Objectives: Hip arthroscopy is an established field within orthopaedic surgery. The majority of the procedures involve repairs of the acetabular labrum and arthroscopic treatment of femoroacetabular impingement (FAI). The procedures are being performed with increasing frequency annually. Fluoroscopic guidance is recommended during these procedures, and radiation exposure to the surgeon, staff, and patient remains a valid concern. The purpose of this study is to measure radiation exposure to the surgeon during hip arthroscopy and determine if this exposure remains below recommended annual occupational radiation exposure thresholds recommended by the International Committee on Radiological Protection (IRCP). Objectives: High tibial osteotomy (HTO) has become a well-established treatment for unicompartmental osteoarthritis of the knee. Over the last 30 years, various techniques have been introduced to advance this procedure. The purpose of this study is to review the outcomes of patients who received medial opening wedge HTO over the last ten years (2002-2012) using a modern, low profile, medially based fixation device. OBJECTIVES: The aim of this study was to compare outcomes and coronary angiographic findings in post-cardiac arrest patients with and without ST-segment elevation myocardial infarction (STEMI).

BACKGROUND: The 2013 STEMI guidelines recommend performing immediate angiography in resuscitated patients whose initial electrocardiogram shows STEMI. The optimal approach for those without STEMI post-cardiac arrest is less clear.

METHODS: A retrospective evaluation of a post-cardiac arrest registry was performed.

RESULTS: The database consisted of 746 comatose post-cardiac arrest patients including 198 with STEMI (26.5%) and 548 without STEMI (73.5%). Overall survival was greater in those with STEMI compared with those without (55.1% vs. …

Background: Georgia’s public health districts first began exploring the idea of national public health accreditation in 2008 when Cobb & Douglas Public Health included accreditation in their strategic plan. In May 2015, Cobb & Douglas Public Health was the first Georgia public health district to achieve national accreditation status. This article discusses the current state of accreditation readiness in Georgia and explores strengths and barriers to accreditation.

This research study focuses on the CMS, pay-for-performance, inpatient initiative regarding hospital acquired conditions (HAC) and present on admission (POA) identification of illnesses and injuries and studies conducted upon the initiatives that have measured outcome since the implementation of the program. This study is a focused assessment of numerous studies regarding the overall outcome of the initiative thus far, which was implemented in 2007. The goal of this research is to ascertain the effectiveness of the HAC initiative and possible factors that have positively or negatively impacted the overall outcomes in regard to Catheter-Associated Urinary Tract Infections (CAUTI).

The Impact Of Ehr Usability Issues On The Perceived Efficiency Of Non-Clinical Hospital-Based Ehr Users, Natasha Ocean

A survey-based study of the impact of the EHR systems usability issues on the perceived efficiency of work among non-clinical users in the hospital settings was performed June 20th to July 24th 2015. There were 160 valid responses. The results were processed in SPSS (IBM statistical application) and I determined the usability dimensions with the greatest impact on efficiency, for all EHRs and for the major EHRs (Meditech, Epic and Cerner). Helpfulness was the most frequent problem for users of all EHRs.

Observational studies suggest that hematomas continue to enlarge during hospitalization in patients with traumatic brain injury (TBI). There is limited data regarding factors associated with hematoma enlargement and on whether hematoma enlargement contributes directly to death and disability in patients with TBI.

We analyzed data collected as part of the Resuscitation Outcomes Consortium Hypertonic Saline and TBI Study. Hematoma enlargement was ascertained and collected as a predefined safety endpoint. Introduction Although venous thromboembolism (VTE) is seen with morbidity and mortality in various surgical specialties, scarce data are available in the head and neck surgery domain. Objective We aim to determine the incidence of VTE in patients receiving surgery for head and neck cancer. Methods Four hundred thirteen patients who underwent head and neck surgery procedures between 2005 and 2013 were reviewed retrospectively. It is estimated that 25% of the alcohol consumed in China as traditional alcohol. This study explored the drinking patterns and the motives to use traditional alcohol among a sample of the Yi minority in southern China. The Theory of Planned Behavior was used to explore motives.

Several studies have linked nosocomial transmission airborne diseases to airflow in healthcare settings. Quasi-experimental methods are developed to observe the aerodynamic transport behavior of synthetic respiratory particles in corridors of a hospital. Computational models, validated by experimental results, are then developed to explore the spatial relationships of supply-exhaust air ventilation in patient corridors. The aim of this study is to determine optimal HVAC design strategies to contain and remove airborne contaminants in healthcare environments.

การปลูกถ่ายอวัยวะ เป็นอีกหนึ่งในรูปแบบการรักษาที่มีการค้นคว้าพัฒนา และประสบความสำเร็จเพิ่มขึ้นต่อเนื่องมากว่าครึ่งศตวรรษ จากความสำเร็จดังกล่าวการปลูกถ่ายอวัยวะจึงกลายเป็นหนึ่งในกุญแจสำคัญทางการรักษา โดยมีจุดมุ่งหมายเพื่อรักษาชีวิต และทำให้ผู้เข้ารับการปลูกถ่ายสามารถกลับไปใช้ชีวิตของเขาอย่างมีคุณภาพชีวิตที่ดีต่อไป ผลพวงอีกประการจากความก้าวหน้าในการปลูกถ่ายคือ ความต้องการอวัยวะเพื่อการปลูกถ่ายมีมากขึ้นจนเกิดความไม่พอเพียง ถึงแม้หน่วยงานต่าง ๆ จะพยายามแก้ไขปัญหาโดยการเปิดศูนย์รับบริจาค ก็ยังพบว่าอวัยวะที่ได้จากการบริจาคไม่เพียงพอต่อความต้องการที่เพิ่มขึ้นการปลูกถ่ายในทุก ๆ ครั้งจึงมีความสำคัญอย่างมากจากการศึกษาที่ผ่านมาพบว่าปัจจัยที่ส่งผลทางลบต่อผลการปลูกถ่ายที่สำคัญปัจจัยหนึ่งคือการเกิดภาวะซึมเศร้า ที่แม้จะมีหลายงานวิจัยที่เน้นย้ำถึงอันตรายของภาวะซึมเศร้าที่มีต่อผู้ป่วยปลูกถ่ายอวัยวะทั้งในช่วงก่อนการปลูกถ่ายและหลังการปลูกถ่าย แต่กลับพบว่ามีงานวิจัยส่วนน้อยที่กล่าวถึงวิธีการจัดการหรือการบำบัดอย่างเป็นขั้นตอนที่มุ่งเน้นเพื่อลดภาวะซึมเศร้า และจากการสืบค้นยังไม่พบการศึกษาดังกล่าวในประเทศไทย ดังนั้น เพื่อให้ผู้ป่วยมีคุณภาพชีวิตที่ดีขึ้น มีความร่วมมือในการรักษา มีอัตราการตายลดลง ทำให้การปลูกถ่ายแต่ละครั้งเกิดผลประโยชน์สูงสุด และมีความคุ้มค่ามากที่สุดการบำบัดหรือสร้างกระบวนการ เพื่อลดภาวะซึมเศร้าในผู้ป่วยปลูกถ่ายจึงมีความสำคัญ.

Background : Colonoscopy is the gold standard investigation for assessing of the intra-luminal colorectal pathology. Effective bowel preparation contributed efficient visualizing of the colon. Osmotic laxatives such as sodium phosphate (NaP) are commonly used for colonic preparation for colonoscopy. A meta-analysis suggested that NaP offered a more effective and more readily completed preparation than PEG-based regimes. However, the use of sodium phosphate solution has been shown to significantly reduce the serum potassium level which is likely to affect cardiac rhythm in particular by prolonging the QT interval. Hemoglobin H disease (Hb H) in adult patients can be diagnosed by identifying theband of Hb H with or without Hb Bart on Hb electrophoresis, and probably confirmed bygenotyping study. However, Hb H may be suppressed by some acquired factors includingiron deficiency anemia. Herein, we report a case of a 55-year-old woman was recognized ashaving chronic anemia since childhood. She was treated with occasional transfusions, oneor two units a year. เหตุผลของการทำวิจัย : ในปัจจุบันการผ่าตัดกระจกทั้งสองข้างในวันเดียวกันยังไม่ได้เป็นมาตรฐานในการรักษาผู้ป่วยที่เป็นโรคต้อกระจกของตาทั้งสองข้างเนื่องด้วยแพทย์ยังมีความกังวลถึงภาวะแทรกซ้อนที่อาจเกิดขึ้นโดยเฉพาะภาวะติดเชื้อในลูกตาทั้งสองข้าง ปัจจุบันยังไม่มีงานวิจัยที่สามารถให้ข้อสรุปที่ชัดเจนวัตถุประสงค์ : เพื่อศึกษาเปรียบเทียบระหว่างการผ่าตัดต้อกระจกทั้งสองข้างในวันเดียวกัน กับการผ่าตัดต้อกระจกแยกวันในตาแต่ละข้าง ในด้านการเกิดภาวะแทรกซ้อนและประสิทธิภาพของการผ่าตัดรูปแบบการวิจัย : การทบทวนวรรณกรรมอย่างเป็นระบบและการวิเคราะห์อภิมานสถานที่ทำการวิจัย : โรงพยาบาลจุฬาลงกรณ์ตัวอย่างและวิธีการศึกษา : คณะผู้จัดทำคัดเลือกวรรณกรรมทั้งประเภท Randomized controlledtrial และ Non-randomized controlled trial รวมถึง Observationalstudy ที่เกี่ยวข้อง หลังจากนั้นจึงให้ผู้ทบทวนวรรณกรรม 2 คนแยกกันพิจารณาวรรณกรรมที่คัดเลือกได้ โดยมีผลลัพธ์ของการศึกษาคือผลข้างเคียงทั้งระหว่างผ่าตัดและหลังผ่าตัด และความสามารถในการเห็นก่อนและหลังผ่า ผู้จัดทำใช้ l² ในการประเมิน heterogeneityและคำนวณ Odd ratio ร่วมกับ 95% interval สำหรับข้อมูลทวิภาคผลการศึกษา : จากการทบทวนวรรณกรรมอย่างเป็นระบบค้นพบวรรณกรรมทั้งสิ้น8 วรรณกรรม ได้แก่ Randomized controlled trial 3 วรรณกรรม Nonrandomizedcontrolled trial 2 วรรณกรรม และ Case series 3วรรณกรรม ประกอบด้วยกลุ่มตัวอย่าง 2161 คนในกลุ่ม ISBP และ933 คนในกลุ่ม DSBP โดยภาวะแทรกซ้อนระหว่างการผ่าตัดที่พบคือเยื่อหุ้มเลนส์ด้านหลังฉีกขาด ส่วนภาวะแทรกซ้อนหลังการผ่าตัด ได้แก่โรคม่านตาอักเสบ ความดันลูกตาสูง และจุดรับภาพตรงกลางบวมโดยไม่พบอุบัติการณ์ของภาวะติดเชื้อในลูกตาเลยในแต่ละวรรณกรรมส่วนด้านความสามารถในการเห็นแต่ละงานวิจัยมีความแตกต่างในเรื่องมาตรวัดและค่ากลางทางสถิติ จึงไม่สามารถนำผลงานวิจัยมาเปรียบเทียบระหว่างกันได้ อย่างไรก็ตามไม่พบความแตกต่างระหว่างสองกลุ่มสรุป : ไม่พบความแตกต่างกันอย่างมีนัยสำคัญทางสถิติระหว่างการผ่าตัดทั้งสองวิธีทั้งด้านภาวะแทรกซ้อนและความสามารถในการเห็น.

เหตุผลของการทำวิจัย : การเรียนพิเศษเพื่อเตรียมสอบเข้ามหาวิทยาลัยส่งผลให้นักเรียนมัธยมศึกษาตอนปลายเกิดความเครียด ขึ้น ซึ่งเป็นสถานการณ์ที่สังคมไทยปัจจุบันเผชิญอยู่ปัจจัยด้านต่าง ๆ เช่น เพศ ความคาดหวังต่อตนเองความคาดหวังของผู้ปกครอง ระยะเวลาการเรียนพิเศษ แผนการเรียน ฯลฯปัจจัยต่าง ๆ เหล่านี้อาจส่งผลต่อความเครียดของนักเรียนได้วัตถุประสงค์ : เพื่อศึกษาระดับความเครียดและความสัมพันธ์ระหว่างปัจจัยที่ก่อให้เกิดความเครียดของนักเรียนมัธยมศึกษาตอนปลายในกรุงเทพมหานครที่เรียนพิเศษเพื่อเตรียมสอบเข้ามหาวิทยาลัยรูปแบบการวิจัย : การศึกษานี้เป็นวิจัยเชิงพรรณนาโดยศึกษา ณ จุดเวลาใดเวลาหนึ่งสถานที่ทำการศึกษา : อาคารวรรณสรณ์ ซึ่งเป็นอาคารแหล่งรวมสถาบันกวดวิชาที่ใหญ่ที่สุดตั้งอยู่บริเวณแยกถนนพญาไท กรุงเทพมหานครตัวอย่างและวิธีการ : เป็นการศึกษาในนักเรียนระดับมัธยมศึกษาตอนปลายทั้งเพศหญิงและชาย อายุ16 - 18 ปี จำนวน 384 คน ในภาคการเรียนที่ 1 ปีการศึกษา2557 โดยใช้แบบสอบถามส่วนบุคคล จำนวน 10 ข้อ และแบบประเมินความเครียดสวนปรุง (SPST-20) วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาในการหาข้อมูลทั่วไปของกลุ่มตัวอย่าง แสดงค่าสถิติเป็นค่าเฉลี่ยเลขคณิตความถี่ ร้อยละและใช้ค่า Mean ± SD และใช้สถิติเชิงอนุมานในการหาข้อมูลสถิติเชิงลักษณะในการเปรียบเทียบปัจจัยที่อาจจะมีผลต่อความเครียดของนักเรียนมัธยมศึกษาตอนปลายที่เรียนพิเศษ โดยใช้สถิติIndependent sample t-test, One-way ANOVA, multiple linearregression, regression analysis and Pearson correlationผลการศึกษา : พบว่านักเรียนมัธยมศึกษาตอนปลายในกรุงเทพมหานคร ที่เรียนพิเศษเพื่อเตรียมสอบเข้ามหาวิทยาลัย ส่วนใหญ่มีความเครียดในระดับสูง(ร้อยละ 48.2) มีความคาดหวังต่อตนเอง (ร้อยละ 70.3) สำหรับความคาดหวังของผู้ปกครองต่อนักเรียน (ร้อยละ 68.5) อยู่ในระดับปานกลางสรุป : นักเรียนมัธยมศึกษาตอนปลายที่เรียนกวดวิชาเพื่อเข้าศึกษาต่อในระดับอุดมศึกษา ในเขตกรุงเทพมหานคร มีความเครียดอยู่ในระดับสูงนักเรียนมีความคาดหวังต่อตนเองและผู้ปกครองมีความคาดหวังต่อตัวนักเรียนอยู่ในระดับปานกลาง.

เหตุผลของการวิจัย : โรคหลอดเลือดสมองเป็นสาเหตุสำคัญที่ส่งผลให้ผู้ป่วยเสียชีวิต และเกิดความพิการระยะยาวซึ่งส่งผลกระทบไปยังผู้ดูแลผู้ป่วยทั้งทางด้านร่างกาย จิตใจ และสังคมของผู้ดูแลผู้ป่วยทำให้เกิดภาวะซึมเศร้าตามมาแต่อย่างไรก็ตามงานวิจัยในประเทศไทยมีการศึกษาในประเด็นนี้เป็นจำนวนน้อย และเป็นระยะเวลานานแล้วที่มีการศึกษา ผู้วิจัยจึงสนใจศึกษาในประเด็นดังกล่าววัตถุประสงค์ : เพื่อศึกษาความชุกของภาวะซึมเศร้าและปัจจัยที่เกี่ยวข้องกับภาวะซึมเศร้าในผู้ดูแลผู้ป่วยโรคหลอดเลือดสมองที่พาผู้ป่วยมารับการรักษาณ โรงพยาบาลจุฬาลงกรณ์รูปแบบการวิจัย : การวิจัยเชิงพรรณนา ศึกษา ณ ช่วงเวลาใดเวลาหนึ่งสถานที่ทำการศึกษา : แผนกอายุรกรรมประสาท และแผนกเวชศาสตร์ฟื้นฟู โรงพยาบาลจุฬาลงกรณ์ตัวอย่างและวิธีการศึกษา : ศึกษาในผู้ดูแลผู้ป่วยโรคหลอดเลือดสมองจำนวน 96 คน โดยแบบสอบถามที่ใช้ในการเก็บข้อมูล ได้แก่ แบบสอบถามข้อมูลส่วนบุคคล,แบบประเมินดัชนีบาร์เทลอินเด็กซ์, แบบทดสอบความรู้สึกเป็นภาระ,แบบสอบถามแรงสนับสนุนทางสังคม และแบบสอบถามภาวะซึมเศร้า(BDI-IA) ใช้สถิติเชิงพรรณนาวิเคราะห์ค่าร้อยละ, ค่าเฉลี่ย, ส่วนเบี่ยงเบนมาตรฐาน เพื่อหาความชุกของภาวะซึมเศร้าสถิติไคสแควร์ เพื่อหาปัจจัยที่สัมพันธ์กับภาวะซึมเศร้า และสถิติถดถอยพหุคูณลอจิสติกเพื่อหาปัจจัยทำนายภาวะซึมเศร้าผลการศึกษา : จากการวิเคราะห์ข้อมูล พบว่าผู้ดูแลผู้ป่วยโรคหลอดเลือดสมองมีภาวะซึมเศร้า ร้อยละ 44.8 ปัจจัยที่มีความสัมพันธ์กับภาวะซึมเศร้าในผู้ดูแลผู้ป่วยโรคหลอดเลือดสมองอย่างมีนัยสำคัญ ทางสถิติในด้านผู้ดูแล ได้แก่ ความไม่เพียงพอของรายได้ การนอนหลับที่น้อยกว่า5 ชั่วโมง ความหนักใจในการดูแลอาการของผู้ป่วยโดยเฉพาะอย่างยิ่งปัญหาด้านพฤติกรรม และการสื่อสารของผู้ป่วย แรงสนับสนุนทางสังคมต่ำ ความรู้สึกเป็นภาระมาก และปัจจัยด้านผู้ป่วย ได้แก่ การไม่สามารถช่วยเหลือตนเองในการทำกิจวัตรประจำวันของผู้ป่วย ปัจจัยทสามารถใช้พยากรณ์ภาวะซึมเศร้าได้อย่างมีนัยสำคัญทางสถิติ ได้แก่ความไม่เพียงพอของรายได้ (OR = 4.20 95% CI = 1.31-13.39)การนอนหลับที่น้อยกว่า 5 ชั่วโมง (OR = 5.03 95% CI = 1.51-16.80)และความรู้สึกเป็นภาระมาก (OR = 9.05 95% CI = 2.68 - 30.55)ซึ่งการมีปัจจัยเสี่ยงดังกล่าวเพิ่มอัตราการเกิดภาวะซึมเศร้าในผู้ดูแลผู้ป่วยโรคหลอดเลือดสมองสรุปผลการวิจัย : แพทย์ หรือผู้ที่เกี่ยวข้องในการดูแลรักษาผู้ป่วยโรคหลอดเลือดสมองควรมีการประเมิน หรือคัดกรองภาวะซึมเศร้าในผู้ดูแลผู้ป่วยร่วมด้วยเพื่อลดภาวะซึมเศร้าในผู้ดูแลผู้ป่วย และยังส่งผลต่อคุณภาพการดูแลผู้ป่วยโรคหลอดเลือดสมองได้.

Abstract

This case report illustrates successful Video Assisted Thoracic Surgery (VATS) performed on a 45-year-old woman and 52-year-old man presenting with a mass in left and right paravertebral space on the CT scan respectively. VATS has many benefits over traditional open operation (thoracotomy), resulting in less pain and shorten recovery time. However, VATS has higher equipment cost but when an experienced surgeon performs the surgery, better outcomes are achieved. VATS is not common in Pakistan's surgical setup as it is an expensive method of eradicating mediastinal pathologies and not every patient undergoes VATS. Despite the longstanding knowledge that blood flow increases in proportion to metabolic activity of skeletal muscle, the underlying mechanisms that govern this response have only recently been identified.1 Given the role of endothelial cells in mediating exercise hyperemia,2 interest has been focused on endothelium-derived vasodilation occurring via the synthesis of nitric oxide (NO) and vasodilating prostaglandins (PGs; i.e. prostacyclin) or endothelium- derived hyperpolarization. A number of studies performed in humans have established a minimal-to-modest role for NO and PGs during mild- and moderate- intensity exercise. Transcription factors that mediate neuronal defenses against diverse stresses were quantified in plasma neural-derived exosomes of Alzheimer's disease or frontotemporal dementia patients and matched controls. Exosomal levels of low-density lipoprotein receptor-related protein 6, heat-shock factor-1, and repressor element 1-silencing transcription factor all were significantly lower in Alzheimer's disease patients than controls (P < 0.0001). In frontotemporal dementia, the only significant difference was higher levels of repressor element 1-silencing transcription factor than in controls. Exosomal transcription factors were diminished 2-10 years before clinical diagnosis of Alzheimer's disease. Low exosomal levels of survival proteins may explain decreased neuronal resistance to Alzheimer's disease neurotoxic proteins.

PURPOSE. An alternative cancer therapy based on RNA interference (RNAi) has shown considerable promise but the possibility of resistance development is not known. This study explored the possibility of therapeutic resistance against siRNA nanoparticles in human cancer cells. METHODS. Two approaches to siRNA treatment were undertaken using lipid-modified polyethylenimines, a single high concentration (shock) and repeated increasing concentrations (gradual). The targets were Mcl-1, RPS6KA5 and KSP in MDA-MB-435 cells. RESULTS.
