การรักษาทางทันตกรรมในผู้ป่วย ซิสเท็มมิกสเคลอโรซิส : รายงานผู้ป่วย, สุวิมล ปิติพานิช

ซิสเท็มมิกสเคลอโรซิส จัดเป็นความผิดปกติที่เกิดจากภูมิต้านเนื้อเยื่อตนเอง รอยโรคที่พบในความผิดปกติ นี้คือมีภาวะพังผืดขึ้นในหลายอวัยวะ เป็นผลให้ผู้ป่วยเกิดความบกพร่องของสภาพร่างกายและการทํางานของ อวัยวะที่เกิดความผิดปกติเหล่านั้น ซิสเท็มมิกสเคลอโรซิสที่เกิดขึ้นกับอวัยวะของช่องปากในผู้ป่วยมักทําให้เกิด ปัญหาเกี่ยวกับสุขอนามัยในช่องปาก แต่การรักษาทางทันตกรรมในผู้ป่วยกระทําได้ยาก เนื่องจากช่องปากที่แคบ เล็กกว่าปกติ บทวิทยาการนี้เป็นการรายงานผู้ป่วยด้วยโรคซิสเท็มมิกสเคลอโรซิส ซึ่งผู้ป่วยมีปัญหาเกี่ยวกับโรคฟันผุ โรคเหงือกอักเสบและการใส่ฟันปลอมที่ไม่พอดีกับช่องปาก ในบทวิทยาการจะได้กล่าวถึงลักษณะอาการของโรคใน ผู้ป่วย วิธีดําเนินการรักษาทางด้านทันตกรรม การทําความเข้าใจกับผู้ป่วยถึงปัญหาที่ผู้ป่วยประสบจากการเกิดโรค และความสําคัญของการรักษาอนามัยของช่องปาก นอกจากนี้ยังบรรยายถึงปริทัศน์วรรณกรรมของโรคซิสเท็มมิกสเคลอโรซิสอย่างละเอียดไว้ด้วย

วัตถุประสงค์ เพื่อศึกษาผลของสารสกัดจากว่านหางจระเข้ที่มีต่อการเพิ่มจํานวนของเซลล์สร้างเส้นใยและเซลล์ สร้างเคอราติน CCL-25 วัสดุและวิธีการ เซลล์ถูกทดสอบด้วยสารสกัดจากส่วนวุ้นและส่วนยางที่ระดับความเข้มข้นของโปรตีนต่าง ๆ กัน เป็นเวลา 24 ชั่วโมง จากนั้นจํานวนเซลล์จะถูกคํานวณโดยวิธีการย้อมเซลล์ด้วยสีเมทิลีนบลู และผลการทดลองจะถูกวิเคราะห์ทางสถิติแบบ One-way Analysis of Variance ผลการศึกษา สารสกัดจากส่วนวุ้นของว่านหางจระเข้ที่ระดับความเข้มข้นของโปรตีนตั้งแต่ 1-50 ไมโครกรัมต่อ มิลลิลิตร สามารถกระตุ้นการเพิ่มจํานวนเซลล์สร้างเส้นใยอย่างมีนัยสําคัญทางสถิติแต่ไม่มีผลต่อเซลล์สร้างเคอราติน ส่วนสารสกัดจากส่วนยางที่ระดับความเข้มข้นของโปรตีน 20-400 ไมโครกรัมต่อมิลลิลิตร สามารถกระตุ้นการเพิ่ม จํานวนเซลล์ของเซลล์สร้างเส้นใย ในขณะที่ความเข้มข้นของโปรตีนในสารสกัดจากส่วนยางของว่านหางจระเข้ที่ 20 ไมโครกรัมต่อมิลลิลิตร กระตุ้นการเพิ่มจํานวนของเซลล์สร้างเคอราติน อย่างมีนัยสําคัญทางสถิติ (p<0.05) สรุป สารสกัดจากส่วนวุ้นของว่านหางจระเข้ที่ระดับความเข้มข้นของโปรตีน 1-50 ไมโครกรัมต่อมิลลิลิตร และสาร สกัดจากยางที่ระดับความเข้มข้น 20-400 ไมโครกรัมต่อมิลลิลิตร สามารถกระตุ้นการเพิ่มจํานวนของเซลล์สร้างเส้นไย

การให้บริการทางทันตกรรมในคลินิกทันตกรรม โรงพยาบาลคณะทันตแพทยศาสตร์จุฬาลงกรณ์มหาวิทยาลัย, สุวิมล ปิติพานิช, มลิวรรณ วงศ์สิทธาจารย์

วัตถุประสงค์ เพื่อสํารวจข้อมูลของผู้ป่วยที่มารับบริการทางทันตกรรมในคลินิกทันตกรรมบริการ คณะทันต แพทยศาสตร์ จุฬาฯ และนําไปกําหนดเป็นนโยบายและวางแผนการให้บริการที่เหมาะสม และสอดคล้องกับ ความต้องการของผู้ป่วย วัสดุและวิธีการ การศึกษาทําโดยสํารวจจากแฟ้มประวัติและรายงานบันทึกการรักษาของผู้ป่วย ในช่วง 6 เดือน ระหว่างเดือนกรกฎาคม-ธันวาคม พ.ศ. 2542 จํานวน 6,354 ราย นําข้อมูลการให้บริการทางทันตกรรมที่ได้มา คํานวณเป็นร้อยละ และเปรียบเทียบความแตกต่างของชนิดการให้บริการแบ่งตามช่วงอายุและเพศ ผลการทดลองและสรุป พบว่าการให้บริการทางทันตกรรมที่มากที่สุด คือ การอุดฟันคิดเป็นร้อยละ 40.84 การ ขูดหินน้ําลายและรักษาโรคปริทันต์คิดเป็นร้อยละ 23.17 และการบําบัดรักษาเร่งด่วนคิดเป็นร้อยละ 13.95 แสดงว่าผู้ป่วยมีปัญหาเกี่ยวกับโรคฟันผุและโรคปริทันต์มากที่สุด และผู้ป่วยส่วนหนึ่งจะมาหาทันตแพทย์เมื่อปวด จึงควรรณรงค์เรื่องทันตกรรมป้องกันและให้ผู้ป่วยมารับการตรวจฟันทุก 6 เดือน

รายงานผู้ป่วยหญิงวัยกลางคน 2 รายที่มีรอยโรคไลเคนนอยด์ที่บริเวณเยื่อเมือกกระพุ้งแก้มที่สัมผัสกับ วัสดุอุดอมัลกัม การเกิดของโรคเป็นแบบค่อยๆ ดําเนินไป รอยโรคมีลักษณะเป็นฝ้าสีขาวมีอาการปวดแสบปวด ร้อนเมื่อรับประทานอาหารรสจัด การรักษาด้วยการรื้อวัสดุอุดอมัลกัมออก แล้วเปลี่ยนเป็นวัสดุคอมโพสิตเรซิน พบว่ารอยโรคและอาการปวดแสบปวดร้อนหายไป จากการติดตามผลการรักษาเป็นระยะ ๆ เป็นเวลา 1 ปี 6 เดือน ไม่พบรอยโรคปรากฏอีกเลย

Drowning and near drowning is a significant problem nationwide and in San Bernardino County. Drowning in and around the home is the leading cause of accidental death of children under 5 years of age. In the United States, drowning is cited as the second most common cause of death for adolescents. This project was undertaken to help establish a funding source to develop a drowning prevention program in San Bernardino County. A request for assistance was obtained from the California Center for Childhood Injury Prevention and the California Kids' Plates Program.
