Background: Suicide has been called the ‘silent crisis’. As rates have increased in recent years, it is now the 10th leading cause of death in the United States. Within the state of Kentucky, females die by suicide most often through the use of firearms, differing from the national trend where poisoning is most common. With a drastic increase in firearm suicides has been observed beginning in 2010. Introduction: Hormone-related reproductive factors have been reported to be associated with breast cancer subtypes. However, the direction and magnitude of these associations were inconsistent. Additionally, for breast cancer defined by estrogen receptor (ER) and progesterone receptor (PR): ER+PR+, ER+PR-, ER-PR+, and ER-PR- subtypes, no meta-analysis was available for ER+PR-and ER-PR+ subtypes. For ER+PR+ and ER-PR- subtypes, only a few reproductive risk factors have been examined in meta-analyses.
Methods: Primary studies published from 2011 to 2017 were retrieved from PubMed, Embase, and Scopus. Following PRISMA guidelines, a total of 98 eligible studies investigated the association between reproductive factors and breast cancer …

Objectives: To examine the inter-laboratory agreement between multiplex realtime quantitative polymerase chain reaction (qPCR) assay results and aerobic culture results on bovine lung samples for detection of Histophilus somni (HS), and to assess associations of laboratory-derived factors on test agreement.
Methods: A survey of records from the University of Kentucky Veterinary Diagnostic Laboratory (UKVDL) was conducted to evaluate test results from grossly pneumonic bovine lung samples submitted during the period April 1, 2015 through August 31, 2018. In eastern Kentucky, some communities experience colorectal cancer (CRC) mortality rates as high as 25.9 deaths per 100,000 people as compared to the national rate of 14.8 deaths per 100,000 people (NCI, 2018; Kentucky Cancer Registry, 2018). Innovative screening mechanisms, coupled with patient navigation services, may be the key to increasing screening rates and preventing unnecessary deaths in the region. This Article contemplates the validity of theft rhetoric in relation to the right of countries to grant compulsory licenses from an unconventional perspective; that of biblical teachings on what it means to steal.
Part I describes the use of theft rhetoric in relation to IP infringement broadly and drug-patent compulsory licenses in particular.
Part II challenges the contention, suggested by theft rhetoric, that compulsory licenses are morally wrong as a form of stealing, by considering the meaning of theft in the context of its Judeo-Christian origins.

The Science Journal of the Lander College of Arts and Sciences
Takotsubo Cardiomyopathy (TTC) is a temporary heart-wall motion abnormality with the clinical presentation of a myocardial infarction. Found predominantly in postmenopausal women, TTC most often appears with apical ballooning and mid-ventricle hypokinesis. Often induced by an emotional or physical stress, TTC is reversible and excluded as a diagnosis in patients with acute plaque rupture and obstructive coronary disease. The transient nature and positive prognosis of this cardiomyopathy leaves a dilemma as to what precipitates it. Drug abuse, currently a national epidemic affecting millions of Americans, causes numerous health issues including increased dental disease. There are several factors which can contribute to an increase in caries and missing teeth. One of the most discussed mechanisms is xerostomia. Drug abusers tend to have large sugar intakes which exacerbates the problems caused by xerostomia. Drug abusers are also at risk for oral infections associated with dental decay due to their altered saliva composition. ประสิทธิภาพของการใช้ยาต้านการทำงานของเกล็ดเลือดรักษาภาวะลมพิษเรื้อรังที่ยากต่อการรักษา, อมรรัตน์ ประเสริฐเจริญสุข

วัตถุประสงค์ เพื่อศึกษาประสิทธิภาพของการรักษาโดยใช้ยาต้านเกล็ดเลือดร่วมกับยาต้านฮีสตามีน ในการรักษาภาวะลมพิษเรื้อรังที่ยากต่อการรักษา เปรียบเทียบกับการใช้ยาต้านฮีสตามีนอย่างเดียว วิธีการวิจัย การวิจัยเชิงทดลองแบบสุ่มกลุ่มตัวอย่างและมีการปกปิดในผู้ป่วยและผู้วิจัย ทำการศึกษาผู้ป่วยลมพิษเรื้อรังที่ไม่ตอบสนองต่อการรักษาด้วยยาต้านฮีสตามีนตามมาตรฐาน โดยมีคะแนนประเมินอาการลมพิษใน 1 สัปดาห์ (weekly urticaria activity scores,UAS7) ตั้งแต่ 16 คะแนนขึ้นไปและมีค่า D-dimer มากกว่า 500 ng/ml แบ่งออกเป็นสองกลุ่ม โดยกลุ่มที่ได้ยาต้านเกล็ดเลือด ได้รับยา cilostazol 150 มิลลิกรัม และ dipyridamole 50 มิลลิกรัม ร่วมกับยาต้านฮีสตามีน (desloratadine 20 มิลลิกรัม) และกลุ่มควบคุมได้รับยาต้านฮีสตามีน (desloratadine 20 มิลลิกรัม) อย่างเดียวต่อวัน โดยทำการรักษา 4 สัปดาห์ ผลการศึกษา ผู้ป่วยลมพิษทั้งหมด 14 คน กลุ่มที่ได้รับยาต้านเกล็ดเลือด 5 คน และ กลุ่มที่ได้รับยาต้านฮีสตามีน 9 คน โดยพบว่ากลุ่มที่ได้ยาต้านเกล็ดเลือดมีคะแนนลมพิษ (UAS7) ลดลงอย่างมีนัยสำคัญ (24.00±4.00 vs 8.00±7.00, P < 0.05) กลุ่มที่ได้ยาต้านฮีสตามีนลดลงจาก19.00±10.00 เป็น 11.00±22.00 คะแนน (P=0.09) และพบว่าระดับค่า D-dimer ในกลุ่มที่ได้ยาต้านเกล็ดเลือดมีการลดลงจาก 1,036.52±746.68 ng/ml เป็น 549.36±582.56 ng/ml (P=0.08) ส่วนกลุ่มที่ได้ยาต้านฮีสตามีนพบว่ามีการเปลี่ยนแปลงเล็กน้อย โดยค่า D-dimer จาก 875.81±539.95 ng/ml เป็น 642.02±661.19 ng/ml (P=0.31) สรุป การศึกษานี้ การใช้ยาต้านเกล็ดเลือด ร่วมกับ ยาต้านฮีสตามีน ทำให้ผู้ป่วยมีภาวะลมพิษดีขึ้น และพบว่าค่า D-dimer ลดลง อาจจะเป็นไปได้ว่า ภาวะเกาะกันของเกล็ดเลือดมีบทบาทในผู้ป่วยที่เป็นลมพิษเรื้อรังและยากต่อการรักษา

วัตถุประสงค์ เพื่อเปรียบเทียบผลของระดับไบคาร์บอเนตในเลือดที่แตกต่างกันต่อการเปลี่ยนแปลงของมวลและความแข็งแรงกล้ามเนื้อ อัตราการทำงานของไต และภาวะโภชนาการ ในผู้ป่วยโรคไตเรื้อรังระยะก่อนการบำบัดทดแทนไตที่มีภาวะเลือดเป็นกรด วิธีการวิจัย การวิจัยเชิงทดลองแบบสุ่มและมีกลุ่มควบคุมในผู้ป่วยโรคไตเรื้อรังระยะที่ 3และ4 ที่มีระดับไบคาร์บอเนตในเลือดน้อยกว่า 22 มิลลิโมลต่อลิตร ทั้งหมด 42 ราย ผู้ป่วยทุกรายจะได้รับประทานยาเม็ดโซเดียมไบคาร์บอเนตตามตารางคำนวณเม็ดยาก่อนแบ่งเป็นกลุ่มที่มีระดับไบคาร์บอเนตเป้าหมาย 26±1 มิลลิโมลต่อลิตร (กลุ่มทดลอง) และกลุ่มที่มีระดับไบคาร์บอเนตเป้าหมาย 22±1 มิลลิโมลต่อลิตร (กลุ่มควบคุม) ทั้งสองกลุ่มได้รับการตรวจติดตามมวลกล้ามเนื้อโดยใช้ multi-frequency bioelectrical impedance analysis (MF-BIA) ตรวจแรงบีบมือ และติดตามค่าการทำงานของไต ระดับพรีอัลบูมินในเลือด ระดับ normalized protein nitrogen appearance (nPNA) รวมถึงผลข้างเคียงทุก 2 เดือนจนครบ 4 ดือน ผลการศึกษา มีผู้ป่วยในกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 21 ราย อายุเฉลี่ยอยู่ที่ 60 ปี มีผู้ป่วยเพศชายร้อยละ 60 และเป็นเบาหวานร้อยละ 60 หลังจากรักษาผ่านไป 4 เดือน ระดับไบคาร์บอเนตเฉลี่ยของทั้งสองกลุ่ม อยู่ที่ 23.1±2.0 และ 21.2±2.7 มิลลิโมล/ลิตร ตามลำดับ พบว่ากลุ่มทดลองมีมวลกล้ามเนื้อทั้งร่างกายเพิ่มขึ้นจาก 26±5.4 เป็น 26.7±5.7 กก. (p=0.046) และกล้ามเนื้อระยางค์เพิ่มขึ้นจาก 19.8±4.1 เป็น 20.6±4.5 กก. (p= 0.03) และพบว่าผู้ป่วยเบาหวานในกลุ่มทดลองมีการเพิ่มขึ้นของมวลกล้ามเนื้ออย่างมีนัยสำคัญทางสถิติ ผลต่อระดับการทำงานของไต พบว่าในกลุ่มทดลองยังไม่มีการเปลี่ยนแปลงอย่างมีนัยสำคัญ แต่ในกลุ่มควบคุมมีอัตราการกรองของไตลดลงเฉลี่ย 3.6 มล./นาที/1.73ตร.ม. รูปแบบการรับประทานโพแทสเซียมในอาหารของผู้ป่วยไตวายเรื้อรังระยะสุดท้ายที่ได้รับการบำบัดทดแทนไตด้วยการล้างไตทางช่องท้องในประเทศไทย, มธุรส วิโรจนวัธน์

ที่มา: ผู้ป่วยล้างไตทางช่องท้องชาวไทยพบปัญหาโพแทสเซียมในเลือดต่ำร้อยละ 46 และภาวะโพแทสเซียมในเลือดต่ำสัมพันธ์กับอัตราการเสียชีวิตที่เพิ่มขึ้น มีความเชื่อว่าปริมาณโพแทสเซียมในอาหารน่าจะเป็นปัจจัยสำคัญ ข้อสงสัยนี้ยังไม่สามารถสรุปได้แน่ชัด เนื่องด้วยยังไม่มีงานวิจัยที่ศึกษารูปแบบการรับประทานโพแทสเซียมในอาหารของผู้ป่วยกลุ่มนี้ในประเทศไทย วัตถุประสงค์: เพื่อศึกษารูปแบบการรับประทานโพแทสเซียมในอาหารของผู้ป่วยล้างไตทางช่องท้องในประเทศไทย วิธีการศึกษา: การศึกษาวิจัยเชิงพรรณนา ศึกษารูปแบบการรับประทานโพแทสเซียมในอาหารของผู้ป่วยล้างไตทางช่องท้องที่ได้รับการรักษาในโรงพยาบาล 4 แห่ง โดยแนะนำวิธีการถ่ายภาพอาหารที่รับประทาน ส่งภาพอาหารทางสมาร์ทโฟนผ่านแอพพลิเคชันไลน์ให้นักกำหนดอาหาร เป็นเวลา 3 วัน (วันธรรมดา 2 วัน และ วันหยุด 1 วัน) ข้อมูลปริมาณอาหารจะถูกนำมาบันทึกในโปรแกรมอินมูแคลโดยนักกำหนดอาหาร 1 คน ตรวจระดับโพแทสเซียมในเลือด นิยามโพแทสเซียมในเลือดต่ำ คือ ระดับโพแทสเซียมในเลือด

Patient adherence to in-clinic rehabilitation is between 30-70% and even lower for home exercise programs (HEPs). Barriers to patient adherence have been identified and include but are not limited to anxiety, depression, lack of positive feedback, lack of social support, lack of time, low levels of physical activity at baseline, pain during exercise, and low self-efficacy. As clinicians prescribing rehabilitation may not be able to influence all of the identified barriers, they may positively influence others. Discharge readiness following total knee replacement (TKR) has often been defined using quantitative factors, such as knee range of motion or walking a specified distance. These measurements fail to include other features that could impact readiness for discharge, such as social support or patient perceptions. Most patients have positive results following TKR surgery, however others experience negative outcomes such as falls, reduced functional performance, and hospital readmission. Purpose: To investigate current diagnostic equipment availability and usage for glaucoma casefinding within community optometric practice, and to explore optometrists’ attitudes towards an enhanced scope of clinical practice. Methods: An anonymous survey was developed, validated, and distributed to all optometrists in Ireland. Results: 199 optometrists (27% of registrants) responded to the survey. 87% had access to the traditional triad of tests necessary to conduct adequate glaucoma case finding. Standard automated perimetry was the most commonly absent (13%) of the three essential screening tests. 64% of respondents indicated that monocular direct ophthalmoscopy was their first choice technique for fundus examination. 47% …

Current treatment strategies for glaucoma are limited to halting disease progression and do not restore lost visual function. Intraocular pressure is the main risk factor for glaucoma, and intraocular pressure–lowering treatment remains the mainstay of glaucoma treatment, but even successful intraocular pressure reduction does not stop the progression of glaucoma in all patients. We review the literature to determine whether nutritional interventions intended to prevent or delay the progression of glaucoma could prove to be a valuable addition to the mainstay of glaucoma therapy.
