Nurse educators need to be well-prepared with essential competencies, to prepare nurses for their future in health care. Nurses who are recruited to teach, must meet at least minimal competencies and be provided appropriate resources to assume their roles in academia. The recently published National League for Nursing (NLN) Core Competencies for Nurse Educators (2005) identify the skills, knowledge and attitudes necessary for both novice and experienced faculty.

Single lung ventilation in small children is extremely challenging. In this case, a paediatric bronchial blocker was successfully inserted in a 19-month-old child to provide single lung ventilation using a modified insertion technique. It provided excellent working conditions during thoracotomy and our method of insertion may help in reducing the cost.

Glaucoma drainage implants are frequently used in children with severe forms of glaucoma, and tube extenders are sometimes required to compensate for ocular growth or when the intraocular portion of the tube is thought to be too short. We report 2 cases of spontaneous disconnection of a tube extender, occurring 8 months after insertion in one case and 3 years later in the other. In both cases the tube was removed and the patient treated with endoscopic diode laser photocoagulation.

Studies have suggested a possible role for Blastocystis hominis and Dientamoeba fragilis in the etiology of irritable bowel syndrome (IBS). We studied the prevalence of B. hominis and D. fragilis in patients with IBS-diarrhea (IBS-D). Three hundred and thirty patients were enrolled, 171 (52%) with IBS-D and 159 (48%) were controls, respectively. Stool microscopy, culture, and polymerase chain reaction (PCR) for B. hominis and D. fragilis were done. B. hominis was positive by stool microscopy in 49% (83/171) of IBS compared to 24% (27/159) in control (p < 0.001). B. hominis culture was positive in 53% (90/171) in IBS compared to 16% (25/159) in control (p < 0.001). B. hominis PCR was positive in 44% (75/171) in IBS compared to 21% (33/159) in control (p < 0.001). D. fragilis microscopy was positive in 3.5% (6/171) in IBS-D compared to 0.6% (1/159) in control (p = 0.123). D. fragilis culture was positive in 4% (7/171) in IBS compared to 1.3% (2/159) in control (p = 0.176). D. fragilis PCR was positive in 4% (6/171) in IBS-D compared to 0% (0/159) in control (p = 0.030). B. hominis is common, while D. fragilis was less prevalent in our patients with IBS-D. B. hominis and D. fragilis culture had a better yield compared to stool microscopy and PCR.

Iatrogenic bile duct injury (BDI) is an uncommon but serious complication of cholecystectomy, with identified risk factors of acute cholecystitis, male sex, older age, and aberrant biliary anatomy. The Nationwide Inpatient Sample (1998-2006) was queried for cholecystectomy performed on hospital day 0 or 1. Bile duct injury repair procedure codes were used as a surrogate for BDI. We identified 377,424 patients who underwent cholecystectomy, with 1124 BDIs (0.3%). On multivariate logistic regression analysis, Asian race/ethnicity was a significant risk factor for BDI (odds ratio [OR], 2.26; 95% confidence interval [CI], 1.59-3.23; P < .001). This persisted for laparoscopic (OR, 2.62; 95% CI, 1.28-5.39; P = .009) and open (2.21; 1.59-3.07; P < .001) cholecystectomies. No other race/ethnicity was identified as a risk factor for BDI. We report a new finding that Asian race/ethnicity is a significant risk factor for BDI in laparoscopic and open cholecystectomies.

BACKGROUND: As the number of angina patients with severe coronary artery disease not amenable to revascularization increases, new therapies will be developed. How patients with depressed compared to normal left ventricular ejection fraction (LVEF) will respond to new therapies may differ.

HYPOTHESIS: We conducted a retrospective chart review to determine the distribution of LVEF in angina patients with severe coronary artery disease (three-vessel disease with >50% stenosis major epicardial vessels or >50% stenosis left main) not amenable to revascularization.

One of the distinguishing features of fungal cells is their highly polarized model of growth. Both yeast cells and hyphal cells grow by cell surface expansion at specified cortical sites. Although the same general mechanisms are likely to be involved in controlling the establishment of hyphal polarity in budding yeast and filamentous fungi, it is noticeable that hyphal cells are organized in a fundamentally different manner to yeast dells. HIV and AIDS remain highly stigmatised. Modified directly observed therapy (m-DOT) supports antiretroviral treatment (ART) adherence but little is known about its association with perceived stigma in resource-constrained settings. In 2003, 234 HIV-infected adults enrolled in a two-arm randomised trial comparing a health centre-based m-DOT strategy with standard self-administration of ART. Data on perceived stigma were collected using Berger's HIV stigma scale prior to starting ART and after 12 months. This was a secondary analysis to examine whether perceived stigma was related to treatment delivery. Perceived stigma scores declined after 12 months of treatment from a mean of 44.9 (sd=7.6) …

The overarching aim of this research was to examine potential boundary conditions to situation model construction (Experiment 1) and narrative-based persuasion (Experiment 3). Variables such as narrative perspective (i.e., 2nd or 3rd person) and matched characteristics with the reader (i.e., participant-protagonist gender match) were first examined using situation model updating (Experiment 1) and behavioral measures (Experiment 3) as dependent measures. It was expected that situation model updating would be more likely for narratives written in the 2nd person perspective and with a participant-protagonist gender match. The purpose of this research was to characterize the neutron energy distribution and flux emitted from the UNLV plutonium-beryllium source, serial number MRC-N-W PuBe 453. This was accomplished through the use of the MCNPX/5 Monte-Carlo particle transport code to simulate radiation interactions within the physical environment of the source and its surroundings. The moderating drum currently containing the source as well as all of the sampling ports were accurately modeled in MCNPX/5. The study of blood vessel growth and remodeling is a complex endeavor. Hypoxia, the lack of oxygen in a tissue, is known to stimulate angiogenesis (the growth of new blood vessels), and have little effect on arteriogenesis (the enlargement of existing blood vessels). However, the role of hypoxia in vessel function is unknown, but may be determined using the results and methods developed in this experiment.

Existing front-line vector control measures, such as insecticide-treated nets and residual sprays, cannot break the transmission cycle of Plasmodium falciparum in the most intensely endemic parts of Africa and the Pacific. The goal of malaria eradication will require urgent strategic investment into understanding the ecology and evolution of the mosquito vectors that transmit malaria. Priority areas will include understanding aspects of the mosquito life cycle beyond the blood feeding processes which directly mediate malaria transmission. Global commitment to malaria eradication necessitates a corresponding longterm commitment to vector ecology.

Understanding what the trypanosome pathogens are, their vectors and mode of transmission underpin efforts to control the disease they cause in both humans and livestock. The risk of transmission is estimated by determining what proportion of the vector population is carrying the infectious pathogens. This risk also depends on the infectivity of the trypanosomes to humans and livestock. Most livestock pathogens are not infective to humans, whereas the two sub-species that infect humans also infect livestock. As with other infectious diseases, we can therefore trace the foundation of many continuing disease control programs for trypanosomiasis to the discovery of the …

The mechanical properties and clinical performance of Turkom-Cera system were evaluated in this study. The mechanical properties evaluated were; flexural strength, microhardness, shear bond strength, fracture resistance and marginal integrity. The biaxial flexural strength and hardness of Turkom-Cera compared to two other allceramic systems (In-Ceram and Vitadur-N) were investigated. The Turkom-Cera exhibited significantly higher flexural strength (506.8 MPa) than In-Ceram (347.4 MPa) and Vitadur-N (128.7 MPa) ceramic materials. However, In-Ceram core has significantly higher hardness (1116.2 VHN) than Turkom-Cera (1002.1 VHN) and Vitadur-N (812.8 VHN) all-ceramic materials. Objectives: This study examined the relationship between substance use disorder (SUD) and intimate partner violence screening (IPV) and management practices in the emergency department (ED). Methods: This was a retrospective cohort study of adult ED patients presenting to an urban, tertiary care teaching hospital over a 4-month period. An automated electronic data abstraction process identified consecutive patients and retrieved visit characteristics, including results of three violence screening questions, demographic data, triage acuity, time of visit, and ICD-9 diagnosis codes. The purpose of this study was to determine if a difference exists in hip power asymmetry between community-dwelling older adult fallers and non-fallers. Hypothesis: fallers would be more asymmetrical than non-fallers. Participants: 21 non-fallers (10 females, 11 males) and 18 fallers (14 females, 4 males) over the age of 65 (76.5 ± 6.9yrs). Method: Isokinetic peak torque during flexion, extension, abduction, and adduction at four velocities was recorded as measures of leg power. The current median survival of patients with glioblastoma multiforme (GBM), the most common type of glioma, remains at 14.6 months despite multimodal treatments (surgery, radiotherapy and chemotherapy). This research aims to study the feasibility of photothermal ablation of glioma using gold nanoshells that are heated upon laser irradiation at their resonance wavelength. The novelty of our approach lies in improving nanoshell tumor delivery by loading them in macrophages, which are known to be recruited to gliomas via tumor-released chemoattractive agents.
