ผลของเรซินโคตทิ้งต่อกําลังแรงยึดดึงระหว่างเรซินซีเมนต์และเนื้อฟัน, กฤษณะ บัญญัติศรีสกุล, นิยม ธํารงค์อนันต์สกุล

วัตถุประสงค์ เพื่อศึกษาผลของเรซินโคดทิ้งต่อกําลังแรงยึดถึงระหว่างเรซินซีเมนต์และเนื้อฟัน วัสดุและวิธีการ ฟันกรามมนุษย์จํานวน 40 ซี่ ถูกตัดด้านบดเคี้ยวในแนวระนาบเพื่อเผยให้เห็นเนื้อฟันด้วย เครื่องตัดฟันความเร็วต่ํา จากนั้นขัดผิวเนื้อฟันด้วยกระดาษซิลิกอนคาร์ไบด์ความละเอียด 600 กริท แบ่งฟันเป็น 4 กลุ่ม ได้แก่ 1) เนื้อฟันที่ไม่เคลือบด้วยเรซินโคดทิ้งแล้วยึดด้วยพานาเวียเอฟทู 2) เนื้อฟันที่เคลือบด้วยเรซินโคดทิ้ง แล้วยึดด้วยพานาเวียเอฟทู 3) เนื้อฟันที่ไม่เคลือบด้วยเรซินโคดทิ้งแล้วยึดด้วยซุปเปอร์บอนด์ซีแอนด์บี และ 4) เนื้อฟันที่เคลือบด้วยเรซินโคดทิ้งแล้วยึดด้วยซุปเปอร์บอนด์ซีแอนด์บี เก็บชิ้นงานทั้งหมดไว้ภายใต้ความชื้น สัมพัทธ์ร้อยละ 100 ที่อุณหภูมิ 37 องศาเซลเซียสเป็นเวลา 24 ชั่วโมง นําชิ้นงานมาเตรียมเป็นรูปมินิดัมเบลล์ (10 ชิ้นตัวอย่างต่อกลุ่มการทดสอบ) และทดสอบค่ากําลังแรงยึดดึงด้วยเครื่องทดสอบแรงแบบสากล ด้วยความเร็ว หัวจับ 0.5 มิลลิเมตรต่อนาที นําข้อมูลที่ได้มาวิเคราะห์ความแปรปรวนแบบทางเดียว ผลการศึกษา ค่ากําลังแรงยึดดึงของกลุ่มที่ 1 และ 2 มีค่าเท่ากับ 9.59 + 3.65 และ 5.54 + 2.07 เมกะปาสคาล ตามลําดับส่วนกลุ่ม 3 และ 4 มีค่าเท่ากับ 17.03 + 2.93 และ 8.81 + 3.85 เมกะปาสคาล ตามลําดับ วิเคราะห์ ความแตกต่างของค่าเฉลี่ยโดยใช้สถิติการวิเคราะห์ความแปรปรวนแบบทางเดียวพบว่าค่ากําลังแรงยึดดึงของกลุ่มที่ 3 มีค่าสูงที่สุดและแตกต่างจากกลุ่มที่เหลืออีก 3 กลุ่มอย่างมีนัยสําคัญทางสถิติ (p < 0.05) และค่ากําลังแรงยึดดึง ของกลุ่มที่ 1 และ 2 แตกต่างกันอย่างมีนัยสําคัญทางสถิติ (p < 0.05) ส่วนค่ากําลังแรงยึดถึงระหว่างกลุ่มที่ 1 กับ กลุ่มที่ 4 และค่ากําลังแรงยึดถึงระหว่างกลุ่มที่ 2 กับกลุ่มที่ 4 แตกต่างกันอย่างไม่มีนัยสําคัญทางสถิติ (p > 0.05) ความล้มเหลวของชิ้นทดสอบ พบว่าร้อยละ 70 ของกลุ่มที่ 1 และ กลุ่มที่ 3 เกิดระหว่างชั้นยึดโดยมีเรซินซีเมนต์ หลงเหลือบนเนื้อฟัน ขณะที่ร้อยละ 60 ของกลุ่มที่ 2 และร้อยละ 70 ของกลุ่มที่ 4 เกิดระหว่างชั้นเรซินโคดทิ้งกับ เรซินซีเมนต์โดยมีเรซินซีเมนต์เหลือบนชั้นเรซินโคดทิง สรุป ค่ากําลังแรงยึดดึงของเนื้อฟันกับเรซินซีเมนต์ซุปเปอร์บอนด์ซีแอนด์บี …

Laser Doppler Flowmetry (LDF) is an accurate and reliable method for assessing microcirculatory function. Several studies have shown that, in comparison with other methods, the measurement of blood flow, especially of blood flow in bones, teeth and surrounding tissues, is reproducible and correlates with clinical features. วัตถุประสงค์ เพื่อเปรียบเทียบผลการรั่วซึมระดับจุลภาคของสารยึดติดระหว่างผิวฟันที่ไม่มีการปนเปื้อนน้ําลายกับผิวฟันที่มีการกําจัดการปนเปื้อนนั้นด้วยสารไพรเมอร์ของระบบสารยึดติดที่นิยมใช้กันในปัจจุบัน วัสดุและวิธีการ เตรียมโพรงฟันคลาสไฟว์บนผิวฟันกรามน้อยด้านใกล้แก้มจํานวน 108 ซี่ แบ่งเป็น 9 กลุ่ม ได้แก่ กลุ่มที่ไม่มีการปนเปื้อนของออปติบอนด์ เอฟแอล ออปติบอนด์ เอ็กซ์ทีอาร์ ออปติบอนด์ ออลอินวัน (กลุ่ม FC XC AC ตามลําดับ) กลุ่มที่มีการปนเปื้อนของน้ําลายและกําจัดด้วยสารไพรเมอร์ก่อนการฉายแสงของสาร ยึดติดทั้ง 3 ชนิด (กลุ่ม FB XB AB ตามลําดับ) และกลุ่มที่มีการปนเปื้อนของน้ําลายและกําจัดหลังการฉายแสง ของสารยึดติดทั้ง 3 ชนิด (กลุ่ม FA XA AA ตามลําดับ) บูรณะด้วยเรซินคอมโพสิต (พรีมิส สีเอ 3 บอดี้) ขัดด้วยแผ่นซอฟเฟลกซ์ จําลองการใช้งานด้วยการเปลี่ยนแปลงอุณหภูมิ แล้วแช่ในสารละลายเมทิลีนบลู ตัดฟัน ตามแนวแกนฟัน และประเมินการรั่วซึมโดยใช้กล้องจุลทรรศน์ด้วยคะแนน 5 ช่วง วิเคราะห์ผลด้วยสถิติคริสคัลวาลิส และแมนวิทนี่ ที่ระดับความเชื่อมั่นร้อยละ 95 ผลการศึกษา การรั่วซึมระดับจุลภาคบริเวณเคลือบฟันและเนื้อฟันของกลุ่ม FB และ FA มีค่ามัธยฐานมากกว่า ของกลุ่ม FC อย่างมีนัยสําคัญทางสถิติ (p < 0.05) ส่วนค่ามัธยฐานการรั่วซึมระดับจุลภาคของกลุ่ม XB และ XA นั้นพบว่าเฉพาะบริเวณเคลือบฟันเท่านั้นที่มีค่ามากกว่ากลุ่ม XC อย่างมีนัยสําคัญทางสถิติ (p < 0.05) ในขณะที่ ไม่พบความแตกต่างกันอย่างมีนัยสําคัญทางสถิติเมื่อเปรียบเทียบค่ามัธยฐานการรั่วซึมของกลุ่ม AC AB AA ทั้งส่วนเคลือบฟันและเนื้อฟัน สรุป ในการศึกษาครั้งนี้ การปนเปื้อนน้ําลายในขั้นตอนการเตรียมผิวฟันด้วยออปติบอนด์ เอฟแอลไม่สามารถ แก้ไขได้ด้วยสารไพรเมอร์ของสารยึดติดนี้ ส่วนออปติบอนด์ เอ็กซ์ทีอาร์สามารถแก้ไขการปนเปื้อนน้ําลายได้ในส่วนของเนื้อฟันเท่านั้น สําหรับออปติบอนด์ ออลอินวันนั้นพบว่าไม่ว่าการปนเปื้อนน้ําลายจะเกิดขึ้นที่เคลือบฟัน หรือเนื้อฟัน เมื่อแก้ไขแล้วการรั่วซึมระดับจุลภาคนั้นไม่แตกต่างจากกลุ่มควบคุม (ว ทันต จุฬาฯ 2557;37:1-14)

การประเมินการรั่วซึมระดับจุลภาคของคอมโพสิต ชนิดไหลแผ่ได้ที่ยึดได้ด้วยตัวเองในโพรงฟันคลาสไฟว์ : ผลของเทอร์โมไซคลิงและสารบอนด์ดึง, กอบกฤษณ์ หทัยอารีย์รักษ์, สุชิต พูลทอง, เอกมน มหาโภคา

วัตถุประสงค์ เพื่อเปรียบเทียบการรั่วซึมระดับจุลภาคที่ขอบเคลือบฟันและเนื้อฟันก่อนและหลังทําเทอร์โมไซคลิงของคอมโพสิตชนิดไหลแผ่ได้ที่ยึดได้ด้วยตัวเองกับการใช้เรซินคอมโพสิตร่วมกับสารบอนด์ดึง วัสดุและวิธีการ ฟันกรามน้อยที่ถูกถอนจํานวน 96 ที่ผ่านการเตรียมโพรงฟันคลาสไฟว์ด้านใกล้แก้ม (1 - 96) ถูกแบ่งเป็น 4 กลุ่ม (n = 24) : 1. เวอร์ทิสโฟลว์ 2. เวอร์ทิสโฟลว์ + ออปติบอนด์ออลอินวัน 3. พรีมิสโฟลว์ + ออปติบอนด์ออลอินวัน 4. พรีมิส + ออปติบอนด์ออลอินวัน โดยฟันที่ผ่านการบูรณะแล้วจะแบ่งเป็น 2 กลุ่มย่อย (n = 12) คือกลุ่มที่ไม่ทําเทอร์โมไซคลิงและกลุ่มที่ทําเทอร์โมไซคลิง (1,000 รอบ) จากนั้นนําไปแช่ในเมทิลลีนบลู เพื่อประเมินระดับการรั่วซึมของสี แล้วทําการวิเคราะห์ข้อมูลทางสถิติด้วยครัสคาลวัลลิสและแมนวิทนียู (p<0.05) ผลการศึกษา ไม่มีความแตกต่างอย่างมีนัยสําคัญของการรั่วซึมที่ขอบเคลือบฟันของวัสดุทั้ง 4 กลุ่มทั้งก่อนและ หลังการทําเทอร์โมไซคลิง (p-0.067 และ p-0.397 ตามลําดับ) ขณะที่เวอร์ทิสโฟลว์มีการรั่วซึมที่ชอบเนื้อฟันสูง กว่าวัสดุกลุ่มอื่นอย่างมีนัยสําคัญทั้งก่อนและหลังเทอร์โมไซคลิง (p < 0.000 และ p = 0.001 ตามลําดับ) โดยการ ทําเทอร์โมไซคลึงไม่มีผลต่อการรั่วซึมระดับจุลภาคอย่างมีนัยสําคัญ สรุป คอมโพสิตชนิดไหลแผ่ได้ที่ยึดได้ด้วยตัวเองมีการรั่วซึมระดับจุลภาคที่ขอบเคลือบฟันไม่แตกต่างจากการใช้เรซินคอมโพสิตร่วมกับสารบอนด์ติง แต่มีการรั่วซึมระดับจุลภาคที่ขอบเนื้อฟันสูงกว่าอย่างมีนัยสําคัญซึ่งการใช้สารบอนด์ดึงร่วมกับคอมโพสิตชนิดไหลแผ่ได้ที่ยึดได้ด้วยตัวเองอาจเป็นทางเลือกเพื่อช่วยลดการรั่วซึมระดับ จุลภาคที่เนื้อฟันได้ โดยการทําเทอร์โมไซคลิงไม่มีผลต่อการการรั่วซึมระดับจุลภาค (ว ทันต จุฬาฯ 2557;37:15-24)

ประสิทธิภาพของแอลกอฮอล์ในการกําจัดเชื้อแบคทีเรียบนหลอดยาชาทางทันตกรรม, รัชนี ปานจินดา, อรนาฏ มาตังคสมบัติ, รัชนี อัมพรอร่ามเวทย์, เกศกัญญา สัพพะเลข

วัตถุประสงค์ เพื่อทําการสํารวจเบื้องต้นถึงความชุกของการปนเปื้อนเชื้อแบคทีเรียบนหลอดยาชา และทดสอบประสิทธิภาพของแอลกอฮอล์ในการกําจัดเชื้อแบคทีเรียที่ปนเปื้อนบนหลอดยาชา วัสดุและวิธีการ เพาะเชื้อแบคทีเรียจากตัวอย่างหลอดยาชาที่เก็บจากคลินิกทันตกรรมในกรุงเทพมหานคร 25 แห่ง ตรวจสอบเชื้อที่พบด้วยการย้อมสีแกรม และทดสอบการกําจัดเชื้อแบคทีเรีย 3 ชนิด คือ สแตฟฟีโลค็อกคัส ออเรียส ซูโดโมแนส แอรูจิโนซา และบาซิลลัส ซับทิลิส ด้วยการแช่หลอดยาชาที่ปนเปื้อนเชื้อในแอลกอฮอล์ ร้อยละ 70 นาน 15 และ 10 นาที แล้วนําไปเพาะเชื้อ และทําการตรวจสอบการรั่วซึมของแอลกอฮอล์เข้าไปใน หลอดยาชาหลังจากแช่ในแอลกอฮอล์ร้อยละ 70 ผสมสีคริสตัลไวโอเลตร้อยละ 5 เป็นเวลา 10 นาที ผลการศึกษา พบเชื้อแบคทีเรียจากตัวอย่างหลอดยาชาร้อยละ 32 ของคลินิกที่ศึกษา เป็นเชื้อแกรมบวกทรงแท่ง แกรมบวกทรงกลม ทั้งแกรมบวกทรงกลมและแกรมลบทรงแท่ง ร้อยละ 50 41.7 และ 8.3 ตามลําดับ การแช่ แอลกอฮอล์ร้อยละ 70 นาน 10 นาที สามารถกําจัดเชื้อสแตฟฟีโลค็อกคัส ออเรียส และซูโดโมแนส แอรูจิโนซาได้ แต่ไม่สามารถกําจัดเชื้อบาซิลลัส ซับทิลิสได้ และไม่พบว่ามีการรั่วซึมของแอลกอฮอล์เข้าไปในหลอดยาชาอย่างมีนัยสําคัญ สรุป พบการปนเปื้อนเชื้อแบคทีเรียบนผิวหลอดยาชาได้ทั่วไปในคลินิกทันตกรรม การแช่หลอดยาชาใน แอลกอฮอล์ร้อยละ 70 เป็นเวลา 10 นาที สามารถกําจัดเชื้อสแตฟฟีโลค็อกคัส ออเรียส และซูโดโมแนส แอรูจิโนซา บนหลอดยาชาได้ โดยไม่พบแอลกอฮอล์รั่วซึมเข้าไปในหลอดยาชา (ว ทันต จุฬาฯ 2557;37:59-68)

บทความปริทัศน์ฉบับนี้มีวัตถุประสงค์เพื่อเสนอเกี่ยวกับรอยโรคในกลุ่มอาการแมคคูนอัลไบรท์ที่พบน้อยมาก ซึ่งกลุ่มอาการดังกล่าวมีสาเหตุจากการกลายพันธุ์ของเซลล์ร่างกายบนตําแหน่งของยีนจีเอนเอเอส 1 ทําให้ไม่สามารถยับยั้งการกระตุ้นของหน่วยย่อยแอลฟาของสายจีโปรตีนในการสร้างไซคลิก อะดีโนซีน โมโนฟอสเฟต ผลทําให้ปริมาณไซคลิก อะดีโนซีน โมโนฟอสเฟต ในเซลล์เพิ่มสูงขึ้น ส่งผลให้เกิดความผิดปกติของร่างกาย หลายระบบ ที่พบบ่อยและเป็นอาการแสดงสําคัญ ได้แก่ ไฟบรัสดิสเพลเซีย ปานสีน้ําตาล และการทําหน้าที่เกิน ต่อมไร้ท่อ ซึ่งไฟบรัสดิสเพลเซียสามารถส่งผลต่อกระดูกใบหน้า กะโหลกศรีษะและเนื้อเยื่อฟัน ทําให้เกิดใบหน้า ผิดรูป ความผิดปกติโครงสร้างเนื้อฟัน และพัฒนาการของฟัน ทันตแพทย์จึงสามารถตรวจพบอาการแสดงที่ ผิดปกติของรอยโรคไฟบรัสดิสเพลเซียและนําไปสู่การวินิจฉัยกลุ่มอาการนี้ได้ ทั้งนี้ต้องอาศัยประวัติ ลักษณะ ทางคลินิกทั้งภายนอกและภายในช่องปาก เช่น ใบหน้าไม่สมมาตร รวมทั้งภาพรังสีของรอยโรคบริเวณกระดูกที่ พบลักษณะเฉพาะเป็นกระจกฝ้า โดยหากสงสัยว่าผู้ป่วยลักษณะที่เข้าเกณฑ์การวินิจฉัยอาจต้องส่งปรึกษาแพทย์ และร่วมกันให้การรักษาผู้ป่วยแบบองค์รวม การรักษาไม่มีเฉพาะเจาะจง ส่วนมากเป็นการรักษาตามอาการที่ตรวจพบในผู้ป่วยแต่ละราย (ว ทันต จุฬาฯ 2557;37:113-22)

การประเมินการเกิดฟันผุก่อนปรากฏลักษณะทางคลินิก หรือก่อนฟันผุเดิมเปลี่ยนแปลงขนาดและความรุนแรงนั้นเป็นมาตรการหนึ่งที่จะช่วยให้บุคลากรทางทันตสาธารณสุขสามารถจําแนกผู้ป่วยที่มีความเสี่ยงต่อการเกิดโรคฟันผุที่แตกต่างกันซึ่งต้องการการเฝ้าระวัง และการวางแผนติดตามการเกิดโรคที่แตกต่างกัน และสามารถจัดสรรทรัพยากรที่มีอยู่อย่างจํากัดได้อย่างเหมาะสม วัตถุประสงค์ของการทบทวนวรรณกรรมนี้เพื่อชี้ให้เห็นความสําคัญและประโยชน์ของการประเมินความเสี่ยงต่อการเกิดโรคฟันผุและการพิจารณาเลือกปัจจัยด้านต่าง ๆ เพื่อทํานายการเกิดฟันผุในอนาคตของเด็กได้ โรคฟันผุเกิดจากหลายปัจจัย ประกอบด้วย ปัจจัยทั่วไป ได้แก่ สถานภาพทางเศรษฐกิจและสังคม โรคประจําตัวและยาที่มีผลต่อสุขภาพช่องปาก และปัจจัยเฉพาะ ได้แก่ ตัวฟัน สิ่งแวดล้อมในช่องปาก พฤติกรรม และเชื้อแบคทีเรีย การพิจารณาเลือกรูปแบบหรือแบบจําลองในการทํานายความเสี่ยงต่อการเกิดโรคฟันผุควร พิจารณาถึง กลุ่มอายุ ความชุกของการเกิดโรค ลักษณะของการผุ และงบประมาณที่มี ซึ่งปัจจัยฟันผุในอดีต ปัจจัยอนามัยช่องปาก และพฤติกรรมการแปรงฟันจะเป็นตัวทํานายความเสี่ยงของการเกิดฟันผุในอนาคตได้ดีที่สุดส่วนการใช้ชุดทดสอบเชื้อจะเหมาะในการทํานายฟันที่ผุเริ่มแรกในเด็ก แต่จะไม่เหมาะในการทํานายฟันผุที่ลุกลาม ไปแล้วซึ่งควรใช้ประกอบการพิจารณาร่วมกับปัจจัยอื่น ๆ (ว ทันต จุฬาฯ 2557;37:97-112)

วัตถุประสงค์ เพื่อเปรียบเทียบความรู้และทัศนคติต่อการใช้และการดื้อยาปฏิชีวนะในทางทันตกรรมของอาจารย์ทันตแพทย์และนิสิตทันตแพทย์ชั้นปีที่ 4 วัสดุและวิธีการ โดยให้อาจารย์ทันตแพทย์และนิสิตทันตแพทย์ชั้นปีที่ 4 ตอบแบบสอบถามเกี่ยวกับความรู้และทัศนคติที่มีต่อการใช้ยาปฏิชีวนะเพื่อรักษาการติดเชื้อในช่องปากและการดื้อยาโดยที่แบบสอบถามไม่มีการระบุข้อมูลของผู้ตอบและนําข้อมูลมาวิเคราะห์โดยใช้สถิติเชิงพรรณนาและสถิติเชิงวิเคราะห์ ผลการศึกษา จํานวนแบบสอบถามที่นํามาวิเคราะห์ข้อมูลมี 117 ชุด จากจํานวนที่แจกทั้งหมด 200 ชุด (ร้อยละ 59) พบว่าร้อยละ 70 ของผู้ตอบแบบสอบถามทั้งสองกลุ่มคิดว่าปัญหาการดื้อยาเป็นปัญหาที่สําคัญของประเทศและ ร้อยละ 74 ของผู้ตอบแบบสอบถามทั้งหมดมีความเห็นว่าการใช้ยาปฏิชีวนะอย่างแพร่หลายและการใช้ยาอย่างไม่ เหมาะสมเป็นสาเหตุสําคัญที่ทําให้เกิดการดื้อยา และร้อยละ 93 ของผู้ตอบแบบสอบถามทั้งหมดมีความเห็น ว่าการกําหนดแนวทางในการสั่งจ่ายยาจะเป็นวิธีที่มีประสิทธิภาพมากที่สุดในการลดปัญหาการดื้อยา สําหรับการใช้ยาปฏิชีวนะเพื่อการป้องกันการติดเชื้อของเยื่อหุ้มหัวใจ พบว่าทั้งกลุ่มอาจารย์ทันตแพทย์และกลุ่มนิสิต ทันตแพทย์มีความเห็นไม่ต่างกัน (ร้อยละ 86 และร้อยละ 98 ตามลําดับ) แต่สําหรับในกรณีของการป้องกันการ ติดเชื้อในผู้ป่วยที่ได้รับการฉายแสงที่ศีรษะและลําคอ ทั้งสองกลุ่มมีความเห็นต่างกันอย่างมีนัยสําคัญทางสถิติ (p < 0.05) คือร้อยละ 36 ของกลุ่มอาจารย์และร้อยละ 60 ของกลุ่มนิสิตที่สั่งจ่ายยาปฏิชีวนะ ทั้งนี้โดยรวมพบว่า ผู้ตอบแบบสอบถามทั้งหมดมีความรู้ในการสั่งจ่ายยาปฏิชีวนะในระดับที่เหมาะสม แต่ก็ยังคงมีแนวโน้มของการสั่งจ่ายยาที่มากเกินในบางกรณี ซึ่งอาจเป็นสาเหตุของการเกิดผลข้างเคียงจากการใช้ยาและเกิดการดื้อยาได้เช่นกัน สรุป ผู้ตอบแบบสอบถามส่วนใหญ่มีความเห็นว่าปัญหาการดื้อยาปฏิชีวนะเป็นปัญหาที่สําคัญในระดับประเทศแต่มีความคิดเห็นที่หลากหลายเกี่ยวกับปัญหาการดื้อยารวมทั้งสาเหตุและวิธีการแก้ไข การจัดให้มีแนวทางการสั่ง จ่ายยาและจัดให้มีการสอนในระดับหลังปริญญาอาจเป็นวิธีที่จะช่วยลดปัญหาการดื้อยา (ว ทันต จุฬาฯ 2557;37:83-96)

วัตถุประสงค์ เพื่อเปรียบเทียบการตอบสนองของเนื้อเยื่อในของฟันสุนัขต่อการตัดเนื้อเยื่อในบางส่วนแล้วปิดทับด้วยพอร์ตแลนด์ซีเมนต์ที่ปรับปรุงคุณภาพกับโปรรูปเอ็มทีเอ วัสดุและวิธีการ ทําการตัดเนื้อเยื่อในบางส่วนในฟันกรามน้อยของสุนัข 4 ตัว จํานวน 35 ปี และแบ่งเป็น 3 กลุ่ม กลุ่ม 1 ปิดด้วยโปรรูทเอ็มทีเอที่ผสมกับน้ํากลั่น (จํานวน 10 ที่) กลุ่ม 2 ปิดด้วยพอร์ตแลนด์ซีเมนต์ที่มีบิสมัต ออกไซด์ ผสมกับแคลเซียมคลอไรด์ความเข้มข้นร้อยละ 5 และเมททิลเซลลูโลสความเข้มข้นร้อยละ 1 (จํานวน 20 ) รองพื้นด้วยกลาสไอโอโนเมอร์ซีเมนต์ และบูรณะด้วยเรซิน คอมโพสิต โดยทั้งสองกลุ่มจะทดลองที่ 7 และ 70 วัน กลุ่ม 3 เป็นกลุ่มควบคุมบวก เนื้อเยื่อในที่ตัดถูกเปิดไว้เป็นเวลา 7 วัน (จํานวน 5 ปี) ทําการถอนฟัน ภายใต้การดมยาสลบ แล้วนําฟันที่ได้ไปผ่านกระบวนการตรวจลักษณะทางจุลพยาธิวิทยา เพื่อประเมินการอักเสบ และการสร้างเนื้อเยื่อแข็งของเนื้อเยื่อใน นําข้อมูลที่ได้มาวิเคราะห์ความแตกต่างทางสถิติระหว่างกลุ่มด้วยการ ทดสอบครัสตัล-วอลลิส และไคสแควร์ ที่ระดับนัยสําคัญน้อยกว่า 0.05 ผลการศึกษา ไม่พบความแตกต่างอย่างมีนัยสําคัญทางสถิติด้านการตอบสนองต่อการอักเสบและการหาย ระหว่างโปรรูทเอ็มทีเอและพอร์ตแลนด์ซีเมนต์ทั้งในระยะเวลา 7 วัน และ 70 วัน โดยกลุ่มทดลองทั้งสองไม่พบ การอักเสบของเนื้อเยื่อใน นอกจากนี้พบว่ามีการสร้างเนื้อเยื่อแข็งที่ต่อเนื่อง ด้วยลักษณะรูปร่างและความหนาที่ ไม่แตกต่างกันระหว่างกลุ่มทดลองทั้งสองอย่างไรก็ตามพบความแตกต่างอย่างมีนัยสําคัญระหว่างกลุ่มทดลอง ทั้งสองกับกลุ่มควบคุมบวก ซึ่งพบการอักเสบในระดับปานกลางถึงรุนแรง สรุป พอร์ตแลนด์ซีเมนต์ของประเทศไทยที่มีบิสมัดออกไซด์ผสมกับแคลเซียมคลอไรด์และเมททิลเซลลูโลส เมื่อนํามาใช้เป็นวัสดุปิดทับเนื้อเยื่อในสามารถคงความมีชีวิตของเนื้อเยื่อในของฟันได้โดยปราศจากการอักเสบส่งเสริมให้เกิดการหายและกระบวนการซ่อมแซม นอกจากนี้ยังสามารถกระตุ้นให้เกิดการสร้างเนื้อเยื่อแข็งที่มี ลักษณะไม่แตกต่างจากโปรรูปเอ็มทีเอ (ว ทันต จุฬาฯ 2557;37:47-58)

As the cost of healthcare continues to raise, the need to address nurse attrition, which is a contributing factor, also rises. While there are various factors that influence nurses’ decision to leave or stay, job satisfaction and ethical climate are significant variables. This study examined the effects of negative previous work experiences on job satisfaction and ethical climate. The results showed previous work experiences moderated both job satisfaction and ethical climate. In addition, ethical climate mediated the effects of previous experiences on job satisfaction. Churches are a promising setting through which to reach Latinas with cancer control efforts. A better understanding of the dimensions of religiousness that impact health behaviors could inform efforts to tailor cancer control programs for this setting. The purpose of this study was to explore relationships between dimensions of religiousness with adherence to cancer screening recommendations among church-going Latinas. During sepsis, neutrophils are inappropriately activated and have prolonged lifespans, thus becoming dysfunctional. Excessive neutrophil activation can lead to tissue injury while neutrophil dysfunction can lead to decreased free radical production and reduced phagocytosis, preventing the host from clearing preexisting infections. Lipoxin A4 (LXA4) is a specialized pro-resolution mediator which reduces neutrophil migration and expression of proinflammatory mediators. Purpose—The goals of the present study were to investigate whether (1) the speech disfluencies of preschool-age children are normally distributed; (2) preschool-age children who do (CWS) and do not stutter (CWNS) differ in terms of non-stuttered disfluencies; (3) age, gender, and speechlanguage ability affect the number and type of disfluencies children produce; and (4) parents’ expressed concern that their child stutters is associated with examiners’ judgments of stuttered disfluency.

PURPOSE: To examine the effectiveness of videoconferencing technology for delivering comprehensive weight management treatment.
METHODS: This retrospective cohort study was conducted by extraction of data from medical records for the years 2008-2010. The treatment included a series of 12 weekly MOVE!® classes delivered using videoconferencing. Data were extracted from the time of baseline weight to 1 year after baseline weight for the MOVE! participants (n = 60) and from a concurrent control group (n = 60) that did not participate in MOVE! treatment.

This study seeks to provide empirical evidence about how health-related questions are answered in text reference service in order to further the understanding of how to best use texting as a reference service venue to fulfill people’s health information needs. Two hundred health reference transactions from My Info Quest, the first nation-wide collaborative text reference service, were analyzed identify the types of questions, length of transactions, question-answering behavior, and information sources used in the transactions. This study explored strategies employed by activists engaged in efforts to change policies and laws related to selling and promoting alcoholic beverages based on in-depth interviews with 184 social activists in seven U.S. major cities. Nine strategies aimed at improving local conditions and influencing policy were described by activists across regional contexts. Grassroots mobilization was central to all other strategies, which included the creation or enforcement of laws, meeting with elected officials, media advocacy, working with police/law enforcement, education and training, direct action, changing community norms, and negotiating with store owners.

This article describes U.S. state policies related to alcohol use during pregnancy, using data from the National Institute on Alcohol Abuse and Alcoholism Alcohol Policy Information System. Specifically, this study examines trends in policies enacted by states over time and types of policies enacted across states in the United States, with a focus on whether laws were supportive or punitive toward women. Findings revealed substantial variability in characteristics of policies (19 primarily supportive, 12 primarily punitive, 12 with a mixed approach, and 8 with no policies). The Use Of Surveillance And Preventative Measures For Methicillin-Resistant Staphylococcus Aureus Infections In Surgical Patients, Kevin T . Kavanagh, Lindsay Calderon, Daniel Saman, Said Abusalem

The Agency for Healthcare Research & Quality (AHRQ) found that Methicillin-resistant Staphylococcus aureus (MRSA) is associated with up to 375,000 infections and 23,000 deaths in the United States. It is a major cause of surgical site infections, with a higher mortality and longer duration of care than Methicillin-sensitive Staphylococcus aureus. A multifactorial bundled approach is needed to control this epidemic, with single interventions unlikely to have a significant impact on attenuating MRSA infection rates.
