Pulmonary Artery Sarcoma: A Rare Cause Of Lung Masses And Tumor Emboli, Wasin Jirisant, Napplika Kongpolprom

We reported here a case of pulmonary artery sarcoma. The patient presented with progressive dyspnea on exertion for 2 months. Chest radiography showed a large lobulated mass in the right lower lung field. Chest CT scan showed 2 pulmonary masses in the rightmiddle lobe and the right-lower lobe, with extensive filling defects in the right and left pulmonary arteries. A core needle biopsy of the larger mass showed an undetermined type of sarcoma. Pulmonary arteries were considered the primary origin of the sarcoma. Pulmonary endarterectomy with tumor debulking was performed to relieve the patient's symptoms in a combination with adjuvant …

INTRODUCTION: Antiepileptic-antidepressant combinations are frequently used by clinicians; their pharmacokinetic (PK) and pharmacodynamic (PD) drug interactions (DIs) have not been well studied but are frequently likely to be clinically relevant.

AREAS COVERED: This article provides a comprehensive review of PK DIs between antiepileptics and antidepressants. In the absence of PD DI studies, PD information on pharmacological mechanisms and studies on efficacy and safety of individual drugs are reviewed.

In eukaryotes proteins of the secretory pathway are synthesized and inserted into the lumen or membrane of the endoplasmic reticulum. This is a case report of cardiac herniation following right lower lobe lobectomy in a non-small-cell lung cancer patient who previously underwent right upper lobe and right middle lobe lobectomies. The patient presented with obstructive shock with refractory hypoxemia at the 8th hour after the lobectomy with radical removal of the tumor and complicated lung abscess via intrapericardial approach. Uremic tumoral calcinosis is primarily found in patients with chronic kidney disease (CKD) or end-stage renal disease (ESRD). Its clinical and radiologic presentations are highly characteristic. We herein report a case of a 33-year-old Thai male with ESRD and tertiary hyperparathyroidism. He presented with multiple cystic nodules on the right shoulder, hands, and feet without causing joint destruction for 2 years. The plain radiographs of the right shoulder and hands showed the pathognomonic "sedimentation sign". เหตุผลของการทำวิจัย : วัยรุ่นตั้งครรภ์ควรสามารถสร้างความรู้สึกที่ดีต่อตนเองและทารกในครรภ์ ทั้งนี้เพราะวัยรุ่นตั้งครรภ์ที่มีความรู้สึกมีคุณค่าในตนเองสูงจะมีจิตใจที่เข้มแข็งมั่นคงพึงพอใจตนเองรับรู้ และยอมรับเหตุการณ์ที่เกิดขึ้นตามความเป็นจริงพร้อมที่จะเผชิญปัญหาทั้งด้านร่างกายจิตใจอารมณ์ และสังคมสามารถปรับตัวได้อย่างเหมาะสมด้วยเหตุนี้ผู้วิจัยจึงมีความสนใจที่จะศึกษาเรื่องดังกล่าวเพื่อจะได้นำผลการวิจัยใช้เป็นแนวทางในการส่งเสริมความรู้สึกมีคุณค่าในตนเองของวัยรุ่นตั้งครรภ์แรกวัตถุประสงค์ : เพื่อศึกษาความรู้สึกมีคุณค่าในตนเองของวัยรุ่นตั้งครรภ์แรก และเพื่อศึกษาปัจจัยที่สัมพันธ์กับความรู้สึกมีคุณค่าในตนเองรูปแบบการวิจัย : เป็นการวิจัยแบบสำรวจภาคตัดขวางสถานที่ทำการศึกษา : จังหวัดอุบลราชธานีตัวอย่างและวิธีการศึกษา : กลุ่มตัวอย่างจำนวน 268 รายใช้วิธีการสุ่มตัวอย่างแบบหลายขั้นตอน(Multi-Stage Random Sampling) จากวัยรุ่นตั้งครรภ์แรกที่มาฝากครรภ์ณ โรงพยาบาลส่งเสริมสุขภาพตำบล จังหวัดอุบลราชธานี เก็บรวบรวมข้อมูล โดยแบบสอบถามที่ผู้วิจัยสร้างขึ้น และแบบประเมินความรู้สึกมีคุณค่าในตนเองของโรเซนเบอร์ก (Rosenberg, 1965) มีค่าความเชื่อมั่นดังนี้ การดูแลตนเอง 0.73 ความรู้สึกมีคุณค่าในตนเอง 0.99 และการสนับสนุนทางสังคม 0.97 การวิเคราะห์ข้อมูลโดยใช้โปรแกรมสำเร็จรูป สถิติที่ใช้คือค่าร้อยละ (Percentage) ค่าเฉลี่ย (Mean)ส่วนเบี่ยงเบนมาตรฐาน (Standard Deviations) การทดสอบไคสแควร์(Chi-square test) สัมประสิทธิ์สหสัมพันธ์แบบเพียร์สัน (Pearson’sProduct Moment Correlation Coefficient)ผลการศึกษา : กลุ่มตัวอย่างมีอายุ 17 ปีมากที่สุด คิดเป็นร้อยละ 26.9 อายุต่ำสุด 13 ปีร้อยละ10.4 อาชีพรับจ้างมากที่สุดร้อยละ 41.4 ฝากครรภ์ครั้งแรกอายุครรภ์ <12 สัปดาห์มากที่สุดร้อยละ 49.3 มีความรู้สึกมีคุณค่าในตนเองระดับปานกลางมากที่สุดร้อยละ 70.9 รองลงมาต่ำและสูงร้อยละ 16.0 และ 13.1 ตามลำดับ อายุครรภ์ที่ฝากครรภ์ครั้งแรกมีความสัมพันธ์กับความรู้สึกมีคุณค่าในตนเองอย่างมีนัยสำคัญทางสถิติ(p <0.05) อายุมีความสัมพันธ์ทางลบกับความรู้สึกมีคุณค่าในตนเอง(r = -0.137) และการดูแลตนเองโดยรวม (r = 0.128) ด้านรักษาสุขภาพจิต (r = 0.167) การสนับสนุนทางสังคมด้านวัตถุสิ่งของ(r = 0.194) มีความสัมพันธ์ทางบวกกับความรู้สึกมีคุณค่าในตนเองอย่างมีนัยสำคัญทางสถิติ (p <0.05) ส่วนระดับการศึกษา สถานภาพสมรส อาชีพ รายได้ครอบครัว และการสนับสนุนทางสังคมโดยรวมไม่มีความสัมพันธ์ทางสถิติกับความรู้สึกมีคุณค่าในตนเอง (p >0.05)สรุป : หน่วยบริการการฝากครรภ์ควรส่งเสริมสิ่งแวดล้อมในการฝากครรภ์และพัฒนาวิธีจัดการรักษาความรู้สึกนับถือตนเอง เพื่อให้วัยรุ่นตั้งครรภ์แรกมีความรู้สึกมีคุณค่าในตนเองระดับสูงเพิ่มชึ้น.

Background : The European League Against Rheumatism (EULAR) recommends primary prophylaxis with aspirin and hydroxychloroquine in systemic lupus erythematosus (SLE) patients with antiphospholipid antibodies without prior thrombosis. However, Asian populations may have lower incidence of thrombosis and the role of primary prophylaxis is unclear. Therefore, we examined the prevalence of thrombosis in Thai antiphospholipid-positive SLE patients. Methods : The medical records of SLE patients (N = 715) admitted to King Chulalongkorn Memorial Hospital from 2002 - 2012 were studied. Objective—Extent of vocal fold injury impacts the nature and timing of wound healing, and voice outcomes. However, depth and extent of the lesion created to study wound healing in animal models vary across studies, likely contributing to different outcomes. Our goal was to create a surgery classification system to enable comparison of postoperative outcomes across animal vocal fold wound healing studies.

Abstract

We report experience of managing Choledochal Cyst (CC) in different paediatric ages. Eleven neonates and infants (aged 0-8 months) and 24 paediatric cases (aged 2.5 - 18 years) were managed over 24 years (1988 to 2012). Neonates and infants presented with jaundice, acholic stools and abdominal mass whereas most of the paediatric cases presented with intermittent non-specific abdominal pain. Morphology of CC was mostly cystic in neonates whereas it was fusiform in majority (62%) of paediatric cases. The prevalence of diabetes mellitus is increasing at an alarming rate worldwide. Cardiovascular (CV) disease is the major cause of morbidity and mortality in diabetic patients. The search for new treatments has led to developing alternative insulin-independent treatment strategies such as sodium/glucose co-transporter (SGLT) inhibitors. OBJECTIVE: Sleeping on sofas increases the risk of sudden infant death syndrome and other sleep-related deaths. We sought to describe factors associated with infant deaths on sofas.

METHODS: We analyzed data for infant deaths on sofas from 24 states in 2004 to 2012 in the National Center for the Review and Prevention of Child Deaths Case Reporting System database. Demographic and environmental data for deaths on sofas were compared with data for sleep-related infant deaths in other locations, using bivariate and multivariable, multinomial logistic regression analyses.

Although scale-up of antiretroviral therapy (ART) since 2005 has contributed to a decline of about 30% in the global annual number of human immunodeficiency (HIV)–related deaths and declines in global HIV incidence, estimated annual HIV-related deaths among adolescents have increased by about 50%, and estimated adolescent HIV incidence has been relatively stable. In 2012, an estimated 2,500 (40%) of all 6,300 daily new HIV infections occurred among persons aged 15–24 years. A qualitative assessment of Kenyan men who have sex with men taking daily and intermittent oral HIV preexposure prophylaxis (PrEP) found stigma, sex work, mobility, and alcohol impacted adherence. We analyzed quantitative data from the same cohort to explore different definitions of intermittent adherence. Volunteers were randomized to daily emtricitabine/tenofovir or placebo, or intermittent (prescription: Mondays/Fridays/after sex, maximum1 dose/day)emtricitabine/tenofovir or placebo (2:1:2:1), and followed for 4 months. By electronic monitoring, median adherence for daily dosing was 80 %. Objective: To compare the plaque index in patients receiving multi-bracket fixed orthodontic treatment for various factors like age, gender, socio-economic status, brushing practices, meal habits, types of brackets, types of ligations, use of mouthwash and duration of treatment.
Study Design: Cross-sectional analytical study.
Place and Duration of Study: Orthodontics Clinic, The Aga Khan University Hospital, from September to November 2011.

With the development of medical specialties beginning in the 1860’s, physicians could devote their time to the study of specific organ systems or surgical approaches. Although Jews had been given full rights in the new Germany by 1871, prejudice and other restrictions often precluded hospital and university appointments. Major specialities like internal medicine and surgery were almost closed to Jews, as were obstetrics and gynecology. Dermatology with its heavy emphasis on sexually transmitted diseases evolved into a suitable domain for Jewish physicians almost by default. Chloroviruses (family Phycodnaviridae) are large DNA viruses known to infect certain eukaryotic green algae and have not been previously shown to infect humans or to be part of the human virome. We unexpectedly found sequences homologous to the chlorovirus Acanthocystis turfacea chlorella virus 1 (ATCV-1) in a metagenomic analysis of DNA extracted from human oropharyngeal samples. These samples were obtained by throat swabs of adults without a psychiatric disorder or serious physical illness who were participating in a study that included measures of cognitive functioning.
