วัตถุประสงค์: เพื่อศึกษาผลของโปรแกรมการส่งเสริมสมรรถนะแห่งตนและการสนับสนุนจากครอบครัว ต่อพฤติกรรมส่งเสริมสุขภาพของผู้ป่วยสูงอายุโรคต้อหิน\n \nรูปแบบการวิจัย: การวิจัยแบบกึ่งทดลอง\n \nวิธีดําเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้สูงอายุโรคต้อหินทั้งชายและหญิง ที่เข้ารับการรักษาในคลินิก โรคต้อหิน แผนกผู้ป่วยนอกของโรงพยาบาลจุฬาลงกรณ์ คัดเลือกตามเกณฑ์ที่กําหนด จํานวน 60 ราย แบ่ง เป็นกลุ่มทดลอง 30 ราย กลุ่มควบคุม 30 ราย โดยจับคู่ด้าน เพศ ระดับการศึกษา รายได้ และโรคประจําตัว กลุ่มควบคุมได้รับการพยาบาลตามปกติ และกลุ่มทดลองได้รับโปรแกรมการส่งเสริมสมรรถนะแห่งตน และการสนับสนุนจากครอบครัว ประกอบด้วย 4 ขั้นตอน คือ 1) การประเมินทางสภาพสรีรวิทยาและ การกระตุ้นทางอารมณ์ 2) การให้คําแนะนําหรือการใช้คําพูดชักจูง 3) การเรียนรู้ผ่านตัวแบบหรือ ประสบการณ์ของผู้อื่น และ 4) ประสบการณ์ที่กระทําสําเร็จได้ด้วยตนเอง และการสนับสนุนจากครอบครัว ใน 4 ด้าน คือ ด้านอารมณ์ ด้านการประเมินค่า ด้านข้อมูลข่าวสาร ด้านทรัพยากร เก็บรวบรวมข้อมูล โดยใช้แบบสอบถามพฤติกรรมส่งเสริมสุขภาพของผู้ป่วยสูงอายุโรคต้อหิน มีค่าดัชนีความตรงเชิง เนื้อหา เท่ากับ .80 มีค่าสัมประสิทธิ์อัลฟ่าของครอนบาค เท่ากับ .84 วิเคราะห์ข้อมูลโดยหาค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน สถิติ Paired-t test และสถิติ One-way ANOVA\n \nผลการวิจัย: 1) ค่าเฉลี่ยของคะแนนพฤติกรรมส่งเสริมสุขภาพของผู้ป่วยสูงอายุโรคต้อหินหลังได้รับโปรแกรมฯ สูงกว่าก่อนได้รับโปรแกรมฯ อย่างมีนัยสําคัญทางสถิติที่ระดับ .05\n2) ค่าเฉลี่ยของคะแนนพฤติกรรมส่งเสริมสุขภาพของผู้ป่วยสูงอายุโรคต้อหิน หลังได้รับโปรแกรมฯสูงกว่าผู้ป่วยสูงอายุโรคต้อหินที่ได้รับการพยาบาลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05\n \nสรุป: โปรแกรมการส่งเสริมสมรรถนะแห่งตนและการสนับสนุนจากครอบครัวส่งผลให้ผู้สูงอายุโรคต้อหิน มีพฤติกรรมส่งเสริมสุขภาพดีขึ้นและดีกว่ากลุ่มที่ได้รับการพยาบาลตามปกติ\n

วัตถุประสงค์: เพื่อศึกษาสมรรถนะพยาบาลเวชศาสตร์ฟื้นฟู\n \nรูปแบบการวิจัย: การวิจัยเชิงพรรณนา โดยใช้เทคนิค EDFR\n \nวิธีดําเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้เชี่ยวชาญทางด้านเวชศาสตร์ฟื้นฟู จํานวน 21 คน ประกอบด้วย แพทย์ผู้เชี่ยวชาญจากราชวิทยาลัยแพทย์เวชศาสตร์ฟื้นฟูแห่งประเทศไทย จํานวน 2 คน ผู้เชี่ยวชาญ จากชมรมพยาบาลเวชศาสตร์ฟื้นฟแห่งประเทศไทย จํานวน 11 คน ผู้บริหารทางการพยาบาลระดับ หัวหน้าแผนกพยาบาลเวชศาสตร์ฟื้นฟู จํานวน 4 คน และอาจารย์ผู้สอนหลักสูตรเฉพาะทางพยาบาล เวชศาสตร์ฟื้นฟู จํานวน 3 คน วิธีดําเนินการวิจัยประกอบด้วย 3 ขั้นตอน คือ ขั้นตอนที่ 1 สัมภาษณ์ เกี่ยวกับสมรรถนะพยาบาลเวชศาสตร์ฟื้นฟู ขั้นตอนที่ 2 นําข้อมูลที่ได้มาวิเคราะห์เนื้อหาแล้วนํามาสร้าง แบบสอบถามให้ผู้เชี่ยวชาญแสดงความคิดเห็นเกี่ยวกับระดับความสําคัญของข้อรายการแต่ละข้อและ ขั้นตอนที่ 3 นําข้อมูลที่ได้มาคํานวณค่ามัธยฐานและค่าพิสัยระหว่างควอไทล์และส่งแบบสอบถามไปให้ ผู้เชี่ยวชาญยืนยันคําตอบ หลังจากนั้น นําข้อมูลที่ได้มาคํานวณหาค่ามัธยฐานและค่าพิสัยระหว่าง ควอไทล์เพื่อสรุปผลการวิจัย\n\n ผลการวิจัย: สมรรถนะพยาบาลเวชศาสตร์ฟื้นฟู ประกอบด้วย 7 ด้าน มีจํานวนทั้งหมด 57 ข้อ ได้แก่ 1) ด้านการปฏิบัติการพยาบาลเวชศาสตร์ฟื้นฟูมีจํานวน 21 ข้อ 2) ด้านการป้องกันภาวะแทรกซ้อนมีจํานวน 7 ข้อ 3) ด้านการสอนและให้คําปรึกษามีจํานวน 5 ข้อ 4) ด้านนวัตกรรมและเทคโนโลยีมีจํานวน 5 ข้อ 5) ด้านวิชาการและการวิจัยมีจํานวน 6 ข้อ 6) ด้านการประสานงานและการดูแลอย่างต่อเนื่องมีจํานวน 4 ข้อและ 7) ด้านการส่งเสริมสุขภาพและพัฒนาคุณภาพชีวิตมีจํานวน 9 ข้อ\n \nสรุป: สมรรถนะพยาบาลเวชศาสตร์ฟื้นฟูประกอบด้วยสมรรถนะ 7 ด้าน ผู้บริหารทางการพยาบาล สามารถนํามาเป็นแนวทางในการพัฒนา และประเมินพยาบาลที่ปฏิบัติงานในหน่วยงานเวชศาสตร์ฟื้นฟูได้\n

Atrial fibrillation is associated with substantial morbidity and mortality rates. The incompletely understood pathogenesis of this cardiac dysrhythmia makes it difficult to improve approaches to primary and secondary prevention. Evidence has accumulated in regard to a relationship between inflammation and atrial fibrillation. Investigators have correlated the dysrhythmia with myocarditis, pericardiotomy, and C-reactive protein levels, suggesting that inflammation causes atrial fibrillation or participates in its onset and continuation. Conversely, other investigators suggest that atrial fibrillation induces an inflammatory response. In this review, we summarize and critically discuss the nature and clinical role of inflammation and C-reactive protein in atrial fibrillation.

OBJECTIVE: To measure neurodevelopment at 3 years of age in children with single right-ventricle anomalies and to assess its relationship to Norwood shunt type, neurodevelopment at 14 months of age, and patient and medical factors.

STUDY DESIGN: All subjects in the Single Ventricle Reconstruction Trial who were alive without cardiac transplant were eligible for inclusion. The Ages and Stages Questionnaire (ASQ, n = 203) and other measures of behavior and quality of life were completed at age 3 years. Autism spectrum disorders (ASDs) are neurodevelopmental disorders of socialization, communication, and repetitive behaviors. Children with ASD have underlying anxiety leading to challenging behaviors in unfamiliar situations. The anxiety impacts timely completion of an imaging procedure. The purpose of the case study was to describe the process of the social script intervention delivered using the iPad application on parent and child anxiety, child behaviors, and imaging procedure length between two parent and child dyads. To describe intensive care unit (ICU) patients' delusional memories and interpretations of those memories.

Background

Delusional memories of the ICU are distressing for patients and may impact psychological recovery.

Methods

This is a secondary analysis from a study of mechanically ventilated patients' recall in relation to sedation. Subjects, recruited from one medical-surgical ICU, participated in structured interviews after extubation.

Results

Subjects (n = 35) with a mean age of 66 (SD 12.9) and on the ventilator a median of 4.5 days provided detailed descriptions of delusional memories of being shackled, caged, strangled, or being in a foreign country. …

ABSTRACT Vogt-Koyanagi-Harada (VKH) syndrome is a rare multisystem disease of melanocyte containing organs. It is characterized by diffuse granulomatous inflammation involving various organs including eye. VKH syndrome is usually sporadic, but some familial cases have also been reported indicating a hereditary basis. VKH is not associated with mortality but it may result in long-term complications such as decreased vision associated with cataract, glaucoma and choroidal neovascularization. This paper explores how technology use in adolescence facilitates adult identity achievement and presents evidence that technological objects, such as smartphones have become adolescent transitional objects. Early and late adolescents were surveyed about technology use and feelings associated with technology. Among older adolescents, anxiety level was related to smart phone use, such that higher anxiety was associated with greater smart phone use. The feelings and behaviors associated with use of the preferred device are consistent with feelings and behaviors associated with use of a transitional object. In contrast, younger adolescents did not appear to use technology as a transitional object. …

Acute hemorrhagic pneumonia caused by Streptococcus zooepidemicus has emerged as a major disease of shelter dogs and greyhounds. S. zooepidemicus strains differing in multilocus sequence typing (MLST), protective protein (SzP), and M-like protein (SzM) sequences were identified from 9 outbreaks in Texas, Kansas, Florida, Nevada, New Mexico, and Pennsylvania. Clonality based on 2 or more isolates was evident for 7 of these outbreaks. The Pennsylvania and Nevada outbreaks also involved cats. Background: The ‘Cash Transfer to Orphans and Vulnerable Children’ (CT-OVC) in Kenya is a government-supported program intended to provide regular and predictable cash transfers (CT) to poor households taking care of OVC. CT programs can be an effective means of alleviating poverty and facilitating the attainment of an adequate standard of living for people’s health and well-being and other international human rights. A 9 – year – old, 6.2 kg female Shitsu dog with a repeated history of pododemodicosis was presented at the animal hospital. The dog had been previously treated with miticides such as ivermectin injections and oral milbemycin oxime, but the clinical signs recurred every time after the therapy had been stopped. Microscopic examination of skin samples from the dog revealed the presence of Demodex canis. A spot – on formulation composed of 2.5% moxidectin and 10% imidacloprid was applied to the dog weekly. BACKGROUND: The purpose of the present study is to evaluate a single surgeon's short, intermediate, and long-term clinical, functional, and radiographic outcomes with a trapeziectomy with flexor carpi radialis (FCR) suspension arthroplasty without tendon interposition (LRSA).

METHODS: Twenty-one patients underwent 26 FCR suspension arthroplasties without tendon interposition by a single senior surgeon. All patients had Eaton stage III and IV carpometacarpal (CMC) osteoarthritis. The aim of this study was to determine which major ovarian hormone modulates the antinociceptive effects of i.c.v. Lactococcosis caused by Lactococcus garvieae is an increasing problem in aquaculture. The current study is, to our knowledge, the first record of L. garvieae isolated from giant freshwater prawn (Macrobrachium rosenbergii de Man) in Thailand. These bacteria were isolated from giant freshwater prawns that were either cultured or in their natural environments in Phatthalung and Songkhla, provinces of southern Thailand. Based on conventional and rapid identification systems, as well as genetic and phylogenetic characterizations, the bacteria were identified as L. garvieae. This study investigated the prevalence of zoonotic pathogens in stray dogs including toxoplasmosis, heartworm disease, Lyme disease, leptospirosis and Q fever in central Taiwan (Taichung, Changhua and Yunlin County) using nested, semi-nested or traditional polymerase chain reaction. Associations between prevalence of the infections and demiographic data of stray dogs in central Taiwan were also analyzed. Infectious endocarditis (IE) is a heart valve or endocardial disease. Bartonella spp. are considered one of the causes of IE. The objective was to study the prevalence of canine IE in Bangkok, Thailand with an emphasis on Bartonella spp. infections. A review of the reports between January 1999 to December 2009 of 3,545 necropsied dogs was performed. Cardiac tissue blocks from 11 dogs were studied for the presence of eubacterial DNA and Bartonella spp. DNA by fluorescence in situ hybridization (FISH) and for Bartonella spp. DNA by conventional polymerase chain reaction (PCR).The prevalence of canine IE was 0.65%. Blunt thoracic trauma may result in cardiac injuries ranging from simple arrhythmias to fatal cardiac rupture. Coronary artery dissection culminating in acute myocardial infarction (AMI) is rare after blunt chest trauma. Here we report a case of a 37-year-old man who had an AMI secondary to coronary dissection resulting from blunt chest trauma after involvement in a physical fight.

Statement of problem

The effect of microwave brazing on the strength properties of dental casting alloys is not yet known.

Purpose

The purpose of this study was to compare the strength properties of preceramic brazed joints obtained by using a microwave oven and a conventional torch flame for a high noble alloy (Au-Pd).

Material and methods

A total of 18 tensile bars made of an Au-Pd ceramic alloy were fabricated. Six specimens were cut and joined with a high-fusing preceramic solder in a specially designed microwave oven, and 6 specimens were joined with a conventional natural gas/oxygen torch. The remaining …
