Introduction: Individualized learning plans (ILPs) are documented personal learning objectives developed by a learner with the help of a facilitator. The Pediatric Residency Review Committee requires residents to accomplish one ILP per year. Faculty members, including program directors, who are newly responsible for facilitating development of resident ILPs may feel unprepared for the task. They are likely unfamiliar with the concepts of ILPs, and most have not created an ILP of their own. Serum prolactinis an indicator of tuberoinfundibulardopamine activity. It is reported to increase in wide variety of mental illnesses. It has close relationship with antipsychotic therapy. However, its relationship with psychopathology and outcome is not clear. Serum prolactinlevel was measured in 30 male and 30 female drug naive patients of schizophrenia. Subsequently, these patients were treated with antipsychotics. The severity of psychopathology at the baseline and subsequent improvement at the end of 3 weeks and 6 weeks was assessed on modified brief psychiatric rating scale (mBPRS). Introduction:We report a case of sero-negative celiac disease in Pakistan. Case Presentation: A 20-year-old female presented with papulovesicular rash for 15 years, diarrhea for 8 years, spasms of hands and twitching of face for 4-5 months. She had mild anemia, low vitamin-D3 and serum calcium. On exclusion of other causes of malabsorption, anti-tissue transglutaminase antibodies (immunoglobulin-A & immunoglobulin-G), anti-endomysial antibodies, total immunoglobulin-A levels and skin biopsy were performed, which were normal. Intestinal biopsy revealed subtotal villous atrophy. Patient was prescribed gluten-free diet, to which she responded with alleviation of symptoms.

Conclusions:

The purpose of this study was to determine if dental aesthetics is perceived by adults to affect an individual’s ability to emerge as a leader. An electronic survey was constructed using facial frontal smiling photographs of 10 patients. Statements were formulated to evaluate four leadership characteristics: intelligence, self-confidence, friendliness, and trustworthiness. Evaluators indicated their level of agreement with each statement for each subject using a 100mm Visual Analog Scale (VAS). Data was obtained from 214 evaluators. Picture ratings were analyzed using repeated-measures mixed-model analyses. Evaluator factors and picture factors were considered when testing for crooked vs. straight teeth differences. There …

Decorin, a member of the small leucine-rich proteoglycan gene family, impedes tumor cell growth by down-regulating the epidermal growth factor receptor. Decorin has a complex binding repertoire, thus, we predicted that decorin would modulate the bioactivity of other tyrosine kinase receptors. We discovered that decorin binds directly and with high affinity (K(d) = approximately 1.5 nM) to Met, the receptor for hepatocyte growth factor (HGF). Binding of decorin to Met is efficiently displaced by HGF and less efficiently by internalin B, a bacterial Met ligand. Interaction of decorin with Met induces transient receptor activation, recruitment of the E3 ubiquitin ligase …

The annual budget for drug advertising in the United States exceeds that for all undergraduate and postgraduate medical education and comes close to the entire budget of the National Institutes of Health. In 2008 drug advertisers spent $4.7 billion, down from a record $4.8 billion in 2006, but up from $2.6 billion in 2002. Since the relaxation of full disclosure rules for television advertising, the amount spent on direct-to-consumer advertisements (DTCA) has soared, leading to increased sales of certain pharmaceuticals, as advertisements are placed in magazines, newspapers, internet websites and on television. The American College of Physicians (ACP) and the Infectious Diseases Society of America (IDSA) have jointly published 3 policy statements on AIDS, the first in 1986 [1], the second in 1988 [2], and the third in 1994 [3]. In 2001, the IDSA created the HIV Medicine Association (HIVMA), and this updated policy paper is a collaboration between the ACP and the HIVMA of the IDSA. Since the last statement, many new developments call for the need to reexamine and update our policies relating to HIV infection. First, there have been major advances …

Perceptions of the relationships between nurses and staff were evaluated from a faculty perspective to uncover the ways that the faculty react, respond, and help to support students. During interviews with faculty, access to practice and clinical unit atmosphere emerged as themes. Faculty described nurses granting access to practice by mentoring students and involving them in the entire care of the patient, or nurses who limited students' access to practice through unprofessional behavior, or "cutting corners." Unit atmosphere influenced learning both positively and negatively depending on inclusion of students, morale, and welcoming behaviors of staff. Faculty acted as guides and …

Permutation tests are useful for drawing inferences from imaging data because of their flexibility and ability to capture features of the brain that are difficult to capture parametrically. However, most implementations of permutation tests ignore important confounding covariates. To employ covariate control in a nonparametric setting we have developed a Markov chain Monte Carlo (MCMC) algorithm for conditional permutation testing using propensity scores. We present the first use of this methodology for imaging data. Our MCMC algorithm is an extension of algorithms developed to approximate exact conditional probabilities in contingency tables, logit, and log-linear models. An application of our non-parametric …

BACKGROUND: The anti-EGFR monoclonal antibody cetuximab is used in metastatic colorectal cancer (CRC), and predicting responsive patients garners great interest, due to the high cost of therapy. Mutations in the KRAS gene occur in ~40% of CRC and are a negative predictor of response to cetuximab. However, many KRAS-wildtype patients do not benefit from cetuximab. We previously published a gene expression predictor of sensitivity to erlotinib, an EGFR inhibitor. The purpose of this study was to determine if this predictor could identify KRAS-wildtype CRC patients who will benefit from cetuximab therapy.

In the fall of 2006, a twenty-three year old male tractor-trailer driver who was hauling coal, died when a rear outer tire on the trailer failed. After the tire failed, the trailer slipped off the right side of the road, overturned, and went over an embankment. The trailer remained attached to the tractor as both went over the embankment, flipping the entire unit upside down. A witness called emergency medical services who arrived and called the county coroner. The coroner arrived and declared the driver dead at the scene. Background: Norepinephrine (NE) modulates the responsiveness of macrophages to proinflammatory stimuli through the activation of adrenergic receptors (ARs). Being part of the stress response, early increases of NE in sepsis sustain adverse systemic inflammatory responses. The intestine is an important source of NE release in the early stage of cecal ligation and puncture (CLP)-induced sepsis in rats, which then stimulates TNF-a production in Kupffer cells (KCs) through the activation of the a2-AR. Introduction: Instrumental variable (IV) methods have been used in econometrics for several decades now, but have only recently been introduced into the epidemiologic research frameworks. Similarly, Mendelian randomization studies, which use the IV methodology for analysis and inference in epidemiology, were introduced into the epidemiologist's toolbox only in the last decade.

Analysis: Mendelian randomization studies using instrumental variables (IVs) have the potential to avoid some of the limitations of observational epidemiology (confounding, reverse causality, regression dilution bias) for making causal inferences. Purpose: The objective of this study was to determine if regular volunteering during adolescence was associated with a delay in first pregnancy until adulthood. Method: Data for this analysis were drawn from Wave III of the National Longitudinal Study of Adolescent Health (Add Health). Logistic regression was used, controlling for demographic characteristics and pregnancy intention, to estimate the independent contribution of volunteering during adolescence to a delay in first pregnancy until after age 18. Results: One-third of respondents reported regularly volunteering during adolescence. Volunteering significantly differed among those who had a teen pregnancy compared to those who delayed pregnancy until …

Creatine Kinase (CK) is used as a measure of exercise-induced muscle membrane damage. During acute eccentric (muscle lengthening) exercise, muscle sarcolemma, sarcoplasmic reticulum, and Z-lines are damaged, thus causing muscle proteins and enzymes to leak into the interstitial fluid.

Strenuous eccentric exercise produces an elevation of oxygen free radicals, which further increases muscle damage. Muscle soreness and fatigue can be attributed to this membrane damage. Estradiol, however, may preserve membrane stability post-exercise (Brancaccio, Maffulli, & Limongelli, 2007; Carter, Dobridge, & Hackney, 2001; Tiidus, 2001). Current perceptions about nurses’ roles and responsibilities are examined in this study, specifically relating to adolescent inpatient MHNs. Psychiatrists and psychiatric advanced practice registered nurses (APRNs), who work with MHNs and have also published scholarly psychiatric articles, were contacted to request their participation in an anonymous survey hosted by SurveyMonkey.com. This research was conducted to examine the stereotypes that exist against nurses within the health care profession itself, as compared to the pre-existing stereotypes displayed by the media’s view of nurses. Autism spectrum disorders (ASDs) are classified as pervasive developmental disorders characterized by social, communicative, and behavioral impairments. According to formal and informal reports, children with ASD present with receptive and expressive language delay. Joint attention (JA: the behavior that occurs when two individuals focus on the same object or event) has been identified as a possible marker of delayed language development in children with ASD. In this study, the JA behaviors in children with ASD were contrasted with initially language-matched typically developing (TYP) children across three visits.

There is increasing evidence that childhood victimization and attachment disruptions impact a child’s development. In this study, children and adolescents from an outpatient psychiatric clinic were assessed, measuring history of trauma, history of out-of-home placement, initial diagnoses, and CBCL internalizing and externalizing problem scores. Multiple regression analyses showed that both violent abuse trauma (physical/sexual abuse) and victim trauma (physical abuse/sexual abuse/witnessing domestic violence/witnessing community violence) are prevalent among patients with externalizing severity problems; concluding that diagnosis alone may not account for a history of victimization, but externalizing problem severity does. Mild Cognitive Impairment- Amnestic Subtype (MCIa) is a putative prodromal stage of Alzheimer’s Disease (AD) characterized by focal deficits in episodic verbal memory. Less is known about relative deficits in visuospatial learning, although there is ample evidence indicating involvement of the hippocampus in visuospatial learning, as well as hippocampal degeneration in early AD. The aim of this study was to better characterize the components of working memory dysfunction in people with MCIa to increase the ability to reliably diagnose this disease. Although many areas of the brain lose their regenerative capacity with age, stem cell niches have been identified in both the subventricular zone (SVZ) along the lateral walls of the lateral ventricles and the subgranular zone (SGZ) of the dentate gyrus (Gage, 2000; Alvarez-Buylla et al., 2001; Alvarez-Buylla and Lim, 2004). The SVZ niche utilizes many mechanisms to determine the migration patterns of neuroblasts along the RMS into the olfactory bulb, one being Eph/ephrin signaling (Conover et al., 2000; Holmberg et al., 2005).
