© 2017 Sarcomatoid renal cell carcinoma (sRCC) is an aggressive variant of renal cell carcinoma (RCC) that has a significantly lower overall survival. Even after prompt surgical extirpation, this histologic variant progresses rapidly. We present a case of an early recurrence and rapid progression of sRCC despite successful radical resection.

Objective: To expand the diagnostic armamentarium for medullary sponge kidney (MSK), we evaluate the use of high-resolution multidetector computed tomography (MDCT) for MSK diagnosis and compare to the standard intravenous urography (IVU). Despite a significant prevalence amongst stone formers, diagnosis of this well described condition has declined. IVU, the gold standard in MSK diagnosis, has largely been replaced by CT, which has previously been shown unable to demonstrate signs of MSK. Purpose: The evolution of health care systems in response to societal and financial pressures has changed care delivery models, which presents new challenges for physicians. Leadership training is increasingly being recognized as an essential component of medical education training to prepare physicians to meet these needs. Unfortunately, most medical schools do not include leadership training. It has been suggested that a longitudinal and integrated approach to leadership training should be sought. Several nomograms exist for ordering palladium-103 seeds for permanent prostate seed implants (PSI). Excess seeds from PSIs pose additional radiation safety risks and increase the cost of care. This study compared five nomograms to clinical data from dynamic modified-peripheral intraoperative PSI to determine (a) the cause of excess seeds and (b) the optimal nomogram for our institution. © 2018 National Academy of Sciences. All Rights Reserved. The nervous system maintains physiological homeostasis through reflex pathways that modulate organ function. This process begins when changes in the internal milieu (e.g., blood pressure, temperature, or pH) activate visceral sensory neurons that transmit action potentials along the vagus nerve to the brainstem. IL-1β and TNF, inflammatory cytokines produced by immune cells during infection and injury, and other inflammatory mediators have been implicated in activating sensory action potentials in the vagus nerve. With the increasing prevalence and high cost of treatment, diabetes places an enormous demand on economic resources. To combat this issue and rising health care costs,Cleveland County Government took a proactive approach and partnered with its local YMCA. The YMCA was contracted to provide Prediabetes Prevention and Diabetes Control program classes to County employees. The programs were designed to (a) decrease the A1c level of program participants, (b) lower health care costs for the County, and (c) provide education about prevention and maintenance.

This project seeks to explore the concept of aromatherapy as an intervention for undergraduate nursing student test anxiety in greater depth. Test anxiety is significant in nursing education because of the high stakes weight of exams and difficulties associated with answering multiple choice and multiple response questions. Historically, the Hunt School of Nursing has not provided specific procedures or interventions to assist students with test anxiety; however, this project seeks to outline a pathway for the development and achievement of a successful aromatherapy program for undergraduate students. Approaching from Jung’s perspective this paper aims to understand how the unconscious communicates through symbolism that may be the basis for synchronicity arising from mantic procedures. A particular ritual of divination called tasseography will be studied whereby the seer interprets patterns in coffee grounds intuitively and by following a standard system of symbolism to foretell the seeker’s future life events or provide answers to seeker’s pressing life questions. Exploring The Career Satisfaction Of Counselor Educators, Regina R. Moro, Rebecca Scherer
Counselor Education Faculty Publications and Presentations
In this article, the authors report counselor educators’ career satisfaction through a descriptive analysis. Seventy-five counselor educators from all across the United States completed an online demographic questionnaire and four self-report instruments related to career satisfaction in general, work environment, and mentorship experiences. The results indicate that counselor educators report satisfaction with most aspects of the job, but report dissatisfaction with pay/promotion and mentorship. This is important for consideration for current and future counselor educators, due to the amount of time and cost associated with obtaining a doctoral degree. The researchers discuss and suggest future research recommendations.

INTRODUCTION: The process of identifying effective responses to the challenges of placing and retaining a rural behavioral health workforce remains elusive. The Virtual Mentorship Network was developed to test the feasibility of using distance technology to connect rural students interested in mental health careers with mentors.
METHODS: In Year 1, college and high school students were virtually mentored using a near-peer approach both live and asynchronously as a cohort over 7 months. Aim: This research was aimed to assess the potential for inhibition of protein binding of phenytoin by tizoxanide in human plasma. Phenytoin was used because studies have proven to be a highly plasma protein-bound drug with a narrow therapeutic index and non-linear pharmacokinetics. Tizoxanide is another highly protein-bound active metabolite of an anti-infective prodrug nitazoxanide. Both drugs are expected to be administered together due to their therapeutic indications.

Spontaneous pneumomediastinum (SPM), also referred to as mediastinal emphysema, is defined as the presence of free air in the mediastinal cavity without a clear and identifiable cause. Spontaneous pneumomediastinum, in general, is a relatively rare condition, more so in the setting of pregnancy or labor. Clinically, SPM may present as dyspnea, chest pain, and subcutaneous swelling, which may be of serious concern in the setting of pregnancy. A comprehensive literature review revealed that the majority of patients are primiparas, of a younger age, and have term or longer durations of pregnancy. The second stage of labor was found to be …

The continually emerging field of biosocial criminology provides a basis for productively merging biology with sociological reasonings for criminal behavior. Mainstream research in criminology focuses on environmental factors as the sole reason individuals exhibit antisocial behavior patterns and may ultimately commit crimes. Criminological research has travelled in this direction for decades. The current climate within this community subscribes heavily to the notion that biology has very little to do with why people behave the way they do, and if it did, government control would be the norm. The effect of GINST15, an enzyme fermented ginseng supplement, on hormonal and inflammatory responses to physical stress in humans is unknown. The purpose of this investigation was to examine the constitutive and stress-induced effects of GINST15 supplement on hypo-pituitary-adrenal (HPA) and antioxidant activity in addition to muscle damage. Ten women (age: 38.7 ± 7.8 years; height: 163.81 ± 4.4 cm; body mass 76.0 ± 11.6 kg) and nine men (age: 41.2. ± 9.7 years; height: 177.4 ± 5.3 cm; body mass: 88.5 ± 5.0 kg) participated in a double-blinded, placebo-controlled, counterbalanced within-group study. Vision is one of a few senses people take for granted but imagine the shift in quality of life if suddenly there was a black curtain covering a patient’s visual field. Symptoms of retinal detachment can present as benign visual disturbances that could be dismissed by both patients and providers. The symptoms can be so subtle that patients present as asymptomatic and are often detected during primary care visits. Retinal detachment is more often diagnosed as people age and the visual structures become weakened or in patients that have chronic medical conditions such as diabetes.
