เหตุผลของการทำวิจัย : ภาวะซึมเศร้า ความว้าเหว่ และการเห็นคุณค่าในตนเองต่ำ เป็นปัญหาที่สำคัญในผู้สูงอายุที่พักในสถานบริการที่พักทั้งรัฐและเอกชน แต่อย่างไรก็ตามงานวิจัยในประเทศไทยยังไม่มีการศึกษาประเด็นดังกล่าวครอบคลุมทั้ง 3 ด้านวัตถุประสงค์ : เพื่อศึกษาความชุกของภาวะซึมเศร้า และศึกษาปัจจัยที่มีความเกี่ยวข้องกับภาวะซึมเศร้าของผู้สูงอายุ นอกจากนั้นยังศึกษาความสัมพันธ์ระหว่างภาวะซึมเศร้า ความว้าเหว่ และการเห็นคุณค่าในตนเองของผู้สูงอายุในสถานสงเคราะห์คนชราบ้านบางแค และที่พักผู้สูงอายุของเอกชนรูปแบบการวิจัย : การวิจัยเชิงพรรณนา ศึกษา ณ ช่วงเวลาใดเวลาหนึ่งสถานที่ทำการวิจัย : บ้านพักคนชราบ้านบางแค 1 สังกัดกรมประชาสงเคราะห์ กระทรวงแรงงาน และสวัสดิการสังคม, บ้านบางแค 2 สังกัดกรุงเทพมหานคร และผู้สูงอายุที่อาศัยในสถานที่พักเอกชนใน 5 แห่งในเขตกรุงเทพมหานครตัวอย่างและวิธีการศึกษา : ศึกษาในผู้สูงอายุจำนวน 295 คน โดยแบบสอบถามที่ใช้ในการเก็บข้อมูลได้แก่แบบสอบถามข้อมูลส่วนบุคคล, แบบสอบถามภาวะซึมเศร้าในผู้สูงอายุ, แบบวัดความว้าเหว่ และแบบสอบถามการเห็นคุณค่าในตนเอง โดยใช้โปรแกรม SPSS version 17 วิเคราะห์ค่าร้อยละ, ค่าเฉลี่ย,ค่าเบี่ยงเบนมาตรฐาน, ไครสแควร์เทส, ทีเทส, ค่าสหสัมพันธ์และสถิติถดถอยลอจิสติกเพื่อหาปัจจัยทำนายภาวะซึมเศร้าผลการวิจัย : กลุ่มตัวอย่างในการศึกษาครั้งนี้เป็นเพศหญิงร้อยละ 70.5 อายุเฉลี่ย78.43 (± 8.3) ปี พบความชุกของภาวะซึมเศร้าในกลุ่มตัวอย่างร้อยละ57.3 ปัจจัยที่มีความสัมพันธ์กับภาวะซึมเศร้า ได้แก่ เพศหญิง,สถานภาพหม้ายหรือโสด, ไม่ได้รับการศึกษา, ไม่มีรายได้, ไม่มีญาติมาเยี่ยม, มีความสัมพันธ์กับสมาชิกในครอบครัวและผู้ดูแลประจำไม่ดี,มีความพึงพอใจระดับต่ำต่อที่พักอาศัย และความใส่ใจของเจ้าหน้าที่,มีการเข้าร่วมกิจกรรมด้านอาชีวบำบัด และกิจกรรมด้านสุขภาพน้อยครั้งเมื่อศึกษาความสัมพันธ์ระหว่างภาวะซึมเศร้า พบว่ามีความสัมพันธ์กับภาวะว้าเหว่ที่สูงและการเห็นคุณค่าในตนเองต่ำ และผลการวิเคราะห์สถิติถดถอยลอจิสติกพบว่าการไม่ได้รับการศึกษา,การไม่มีญาติมาเยี่ยม, ความพึงพอใจต่อที่พักในระดับต่ำ, ภาวะความว้าเหว่ระดับปานกลางถึง สูง เป็นปัจจัยทำนายภาวะซึมเศร้าสรุป : ผลการวิจัยพบว่าประมาณครึ่งหนึ่งของผู้สูงอายุมีภาวะซึมเศร้า และทราบถึงปัจจัยที่สัมพันธ์ การคัดกรองและการรักษาภาวะซึมเศร้ารวมถึงการดูแลด้านจิตสังคม เช่น การลดความว้าเหว่, การส่งเสริมการเห็นคุณค่าในตนเอง และการเพิ่มระดับความพึงพอใจต่อที่พักอาศัยมีส่วนช่วยด้านสุขภาพจิตของผู้สูงอายุในสถานสงเคราะห์และที่พักผู้สูงอายุ.

เหตุผลของการวิจัย : จากหลายการศึกษาก่อนหน้านี้ ในกลุ่มผู้ป่วยที่เข้ารับการผ่าตัดมะเร็งเต้านมมีภาวะซึมเศร้าและวิตกกังวลสูง ซึ่งอาจจะเกิดจากการสูญเสียภาพลักษณ์ทาง กาย อย่างไรก็ดี จนถึงปัจจุบัน ยังไม่ค่อยมีการศึกษาโดยตรงเกี่ยวกับภาพลักษณ์ทางกายในผู้ป่วยกลุ่มนี้ทางผู้วิจัยจึงสนใจที่จะศึกษาในเรื่องนี้ เพื่อทำความเข้าใจให้มากขึ้นและเพื่อนำไปพัฒนาการดูแลผู้ป่วยกลุ่มนี้ให้ดียิ่งขึ้นวัตถุประสงค์ : เพื่อศึกษาภาพลักษณ์ทางกาย และปัจจัยที่เกี่ยวข้องในผู้ป่วยมะเร็งเต้านมที่ทำผ่าตัดเต้านมออกทั้งหมด ณ โรงพยาบาลจุฬาลงกรณ์รูปแบบการวิจัย : การวิจัยเชิงพรรณนา ณ จุดเวลาใดเวลาหนึ่งสถานที่ทำการศึกษา : โรงพยาบาลจุฬาลงกรณ์ตัวอย่างและวิธีการศึกษา : เก็บรวบรวมข้อมูลจากผู้ป่วยมะเร็งเต้านมที่ทำผ่าตัดเต้านมออกจำนวน 126 ราย โดยให้ผู้ป่วยตอบแบบสอบถามทั้งหมด 7 ส่วนด้วยตนเอง ได้แก่ 1) ข้อมูลส่วนบุคคล 2) ประวัติการเจ็บป่วยและการรักษามะเร็งเต้านม 3) แบบประเมินการรับรู้ภาพลักษณ์ทางกายBody Image Scale (BIS) 4) แบบประเมินการเผชิญความเครียดJalowiec Coping Scale 5) แบบประเมินการสนับสนุนจากครอบครัว6) แบบประเมินคุณภาพชีวิต 7) แบบสอบถามการปรับตัวและแก้ไขปัญหาความขัดแย้ง การวิเคราะห์ข้อมูลใช้ Univariate analysis และMultivariate Analysis เพื่อหาปัจจัยที่เกี่ยวข้องและปัจจัยทำนายภาพลักษณ์ทางกายในผู้ป่วยกลุ่มดังกล่าวผลการศึกษา : พบว่ากลุ่มตัวอย่างส่วนใหญ่ (ร้อยละ 56.3) มีการรับรู้ภาพลักษณ์ทางกายในระดับปานกลาง โดยปัจจัยที่เกี่ยวข้องกับภาพลักษณ์ทางกายจาก Multivariate Analysis มี 5 ปัจจัย ได้แก่ อายุ ระดับการศึกษาคุณภาพชีวิต 2 ด้าน (ได้แก่ ด้านการตอบสนองต่อการวินิจฉัยและการรักษาด้านผ่าตัด และ ด้านการตอบสนองต่อการวินิจฉัย และการรักษาด้านโภชนาการ) และการเผชิญความเครียดด้านการจัดการกับอารมณ์ (p <0.01)สรุป : การรับรู้ภาพลักษณ์ทางกาย ในผู้ป่วยมะเร็งเต้านมที่ได้รับการรักษาโดยการผ่าตัดจากการศึกษาครั้งนี้ได้ผลใกล้เคียงกับการศึกษาอื่นโดยมีหลายการศึกษาพบว่าผู้ป่วยมะเร็งเต้านมที่ได้รับการรักษาโดยการผ่าตัด มีการรับรู้ภาพลักษณ์ทางกายในระดับปานกลางซึ่งการทราบปัจจัยที่เกี่ยวข้อง จะทำให้บุคลากรทางการแพทย์สามารถคัดกรองกลุ่มเป้าหมายได้ง่ายขึ้น ซึ่งนำไปสู่การให้การช่วยเหลือทางจิตใจต่อไป.

Background: The East Tennessee State University Quillen College of Medicine Library has participated for several years in projects to provide rural clinicians with health information resources.
Objectives: To determine whether a strategy of hand‐held devices with a best‐evidence point‐of‐care disease tool and a drug database paired with access to a medical library for full‐text articles and training to use the tools would be an affordable way to meet the information needs of rural underserved clinicians.

Because we found UV-exposed oral tissue cells have reduced DNA repair and apoptotic cell death compared with skin tissue cells, we asked if a correlation existed between personal UV dose and the incidences of oral and pharyngeal cancer in the United States. We analyzed the International Agency for Research on Cancer's incidence data for oral and pharyngeal cancers by race (white and black) and sex using each state's average annual personal UV dose. We refer to our data as ‘white’ rather than ‘Caucasian,’ which is a specific subgroup of whites, and ‘black’ rather than African-American because blacks from other countries …

There is a lack of research related to return to work (RTW) after acute ischemic stroke. Historically considered a disease of the elderly, acute ischemic stroke studies have not routinely used return to work as an outcome. Major stroke trials have not routinely collected this data as an endpoint. However, the mean age for acute ischemic stroke (AIS) has declined to 69 years of age, while the incidence in patients under the age of 55 has increased to 19%1-3. This phenomenon has not been studied in Pakistan to-date. OBJECTIVE:
To determine the accuracy of three-dimensional bladder ultrasonography (US, using the BVI 3000, Verathon, WA, USA) for determining the residual urinary volume, compared with the conventional catheterisation method.
PATIENTS AND METHODS:
We conducted a cross-sectional study at day-care unit of a University hospital after obtaining approval from the Ethics Review Committee of the hospital. Thirty-four patients with lower urinary tract symptoms requiring cystometrography were included. A study was conducted by Spandan holistic institute of applied Homoeopathy, Mumbai, with the objective of demonstrating the usefulness of homeopathic management in autism. The ventral bed nucleus of the stria terminalis (vBNST) has been implicated in stress-induced cocaine use. Here we demonstrate that, in the vBNST, corticotropin releasing factor (CRF) is expressed in neurons that innervate the ventral tegmental area (VTA), a site where the CRF receptor antagonist antalarmin prevents the reinstatement of cocaine seeking by a stressor, intermittent footshock, following intravenous self-administration in rats. The vBNST receives dense noradrenergic innervation and expresses β adrenergic receptors (ARs). Footshock-induced reinstatement was prevented by bilateral intra-vBNST injection of the β-2 AR antagonist, ICI-118,551, but not the β-1 AR antagonist, betaxolol. Objective—Routine monitoring of plasma HIV RNA among HIV-infected patients on antiretroviral therapy (ART) is unavailable in many resource-limited settings. Alternative monitoring approaches correlate poorly with virologic failure and can substantially delay switch to second-line therapy. We evaluated the impact of delayed switch on mortality among patients with virologic failure in Africa.
Design—A cohort.

Introduction: Pre-exposure prophylaxis (PrEP) may be an important safer conception strategy for HIV-1–uninfected women with HIV-1–infected partners. Understanding medication adherence in this population may inform whether PrEP is a feasible safer conception strategy.
Methods: We evaluated predictors of pregnancy and adherence to study medication among HIV-1–uninfected women enrolled in a randomized placebo-controlled trial of PrEP among African HIV-1– serodiscordant couples. Participants were counseled on HIV-1 risk reduction, contraception, and adherence and tested for pregnancy at monthly study visits. Pill counts of dispensed drug were performed and, at a subset of visits, plasma was collected to measure active drug concentration.

BACKGROUND: Polyethylene glycol 3350 plus sports drink (PEG-SD) is a hypo-osmotic purgative commonly used for colonoscopy, though little safety data are available.
AIM: To evaluate the effect of PEG-SD on serum sodium (Na) and other electrolytes compared with PEG-electrolyte solution (PEG-ELS).
METHODS: We performed a single center, prospective, randomised, investigator-blind comparison of PEG-ELS to PEG-SD in out-patients undergoing colonoscopy. Vaccination of Mongolian gerbils with Brugia malayi cysteine protease inhibitor-2 in which the amino acid Asn66 was mutated to Lys66 (Bm-CPI-2M) resulted in reduced parasite numbers of 48.6% and 48.0% at 42 and 90 days p.i. with B. malayi L3s. Fertility of female worms was also affected at 90 days p.i. In vitro killing of L3s observed in the presence of gerbil peritoneal exudate cells and anti-Bm-CPI-2M sera suggests antibody-dependent cell-mediated cytotoxicity as a putative protective mechanism. These observations suggest that Bm-CPI-2M is a promising prophylactic and anti-fecundity vaccine candidate.

Objective: To determine the frequency of cytogenetic abnormalities in patients diagnosed as primary myelodysplastic syndrome (MDS) using conventional karyotyping. Study Design: Case series. Place and Duration of Study: The Clinical Laboratory, The Aga Khan University Hospital, Karachi, between January 2006 - June 2012. Methodology: Patients of all ages and either gender who fulfilled WHO criteria for MDS were included. Cytogenetic analysis was conducted at the time of diagnosis. MicroRNAs (miRNAs) are short noncoding RNAs that regulate the expression of their targets in a sequence-dependent manner. For protein-coding transcripts, miRNAs regulate expression levels through binding sites in either the 3' untranslated region (3' UTR) or the amino acid coding sequence (CDS) of the targeted messenger RNA (mRNA). Currently, for the 5' untranslated region (5' UTR) of mRNAs, very few naturally occurring examples exist whereby the targeting miRNA down-regulates the expression of the corresponding mRNA in a seed-dependent manner. วัตถุประสงค์: เพื่อศึกษาผลของโปรแกรมส่งเสริมการรับรู้สมรรถนะแห่งตนต่อพฤติกรรมทางเพศ \nที่ปลอดภัยของนักเรียนอาชีวศึกษาชาย\n\nรูปแบบการวิจัย: การวิจัยแบบกึ่งทดลอง\n\nวิธีดําเนินการวิจัย: กลุ่มตัวอย่าง คือ นักเรียนอาชีวศึกษาชายที่กําลังศึกษาหลักสูตร ปวส. ที่มีอายุระหว่าง 18-24 ปี จากโรงเรียน 2 แห่ง ที่มีลักษณะคล้ายกันในจังหวัดน่าน โดยสุ่มโรงเรียนเพื่อจัดเป็นกลุ่ม ทดลองหรือควบคุม แล้วคัดเลือกโรงเรียนละ 2 ห้อง แบ่งเป็นกลุ่มควบคุม จํานวน 30 คน และกลุ่ม ทดลอง จํานวน 30 คน กลุ่มควบคุมได้รับความรู้จากหลักสูตรการเรียนการสอนตามปกติ กลุ่มทดลองได้ รับโปรแกรมส่งเสริมการรับรู้สมรรถนะแห่งตน ซึ่งประกอบด้วย 4 ขั้นตอน คือ 1) การใช้คําพูดชักจูง 2) การได้เห็นตัวแบบหรือประสบการณ์ของผู้อื่น 3) ประสบการณ์ที่กระทําสําเร็จด้วยตนเอง และ 4) สภาวะด้านร่างกายและอารมณ์ โดยมีสื่อประกอบการดําเนินกิจกรรม คือ แผนการสอน ภาพสไลด์ วีดิทัศน์ และคู่มือ "อาชีวะยุคใหม่ ทันสมัย ไม่เข้าใกล้เอดส์" ซึ่งได้รับการตรวจสอบความตรงของ เนื้อหาจากผู้ทรงคุณวุฒิ จํานวน 5 คน เครื่องมือกํากับการทดลอง ได้แก่ แบบประเมินการรับรู้สมรรถนะ ของตนเองในการมีพฤติกรรมทางเพศที่ปลอดภัย เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูลประกอบด้วย แบบสอบถามข้อมูลส่วนบุคคล และแบบวัดพฤติกรรมทางเพศที่ปลอดภัย มีค่าความเที่ยง .81 วิเคราะห์ ข้อมูลโดยใช้สถิติเชิงบรรยายและการทดสอบค่าที่\n\nผลการวิจัย: 1) ค่าเฉลี่ยของคะแนนพฤติกรรมทางเพศที่ปลอดภัยของนักเรียนอาชีวศึกษาชายหลังได้รับ โปรแกรมส่งเสริมการรับรู้สมรรถนะแห่งตน (x̄ = 66.40, SD = 2.43) สูงกว่าก่อนได้รับโปรแกรมฯ (x̄ = 55.77, SD = 6.09) อย่างมีนัยสําคัญทางสถิติ (p < .01) และ 2) ค่าเฉลี่ยของคะแนนพฤติกรรมทางเพศ ที่ปลอดภัยของนักเรียนอาชีวศึกษาชายในกลุ่มที่ได้รับโปรแกรมส่งเสริมการรับรู้สมรรถนะแห่งตน (x̄ = 66.40, SD = 2,43) สูงกว่ากลุ่มที่ได้รับความรู้ตามปกติ (x̄ = 57.30, SD = 5.36) อย่างมีนัยสําคัญ ทางสถิติ (p < .01)\n \nสรุป: โปรแกรมส่งเสริมการรับรู้สมรรถนะแห่งตนส่งผลให้นักเรียนอาชีวศึกษาชายมีพฤติกรรมทางเพศ ที่ปลอดภัยเพิ่มขึ้นและเพิ่มมากกว่านักเรียนอาชีวศึกษาชายที่ได้รับความรู้ตามปกติ\n

Objective. The aim of this study was to document the survival advantage of lowering stage at detection from Stage IIIC to Stage IIIA epithelial ovarian cancer. Methods. Treatment outcomes and survival were evaluated in patients with Stage IIIA and Stage IIIC epithelial ovarian cancer treated from 2000 to 2009 at the University of Kentucky Markey Cancer Center (UKMCC) and SEER institutions. Results. Cytoreduction to no visible disease (P < 0.0001) and complete response to platinum-based chemotherapy (P < 0.025) occurred more frequently in Stage IIIA than in Stage IIIC cases. Time to progression was shorter in patients with Stage IIIC ovarian cancer (17 ± 1 months) than in those with Stage II1A disease (36 ± 8 months). Five-year overall survival (OS) improved from 41% in Stage IIIC patients to 60% in Stage IIIA patients treated at UKMCC and from 37% to 56% in patients treated at SEER institutions for a survival advantage of 19% in both data sets. 53% of Stage IIIA and 14% of Stage IIIC patients had NED at last followup. Conclusions. Decreasing stage at detection from Stage IIIC to stage IIIA epithelial ovarian cancer is associated with a 5-year survival advantage of nearly 20% in patients treated by surgical tumor …

Speech sensorimotor adaptation is the short-term learning of modified articulator movements evoked through sensory-feedback perturbations. A common experimental method manipulates acoustic parameters, such as formant frequencies, using real time resynthesis of the participant's speech to perturb auditory feedback. While some studies have examined phrases comprised of vowels, diphthongs, and semivowels, the bulk of research on auditory feedback-driven sensorimotor adaptation has focused on vowels in neutral contexts (/hVd/). The current study investigates coarticulatory influences of adjacent consonants on sensorimotor adaptation. The selection of effective articulatory features is an important component of tasks such as acoustic-to-articulator inversion and articulatory synthesis. Although it is common to use direct articulatory sensor measurements as feature variables, this approach fails to incorporate important physiological information such as palate height and shape and thus is not as representative of vocal tract cross section as desired. We introduce a set of articulator feature variables that are palate referenced and normalized with respect to the articulatory working space in order to improve the quality of the vocal tract representation. Silent speech interfaces (SSIs), which recognize speech from articulatory information (i.e., without using audio information), have the potential to enable persons with laryngectomy or a neurological disease to produce synthesized speech with a natural sounding voice using their tongue and lips. Current approaches to SSIs have largely relied on speaker-dependent recognition models to minimize the negative effects of talker variation on recognition accuracy. A computational framework was implemented and validated to better understand the hypertensive artery remodeling in both geometric dimensions and material properties. Integrating the stress-modulated remodeling equations into commercial finite element codes allows a better control and visualization of local mechanical parameters. Both arterial thickening and stiffening effects were captured and visualized. An adaptive material remodeling strategy combined with the element birth and death techniques for the geometrical growth were implemented. Public health services in sub-Sahara Africa countries face severe health workforce shortages exacerbated by both outward migration and internal public to private sector migration—Tanzania is no exception. This review was conducted to characterize the extent of health workforce shortages in Tanzania, and the factors impacting on the shortage. The authors reviewed publicly available data to assess the extent of health workforce shortages within Tanzania and the range of ‘pull’ and ‘push’ factors contributing to the shortages.
