Brain Training And Meditation’S Effects On Memory In Subjects With Vascular Cognitive Impairment, Sarah Savoia
Brain Training And Meditation’S Effects On Memory In Subjects With Vascular Cognitive Impairment, Sarah Savoia
Yale School of Medicine Physician Associate Program Theses
Vascular Dementia (VaD) is an important public health concern, which causes significant morbidity and mortality amongst populations around the world. With the increases in average age of individuals and prevalence of cardiovascular risk factors, the incidence of vascular cognitive impairment (VCI) and VaD are on the rise. Most of this increase will come from cerebral small vessel disease (CSVD) as treatment for large vessel disease improves. Yet, very few interventions are recommended for CSVD beyond control of risk factors. In this thesis, we propose a non-pharmacological intervention, which we believe may address executive dysfunction in VCI due to CSVD. CSVD …

Does Use Of A Molecular Rapid Pathogen Kit Improve Outcomes In The Bacteremic And Critically Ill?, Julie P. Gedalecia
Yale School of Medicine Physician Associate Program Theses
Sepsis is a pervasive condition that carries a tremendous burden of disease in the form of financial cost, morbidity, and mortality. Culturing methods slow clinicians’ ability to begin focused treatment, and increased antimicrobial resistance only intensifies the need for improved diagnostic tools. Rapid molecular diagnostic tests can shorten time to identify organisms, reduce inappropriate antibiotic treatment, and improve patient outcomes. A newly approved test has proven fast and accurate for identification and susceptibility, but has not been studied in regard to clinical outcomes. Our study will compare the effect of the Accelerate™ system versus standard identification and susceptibility tests on …

INTRODUCTION:
The purpose of this study was to describe and compare the performance of balance and walking tests in relation to self-reported fall history in adults with myotonic dystrophy type 1 (DM1).
METHODS:
Twenty-two (13 male) participants with DM1 completed, a 6-month fall history questionnaire, the modified Dynamic Gait Index (mDGI), limits of stability (LoS) testing, and 10-m walking tests.
RESULTS:
Mean (SD) falls in 6 months was 3.7 (3.1), and 19 (86%) participants reported at least 1 fall. Significant differences in mDGI scores (P = 0.006) and 10-m fast walking gait velocity (P = 0.02) were found between those …

Clinical and Translational Science Institute
Objectives—We sought to investigate the potential association between maternal age and the need for active obstetrical intervention intrapartum in primiparas. Study design—Observational study over 14 years (2001–2014) of all consecutive primiparous singleton births having delivered at the Centre Hospitalier Universitaire Hospitalier Sud Reunion's maternity (French overseas department, Indian Ocean). Results—Of the 21,235 singleton primiparous births, there were three significant linear associations between maternal age from 12 years of age to 42 +(all χ2 for linear trend, p < .0001) (a) vaginal deliveries without any medical intervention, (b) rate of cesarean sections, and (c) rate of operative vaginal procedures. These three linear associations persisted when controlling for maternal obesity (±30 kg/m2 ), “heavy babies” (>3.5 kg), and ethnicity. Using maternal age remained significantly an independent risk factor (p < .0001), after controlling for the major confounders: maternal BMI, maternal height, birthweight ≥3500 g, p < .0001. Conclusions—Increasing maternal age has a linear association with vaginal deliveries without any medical intervention, rate of cesarean sections, and rate of operative vaginal procedures. These associations are independent of maternal BMI and maternal height. We currently do not have a specific explanation why younger women appear to be protected from requiring intrapartum obstetric intervention. Nevertheless, these strong facts deserve acknowledgement and further research.

Clinical and Translational Science Institute
We examine the dynamic relationships between economic status and health measures using data from 8 waves of the Panel Study of Income Dynamics from 1999 to 2013. Health measures are self-rated health (SRH) and functional limitations; economic status measures are labor income (earnings), family income, and net wealth. We use 3 different types of models: (a) ordinary least squares regression, (b) first-difference, and (c) system-generalized method of moment (GMM). Using ordinary least squares regression and first difference models, we find that higher levels of economic status are associated with better SRH and functional status among both men and women, although …

Light has substantial influences on the physiology and behavior of most laboratory animals. As such, lighting conditions within animal rooms are potentially significant, and often underappreciated variables within experiments. Disruption of the light/dark cycle, primarily by exposing animals to light at night (LAN), disturbs biological rhythms and has widespread physiological consequences due to mechanisms such as melatonin suppression, sympathetic stimulation, and altered circadian clock gene expression. Thus, attention to the lighting environment of laboratory animals and maintaining consistency of a light/dark cycle is imperative for study reproducibility. Background Breast cancer patients report a perception of increased muscle fatigue, which can persist following surgery and standardized therapies. In a clinical experiment, we tested the hypothesis that pathways regulating skeletal muscle fatigue are down-regulated in skeletal muscle of breast cancer patients and that different muscle gene expression patterns exist between breast tumour subtypes. In a preclinical study, we tested the hypothesis that mammary tumour growth in mice induces skeletal muscle fatigue and that overexpression of the cytokine interleukin-15 (IL-15) can attenuate mammary tumourinduced muscle fatigue. Methods Early stage non-metastatic female breast cancer patients (n = 14) and female non-cancer …

BACKGROUND: Rib fractures (RFx) continue to be a source of morbidity and mortality. A RFx care pathway has been used based on forced vital capacity (FVC). The objective of this study was to test the hypothesis that deterioration of FVC to less than 1 after admission is a marker for high-risk patients and affects outcomes. METHODS: A retrospective study of patients enrolled in an RFx care pathway at a Level 1 trauma center from 2009 to 2014. All patients had an admission FVC greater than 1. 2 groups were analyzed: patients with a lowest inpatient FVC less than 1 (Group …
