Objectives: The primary objective was to expand upon results of a previously piloted patient perception survey with Healthcare Failure Mode and Effect Analysis (HFMEA), to identify areas within pharmacist-managed clinics needing improvement.

Methods: The survey was adapted for use in pharmacist-managed clinics. Patients completed the survey following regularly scheduled pharmacist appointments. Data were analyzed with a method adapted from HFMEA. Product scores could range from five to 25. A product of five indicates that pharmacists are doing a good job on the items that patients place the most value on, while a product score of 25 indicates that pharmacists are …

The purpose of this study is to gauge the attitude, behaviour and knowledge of students in the age group of 18 to 30 in relation to drinking and driving. The student population was focused on as they are a high risk group on Irish roads. The topic of drink driving was brought to the forefront in Ireland with the introduction of the 2010 Road Traffic Act. This introduced the reduction in the legal BAC limit from 80mg/100ml to 50mg/100ml for normal drivers and 20mg/100ml for specified drivers to come in line with the rest of Europe. Purpose: 1) To examine the spontaneous speech fluency of typically developing bilingual children obtained during an oral narrative task involving a conversational interview. 2) To determine what type of disfluencies may be present and 3) To investigate if there is a difference between fluency data obtained live and fluency data obtained from audio recordings during fluency sampling.

This pilot study created a comprehensive orientation module to help Midwives for Haiti prepare volunteers to serve in the organization’s midwifery school in Haiti. The module was evaluated to assess its perceived effectiveness and) revise it to better fit volunteer needs. The volunteers completed the module prior to their trip to Haiti and then completed an evaluation survey upon return; only three volunteers completed the entire pilot study process. Two out of three responded that the orientation fully prepared them for their time abroad. Other comments included the need for (a) additional emphasis on basic math and drip rate calculations, …

The incidence and mortality rates of prostate cancer (PCa) are higher in African American (AA) compared to Caucasian American (CA) men. To elucidate the molecular mechanisms underlying PCa disparities, we employed an integrative approach combining gene expression profiling and pathway and promoter analyses to investigate differential transcriptomes and deregulated signaling pathways in AA versus CA cancers. A comparison of AA and CA PCa specimens identified 1,188 differentially expressed genes. Interestingly, these transcriptional differences were overrepresented in signaling pathways that converged on the androgen receptor (AR), suggesting that the AR may be a unifying oncogenic theme in AA PCa. The TNM staging system is based on three anatomic prognostic factors: Tumor, Lymph Node and Metastasis. However, cancer is no longer considered an anatomic disease. Therefore, the TNM should be expanded to accommodate new prognostic factors in order to increase the accuracy of estimating cancer patient outcome. The ensemble algorithm for clustering cancer data (EACCD) by Chen et al. reflects an effort to expand the TNM without changing its basic definitions. The “Hispanic Paradox” suggests that despite rates of poverty similar to African Americans, Hispanics have far better health and mortality outcomes, more comparable to non-Hispanic White Americans. Three prominent possible explanations for the Hispanic Paradox have emerged. The “Healthy Migrant Effect” suggests a health selection effect due to the demands of migration. The Hispanic lifestyle hypothesis focuses on Hispanics’ strong social ties and better health behaviors. The reverse migration argument suggests that the morbidity profile in the USA is affected when many Hispanic immigrants return to their native countries after developing a serious illness. Vascular diseases have been the major cause of death and disability in presidents and vice presidents of the United States. Untreated hypertension and cigarette smoking have contributed greatly to this increased morbidity and mortality risk, which has impacted on historical events, especially in the 20th century. In this article, the medical histories of those incumbent Presidents and Vice Presidents who suffered from coronary artery and cerebrovascular diseases will be reviewed. A discussion of how atrial fibrillation has affected the Presidents is also included.

In the current study, we investigated the effect of a long-acting β -agonist (salmeterol) and a phosphodiesterase 4 (PDE4) inhibitor (cilomilast) on human lung fibroblast-mediated collagen gel contraction. Higher concentrations of salmeterol (10(-7) and 10(-6) M) inhibited fibroblast-mediated collagen gel contraction. No effect was observed with cilomilast alone (up to 10(-5) M). In the presence of 10(-8) M salmeterol, however, cilomilast could significantly inhibit fibroblast-mediated collagen gel contraction in a concentration-dependent manner (10(-7) ~10(-5) M). Blockade of endogenous PGE2 by indomethacin further potentiated the inhibitory effect of salmeterol on fibroblast-mediated collagen gel contraction, but it did not affect cilomilast's effect. …

Introduction: De Quervain’s tenosynovitis is a repetitive stress disorder occurring at the first dorsal compartment of the wrist at the radial styloid and is commonly treated by hand therapists.^1-3 Conservative treatments include activity modification, modalities, orthotics, and manual therapy.^4-7The literature is unclear regarding best practice for treating de Quervain’s tenosynovitis with conservative methods.^5,8,9

A 56 year-old male presented with symptoms of pneumonia and findings consistent with acute lung injury (ALI). Deterioration of respiratory function occurred over the first 24 hours of hospitalization leading to placement in an intensive care unit (ICU) followed by tracheal intubation and mechanical ventilation (ETMV). At that time criteria defining acute respiratory distress syndrome (ARDS) were present. The palliative medicine service was asked to address concerns expressed by the patient’s spouse reflecting uncertainty regarding outcome expectations.

A local school district, Muskegon Public Schools, is restructuring due to budget constraints. Therapists at Muskegon Public Schools were concerned about the safety and accessibility of the current playgrounds. The purpose of this needs assessment was to determine if any of Muskegon Public Schools’ playgrounds met the students’ safety and accessibility needs. To determine this, the researchers created a checklist using the Consumer Product Safety Commission’s Handbook on Public Playground Safety (1997) and the U.S. Access Board Summary of Accessibility Guidelines for Play Areas (2005). The purpose of this study was to investigate whether there were differences in mental health outcomes between first generation college students and non-first generation college students. The sample (n = 6,449) consisted of undergraduate students, aged 18-22, in bachelor's degree programs, and was drawn from 15 colleges and universities throughout the United States. Acculturative stress was used as a theoretical framework for why first generation college students (pioneers) may screen higher in prevalence and severity of mental health outcomes. The purpose of the capstone project was to explore the efficacy of the ATAQ to assess for asthma control at each patient encounter. In addition, addressing asthma control at each care visit would improve asthma outcomes by implementing education that promotes asthma self-management. The goal of the capstone project was to improve asthma control assessment in the primary care setting and provide asthma education for children and the caregiver to improve asthma outcomes. The purpose of this study is to evaluate why current treatment goals for patients with Acute Myocardial Infarction (AMI) are not being achieved despite a large body of evidence supporting regionalized ST Elevation Myocardial Infarction (STEMI) systems. An online survey tool,developed by the research project team, was used to poll the Critical Access Hospitals in Nebraska in order to understand delay factors and other barriers to executing a STEMI system in a rural area. Using telemedicine as a screening tool addresses both issues for diabetic retinopathy (DR) screening, as fundus photography has been determined to be sensitive and specific when screening for DR.^1,2 The American Diabetes Association places a Grade E recommendation on fundus photography as a screening tool.³ We analyze the financial impact of ophthalmic telemedicine in a mountainous, rural health clinic in West Virginia over a seven year period from 2003-2009. Gender difference has been suggested as a risk factor for developing cardiovascular and renal diseases in humans and experimental animals. As a major sex hormone, progesterone was reported to compete with cardiotonic steroid binding to Na/K-ATPase. Our previous publication demonstrated that cardiotonic steroids (e.g., marinobufagenin) play an important role in the development of experimental uremic cardiomyopathy. We also observed that the putative mineralocorticoid antagonists, spironolactone and its major metabolite canrenone, antagonize binding of cardiotonic steroids to Na/K-ATPase in a competitive manner and also ameliorate experimental uremic cardiomyopathy induced by partial nephrectomy. The lived experiences of seven individuals who are deaf blind (DB) were explored through the use of semi-structured face-to-face interviews. Two of the emerging themes from this phenomenological analysis were: (1) the daily challenges faced by the participants (e.g., difficulties with communication; compromised activities of daily living; lack of independence; and (2) their lack of preparedness for the future resulting from the uncertainties associated with their degenerative diseases. The findings from this study clearly articulate the importance of intervenor services from the perspective of these individuals.
