While musculoskeletal problems are leading sources of disability, there has been little research on measuring the number of functionally limiting musculoskeletal problems for use as predictor of outcome in studies of chronic disease. This paper reports on the development and preliminary validation of a self administered musculoskeletal functional limitations index.

A Health Insurance Exchange: Prototypes And Design Issues, Mark Merlis

National Health Policy Forum

Many reform proposals call for the creation of one or more health insurance exchanges, intermediaries that can help individuals or small employers navigate the insurance market. An exchange might be public or private, national, or local. It might serve simply as a clearinghouse for plan information or could play an active role in setting benefit packages, choosing high-quality plans, and negotiating premium rates. This paper begins with a summary of recent experience with insurance exchanges and similar systems. It then reviews basic issues in the design of an exchange.

Non-group health insurance is coverage that individuals purchase on their own rather than as part of a group. Most states currently permit non-group insurers to underwrite, a process whereby an insurer assesses the health and other characteristics of individuals to determine their likely utilization of health services or risk; insurers then use this assessment to determine whether they will offer coverage and the premium they will charge. Policymakers have identified underwriting and related practices in non-group markets as a target for reform to enable broader access for the currently uninsured. The reported prevalence of decalcification in orthodontic patients varies from 2 to 96% mainly due to the lack of a standard examination technique. The aims of this study were: 1) to determine the prevalence of white spot lesions around brackets using visual examination and the DIAGNOdent; 2) to determine which teeth were the most susceptible to decalcification; and 3) to test the accuracy of the DIAGNOdent by comparing to the visual examination. Aim: The aim of this study is to determine the frequency of single nucleotide polymorphisms (SNPs) in various pattern recognition receptor (PRR) genes, including Toll like receptors (TLR) -2, -4, -9, and CD14 in chronic (CP), localized (LAP) and generalized aggressive (GAP) periodontitis and periodontally healthy (NP) patients in an African American population. This is the first study conducted to determine role of SNPs in TLR genes and CD14 gene in a periodontally-diseased African-American population. Additionally, this is the first study to assess the role of TLR-9 polymorphism in periodontitis patients. METHODS: A total of 130 subjects were involved in the study. Objectives: To review evidence supporting use of fluoroquinolones as first line agents over other antibiotics for treating typhoid and paratyphoid fever (enteric fever).
Design: Meta-analysis of randomised controlled trials. Data sources: Cochrane Infectious Diseases Group specialised register, CENTRAL (issue 4, 2007), Medline (1966-2007), Embase (1974-2007), LILACS (1982-2007), selected conferences, reference lists, and ongoing trial register (November 2007).
Review methods: Trials comparing fluoroquinolones with chloramphenicol, cephalosporins, or azithromycin in culture-proven enteric fever were included. Phonemic categorization is the ability to discriminate and organize speech sounds into categories. This ability begins soon after birth and continues to refine as an individual matures. An association between categorical labeling and phonological awareness has been reported. A strong link between perception and production has been established. The present study examined phonemic categorization of two groups of four listeners. Eight-to ten-year-old children with an articulation disorder were compared with typically speaking peers to determine if the two groups differed in their ability to categorize speech sounds. Indoor and ambient concentrations of 21 Volatile Organic Compounds (including 14 Hazardous Air Pollutants) were measured in the homes of nearly 80 western Montana (Missoula) high school students as part of the ‘Air Toxics Under the Big Sky’ program during the 2004/2005 and 2005/2006 school years. Target analytes were measured using low flow air sampling pumps and sorbent tubes, with analysis of the exposed samples by Thermal Desorption/Gas Chromatography/Mass Spectrometry (TD/GC/MS). The results reported here present the findings of the first indoor/ambient air toxics monitoring program conducted in a semi-rural valley location located in the Northern Rocky Mountain/western Montana region. …

A large woodstove changeout program was carried out in Libby, Montana, with the goal of reducing ambient levels of PM_2.5. This provided researchers the opportunity to measure ambient concentrations of phenolic and polycyclic aromatic hydrocarbons (PAHs) before, during, and after the changeout of nearly 1200 stoves to evaluate the effectiveness of the intervention. Starting in the heating season of 2004/2005 and ending in the heating season of 2007/2008, 19 compounds were measured every three days using a high-volume polyurethane foam (PUF) sampler followed by gas chromatography and mass spectrometry analysis. Methods assessing non-daily smoking are of concern because biochemical measures can not verify self-reports beyond 7 days. This study compares two self-reported smoking measures for non-daily smokers. A total of 389 college students, (48% female, 96% white, mean age of 19) smoking between 1 and 29 days out of the past 30, completed computer assessments in three cohorts with the order of administration of the measures counterbalanced. Values from the two measures were highly correlated. Comparisons of Timeline Follow-Back (TLFB) with the global questions for the total sample of non-daily smokers yielded statistically significant differences (p<.001), albeit small, between measures with the TLFB resulting on average in 2.38 more total cigarettes smoked out of the past 30 days, 0.46 less smoking days, and 0.21 more cigarettes smoked per day. Analyses by level of smoking showed that the discordance between the measures differed by frequency of smoking. Global questions of days smoked resulted in frequent reporting in multiples of five days, suggesting digit bias. Overall the two measures of smoking were highly correlated and equally effective for identifying any smoking in a 30-day period among non-daily smokers.

More than twenty years of research has demonstrated that rural residents are at greater risk of being uninsured compared to urban residents and more recent studies point to problems of underinsurance as well. Most studies have shown that the problems of uninsurance and underinsurance are greatest among rural residents living in smaller communities located further from more urbanized areas. Section I examines recent estimates and changes since 1997 in rural health insurance coverage. The world is guaranteed a certain level of climate change due to the emissions already released into the atmosphere. Therefore, adaptation to climate change is necessary. Increases in diarrheal disease and malnutrition due to climate change are analyzed for Bangladesh. Relative risks determined by the World Health Organization, estimates obtained from agricultural models, and migration estimates are utilized to determine the costs associated with each of these diseases, as well as reductions in rice production and increases in urban migration. We restrict the discussion for two exponential rates, which are commonly used for modeling failure times of components, machines, or systems. Is Animal Cruelty A “Red Flag” For Family Violence?: Investigating Co-Occurring Violence Toward Children, Partners, And Pets, Sarah Degrue, David K. Dilillo

Cross-reporting legislation, which permits child and animal welfare investigators to refer families with substantiated child maltreatment or animal cruelty for investigation by parallel agencies, has recently been adopted in several U.S. jurisdictions. The current study sheds light on the underlying assumption of these policies—that animal cruelty and family violence commonly co-occur. Exposure to family violence and animal cruelty is retrospectively assessed using a sample of 860 college students. Results suggest that animal abuse may be a red flag indicative of family violence in the home. Specifically, about 60% of participants who have witnessed or perpetrated animal cruelty as a child …

Understanding which specific foods easily come to mind when individuals consider categories of dietary behavior may help explain observed patterns of dietary intake. We examined which specific behavioral exemplars are most easily retrieved from memory when individuals consider broad dietary categories. Participants reported specific foods that come to mind when considering high fat foods, low fat foods, and fruits/vegetables. Salient foods differed from those suggested in dietary guidelines, were distinct from those known to be major diet components, and showed a non-trivial number of incorrect responses. Expression of GJA1 (commonly known as connexin43 or Cx43), a major myometrial gap junction protein, is upregulated before the onset of delivery, suggesting an essential role for Cx43-mediated gap junctional intercellular communication (GJIC) in normal uterine contraction during parturition. To determine how a disease-linked Cx43 mutation affects myometrial function, we studied a mutant mouse model carrying an autosomal dominant mutation (Gja1Jrt) in the gene encoding Cx43 that displays features of the human genetic disease oculodentodigital dysplasia. Environmental dispossession disproportionately affects the health of Canada’s Aboriginal population, yet little is known about how its effects are sustained over time. We use a critical population health approach to explore the determinants of health in rural and remote First Nation and Inuit communities, and to conceptualize the pathways by which environmental dispossession affects these health determinants. We draw from narrative analysis of interviews with 26 Community Health Representa- tives (CHRs) from First Nation and Inuit communities across Canada. CHRs identified six health determinants: balance, life control, education, material resources, social resources, and environmental/ cultural connections. CHRs articulated the role …
