The umbilical cord provides life-sustaining blood flow between the growing fetus and its mother throughout the length of the pregnancy. Upon delivery, the cord continues to pump nutrient-rich blood to the fetus until impeded by clamping or natural occlusion. A literature review of articles published from 2011 to 2018 was conducted in order to determine the effects of delaying cord clamping on extrauterine transition in full-term infants. It was found that a delay can in fact provide the infant with vital stores of hemoglobin, increasing iron status and improving not only hematologic function, but cardiovascular stability, neurologic development, and whole-body …

A literature review was performed between September 2018 and November 2018 to determine the effects of community water fluoridation cessation in rural communities, namely in Ida Grove, Iowa. Eleven articles were selected using inclusion and exclusion parameters, analyzed by the researchers, and summarized. Results of the literature review highlighted the benefits of fluoridation, which outweighed the possible risks. A plan of action was set out by the researchers to best promote optimal fluoridation in rural communities.

This literature review aims at exploring different treatment options for individuals diagnosed with intertrigo. Ten articles were reviewed between August and November 2018 with the purpose of helping Village Northwest Unlimited develop a therapeutic guideline for patients at risk or that are currently afflicted with the skin condition. After reviewing the literature, the researchers have concluded that there are multiple options for treatment rather than a single best treatment option for intertrigo. However, throughout this literature review, the researchers have provided many research articles that share possible treatment options for patients with intertrigo.

A literature review was performed between the months of August 2018 to December 2018 on ways to decrease unnecessary usage of Foley catheters. Ten articles were reviewed focusing on bed-bound, incontinent women. Research was performed for Spencer Hospital looking at the various ways to decrease the usage, decrease catheter-associated urinary tract infections (CAUTI’s), and decrease skin breakdown from incontinence. Through the research, it was found that a large proportion of catheters are used when they are unnecessary. A literature review was conducted between the months of August 2018 and November 2018 to determine the effects of weighted blankets in patients with symptoms of anxiety. Ten articles were reviewed focusing on patients with anxiety-related symptoms. The purpose was to advocate for the purchase of more weighted blankets for the hospital in Spencer, Iowa. Medical staff currently do not have protocol in place regarding the use of the three weighted blankets the Spencer Hospital already owns. Conclusive evidence was found supporting the use of deep pressure touch stimulation through weighted blankets in patients with anxiety.

Purpose: The purpose of this multiple-case study was to explore the experiences of post- 9/11 female combat veterans transitioning from military service to civilian careers, using the Schlossberg transition model factors of situation, self, social support, and strategies.

Methodology: This multiple-case study identified and interviewed post-9/11 female combat veterans transitioning from active-duty to civilian career. Respondents were purposively chosen based on specific criteria and expert panel recommendations.

Findings: Examination of case-study interview data and artifacts from the 3 post-9/11 female combat veterans indicated nine major findings:

Despite abundant focus on responsible care of laboratory animals, we argue that inattention to the maltreatment of wildlife constitutes an ethical blind spot in contemporary animal research. We begin by reviewing significant shortcomings in legal and institutional oversight, arguing for the relatively rapid and transformational potential of editorial oversight at journals in preventing harm to vertebrates studied in the field and outside the direct supervision of institutions. Straightforward changes to animal care policies in journals, which our analysis of 206 journals suggests are either absent (34%), weak, incoherent, or neglected by researchers, could provide a practical, effective, and rapidly imposed …

© 2019 The Author(s). Genetic and pharmacological manipulations targeting metabotropic glutamate receptor 5 (mGluR5) affect performance in behavioural paradigms that depend on cognitive flexibility. Many of these studies involved exposing mice to highly stressful conditions including electric foot shocks or water immersion and forced swimming. Because mGluR5 is also implicated in resilience and stress responses, however, apparent impairments in inhibitory learning may have been an artifact of manipulation-induced changes in affective state. Transfers are high-risk. Intra-hospital transfer processes research has improved protocols used nationally.1 Outside-hospital admissions remain without standards ensuring the transfer of the right information to the right clinician at the right time; presently, patients can arrive at a new hospital with limited-to-no information.2We trialed a novel process for outside (hospital, provider, or skilled nursing facility) admissions to a single academic medical center’s resident and- hospital-medicine-attending-based (“teaching”) medicine services.

We assessed online health insurance plan indication of coverage and accessibility of information for recommended services for transgender youth (TY). Content analysis was performed for plans used at a pediatric Gender Clinic by reviewing information about coverage of puberty blockers, hormones, masculinizing chest surgery, and counseling. Transgender-specific exclusions and the time required for the research assistant to review each plan's online information were noted. No plan (0%; n=36) indicated coverage of all four categories of recommended services online. Forty-nine percent indicated ≥1 transgender-specific exclusion. Obstetric conditions like placenta percreta and multiple pregnancies can be extremely challenging as they pose a high risk for both the mother and infants. In placenta percreta, placental villi penetrate through the wall of uterus into the surrounding organs including the bladder and carry a risk of massive maternal bleeding. Multiple pregnancies have greater complication rate than that in singleton pregnancy. Successful management of such challenges are best done by a multidisciplinary teamwork, where all members of a perinatal team are involved in decision making and management. We report the successful management of a triplet gestation associated with placenta percreta.

Introduction: Adequate nutrition, including intake of dietary calcium and vitamin D, is important to maintain bone health. Evidence suggests that a deficiency in micronutrients may contribute to bone loss during aging and exert generalized effects on chronic inflammation. Recently, the Dietary Inflammatory Index (DII) was developed to assess the inflammatory potential of individual diets. Our aim was to evaluate the DII in a representative sample and verify its association with low-impact fractures.

Financial incentives can increase physical activity (PA), but differences in the immediacy of reward delivery and individual differences in delay discount rates (i.e., higher discount values associated with less tolerance for delayed rewards) may explain differential responding. The current study tested whether delay discount rate moderated the relative effectiveness of immediate financial rewards on increasing daily PA. Inactive, overweight adults (ages 18–60, N = 96) were randomized to receive either smaller, immediate goal-contingent rewards or larger, delayed rewards for participation. Objectives: Trauma is the leading cause of death in children. The lack of an accepted definition of what constitutes a high-quality stabilisation of a traumatically injured child has limited the evaluation of direct interventions in simulation-based education and service-delivery models to improve trauma care. Maternal obesity has a wide range of health effects on both the pregnant woman and developing fetus. The clinical significance of these disorders, combined with a dramatically increasing prevalence of obesity among pregnant women has precipitated a major health crisis in the United States. Gasdermin E (GSDME/DFNA5) cleavage by caspase-3 liberates the GSDME-N domain, which mediates pyroptosis by forming pores in the plasma membrane. Here we show that GSDME-N also permeabilizes the mitochondrial membrane, releasing cytochrome c and activating the apoptosome. Cytochrome c release and caspase-3 activation in response to intrinsic and extrinsic apoptotic stimuli are significantly reduced in GSDME-deficient cells comparing with wild type cells. GSDME deficiency also accelerates cell growth in culture and in a mouse model of melanoma. Phosphomimetic mutation of the highly conserved phosphorylatable Thr6 residue of GSDME, inhibits its pore-forming activity, thus uncovering a potential mechanism by which GSDME …

Context: Lacrosse is a fast growing contact sport, minimal studies have examined head impacts in women’s lacrosse.

Objective: To compare head impact mechanisms and type of play in women’s lacrosse.

Design: Descriptive epidemiology study.

Setting: Collegiate lacrosse fields.

Participants: 29 women’s lacrosse players (age=20.39±0.39 years, height=162.13+6.50 cm, mass=60.99+4.82 kg) participated over years. Intervention: Participants wore xPatch sensors during games and practices.

Main Outcomes Measures: Peak linear (PLA; g) and rotational (PRA; deg/sec2) accelerations of head impacts.

By studying head impacts over multiple seasons, injury patterns may be recognized which can lead to rule changes and other injury reduction measures. The purpose of our study was to compare head impact characteristics (impact location and mechanism) in men’s soccer over 3 years. We recruited 26 NCAA Division III intercollegiate male soccer players (age=19.64±1.25 years, height=181.70±5.80 cm, mass=76.98±10.19 kg) over 3years. X2 Biosystems xPatch sensors measured frequency and magnitude of all head impacts over a 10 g threshold. The overall male soccer player IR was 7.50/1000 athlete-exposures (CI95=2.85-12.15). Blood flow to the tissue of the hands and digits is efficiently regulated by vasoconstriction and vasodilation. Through a series of cadaveric dissection, we examined arteries in the hands and digits, including ulnar artery, radial artery, palmar arteries, and digital arteries, for their distribution (branching) patterns and morphological parameters (e.g., thickness, length between branches, external and internal diameters). Using data directly collected from three female cadavers as input variables to our mathematical model, we simulated vasoconstriction (-20% and -10% diameter) and vasodilation (+10% and +20 diameter) to evaluate the extent of changes in blood volume and flow within the arteries. …

The second most common cancer in men is prostate cancer. Prostate cancer is traditionally diagnosed by a digital rectal exam. Blood tests can also be used to test for PSA (prostate-specific antigen). These two methods can be used together but can sometimes provide both false negative and false positive results. A new method of testing for prostate cancer could prove to be beneficial. Bones in many higher teleost fish, such as percids and centrarchids, lack osteocytes that function as both chemo- and mechano-sensors, and thus, are not actively involved in homeostasis of blood calcium level. Nonetheless, the vertebrae of yellow perch, Perca flavescens (P. flavescens), living in darker and low calcium (Ca) water, have demonstrated adaptive modeling and remodeling of vertebrae to be more mineralized and thus enhance transmission of the axial muscle torque into the thrust force for the C-start predator escape behavior. Health-Related Quality of Life (HRQL) assessments can provide Athletic Trainers with information about student-athletes’ overall well-being and health. The objective of this study was to examine the association between personal and family reports of medical history and HRQL in collegiate student-athletes. We recruited 271 student-athletes (age=19.00±1.15 years, height=175.18±4.29 cm, mass=72.29±12.03 kg).
