The Clinical Nurse Leader (CNL) is a master-prepared generalist accountable for patient outcomes through application of evidenced-based practice at the microsystem level. Accelerated nursing programs are educating entry level nurses as CNLs in a novel Model C program. This doctoral project evaluates entry-level master’s CNL graduates with the CNL end-of-program competencies to determine whether these graduates are able to have positive effects on patient, systems, and leadership outcomes in clinical settings.

Although scientific evidence indicates that children with chronic illness have more mental health issues than their healthy peers, many controversies and gaps in the literature exist. It is imperative that an understanding of the effects of chronic illness upon the mental health status of children and adolescents be undertaken. Youth suicide continues to be one of the leading causes of death in the United States. Nation wide it is the third leading cause of death in the 10-24 year old age group. The rate of suicide varies somewhat from state to state. In the state of Kentucky, youth suicide is the second leading cause of death. This article describes a grass roots, community based program for youth suicide prevention and its impact on the community. A significant number of adolescent pregnancies are unintended and therefore providing comprehensive counseling is important even before the initiation of sexual activity. By understanding the risk factors the patient may further be provided delivery of appropriate health prevention/promotion services. It is also important for the health provider to continue to be an integral part of the life of the teenager and her child for an optimal development and growth.

Children diagnosed with Autism Spectrum Disorders (ASD) tend to show under-activation of the right fusiform face area of the ventral temporal cortex when viewing emotional faces, which may explain their affect comprehension deficits. This left hemisphere dominance, indicative of a piecemeal processing strategy, has been shown a less effective method of understanding true emotion. The present study aimed to condition the left-visual-field-to right-FFA pathway by allowing children with ASD to work through an emotion-matching computer program. Objective: To determine the frequency, level and flow patterns of lower limb arterial insufficiency in diabetic patients on Doppler ultrasound study.
Study Design: Cross-sectional study.
Place and Duration of Study: Radiology Department, Civil Hospital, Karachi, from February 2007 to September 2008.
Methodology: One hundred adult diabetic patients with suspected peripheral vascular insufficiency irrespective of gender were included. Demographic data, presenting complaints, treatment history, and level of HbA1c were recorded. Doppler evaluated arterial status and ankle brachial index (ABI) were recorded on proforma. The pulse transit time (PTT) has been investigated as an early noninvasive marker of hypovolemia, reflecting a combination of changes in the pre-ejection period (PEP) and vascular transit time (VTT). The use of photoplethysmography (PPG) has limited the analysis to the macrovascular peripheral circulation by nature of the detection mechanism of the PPG itself. A representative, cross-sectional clinical skills assessment of 163 mid-level, rural-based, government health care workers was conducted in four districts of Nepal in June 2007. All Health Assistants and Auxiliary Healthcare Workers within the target districts were scored using checklists of standardized key skills in clinical encounters with model patients or clinical models. Participant scores were reported as a mean percentage in adult medicine 28(%), pediatric medicine 56(%), maternity medicine 35(%), orthopedic medicine 45(%), clinical procedures 59(%), and management 46(%). Purpose: The purpose of this investigation was to pilot the use of and explore the feasibility of an individualized professional learning plan (PLP) designed to guide the continuing education of athletic trainers (AT) based on their individual learning needs and explore the participants’ perceptions of its usefulness and feasibility. Obesity is a common problem that can lead to numerous comorbid conditions, including Type 2 diabetes. Currently, there are few pharmacologic options available to help obese patients lose weight. Pramlintide is an injectable, amylin analogue that is indicated in patients with Type 1 and Type 2 diabetes for use in conjunction with insulin to improve glycemic control. In addition to helping patients decrease hemoglobin A1c levels, pramlintide has also been shown to minimize weight gain, especially in patients with Type 2 diabetes. BACKGROUND: Antimicrobial lock therapy (ALT) may be considered as adjunctive therapy in the treatment of catheter-related bloodstream infections (CRBSI) when catheter removal is not a favorable option. OBJECTIVE: To evaluate the outcomes associated with ALT as adjunctive treatment of CRBSI. METHODS: This was a 24-month retrospective case series analysis evaluating patients treated for more than 24 hours with ALT. The primary outcome was blood culture sterilization for 30 days posttherapy. The impact of ALT duration and time to initiation on central venous catheter (CVC) salvage were evaluated. The single case study presents the story of the implementation of a three year Coordinated School Health Program (CSHP) at a rural North Mississippi school. The school was recognized by the Mississippi Department of Education, Office of Healthy Schools, as one of ten selected model Mississippi healthy schools. Six days after transfer of eyed stage rainbow trout (Oncorhynchus mykiss) eggs into an incubator, egg mortality occurred during the winter of 2007. During the outbreak, approximately 30% of the eggs died. Rheocricotopus fuscipes (Kieffer, 1909) and Eukiefferiella claripennis (Lundbeck, 1898) (subfamily Orthocladiinae), Chironomid midge larvae were observed on the eggs by examining the eggs under the microscope. Histological sections of the infested eggs exhibited midge bite marks. This is the first report of Chironomid midges preying on fish eggs.

The effects of an anionic versus cationic (control) diet on plasma mineral levels and health status were investigated in 24 dry pregnant Holstein-Friesian cows during the last 3 weeks of pregnancy. The cows were in their third or fourth parity, had a moderate level of milk production in their previous lactation, and did not have a history of milk fever. The anionic diet significantly improved plasma calcium status, as compared to the control (2.18 vs. 1.99 mmol L^{-1}) and nonsignificantly reduced the incidence of subclinical hypocalcemia (1/12 vs. 5/12) without affecting other periparturient diseases. Extracellular calcium-sensing receptor (CaSR) is a recently discovered G-protein-coupled receptor that may play a key role in regulating parathyroid hormone secretion and calcium homeostasis. In the present study, based on the reported sequence of the chicken CaSR gene (GenBank accession: XM_416491), specific primers were designed and synthesized, and CaSR gene mRNA expression and tissue distribution in chickens were analyzed using semiquantitative reverse transcriptase polymerase chain reaction (RT-PCR) analysis. It was observed that CaSR mRNA was expressed in the kidney and parathyroid to a greater degree than in the small intestine, oviduct, and brain (P < 0.01). Sequence analysis of the PCR products obtained from the 5 tissues showed that these cDNAs shared 100% homology with each other and the corresponding region of the reported CaSR cDNA. The results may provide a basis and additional insight for understanding the physiological function of CaSR in chickens.

The present study examined the haematological effects of ichthyophthiriasis in common carp. Blood was collected from 4 groups of juvenile carp: 1. healthy; 2. with minor symptoms of ichthyophthiriasis; 3. with severe symptoms of ichthyophthiriasis (moribund); survivors of ichthyophthiriasis after a 3-week recovery period. Blood was subjected to a standard haematological procedure and the following parameters were evaluated: haematocrit (Ht), haemoglobin concentration (Hb), erythrocyte (RBC) and leukocyte (WBC) counts, mean cell volume (MCV), mean cell haemoglobin (MCH), and mean cell haemoglobin concentration (MCHC).
