NAD is a coenzyme for redox reactions and a substrate of NAD-consuming enzymes, including ADP-ribose transferases, Sir2-related protein lysine deacetylases, and bacterial DNA ligases. Microorganisms that synthesize NAD from as few as one to as many as five of the six identified biosynthetic precursors have been identified. De novo NAD synthesis from aspartate or tryptophan is neither universal nor strictly aerobic. Salvage NAD synthesis from nicotinamide, nicotinic acid, nicotinamide riboside, and nicotinic acid riboside occurs via modules of different genes. Nicotinamide salvage genes nadV and pncA, found in distinct bacteria, appear to have spread throughout the tree of life …

Differentiated pancreatic acinar cells expressing endogenous levels of mutant K-Ras do not spontaneously develop pancreatic ductal adenocarcinoma (PDAC). However, we hypothesized that acinar cells would develop PDAC in the presence of Ras activity levels mimicking those of human tumor cells.We measured Ras activity in PDAC cells from mice and humans using a Raf pull-down assay. We compared the effects of acinar cell expression of mutant K-Ras at endogenous and elevated levels on Ras activity and on the development of PDAC.

In recent years, important gains and changes have been observed in the life of teenagers with Down syndrome (DS) with increased inclusion into society. This review will discuss adolescence and sexuality in teenagers with DS from a descriptive study of 50 patients with DS between the ages of 10 and 20 years. The mean age was 13.5 years, 50% females; 86% went to school with 62.2% in school for over six years. Of the patients that attended school, 60% went to special education school and only 10% read and wrote correctly. In an evaluation of autonomy, 66% took showers, 78% …

Diabetes mellitus is estimated to affect ∼24 million people in the United States and more than 150 million people worldwide. There are numerous end organ complications of diabetes, the onset of which can be delayed by early diagnosis and treatment. Although assays for diabetes are well founded, tests for its complications lack sufficient specificity and sensitivity to adequately guide these treatment options. In 2005, the Texas State Legislature passed Senate Bill 42 (SB42), which required public middle school students (grades 6–8) to participate in 30 minutes of daily structured physical activity. The purpose of this study was to assess awareness of and adherence to SB42 in Texas middle schools, and to assess the impact of SB42 on the frequency and quality of structured physical activity.

Vibrio cholerae is highly motile by the action of a single polar flagellum. The loss of motility reduces the infectivity of V. cholerae, demonstrating that motility is an important virulence factor. FlrC is the sigma-54-dependent positive regulator of flagellar genes. Recently, the genes VC2206 (flgP) and VC2207 (flgO) were identified as being regulated by FlrC via a microarray analysis of an flrC mutant (D. C. Morris, F. Peng, J. R. Barker, and K. E. Klose, J. Bacteriol. 190:231-239, 2008). FlgP is reported to be an outer membrane lipoprotein required for motility that functions as a colonization factor. The study reported …

Deficiency of 21 hydroxylase enzyme deficiency (21OH) activity accounts for 90% cases of congenital adrenal hyperplasia (CAH). This results in deficient cortisol, increased ACTH, adrenal hyperplasia and increased adrenal androgen secretion. There is marked virilization in genetic females which is the hallmark of this disorder. Genetic heterogeneity in 21 OHD is well recognized, and both severe and mild forms occur. Vitiligo, as a cosmetic problem, is a source of great embarrassment to dark skinned people. An attempt was made by the Central Council for Research in Homoeopathy (CCRH) to ascertain the role of homoeopathic therapy and to identify homoeopathic medicines useful in the management of Vitiligo. Here is a case from the ongoing clinical research study on vitiligo at Regional Research Institute (Homoeopathy), Puri, treated by Sulphur. The case was followed up over a period of 2 years with marked improvement in white patches which is evident from the photographs.

Recent theoretical writings suggest that the ineffective regulation of negative emotional states may reduce the ability of women to detect and respond effectively to situational and interpersonal factors that increase risk for sexual assault. However, little empirical research has explored this hypothesis. In the present study, it was hypothesized that prior sexual victimization and negative mood state would each independently predict poor risk recognition and less effective defensive actions in response to an analogue sexual assault vignette. The authors describe a seven-step consensus development process used to create the two most recent editions of the Academic Pediatric Association’s (APA’s) educational guidelines for pediatric residency. The 1996 (printed) and 2004 (online) editions of the guidelines were designed as flexible tools to help residency programs meet changing accreditation requirements by providing lists of goals and objectives and objective-based evaluation tools. Purpose

To evaluate pediatric educators’ use of and satisfaction with the Academic Pediatric Association’s Educational Guidelines for Pediatric Residency.

Method

The authors used customized programming to document all registered users and downloaded files from the Guidelines Web site for a 30-month period after site completion. An online survey of volunteer users was conducted.

Results

Between July 2005 and December 2007, 1,747 individuals registered on the Web site, and 8,754 files were downloaded. Registrants who downloaded files (n = 1,239) represented 97% of the pediatric residency programs in 2008 that were approved by the Accreditation Council for Graduate Medical Education. During …

Congenital cystic adenomatoid malformation of the lung is a rare lesion that typically manifests as severe progressive respiratory distress in the neonate secondary to expansion of the affected lung. We present a neonate in whom this condition was diagnosed and managed at the Aga Khan University Hospital. In presenting this patient, we hope to highlight the radiological aspects of this condition which, with prompt recognition and management and in the absence of pulmonary hypoplasia, foetal hydrops or other congenital anomalies, has a favourable prognosis.

In Vitro Antibiotic Susceptibility Of Aeromonas Hydrophila Isolated From Disease Ornamental Fish, Malinee Jongjareanjai, Nongnut Assawawongkasem, Nantarika Chansue

Thirty bacterial isolates were derived from ornamental fish patients (n=30) of the Veterinary Medical Aquatic Animal Research Center (VMARC), Faculty of Veterinary Science, Chulalongkorn University. The isolated bacterial strains were identified by using commercial biochemical identification kit (API Kits, bioMerieux® SA France). The antibiotics susceptibility testing was performed by disc diffusion method, using 24 types of antibiotic discs (OXOID, Oxoid Ltd, UK). The result showed that the majority of the isolated bacteria was Aeromonas hydrophila (27/30). In the A. hydrophila susceptibility study, 66.30% of bacteria were antibiotic resistant, 9.35% were intermediately sensitive and 24.35% were sensitive strains against the tested …

Individual change and variation in emotional/behavioral disturbance in children and adolescents with intellectual disability has received little empirical investigation. Based on 11 years of longitudinal data from the Australian Child to Adult Development Study, we report associations among individual differences in level, rate of change, and occasion-specific variation across subscales of theDevelopmental Behavior Checklist (DBC) with 506 participants who had intellectual disability and were ages 5 to 19 years at study entry. Case Report: A 61-year-old-woman with a past medical history significant for hypertension, bipolar disorder, obstructive sleep apnea and chronic obstructive pulmonary disease presented with diffusive lymphadenopathy suspicious for lymphoma. A lymph node in her right groin was biopsied by fine needle aspiration and showed atypical cells, but was not diagnostic. An excisional biopsy of the 2 x 2 cm lymph node demonstrated metastatic Merkel cell carcinoma (MCC). No skin lesions were detected. A CT scan revealed the presence of a large nasopharyngeal mass that following surgical removal was diagnosed as MCC. Treatment with etoposide and cisplatin and radiation were instituted. …

Because of the high costs associated with new graduate nurse turnover, an academic-service partnership developed a nurse residency program that provides a comprehensive support system that spans 15 months. Now in its fourth year, involving more than 50 urban and rural hospitals of varying sizes and geographic locations, the program provides formalized preceptor training, monthly daylong educational sessions, and mentoring by clinical coaches. Given the shortage of nurse educators, we sought to better understand teaching excellence because it is crucial for developing the next generation of nurses. A grounded theory approach was used. The sample included 17 respondents, thought to be excellent teachers, from universities across the United States. Consenting respondents were asked, “What do you do to bring nursing to life with your students?” Using line-by-line coding and the constant comparative method, five major themes emerged: (a) engagement, (b) relevance, (c) student centeredness, (d) facilitation of learning, and (e) dynamic process of becoming an excellent nursing educator. Significant increases in childhood asthma prevalence and mortality has prompted federal and regulatory agencies to work towards a national framework to address childhood asthma. Quality of life is an important component of chronic disease, and the evaluation of healthcare outcomes. There is little discussion in the literature regarding the implications of QoL as a theoretical construct. This study determined the effect of asthma on the QoL of our study population and caregiver’s missed workdays on the child’s QoL scores.
