Traumatic brain injury (TBI) affects 3 out of every 1000 Americans each year, and is the leading cause of morbidity and mortality after trauma, accounting for as many as 56,000 deaths per year (Dutton and McCunn, 2003). The Centers for Disease and Control and Prevention found that TBI most commonly occurs in adolescents and young adults aged from 15 to 24 years and in the elderly (75 years and older). Introduction:Aggressive fibromatosis is a benign tumor, thought to arise from deep musculoaponeurotic structures, rarely found in the head or neck. However, when it does occur in the head and neck region, it tends to be more aggressive and associated with significant morbidity, which may be attributed to the vital vascular, neurological or anatomical structures in close proximity. Case Presentation: We report the case of a 39-year-old Pakistani man who presented with a two-month history of a lump on the right side of his neck. The mass was excised and histopathological analysis revealed a case of aggressive fibromatosis.

Despite the resurgence of interest in policy implementation, communities, community-based coalitions, and other community- based groups have had mixed success in effecting state and local health policy changes. However, policy changes have been deemed more effective than individual behavior change programs. The creation of a policy advocacy framework may simplify the policy analysis process and arm community groups with practical tools to select, modify, and promote effective public health policies. This paper applies a hypothetical framework for equipping community groups to apply an evidence base to focus policy advocacy efforts systematically.

Purpose: Oral propranolol has become a promising treatment of capillary hemangiomas (CHs) despite concerns of side effects associated with systemic beta-blockers. The objective of this study was to investigate the distribution of propranolol in periocular tissues and in plasma after topical application of propranolol as compared with intravenous and oral administration of propranolol.

Methods: Each rabbit received propranolol as ophthalmic solution (1%) in one eye (1.5 mg dose), intravenous injection (1.5 mg dose), or commercially available propranolol oral solution (5 mg dose). Perbandingan Rapid Diagnostic Test Dan Pemeriksaan Mikroskopik Pada Diagnosis Malaria, Lombok Siahaan

Penelitian ini dilakukan untuk membandingkan penggunaan Rapid diagnostic test dengan pemeriksaan mikroskopik (apusan darah) dalam menegakkan diagnosis malaria. Penelitian uji diagnostik ini dilakukan di Kabupaten Nias Selatan dan Kotamadya Sabang, Sumatera Utara. Apusan darah diwarnai dengan larutan Giemsa 10% dan dibaca oleh pemeriksa yang sudah terlatih. Rapid diagnostic test dilakukan dengan menggunakan parascreen. Uji diagnostik rapid diagnostic test secara umum diperoleh hasil yaitu sensitivitas 63,8%; spesifisitas 100%; Positive Predictive Value (PPV) 100%; dan negative predictive value (NPV) 93,5%. Penelitian ini bertujuan untuk mengetahui situasi pelayanan KIA dan mengkaji faktor perilaku sebagai penyebab masalah KIA di Provinsi Riau. Riset ini menggunakan rancangan studi deskriptif dengan data kuantitatif dan kualitatif mengenai indikator keberhasilan pelayanan KIA. Hasil penelitian ini menunjukkan bahwa indikator-indikator kinerja pelayanan KIA ada yang tercapai, ada pula yang tidak. Jumlah penderita kanker serviks di Rumah Sakit Umum Pemerintah Dr. Wahidin Sudirohusodo pada tahun 2007 sebanyak 231 kasus, menurun menjadi 220 kasus tahun 2008 dan menurun sebanyak 167 kasus pada tahun 2009. Tujuan penelitian ini adalah untuk melihat besar risiko kejadian kanker serviks terhadap penggunaan pembalut, penggunaan sabun, status ekonomi, dan pasangan pria yang tidak disirkumsisi. This study describes and validates a new method for metagenomic biomarker discovery by way of class comparison, tests of biological consistency and effect size estimation. This addresses the challenge of finding organisms, genes, or pathways that consistently explain the differences between two or more microbial communities, which is a central problem to the study of metagenomics. We extensively validate our method on several microbiomes and a convenient online interface for the method is provided at http://huttenhower.sph.harvard.edu/lefse/.

Prescribing information of nesiritide mandates priming

of intravenous tubing with 25 mL of nesiritide prior to

connecting the intravenous (IV) line to the patient, since

the drug may partially absorb to the line. Thus, 10% of

a reconstituted vial is wasted, with a cost of $40-50 per

line used. No study has quantified the binding effect of

nesiritide to intravenous tubing, tested binding

properties of different materials, or analyzed binding

effect of central lines, where priming cannot occur.

Furthermore, prescribing information states that

nesiritide must not be administered through a central

heparin-coated catheter, since it may bind to heparin. …

Feedback from faculty members and residents is a critical component

of undergraduate medical education. Little is known about how

students assess the adequacy of feedback from faculty and residents.

Purpose

To report medical students’ evaluations of clinical teacher feedback

and to determine if students assess feedback given by residents

differently than they assess feedback given by faculty.

Methods

Data were retrospectively collected from medical students’ responses

to end-of-clerkship questionnaire items that evaluated feedback from

residents and attending physicians. The mean ratings of faculty

feedback in four clerkships were compared with the mean ratings of

resident feedback within each academic …

Dose equivalency of metoprolol and carvedilol for beta-

1 blockade remains subject of ongoing debate. The

degree of beta-1 blockade is best assessed in vivo by

blunting of the exercise induced heart rate. Accordingly,

we examined baseline and peak exercise heart rate in

subjects with chronic heart failure (CHF) treated with

carvedilol or metoprolol. To correct for possible

differences in norepinephrine (NE) release due to beta-2

blockade, we also measured NE levels at baseline and

peak exercise.

Mr. B is an 84 year-old African-American male veteran

with a history of type 2 diabetes and hypertension who was

admitted to the VA hospital for hypertensive urgency. In

the ICU, he became unresponsive and pulseless. Telemetry

showed “ventricular fibrillation” which spontaneously

converted to sinus rhythm. An echocardiogram showed an

ejection fraction of 60%. An ECG performed prior to the

A 48 year-old male with a past medical history of gout,

degenerative disc disease, and T cell lymphoma treated

with chemotherapy and autologous peripheral blood

stem cell transplant, currently in remission, presented

seven days status-post a non-myeloablative allogeneic

transplant with an acute onset of excruciating left hip

and groin pain and a low-grade fever. The physical exam

did not reveal any obvious cause of his extreme pain.

A 43 year-old Chinese woman who

immigrated to the United States in

1994 was seen because of several years

of chronic, intermittent coughing

that was productive of voluminous,

thick, green sputum with occasional

blood streaking. The cough was

accompanied by right anterior

pleuritic chest pain. She had no

fevers, chills, or night sweats. Her

medical history included childhood

pneumonia at the age of 18 months,

without esophago-gastric varices as an unusual cause

of gastrointestinal bleeding in a patient with chronic

liver disease.

The patient is a 47 year-old man who presented to the

hospital with several days of back pain and weakness.

The work-up of his symptoms revealed a L3-L4

phlegmon on MRI of the lumbar spine. A biopsy of this

lesion yielded focal nonviable bone.
