The purpose of this study was to explore job satisfaction and retention of front line nurse managers. Retention of nurses in the workplace is a primary focus of health care systems. Helen Erickson, Evelyn Tomlin, and Mary Ann Swain’s Theory of Modeling and Role Modeling served as the theoretical framework for this study. The literature shows that there is prevalence of nurse manager job satisfaction and retention. Limited information exists on funding models for interprofessional education (IPE) course delivery, even though potential savings from IPE could be gained in healthcare delivery efficiencies and patient safety. Unanticipated economic barriers to implementing an IPE curriculum across programs and schools in University settings can stymie or even end movement toward collaboration and sustainable culture change. Clarity among stakeholders, including institutional leadership, faculty, and students, is necessary to avoid confusion about IPE tuition costs and funds flow, given that IPE involves multiple schools and programs sharing space, time, faculty, and tuition dollars. Background

There is a well-established inverse relationship between cardiorespiratory fitness (CRF) and mortality. However, this relationship has almost exclusively been studied using estimated CRF.

Objectives

This study aimed to assess the association of directly measured CRF, obtained using cardiopulmonary exercise (CPX) testing with all-cause, cardiovascular disease (CVD), and cancer mortality in apparently healthy men and women.

Methods

Participants included 4,137 self-referred apparently healthy adults (2,326 men, 1,811 women; mean age: 42.8 ± 12.2 years) who underwent CPX testing to determine baseline CRF. Participants were followed for 24.2 ± 11.7 years (1.1 to 49.3 years) for mortality. Cox-proportional hazard models were …

Purpose: Cardiopulmonary exercise testing (CPX) provides valuable clinical information, including peak ventilation (V˙Epeak), which has been shown to have diagnostic and prognostic value in the assessment of patients with underlying pulmonary disease. This report provides reference standards for V˙Epeak derived from CPX on treadmills in apparently healthy individuals.

Methods: Nine laboratories in the United States experienced in CPX administration with established quality control procedures contributed to the Fitness Registry and the Importance of Exercise National Database from 2014 to 2017. Over the past three decades numerous studies have illustrated the inverse relationship between cardiorespiratory fitness (CRF) and mortality. However, this relationship has almost exclusively been studied using estimated CRF (CRFe), with known error of ± 3 to 7 ml/kg/min, with no studies assessing this association using directly measured CRF in both men and women that were apparently healthy at baseline testing.

The purpose of this evidenced-based project was to answer the question: Does using a formalized discharge list identifying key educational topics regarding the patient’s disease process decrease readmission risk for COPD patients? This study used all COPD patients admitted over a two-week period who consented to receiving one-on-one detailed COPD education (n=33). This Doctor of Nursing Practice Project examined the effect of an evidence-based staff development education program on the attitudes and knowledge levels of nursing staff regarding the mentally ill adolescent patient population, for which they provide treatment and supervision. A convenience sample of 61 nursing staff members participated in the Improving Therapeutic Relationships among Psychiatric Nursing Staff and Patients project, with 74% of the available sample completing all project surveys. Dynamic issues associated with infection create unprecedented demands for quality healthcare improvements and modifications. Researchers found extended antibiotic infusions to be an effective treatment option with significant clinical benefits to patients and astounding cost savings. This scholarly project identified the emergence of several clinical issues threatening to undermine the intended patient responses to extended antibiotic infusion therapy in one large hospital system. Short bowel syndrome is the congenital or surgical alteration of the small bowel resulting in a shorter than normal length of small bowel. This condition affects approximately 20,000 people in the Unites States and is diagnosed in 3-5 times per 100,000 live births annually. study objective: To evaluate the implementation of the Ohio Emergency and Acute Care Facility Opioids and Other Controlled Substances Prescribing Guidelines and their perceived impact on local policies and practice.

Methods: The study design was a cross-sectional survey of emergency department (ED) medical directors, or appropriate person identified by the hospital, perception of the impact of the Ohio ED Opioid Prescribing Guidelines on their departments practice. From Bouveret's Syndrome To Gallstone Ileus: The Journey Of A Migrating Stone!, F. G. Haddad, W. Mansour, J. Mansour, L. Deeb

Bouveret's syndrome, first described in 1896, is an unusual cause of gastric outlet obstruction secondary to large gallstone impaction in the proximal duodenum after migration through a cholecystoduodenal fistula. Stone migration has been previously described after endoscopic or surgical fragmentation. However, this is the first reported case, in our knowledge, where the stone migrated after oral contrast administration a few days after the onset of symptoms, causing a distal gallstone ileus.

Myotonic muscular dystrophy (DM) is a multi-system disorder affecting skeletal muscles as well as smooth and cardiac muscles. Patients with DM experience disturbances in gastrointestinal motility; however, pancreatobiliary manifestations have rarely been described. We report the case of a 58-year-old male with MD who presented with a sudden onset of vomiting and abdominal pain. Laboratory and radiological findings were consistent with acute pancreatitis. No identifiable cause of pancreatitis could be identified despite an extensive workup. Sphincter of Oddi dysfunction (SOD) was felt to be the most likely cause of our patient's acute pancreatitis. Treatment of lung cancer has been revolutionized with development of drugs that target key driver mutations and immune checkpoints. Until recently, it was believed that these driver mutations are mutually exclusive. However, few reports have emerged citing the presence of both mutations either synchronously or metachronously. We describe a case report of lung adenocarcinoma harboring two driver mutations in the same tumor cells as well as exhibiting high PDL1 expression. We further discuss the possible association of these driver mutations with PDL1 expression.

Alzheimer's disease (AD) is the most common type of dementia, which has a substantial genetic component. AD affects predominantly older people. Accordingly, the prevalence of dementia has been rising as the population ages. To date, there are no effective interventions that can cure or halt the progression of AD. The only available treatments are the management of certain symptoms and consequences of dementia. Primary cilia are microtubule-based organelles, which protrude from the plasma membrane and receive a wide range of extracellular signals. Various cilia use G protein-coupled receptors (GPCRs) for the detection of these signals. For instance, vertebrate rod photoreceptors use their cilia (also called outer segments) as antennae detecting photons by GPCR rhodopsin. Rhodopsin recognizes incoming light and activates its G protein, transducin, which is composed of three subunits α, β, and γ. Similar to all G protein γ subunits, the transducin Gγ1 subunit undergoes C-terminal prenylation resulting in the addition of an isoprenoid farnesyl; however, the significance of this posttranslational modification …

Background: The nonsurgical treatment of chest wall deformity by a vacuum bell or external brace is gradual, with correction taking place over months. Monitoring the progress of nonsurgical treatment of chest wall deformity has relied on the ancient methods of measuring the depth of the excavatum and the protrusion of the carinatum. Background: Cardiovascular disease is the leading cause of death worldwide. Outcomes of patients with ST-segment–elevation myocardial infarction (STEMI) have improved through widespread implementation of systems-of-care, yet sex disparities continue to be reported. A comprehensive, global study of sex disparities in contemporary STEMI care and outcomes has not been undertaken.

Objective: To examine whether sex differences in STEMI management and mortality outcomes persist worldwide and by geographic region.

Monitoring the resistance training volume load (VL) (sets × reps × load) is essential to managing resistance training and the recovery–adaptation process. Subjects: Eight trained weightlifters, seven of which were at national level, participated in the study. Methods: VL was measured both with (VLwD) and without (VL) the inclusion of barbell displacement, across twenty weeks of training, in order to allow for comparisons to be made of these VL calculating methods. Cardiorespiratory fitness (CRF) has been documented as a strong, independent predictor of non-communicable disease and mortality in both clinical and apparently healthy populations. This well-established relationship has impelled organizations, including the American Heart Association, to release scientific statements highlighting the importance of accurate quantification of CRF. Current knowledge of the relationship between CRF and mortality is predominantly based on estimated CRF obtained from varying indirect methods. Cardiopulmonary exercise testing (CPX), the gold standard method of CRF measurement, provides a more accurate and reliable quantification of CRF compared to estimated methods.
