นตัวชี้วัดมาตรฐานสำหรับการประเมินประสิทธิภาพใช้ในการใช้ยาวาร์ฟารินในอยู่ในช่วงระดับของการรักษา อย่างไรก็ตามในประเทศไทยยังไม่มีการนำทีทีอาร์มาเป็นเครื่องมือที่ใช้กันอย่างแพร่หลาย จุดประสงค์: เพื่อวิเคราะห์การปรับปรุงของทีทีอาร์ หลังใช้แนวทางการปรับยาวาร์ฟารินโดยการใช้ทีทีอาร์และไอเอ็นอาร์ เป็นตัวช่วยในผู้ป่วยที่มีภาวะหัวใจห้องบนเต้นระริกที่รักษาด้วยยาต้านการแข็งตัวของเลือดกลุ่มออกฤทธิ์ต้านวิตามินเค และศึกษาความสะดวกในการใช้งานแนวทางการปรับยาวาร์ฟารินโดยการใช้ทีทีอาร์และไอเอ็นอาร์เป็นตัวช่วย ระเบียบวิธีวิจัย: การวิจัยนี้เป็นการศึกษาแบบไปข้างหน้า ศึกษาผู้ป่วยภาวะหัวใจต้องบนเต้นระริกที่รักษาและติดตามที่คลินิกวาร์ฟาริน โรงพยาบาลจุฬาลงกรณ์ ตั้งแต่เดือนกุมภาพันธ์ 2564 ถึงเดือนกุมภาพันธ์ 2565 โดยจะทำการเก็บข้อมูลทั่วไป, โรคประจำตัว, ประเมินความเสี่ยงต่อภาวะหลอดเลือดสมองและภาวะเลือดออก การรักษาที่ได้รับ และผลลัพธ์ที่ต้องการ คือ การเปลี่ยนแปลงของทีทีอาร์, การเสียชีวิต การนอนโรงพยาบาล อุบัติการณ์การเกิดภาวะหลอดเลือดสมองและเลือดออก รวมทั้งเจาะเลือดเพื่อวัดค่าทีทีอาร์ โดยทำการวัด 2 ครั้ง (ครั้งแรกที่ 6 เดือนหลังการใช้แนวทางการปรับยาฯ และครั้งที่ 2 คือ 1 ปีหลังการใช้แนวทางฯ สถิติที่ใช้วิเคราะห์ข้อมูลใช้ McNemar’s Chi-square test และ paired t-test เพื่อหาความสัมพันธ์ดังกล่าว ผลการศึกษา: ผลการวิเคราะห์ข้อมูลพบว่า คัดกรองอาสาสมัครทั้งหมด 95 คน (อาสาสมัครที่ตรงเกณฑ์และคุณสมบัติในการคัดเลือกเข้าในงานวิจัยทั้งหมด 74 คน และอาสาสมัคร 18 คน ขาดความสมบูรณ์ของข้อมูล) อายุเฉลี่ยของอาสาสมัครเท่ากับ 72 ± 11 ปี ร้อยละ 50 เป็นเพศหญิง มีผู้ป่วยร้อยละ 32.14 มีโรคประจำตัวเป็นกล้ามเนื้อหัวใจขาดเลือด ผู้ป่วยร้อยละ 21.43 มีเคยมีประวัติหลอดเลือดสมองตีบมาก่อน และผู้ป่วยร้อยละ 35.7 เป็นเบาหวาน ค่าคะแนนประเมินความเสี่ยง CHA2DS2-VASc เฉลี่ย 3.82 ± 1.82 HAS-BLED เฉลี่ย 2.13 ± 1.01 และ SAMe-TT2R2 เฉลี่ย 3.30 ± 0.74 จากผลการวิจัยพบว่า ค่าเฉลี่ยของทีทีอาร์ตั้งต้นก่อนใช้แนวทางการปรับยาฯ …

Canine atopic dermatitis (CAD) is a pruritic allergic skin disease caused by an overreaction to environmental allergens. The diagnosis of CAD is made on the basis of the patient's history, physical examination, and exclusion of other pruritic skin diseases. BVDV is a major pathogen of ruminants that causes immune dysfunction and lead to secondary bacterial infection. The underlying mechanism of this dysfunction is not well understood. Based on previous results, we believe that macrophages play a central role in BVDV-associated immune dysfunction. Previously, we have shown that the infected monocyte-derived macrophage (MDM) supernatant of the high virulent strains of BVDV significantly induced lymphocyte apoptosis. Aphasia is an acquired language disorder resulting from damage to the brain, most typically a stroke in the left cerebral hemisphere. The degree and type of impairment vary depending on the site and size of the lesion, and the extent to which people with aphasia recover – with time and therapeutic intervention – their pre-stroke linguistic skills varies, depending on exogenous and endogenous variables. In multilingual people, semantic knowledge is predominantly shared across languages.
Providing semantic-focused treatment to people with aphasia has been posited to strengthen
connectivity within association cortices that subserve semantic knowledge. In multilingual people, such treatment should result in within- and cross-language generalisation to all languages, although not equally. We investigated treatment effects in two multilingual participants with aphasia who received verb-based semantic treatment in two pre-stroke highly
proficient languages. We compared within- and cross-language generalisation patterns across languages, finding within- and cross-language generalisation after treatment in the less-impaired, pre-morbidly more-proficient first-acquired language (L1). We summarize recent published work concerning assessment and treatment of aphasia in bilingual and multilingual people and review current related models of treatment outcomes. As well, we discuss studies that address the recently debated topic of cognitive processes in bilingual individuals with aphasia, with a focus on the effects of bilingualism on aphasia recovery and its potential protective effects.

ATP-binding cassette (ABC) transporters are a large class of integral membrane proteins that contribute to key physiological functions in all organisms by utilizing ATP binding and hydrolysis to transport diverse substrates across membrane barriers. P-glycoprotein (P-gp/ ABCB1) and Multidrug Resistance protein 1 (MRP1/ABCC1) are widely reported ABC transporters associated with multidrug resistance in cancer. Employee turnover has significant negative costs to healthcare organizations. While leadership communication styles and quality have been empirically supported to reduce turnover intentions, our understanding of specific modalities of leader intervention is not well understood. This paper sought to understand how the specific act of communication frequency reduces turnover. Through the lens of Leader-Member Exchange (LMX) and role dynamic theories, mediating effects of LMX quality and job satisfaction were investigated to further understand this proposed relationship in a sample of healthcare workers in the Midwest/US. Objective: The main purpose of the present study was to investigate the effectiveness of behaviour therapy training to mothers of autistic children on verbal and non-verbal improvement of their children.
Methods: A total of 33 mothers of autistic children were selected for training based on non-probability purposive sampling procedure. Autism Spectrum Disorder (ASD) Knowledge Scale was used to assess mothers' knowledge about autism and Vineland Adaptive Behaviour Scale was used for assessment of the communication skill in autistic children.

Scholarly literature and non-scholarly news sources have highlighted stressors in higher education. The rigor of occupational therapy education can compound student stress. Although the profession recognizes these stressors, insufficient literature exists regarding strategies to cope with and manage these sources of stress. This scoping review explored the literature examining stress and the implications for occupational therapy student well-being and coping, and also identified strategies for faculty to support student well-being. The purpose of this study was to identify occupational therapy student stress levels and determine factors influencing this emotional response. The study also sought to obtain the perspective of occupational therapy professors on this issue. This mixed-methods study utilized an online quantitative descriptive survey and a telephone or face-to-face qualitative open-ended interview. Online survey respondents included a nationwide sample of 340 occupational therapy professors and 459 occupational therapy students. The integrative seminar is an evidence-based teaching-learning approach that has recently been used to help prepare occupational therapy students for Level II fieldwork. Integrative seminars provide a dedicated context for students to synthesize their learning across various subjects in a professional curriculum. Hybrid occupational therapy (OT) students transitioning from certified OT assistants (COTAs) to OTs can successfully learn graduate-level anatomy in a compressed format with limited synchronous instruction time. The effectiveness of a human anatomy course with limited synchronous instruction time for transitional hybrid occupational therapy students was investigated. A retrospective, non-randomized study was used. A university graduate level human anatomy course for transitional OT students used prosected (previously dissected) cadavers. An educational and simulation-based training model, the Professional ACEs-Informed Training for Health Professionals designed for allied health students was evaluated using a pre-and-post design. The training model emphasizes trauma-informed care and uses social simulation to educate and train students with skills to address adverse childhood experiences. This study evaluated occupational therapy students’ (N=70) levels of self-efficacy and knowledge of trauma-informed care at both pre- and post-training. Analysis of variance results indicated a statistically significant improvement in students’ levels of general self-efficacy and knowledge from pre- to post-assessment (p < .01). International service learning experiences provide invaluable opportunities for occupational therapy practitioners and students to practice clinical skills while also being exposed to different cultures and experiences not available in their home country. Experiential learning is not without its challenges. Following a pilot examination of student feedback on practical and learning difficulties they encountered, students’ experiences of their occupation-focused, service-learning module was systematically explored using a case study design. Concurrent mixed methods were employed to collect data with emphasis on qualitative data gathered through weekly, guided reflective journals and online forums. Students were also surveyed pre and post module to validate data on their experiences regarding their concerns, perceived learning, and evaluation of the module. Four themes were identified. Worldwide, over 800,000 people die each year by suicide, leaving many behind to grieve the loss. Preventing suicide involves reaching people before they are in crisis (prevention), helping them to navigate a crisis that could result in suicide (intervention), and addressing the aftermath of a suicide loss or attempt (postvention). Healthcare professionals, including occupational therapists, unitedly acknowledge the lack of skills, knowledge, and competence in all facets of suicide awareness and prevention in their professional training and in practice. The purpose of this research was to use design-based research (DBR) methods to design a novel educational intervention to promote professional engagement of diverse students over three subsequent years. Accredited occupational therapy education programs are required to include telehealth technology in their curricula as outlined by the Accreditation Council for Occupational Therapy Education. An innovative Doctor of Occupational Therapy program piloted a telehealth module with first- and second-year students. Both dynamic lecture content and active learning lab exercises were created to advance student knowledge in the use of telehealth technology and to inform occupational therapy telehealth education. Promoting health requires coordinated, team-based interventions responsive to multiple determinants of health. This study aimed to determine if interprofessional competencies and knowledge of and confidence in addressing the social determinants of health improved following an interprofessional learning event. A two-group randomized controlled trial was used to determine study outcomes among 408 health science students from 14 health profession programs in the Midwest. Formed groups were randomly assigned to the customary medical-based (control) or social determinants of health focused (experimental) case study.
