© 2018, Spandidos Publications. All rights reserved. The incidence of rectal neuroendocrine tumor (NET), which is often diagnosed during routine surveillance endoscopy, is increasing. The majority of these tumors are small and asymptomatic, possessing benign features with favorable prognoses. At present, small rectal NETs without high-risk factors are typically treated by local resection, including endoscopic mucosal resection, endoscopic submucosal dissection, or transanal endoscopic microsurgery, with or without additional imaging follow-up by abdominal computed tomography or magnetic resonance imaging. The present study, however, describes a case of a small rectal NET without any known risk factors, which was accompanied by substantial …

Reminder Duration Determines Threat Memory Modification In Humans, J. Hu, W. Wang, P. Homan, P. Wang, X. Zheng, D. Schiller

© 2018 The Author(s). Memory reminders can return a memory into an unstable state such that it will decay unless actively restabilized into long-term memory through reconsolidation. Exposure to a memory reminder, however, does not always lead to destabilization. The 'trace dominance' principle posits that the extent of exposure to memory reminders governs memory susceptibility to disruption. Here, we provide a first systematic investigation of reminder duration effects on threat memory modification in humans. Reminder duration was parametrically varied across 155 participants in a three-day protocol. We found that short reminders (1 s and 4 s) made the memory prone …

© 2018 American Society for Microbiology. The transcription factor interferon regulatory factor 5 (IRF5) exerts crucial functions in the regulation of host immunity against extracellular pathogens, DNA damage-induced apoptosis, death receptor signaling, and macrophage polarization. Tight regulation of IRF5 is thus warranted for an efficient response to extracellular stressors and for limiting autoimmune and inflammatory responses. Here we report that the COP9 signalosome (CSN), a general modulator of diverse cellular and developmental processes, associates constitutively with IRF5 and promotes its protein stability. The constitutive CSN/IRF5 interaction was identified using proteomics and confirmed by endogenous immunoprecipitations. The CSN/IRF5 interaction occurred on …

Maxillary Expansion: From Past To Present, Shih-Chieh Chen, Hong-Po Chang, Yu-Chuan Tseng Jan 2018

Maxillary Expansion: From Past To Present, Shih-Chieh Chen, Hong-Po Chang, Yu-Chuan Tseng

Taiwanese Journal of Orthodontics

Maxillary expansion is a common treatment option for upper crowded dentition and posterior lingual crossbite in children and adolescents. As the midpalatal suture is not yet completely interlocked at these ages, the midpalatal suture can be opened via maxillary expansion to increase the length and width of the upper arch. This solves the problems of posterior crossbite and upper crowded dentition. Since the midpalatal suture is stiff in adults, many clinicians believe that adults require surgical intervention to achieve maxillary expansion. However, recent reports indicated that several cases have been treated successfully with the aid of TADs for maxillary expansion. …

Correction Of Bimaxillary Protrusion By Extraction Of Incisors And Canine Substitution, Chung-Li Wang, Chun-Liang Kuo, I-Hua Liu, Chun-Hsiu Yang

Taiwanese Journal of Orthodontics

The maxillary six anterior teeth play important roles in esthetics and oral function. Treatments for missing maxillary lateral incisors either by space closure or space preservation need multiple clinical considerations. This case report presents an orthodontic treatment of a 33-year-old female patient who had a convex profile with lip incompetence, missing lower molars and poor prognosis of bilateral maxillary lateral incisors. The atypical extraction in treatment plan consisted of removing lateral incisors instead of premolars to retract the anterior teeth. The improvement of facial profile and lip posture was achieved and the anterior teeth esthetics was further rehabilitated with prosthesis. …

BACKGROUND: Youth living with disabilities are at risk of experiencing poor health outcomes. Coordinated school health programs have an opportunity to help youth with disabilities and their families through health education, health services, and community engagement. The World Health Organization developed the International Classification of Functioning, Disability, and Health (ICF) as a framework to analyze factors related to health conditions. We used the ICF to examine parental perceptions of health and function among students with disabilities living in rural and urban areas.

METHODS: We surveyed parents (N = 71) using the parent-report versions of the Pediatric Outcomes Data Collection Instrument …

Etiology Of Teen Dating Violence Among Adolescent Children Of Alcoholics, Jennifer A. Livingston, Rina D. Eiden, Jared Lessard, Meghan Casey, James Henrie, Kenneth E. Leonard

United States Food and Drug Administration: Publications

Family processes in early life have been impli- cated in adolescent involvement in teen dating violence, yet the developmental pathways through which this occurs are not well understood. In this study, etiological pathways from parental psychopathology and marital conflict in infancy to involvement in dating violence in late adoles- cence were examined in a sample of children at high-risk due to parental alcohol problems. Families (N = 227) recruited when the child was 12 months of age were assessed at 12-, 24-, 36-months, kindergarten, 6th, 8th, and 12th grades. Slightly more than half of the children were female (51%) and …

Transitional Care Medical House Call: A Pilot Project, Ron B. Ordona Jan 2018

Transitional Care Medical House Call: A Pilot Project, Ron B. Ordona

Doctor of Nursing Practice Projects

Problem Description: Vulnerable, homebound older adults are highly susceptible to unplanned 30-day hospital readmissions, which is costly for the healthcare system. As a result, health care expenditures for this population continue to rise. Studies have shown that transition of care programs, when complemented with home-based primary care delivery, may improve health care outcomes for this population.

Purpose: The purpose of this quality improvement pilot project was to implement medical house calls as a component of transitional care management (TCM) and measure patient outcomes such as unplanned 30-day readmission rates and correlate predictors of readmission. As a secondary outcome, the project …

Improving Care For Adult Clinic Patients With A History Of Poor Glycemic Control, Bonnie M. Clark Jan 2018

Improving Care For Adult Clinic Patients With A History Of Poor Glycemic Control, Bonnie M. Clark

Doctor of Nursing Practice Projects

Problem Description: Patients of one internal medicine clinic were found to have nearly twice the rate of diagnosed diabetes and poor glycemic control, when compared with national rates. Given this, certain aspects of the patient-provider dyad system, such as inadequate provider time, knowledge, and resources; may have contributed to the ability of some patients to adapt to a lifestyle with consistent diabetes self-care.

Intervention: An evidence-based diabetes protocol was developed, a diabetes self-management training (DSMT) curriculum was adapted to local context, and three cycles of patient-centered DSMT classes were delivered to provide individual and group-based support to participants. Completion of …

Body Consciousness Of Clinical Staff Working In Plastic And Cosmetic Surgery Environments, Jodie Alexander Jan 2018

Body Consciousness Of Clinical Staff Working In Plastic And Cosmetic Surgery Environments, Jodie Alexander

Nursing Theses and Capstone Projects

A quantitative research study was conducted to determine body consciousness of plastic surgery nurses and other employees of plastic offices. The theoretical basis was the Objectification Theory that states that a person is at risk of being viewed as an object. Social and other media has made an impact on how a person views themselves and others and it has also glamourized plastic surgery and cosmetic procedures. The social media explosion of the past decade has changed the way the public perceives their image. Nurses working in plastic surgery are exposed to body objectification on a daily basis and it …

Evidence-Based Interventions To Address Workplace Violence In The Emergency Department, Gari Leigh Adams Jan 2018

Evidence-Based Interventions To Address Workplace Violence In The Emergency Department, Gari Leigh Adams

Nursing Theses and Capstone Projects

Workplace violence, as a result of violent patient behavior, is a problem in health care settings and the incidence is increasing. The vulnerability of the emergency department (ED) contributes to the high incidence of violence and includes long waits, crowded spaces, high anxiety levels, substance abuse, and a large number of behavioral health patients seeking treatment. ED team members admit they do not always feel safe when caring for patients due to the risk of violent behavior. Workplace violence effects team member productivity, job satisfaction, and burnout. Team members are at risk for injury or harm and coping challenges as …

Evidence-based practice is a necessary part of providing quality patient care in nursing, and has been linked to lowering costs and bettering patient outcomes. However, evidence-based practice is commonly misunderstood or thrown aside by nurses due to time constraints in the nursing profession. The purpose of the MSN thesis was to explore nurses’ current knowledge and appreciation of evidence-based practice based on previous research, as well as identifying gaps in the knowledge of the concept. A descriptive research design was utilized. Results indicated nurses do have an open mind and appreciation for evidence-based practice, but consider it time consuming and …

Graduate Nurses’ Perceived Confidence In Clinical Competence, Jennifer Dobbins Jan 2018

Graduate Nurses’ Perceived Confidence In Clinical Competence, Jennifer Dobbins

Nursing Theses and Capstone Projects

Upon entering the practice environment, newly graduated nurses often find themselves feeling underprepared for new professional role expectations (Kim, Lee, Eudey, Lounsbury, & Wede, 2015). New graduate nurses’ stress and lack of confidence forges a higher turnover potential (Kim et al., 2015), and increased stress among new graduate nurses may impact their ability to provide quality care (Fink, Krugman, Casey, & Goode, 2008; Theisen & Sandau, 2013). Implementing nurse residency programs have been shown to improve new graduate nurse retention, confidence, and competence (Silvestre, Ulrich, Johnson, Spector, & Blegen, 2017) and are designed to ease transition of new graduate nurses …
