Background: Medical students confront significant academic, psychosocial, and existential stressors throughout their training. Mindfulness-based stress reduction (MBSR) is an educational intervention designed to improve coping skills and reduce emotional distress.

Purpose: The purpose of this study was to examine the effectiveness of the MBSR intervention in a prospective, nonrandomized, cohort-controlled study.

Methods: Second-year students (n = 140) elected to participate in a 10-week MBSR seminar. Controls (n = 162) participated in a didactic seminar on complementary medicine. Profile of Mood States (POMS) was administered preintervention and postintervention.

OBJECTIVE:

To examine the impact of introduction of an alcohol-based hand rub on hand hygiene knowledge and compliance and hand colonization of healthcare workers (HCWs) in a long-term-care facility (LTCF).

METHODS:

Two floors of an LTCF participated. Ward A used the hand rub as an adjunct to soap and water; ward B was the control. HCWs' hands were cultured using the bag-broth technique for Staphylococcus aureus, gram-negative bacilli (GNB), Candida, and vancomycin-resistant enterococci (VRE). HCWs completed a questionnaire at baseline and after an educational intervention and introduction of rub.

RESULTS:

Despite advances in neuroimaging technology and neurosurgical treatment, spinal epidural abscess remains a challenging problem. Early diagnosis is difficult and thus treatment is often too late to prevent severe neuro- logical sequelae.A 46 years old male was admitted to our emergency department due to fever and general weakness. Under the impression of sepsis due to Staphylococcus aureus, he was admitted for further antibiotic treatment. Fever subsided one day after parenteral antibiotic usage. Unfortunately, fever recurred three weeks later accompaning with neck pain, cough, dypsnea and skin rash. Chest roentgenogram revealed rapid progressive cardiomegaly. Myxoma is a rare cardiac tumor and also an unusual cause of cerebrovascular accident, which may result in significant permanent neurological deficit. There was no age preference for the development of myxoma. Most of the reported cases in the past were diagnosed between 20 and 50 years of age, and some of them were complicated by young (＜45 y/o) stroke. We presented a 44 y/o man who had ischemic stroke with initial manifestation of left hemiparesis. A cardiac myxoma at left atrium was later detected by transthorasic echocardiography. In this study, food samples were intentionally contaminated with Escherichia coli O157:H7, and then DNA was isolated by using four commercial kits. The isolated DNA samples were compared by using real-time PCR detection of the Shiga toxin genes. The four kits tested worked similarly.

The phase I cytochrome P450 (CYP) isoenzymes have received substantial attention in the pharmacogenetic literature. Researchers are beginning to examine the role of the phase II UDP-glucuronosyltransferase (UGT) enzymes, which produce products that are more water-soluble, less toxic and more readily excreted than the parent compounds. Several reasons may have contributed to neglect of UGTs (compared to CYPs) including: (1) the overlapping activity of UGTs and lack of selective probes; (2) the complexity of the glucuronidation cycle; and (3) the difficulty in developing analytic methods to measure glucuronides. This study explored the meaning of the phenomenon of the midwife's experience of being present with a woman during childbirth. The value and importance of being with a woman during childbirth, specific and unique to midwifery care, is reflected in midwifery philosophies and models of care. This study looked through the lens of midwives who have written poetry about births they have attended to learn more about the experience of being with a woman. Hermeneutical phenomenology was selected for the philosophical methodology and method because it looked at both the phenomenon and the use of language to describe the phenomenon. …

According to a joint report of the World Health Organization and the Pan American Health Organization in September 2002 there were approximately 1.4 million HIV+ people in Latin America, and a further 420,000 HIV+ people in the Caribbean. The number of infections had increased by nearly 10% from the previous year in Latin America, and 16% in the Caribbean. While striking, these figures may obscure the diversity of the HIV epidemic in the region. This grounded theory study delineates the process inpatient psychiatric nurses use to respond to the challenging nursing problem of medicating resistant involuntary patients. Since approximately one third of all admissions to psychiatric units in the United States are involuntary (Durham, 1996), caring for involuntary patients is a significant part of psychiatric nursing. Medication administration is a major treatment modality that is expected in caring effectively for psychiatric patients (American Psychiatric Association (APA), 1994; APA, 1997; Patel & Hardy, 2001). The purpose of this in vitro investigation was to evaluate the antibacterial effect of calcium hydroxide on a mixture of nine bacteria associated with endodontic infections at different time intervals and to examine the survival of the same bacteria when in mixture. A total of 102 teeth were used in the first part of the experiment. Eighty teeth were used in eight experimental groups of 10 each, 16 teeth were used for positive and negative controls, and six teeth were used for scanning electron microscopy (SEM). Despite improved knowledge and techniques of care, there have been signs of worsening morbidity/mortality and a seeming disconnect between physician and patients as regards communication of care strategies. In an attempt to fashion innovative educational strategies to enhance primary care physician (PCP) and caregiver efficiencies in improving patient outcomes (clinical and financial), the "Preceptor" model of one-to-one PCP/asthma specialist has been developed. Objective: To review cases of male breast cancer.
Settings: Histopathology Section, Department of Pathology, The Aga Khan University, Karachi.
Method: Fifty one cases of male breast cancer specimens, received during a period of 10 years, routinely processed and stained with Haematoxylin and Eosin were analyzed. Special stains and Immunohistochemistry were used in difficult cases.
Results: Male breast cancer affected individuals in the sixth and seventh decades of life with a mean age of 56.2 years. Infiltrating ductal carcinoma (IDC) was the predominant type. Skin involvement was seen in 27.45% of the cases. Breast lump was the most common presenting symptom …

Objective: To know the frequency of breast diseases in Pakistani females.

Methods: A retrospective analysis of 3279 breast specimens received over a period of 4 years (1 993-1996) at the department of pathology, the Aga Khan University Hospital.

Results: Out of a total of 3279 breast specimens, common breast lesions included infiltrating duct carcinoma 37%, followed by fibro adenoma 16.95%, fibrocystic change13.96%, mastitis 6.83% and duct ectasia 5.33%. Majority of the cases of infiltrating duct carcinoma were encountered in the 5th and 6th decades of life. Tumour size was 2 or >2 cms. in 93% of cases and 40% of …
