There continues to be a nurse shortage particularly in remote regions of Australia where the poor health status of Australian Indigenous people, and an increasing ageing population are a perennial problem. The Kimberley, the setting for this study, is a remote region in the north of WA. Human immunodeficiency virus (HIV) infection is now considered a chronic, treatable disease, although treatment is associated with increased rates of coronary artery disease (CAD). Screening of this at-risk population for CAD remains contentious. The aim of this thesis is to provide some guidance into the management of cardiovascular disease in HIV and examine the utility of screening CTCA in this population group.

Esophageal adenocarcinoma has one of the poorest outcomes of all solid tumors, attributable, at least in part, to lack of an early stage diagnostic test. Aberrant methylation is an early and frequent event in carcinogenesis providing an opportunity for early cancer detection. The overall aim of this thesis is to identify and validate regions of aberrant methylation as a biomarker for early detection of esophageal adenocarcinoma and the dysplastic stages of its precursor disease, Barrett’s esophagus. Physical inactivity is a leading cause of death in the United States (U.S.), and is a public health priority. The majority of individuals do not engage in regular physical activity (PA). Hispanics have the highest inactivity rates (51.3%) in the U.S. (HealthyPeople.gov, 2012). This paper reports a gap in knowledge concerning factors that affect PA. Data was collected in Kent and Ottawa counties in Michigan, with Hispanic mothers (N=79) and their daughters (N=79). This study supports the relationship among societal norms, exercise self-efficacy and activity related affect, on commitment to a plan of PA. Theorized relationships appear to be more …

In 2011, changes to the specific type, timing, and amount of food benefits to mothers in the WIC program were rolled out nationwide. These were the first substantive changes since 1980 and were designed to increase breastfeeding initiation rates and extend breastfeeding duration among WIC program participants. Effectiveness of the program changes have been mixed and variable by geographic area of the U.S., however. In Michigan, breastfeeding duration has not increased. This analysis examines the implementation of the national policy change at the state level for Michigan and suggests additions to program to specifically target duration breastfeeding by Michigan mothers.

Circulating serum adiponectin levels in obese asthmatics have been reported to be low. Therefore, we aimed to investigate the role of adiponectin in a mucus hypersecretion model and hypothesized that adiponectin would decrease IL-13 induced MUC5AC expression from differentiated NHBE cells and that increasing concentrations of IL-13 would cause a decrease in Adipo R1 and Adipo R2 expression. Risperidone is an ‘atypical’ antipsychotic and is approved by the USFDA mainly for the treatment of schizophrenia and symptoms of bipolar disorder. Risperidone (an SDA or serotonin-dopamine antagonist) has ~20-fold higher affinity at 5-HT_2A receptors over dopamine D₂ receptors, which makes it more efficacious against the negative symptoms of schizophrenia and less liable to causing extrapyramidal side effects than ‘typical’ antipsychotics.

Potential risk factors for development of Post-Traumatic Stress Disorder (PTSD) are still unclear. One potential risk factor for the development of PTSD is an individual’s cardiovascular reactivity and recovery in response to stressor tasks. The current study was conducted with 763 Army National Guard and Army Reserve soldiers. Participants completed a stressful induction along with self-report measures prior to deployment. Post-deployment, self-report measures were completed to assess PTSD symptomatology and experiences related to deployment and combat. Isocyanate thermal degradation characterization by liquid chromatography coupled with electrospray tandem mass spectrometry has been performed to elucidate the methylene diphenyl diisocyanate (MDI) thermal degradation structure emitted in a generation chamber using a temperature between 50°C and 180°C to produce MDI vapors. [M+H]⁺ ions containing an isocyanate functional group were studied by tandem mass spectrometry. The [M+H]⁺ ion analyses based on the combination of full scans and precursor ion scans were useful for identifying all structures. Accurate measurement of 4,4'-methylene diphenyl diisocyanate (MDI) atmospheres is a challenge since the molecule is both chemically reactive and likely to be present in aerosol form when heated and sprayed because of its low vapor pressure. Meeting this challenge requires optimizing both the sampling device used and the derivatization agent employed to stabilize the isocyanate functional group. This study describes the use of a novel derivatization reagent for isocyanate sampling to address the challenge of MDI aerosol exposure sampling. Like most conventional derivatizing agents for isocyanates, 1,8-diaminonapthalene (DAN) reacts with isocyanate functional groups to form a urea. However, unlike other …

Two patients with no exposure to gardening compost had related Legionella longbeachae infections in Quebec, Canada. Epidemiologic investigation and laboratory results from patient and soil samples identified the patients’ workplace, a metal recycling plant, as the likely source of infection, indicating a need to suspect occupational exposure for L. longbeachae infections.

Evidence for adherence to the American Diabetes Association's (ADA) screening and treatment guidelines for various health complications of diabetes in rural populations is limited. The ADA recommends annual foot and eye exams and supports the use of the Semmes-Weinstein monofilament (SWM) to detect signs of diabetic peripheral neuropathy (DPN). This study evaluated the prevalence and treatment trends of DPN among patients with diabetes in a rural population. Background: Considerable research has documented that exposure to traumatic events has negative effects on physical and mental health. Much less research has examined the predictors of traumatic event exposure. Increased understanding of risk factors for exposure to traumatic events could be of considerable value in targeting preventive interventions and anticipating service needs.

Method: General population surveys in 24 countries with a combined sample of 68 894 adult respondents across six continents assessed exposure to 29 traumatic event types. Differences in prevalence were examined with cross-tabulations. Exploratory factor analysis was conducted to determine whether traumatic event types clustered into interpretable factors. …

Understanding biological mechanisms that shape brain development and susceptibility to emotional dysfunction is crucial for generating improved treatments for depression and anxiety disorders. To study neurodevelopmental factors that influence emotionality, we use model rats that were bred for distinct behavioral responses to novelty. Rats bred for low novelty response (LRs) exhibit a high anxiety-/depressive-like phenotype compared to high novelty responder rats (HRs), which vigorously explore novelty and exhibit high impulsivity, aggression, and risk-taking. The Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) survey is a publically reported comparative data tool measuring hospital patient experiences. Alternative pre-mRNA splicing provides an important mechanism for generating the diverse array of proteins required to generate complex tissue and cell types from a limited genome. Therefore, the proper regulation of alternative splicing is vital to shape cellular identity and function. As consequence, defects in alternative splicing are associated with disease phenotypes that can range from systemic syndromes to the dysfunction of single cell types. Wrist orthoses (also known as splints, braces, or supports) are commonly used to support or restrict the motion of a weak or injured wrist. These orthoses generally function by stiffening the wrist joint. Therefore, choosing the proper orthosis (or improving orthoses) requires that we understand their stiffness properties. In this study, we present a method for measuring the stiffness of wrist orthoses, and we apply this method to 12 of the most common wrist orthoses. The purpose of this cross sectional retrospective study was to use three dimensional surface imaging to determine gender dimorphism and facial morphological changes from adolescence to adulthood in African American and Caucasian populations. Materials and Methods: Three-dimensional images were captured using a laser scanner (Minolta Vivid VI900) and a stereophotogrammatic camera system (3dMDfaceTM). The total sample size included 371 subjects, including the following: 107 African-American adolescents 10–13 years of age, 100 African-American adults 19–30 years of age, 64 Caucasian adolescents 10–13 years of age, and 100 Caucasian adults 18–30 years of age.
