The gating of Kir channels depends critically on phosphatidylinositol 4,5-bisphosphate (PIP2), but the detailed mechanism by which PIP2 regulates Kir channels remains obscure. Here, we performed a series of Targeted molecular dynamics simulations on the full-length Kir2.1 channel and, for the first time, were able to achieve the transition from the closed to the open state. Our data show that with the upward motion of the cytoplasmic domain (CTD) the structure of the C-Linker changes from a loop to a helix. The efforts to improve health care delivery usually involve studies and analysis of patient populations and healthcare systems. In this dissertation, I present the research conducted in the following areas: identifying patient groups, improving treatments for specific conditions by using statistical as well as data mining techniques, and developing new operation research models to increase system efficiency from the health institutes’ perspective. The results provide better understanding of high risk patient groups, more accuracy in detecting disease’ correlations and practical scheduling tools that consider uncertain operation durations and real-life constraints.

Purpose—Several novel techniques have been developed recently to assess the breadth of speaker’s vocabulary exhibited in a language sample. The specific aim of this study was to increase our understanding of the validity of the scores generated by different lexical diversity (LD) estimation techniques. Four techniques were explored: D, Maas, Measure of Textual Lexical Diversity (MTLD), and the Moving Average Type Token Ratio (MATTR).

Method—Four LD indices were estimated for language samples on four discourse tasks (procedures, eventcasts, story retell, and recounts) from 442 neurologically intact adults. The resulting data were analyzed using structural equation modeling.

Economic pressures and the challenge to maintain competitive advantage have resulted in many health-care entities requiring their practitioners to contractually enter into noncompete clauses (NCCs). Many student registered nurse anesthetists (SRNAs) and Certified Registered Nurse Anesthetists (CRNAs) are unaware of NCCs in employee contracts.

An anonymous, web-based questionnaire regard-ing NCCs was distributed to SRNAs and CRNAs nation-wide. Of 242 practicing CRNAs who responded, 147 (60.7%) were employed without a noncompete clause and 22 (9.1%) were unaware whether they had such a provision in their employment contracts. The knowledge level of the nurse anesthetist respondents was low (average score of 55.3%). …

AIMS: The purpose of our study was to evaluate the diagnostic performance of breast magnetic resonance imaging (MRI) in the evaluation of contralateral breast in patients with diagnosed breast cancer. Social networking sites (SNSs) are an increasingly popular channel for communication among college students. Often students disclose more freely via social networking sites than they would in other situations. These disclosures commonly include information about engaging in risky health behaviors (e.g., binge drinking). Study 1 examined students’ impression management goals and self-presentation tactics specifically related to self-disclosures of drinking behavior on SNSs. Findings suggest that students use differing self-presentation tactics across various SNSs in order to achieve their impression management goals and to avoid consequences associated with disclosing about risky health behaviors to certain audiences. Stratification Of The Impact Of Inappropriate Empirical Antimicrobial Therapy For Gram-Negative Bloodstream Infections By Predicted Prognosis, Sarah E. Cain, Joseph Kohn, P. Brandon Bookstaver, Helmut Albrecht, Majdi N. Al-Hasan

The bloodstream infection mortality risk score (BSIMRS) predicts the outcome of patients with Gram-negative bloodstream infections (BSI) with high discrimination. This retrospective cohort study examined the impact of inappropriate antimicrobial therapy on mortality in adult patients with Gram-negative BSI admitted to Palmetto Health Hospitals in Columbia, SC, USA, from 1 January 2011 to 31 December 2012 after stratification by predicted prognosis at initial presentation using BSIMRS. A multivariate Cox regression model was used to identify independent risk factors for 28-day mortality overall and within each predefined BSIMRS category (<5, 5 to 9, and ≥10). Relative risk reduction (RRR), absolute risk reduction (ARR), and number needed to treat (NNT) were calculated from a predictive logistic regression model of mortality. Overall, 390 unique patients with first episodes of Gram-negative BSI were identified. The median age was 66 years, and 229 (59%) were women. There was significant association between inappropriate antimicrobial therapy and mortality in patients with BSIMRS of 5 to 9 (adjusted hazard ratio [aHR], 3.55; 95% confidence intervals [CI], 1.22 to 8.31; P = 0.02) and BSIMRS of ≥10 (aHR, 4.99; 95% CI, 1.09 to 22.87; P = 0.04) but not in those with BSIMRS of <5 (aHR, 3.34; 95% CI, 0.17 to 22.77; P = 0.34). RRR, ARR, and NNT were 0.25, 0.02, and 63 for BSIMRS of <5; 0.56, 0.32, and 3 for BSIMRS of 5 to 9; and 0.39, 0.39, and 3 for BSIMRS of ≥10, respectively. There is a significant benefit from appropriate antimicrobial therapy in patients with Gram-negative BSI with guarded (BSIMRS of 5 to 9) and poor (BSIMRS of ≥10) predicted prognosis. Survival difference remains unclear among those with good predicted prognosis (BSIMRS of <5) at initial presentation.

BACKGROUND: There is little research on the relationship of Hispanic ethnicity, drug use, and bully victimization with suicidal ideation among adolescent females. This study investigates all components and their association with suicidal ideation using the 2013 Youth Risk Behavior Surveillance Survey (YRBSS).

METHODS: Participants were 6,621 adolescent females who completed a self-administered questionnaire (YRBSS). Students answered a series of questions regarding unintentional injuries and violence and alcohol and other drug use.

We evaluated the influence of financial strain and perceived stress on smoking status among United Way Missouri 2-1-1 callers of predominantly low socioeconomic status.

Data for this study were collected as part of a randomized, controlled intervention trial integrating cancer control referrals into regular United Way 2-1-1 Missouri services. วัตถุประสงค์: เพื่อศึกษาความสัมพันธ์และปัจจัยทำนายการปรับตัวในชีวิตสมรสของสตรีตั้งครรภ์วัยรุ่น\n\nรูปแบบการวิจัย: การวิจัยแบบความสัมพันธ์เชิงทำนาย\n \nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ สตรีตั้งครรภ์วัยรุ่น ครรภ์แรก อายุระหว่าง 10-19 ปี ที่อาศัยอยู่กับสามี มารับบริการฝากครรภ์ที่หน่วยฝากครรภ์ โรงพยาบาลศิริราช จำนวน 145 ราย รวบรวมข้อมูลโดยใช้ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามความรู้สึกมีคุณค่าในตนเอง แบบสอบถามการรับรู้การสนับสนุนทางสังคม และแบบสอบถามการปรับตัวในชีวิตสมรส ตรวจสอบความเที่ยงของแบบสอบถามทั้งสามด้วยการหาค่าสัมประสิทธิ์อัลฟ่าของครอนบาค ได้ค่าเท่ากับ .76, .82 และ .78 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงบรรยาย สถิติอีต้า สถิติสหสัมพันธ์สเปียร์แมน สถิติสัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน และสถิติถดถอยพหุคูณแบบขั้นตอน\n \nผลการวิจัย: 1) สตรีตั้งครรภ์วัยรุ่นมีการปรับตัวในชีวิตสมรสอยู่ในระดับปานกลาง (x̄ = 64.86, SD = 13.36) 2) ปัจจัยที่มีความสัมพันธ์ทางบวกกับการปรับตัวในชีวิตสมรสในสตรีตั้งครรภ์วัยรุ่นอย่างมีนัยสำคัญทางสถิติที่ระดับ.05 ได้แก่ ความรู้สึกมีคุณค่าในตนเอง (r = .357) การรับรู้การสนับสนุนทางสังคม (r = .349) และความเพียงพอของรายได้ครอบครัว (η = .303) ทั้งนี้ อายุ ระดับการศึกษา และลักษณะครอบครัว ไม่มีความสัมพันธ์กับการปรับตัวในชีวิตสมรส (r = .036 , r = .012 และ η = .005 ตามลำดับ) 3) ปัจจัยทำนายการปรับตัวในชีวิตสมรสในสตรีตั้งครรภ์วัยรุ่น ได้แก่ ความรู้สึกมีคุณค่าในตนเอง ( β = .275) การรับรู้การสนับสนุนทางสังคม ( β = .264) และความเพียงพอของรายได้ครอบครัว (β = .254) โดยสามารถร่วมกันอธิบายความแปรปรวนของการปรับตัวในชีวิตสมรสในสตรีตั้งครรภ์วัยรุ่นได้ร้อยละ 26.8 (R2 =.268) ที่ระดับนัยสำคัญทางสถิติ .05\n\nสรุป: ปัจจัยทำนายการปรับตัวในชีวิตสมรสในสตรีตั้งครรภ์วัยรุ่น คือ …

เหตุผลของการทำวิจัย : จากการศึกษาผลของการสนับสนุนด้านข้อมูล และอารมณ์ต่อความวิตกกังวลที่ผ่านมาเป็นการศึกษาในผู้ดูแลผู้ป่วยที่อยู่ในระยะของโรคสมองเฉียบพลันยังไม่มีการศึกษาผู้ดูแลผู้ป่วยที่อยู่ในระยะของโรคสมองเรื้อรังเช่น ผู้ดูแลผู้ป่วยเนื้องอกสมองซึ่งจะทำให้ทราบผลของค้นหาแนวทางในการช่วยเหลือต่อไปวัตถุประสงค์ : เพื่อศึกษาผลของการสนับสนุนด้านข้อมูลและอารมณ์ต่อความวิตกกังวลของ ผู้ดูแลผู้ป่วยเนื้องอกสมองรูปแบบการวิจัย : การศึกษาวิจัยแบบกึ่งทดลองสถานที่ทำการศึกษา : โรงพยาบาลจุฬาลงกรณ์ตัวอย่างและวิธีการศึกษา : กลุ่มตัวอย่าง คือ ผู้ดูแลผู้ป่วยเนื้องอกสมอง ที่มาเยี่ยมและดูแลผู้ป่วยที่หอผู้ป่วยธนาคารกรุงเทพ ชั้น 2 หอผู้ป่วยวิกฤตศัลยกรรมประสาท 1ตึก สก. 8 และหอผู้ป่วยวิกฤตศัลยกรรมประสาท 2 ธนาคารกรุงเทพชั้น 2 จำนวน 24 ราย แบ่งเป็นกลุ่มทดลอง 12 ราย และกลุ่มควบคุม12 ราย ซึ่งได้มาจากการสุ่มกลุ่มตัวอย่างอย่างง่าย และใช้แบบสอบถามความวิตกกังวลของสปิลเบอร์เกอร์และคณะ ประเมินความวิตกกังวลแฝงและความวิตกกังวลขณะเผชิญสถิติที่ใช้ ได้แก่ สถิติ chisquare การแจกแจงแบบไฮเปอร์ยีออเมตริก, สถิติ non-parametricแบบ Mann - Whitney U test, non - parametric แบบ Wilcoxon,ANCOVAผลการศึกษา : การศึกษาในครั้งนี้พบว่าความวิตกกังวลขณะเผชิญ และความวิตกกังวลแฝงของกลุ่มทดลองและกลุ่มควบคุมใช้การประเมิน STAI from Y-1ในวันที่ 1 และ วันที่ 3 ก่อนได้รับการสนับสนุนด้านข้อมูล และอารมณ์ทั้งสองกลุ่ม พบว่าไม่มีความแตกต่างกันอย่างมีนัยสำคัญ หลังทำการทดลองความวิตกกังวลขณะเผชิญหลังได้รับการสนับสนุนด้านข้อมูล และอารมณ์ของผู้ดูแลผู้ป่วยเนื้องอกสมองในกลุ่มทดลองและกลุ่มควบคุมแตกต่าง อย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 ส่วนค่าคะแนนรวมของความวิตกกังวลแฝงหลังได้รับการทดลองกลุ่มทดลองและกลุ่มควบคุมไม่มีความแตกต่างกันสรุป : การให้การสนับสนุนด้านข้อมูลร่วมกับการสนับสนุนด้านอารมณ์ด้วยการสร้างสัมพันธภาพเพื่อให้เกิดความไว้วางใจ และเกิดความมั่นใจการพูดคุยใช้คำถามปลายเปิด เป็นปัจจัยหนึ่งที่ทำให้ความวิตกกังวลในกลุ่มตัวอย่างมีแนวโน้มลดลง ความวิตกกังวลที่เกิดขึ้นกับบุคคลในสถานการณ์ทั่ว ๆ ไป เกิดอย่างต่อเนื่องและเป็นระยะเวลานานจนเป็นลักษณะนิสัยประจำตัว หรือเป็นส่วนหนึ่งของบุคลิกภาพ และจะเป็นตัวเพิ่มหรือเสริมความรุนแรงความวิตกกังวลที่เกิดขึ้นในสถานการณ์ปัจจุบัน.

เหตุผลของการทำวิจัย : เพื่อเป็นการคัดกรองและสร้างการตระหนักให้แก่สถานสงเคราะห์ในการดูแลเอาใจใส่วัยรุ่น ซึ่งหากวัยรุ่นกลุ่มดังกล่าวสามารถปรับตัวได้สำเร็จจะช่วยให้พัฒนาตนเองเกิดบุคลิกภาพที่ดี ซึ่งจะเป็นพื้นฐานสำคัญของการดำเนินชีวิตต่อไปวัตถุประสงค์ : เพื่อศึกษาความชุกของภาวะซึมเศร้าและพฤติกรรมไม่พึงประสงค์ปัจจัยที่เกี่ยวข้องกับภาวะซึมเศร้าและพฤติกรรมไม่พึงประสงค์ และศึกษาความสัมพันธ์ระหว่างภาวะซึมเศร้ากับพฤติกรรมไม่พึงประสงค์ในวัยรุ่นอายุระหว่าง 10 - 15 ปี ที่มารับบริการที่สถานสงเคราะห์เด็กหญิงบ้านราชวิถีรูปแบบการวิจัย : การวิจัยเชิงพรรณนา ณ จุดเวลาใดเวลาหนึ่งสถานที่ทำการศึกษา : สถานสงเคราะห์เด็กหญิงบ้านราชวิถีตัวอย่างและวิธีการศึกษา : การวิจัยเชิงพรรณนา ณ จุดเวลาใดเวลาหนึ่ง (Cross-sectionalDescriptive Study) เก็บรวบรวมข้อมูลจากผู้รับบริการที่มีอายุระหว่าง10 - 15 ปี ที่มารับบริการที่สถานสงเคราะห์เด็กหญิงบ้านราชวิถีจำนวน119 คน โดยเก็บกลุ่มตัวอย่างจากจำนวนผู้รับบริการทั้งหมด เครื่องมือในการคัดกรองมีทั้งหมด 3 ชุด ได้แก่ 1) แบบสอบถาม ข้อมูลพื้นฐาน2) แบบประเมินภาวะซึมเศร้าด้วยตนเอง Children’s DepressionInventories 3) แบบประเมินพฤติกรรมเด็ก The Strength andDifficulties Questionnaire ฉบับผู้ปกครองผลการศึกษา : พบอัตราความชุกของภาวะซึมเศร้าในวัยรุ่นอายุระหว่าง 10 - 15 ปีร้อยละ 52.9 อัตราความชุกของพฤติกรรมไม่พึงประสงค์ ร้อยละ 12.6และพบความชุกของการเกิดทั้งภาวะซึมเศร้า และพฤติกรรมไม่พึงประสงค์ ร้อยละ 10.1 โดยระดับการศึกษาของผู้รับบริการมีความสัมพันธ์กับคะแนนรวมด้านพฤติกรรมที่เป็นปัญหา พฤติกรรมด้านอารมณ์และพฤติกรรมด้านความสัมพันธ์กับเพื่อน อย่างมีนัยสำคัญทางสถิติ ที่ระดับ p < 0.01 นอกจากนี้ยังพบว่าภาวะซึมเศร้ามีความสัมพันธ์ กับพฤติกรรมไม่พึงประสงค์ในระดับที่น้อย (p > 0.05)สรุป : ความชุกของภาวะซึมเศร้าและพฤติกรรมไม่พึงประสงค์ จากการศึกษาในครั้งนี้สูงกว่าการศึกษาอื่น ๆ ในวัยรุ่นทั่วไป การทราบปัจจัยเสี่ยงจะช่วยให้บุคคลหรือหน่วยงานที่เกี่ยวข้องสามารถวางแผนช่วยเหลือแก้ไข และป้องกันได้อย่างเหมาะสมต่อไป.

Both wild-type and mutated beta-amyloid (Aβ) peptides can elicit an immune response when delivered subcutaneously. However, only mutated forms of Aβ can sensitize dendritic cells when administered intravenously or intraperitoneally. To understand the role of mutation and delivery routes in creating immune responses, and the function of dendritic cells as therapeutic agents, we used fluorescent-conjugated WT Aβ1-40 (WT40) and artificially mutated Aβ1-40 (22W40) peptides to treat dendritic and Langerhans cells from young and/or old mice at different time points. Chronic Obstructive Pulmonary Disease (COPD) people suffer from severe physical impairments, which often elicit significant psychological distress and impact their quality of life. This meta-analysis aimed to assess evidence from the scientific literature on the effects of relaxation techniques. Methods. We investigated 9 databases to select 25 RCTs. Studies included both inpatients and outpatients with COPD. Both respiratory and psychological outcomes were considered.

HPV infection is a causal agent in many epithelial cancers, yet our understanding of genetic susceptibility to HPV infection and resultant cancer risk is limited. Epidermodysplasia Verruciformis is a rare condition of extreme susceptibility to cutaneous HPV infection primarily attributable to mutations in TMC6 and TMC8. Genetic variation in the TMC6/TMC8 region has been linked to beta-type HPV infection and squamous cell carcinoma of the skin, cervical cancer, HPV persistence and progression to cervical cancer.
