The Long-Term Clinical Prognosis In Patients With Anterior Wall And Non-Anterior Wall Acute Myocardial Infarction Referred To Primary Percutaneous Transluminal Coronary Angioplasty, Mingzhong Zhao, Dayi Hu, Tianchang Li, Sanqing Jia

Objective: To investigate the long-term clinical prognosis in patients with anterior wall acute myocardial infarction (AW-AMI) and non AW-AMI (NAW-AMI) referred to primary percutaneous transluminal coronary angioplasty (P-PTCA). Methods: 287 patients with AMI who underwent P-PTCA were divided into AW-AMI group (142 cases) and NAW AMI group (145 cases) according to different sites of AMI. The baseline characteristics and coronary artery lesions of patients were analysed. The primary end points were in-hospital mortality and the major cardiovascular events (MACE) during a mean 17.3±9.8 months follow-up including the occurrences of non-fatal myocardial infarction, non-fatal congestive heart failure, revascularization of target vessels …

Clopidogrel (Plavix®) is an orally administered pro-drug, whose pharmacology and therapeutic role has been extensively reviewed in recent publications.1-5 In essence, it results in selective and irreversible antagonism of ADP-induced platelet aggregation. Such aggregation normally results in the surface expression of platelet glycoprotein (GP) IIb/IIIa receptors, which facilitate fibrinogen binding as well as consequential further platelet aggregation – the final common pathway of vascular occlusion. Specific cytochrome P450 enzymes are thought to generate the responsible active metabolite (believed to persist transiently). However, dosage adjustment is not recommended in hepatic impairment (unless severe) and drug-drug interactions and co-administration with meals are …

Astrocytes provide the glutamine required by neurons to synthesize glutamate and GABA. However, the mechanisms involved in glutamine transfer from glia to neurons have remained poorly understood. Recent work has implicated the System N transporter SN1 in the efflux of glutamine from astrocytes and the very closely related System A transporters SA1 and SA2 in glutamine uptake by neurons. To understand how these closely related proteins mediate flux in different directions, we have examined their ionic coupling. In the brain, the neurotransmitter glutamate is removed from the synaptic cleft by (Na⁺ + K⁺)-coupled transporters by an electrogenic process. Moreover, these transporters mediate a sodium- and glutamate-dependent uncoupled chloride conductance. In contrast to the wild type, the uptake of radiolabeled substrate by the I421C mutant is inhibited by the membrane-impermeant [2-(trimethylammonium)ethyl]methanethiosulfonate and also by other sulfhydryl reagents. In the wild-type and the unmodified mutant, substrate-induced currents are inwardly rectifying and reflect the sum of the coupled electrogenic flux and the anion conductance. Remarkably, the I421C mutant modified by sulfhydryl reagents exhibits currents that are non-rectifying …

PROBLEM/CONDITION: Since 1971, CDC, the U.S. Environmental Protection Agency (EPA), and the Council of State and Territorial Epidemiologists (CSTE) have maintained a collaborative surveillance system for the occurrences and causes of waterborne-disease outbreaks (WBDOs).This surveillance system is the primary source of data concerning the scope and effects of waterborne diseases on persons in the United States.

REPORTING PERIOD COVERED: This summary includes data regarding outbreaks occurring during January 1999-December 2000 and previously unreported outbreaks occurring in 1995 and 1997.

Concentrations Of Hair Minerals Determined By Inductively Coupled Plasma-Mass Spectrometry In People With Headaches, Joel R. Mundall

Capstone Research Projects

Calculations were made to determine what relationships exist between headaches and hair mineral levels of 1,995 hair tissue mineral analyses by inductively coupled plasma-mass spectrometry. Of the 1,995 samples (1,089 females, 906 males), 163 (100 females, 63 males) were taken from patients who had been diagnosed with headaches. Above average hair levels of bismuth and the earth metals were found among the both the males and females of the headache group. An age correlation analyses showed highest headache incidence in teens and early forties. A direct correlation was also found between calcium and magnesium levels, which peaked in the late …

Attachment Disorders And Personality Disorders : A Possible Connection, Jen Sacora

Researchers now propose that attachment theory can be applied beyond childhood. They believe attachment exists in adulthood and influences adult relationships. Possible connections between personality disorders and well-known approaches to personality development such as attachment theory have been made (Brennan & Shaver, 1998).

The purpose of this paper is to examine the relationships between parent-infant attachment which influence adult relationships and psychopathology. Theories of personality development as they relate to attachment will be explored as well as influence of early childhood experiences on personality. Finally, possible connections between attachment disorders and personality disorders will be speculated.

Due to the recent explosion of information in the biomedical field, it is hard for a single researcher to review the complex network involving genes, proteins, and interactions. We are currently building GeneScene, a toolkit that will assist researchers in reviewing existing literature, and report on the first phase in our development effort: extracting the relevant information from medical abstracts. We are developing a medical parser that extracts information, fills basic prepositional-based templates, and combines the templates to capture the underlying sentence logic. In "Depression as a Mind-Body Problem," Walter Glannon outlines a psychosocial-physiological explanation of depression as a psychological response to chronic stress—today, especially social stress—in which cortisol imbalances disrupt neurotransmitters. Accordingly, treatment for depression should combine psychopharmacology and psychotherapy—a valuable reminder in light of the current restrictions on funding for health care (Hobson and Leonard 2001). My comments focus, however, on Glannon's objections to evolutionary theorists who explain our capacity for depression as adaptive to the natural and social environment. His objections are implausible because he fails to distinguish depression as a mood and a disorder.

The purpose of this descriptive study was to identify factors that perspective patients perceive as ones that influence their satisfaction with healthcare. A distinctive feature of the study was that it addressed factors that influence satisfaction with healthcare from the patient's perception (emic perspective) rather than the provider's (etic) assumptions.

การติดตามและพัฒนาความรู้ทางการรักษา คลองรากฟันของทันตแพทย์ไทยกลุ่มหนึ่ง, วีระ เลิศจิราการ, ภควัตร ชาคริยานุโยค, วิชชา พิชิตพิเชษฐกุล, ชาญชัย สุนพคุณศรี

วัตถุประสงค์ เพื่อศึกษา เปรียบเทียบขั้นตอน ในการรักษาคลองรากฟันของทันตแพทย์ไทยกลุ่มหนึ่งที่ปฏิบัติอยู่ ในปัจจุบัน กับขั้นตอนที่ปฏิบัติขณะที่ศึกษาอยู่ในโรงเรียนทันตแพทย์ วัสดุและวิธีการ ส่งแบบสอบถาม 600 ชุด ไปยังกลุ่มทันตแพทย์ไทยทั่วประเทศโดยการสุ่ม (Convenience Sampling) จากรายชื่อสมาชิกของทันตแพทยสมาคมแห่งประเทศไทยฯ และทําการเปรียบเทียบข้อมูลในสมัยเรียน กับในปัจจุบันที่ปฏิบัติในทางคลินิก ผลการศึกษา พบว่าทันตแพทย์ในประเทศไทยกลุ่มที่ตอบแบบสอบถามมีวิธีการรักษาคลองรากฟันในปัจจุบัน แตกต่างจากวิธีการรักษาคลองรากฟันที่ใช้ปฏิบัติกันในขณะศึกษาอยู่ในโรงเรียนทันตแพทย์ในระดับปริญญาตรี อย่างมีนัยสําคัญ (P<0.001) ทั้งในขั้นตอนการล้างคลองรากฟันและการใส่ยาในคลองรากฟัน รวมทั้งพบว่ามี ทันตแพทย์ถึง 91.95% ซึ่งติดตามพัฒนาความรู้ใหม่ ๆ อยู่ตลอดเวลา สรุป ทันตแพทย์ในประเทศไทยกลุ่มที่ตอบแบบสอบถามมีการติดตามและพัฒนาความรู้อยู่ตลอดเวลา จากการฟัง บรรยายต่าง ๆ ซึ่งจัดโดยสมาคม ชมรม หรือมหาวิทยาลัยต่าง ๆ ซึ่งแสดงให้เห็นว่า การจัดอบรมระยะสั้น มีความ สําคัญต่อการพัฒนาความรู้ของทันตแพทย์ในปัจจุบัน

การรักษาฟันเดนซ์อีแวกจิเนทัสในฟันกรามน้อย ซี่ที่ 2 ล่างขวา ซึ่งมีรอยโรคในช่องปากร่วมด้วย : รายงานผู้ป่วย 1 ราย, ผ่องผุด หาญนรเศรษฐ์

ฟันเดนซ์อีแวกจิเนทัสเป็นฟันที่มีความผิดปกติของผิวเคลือบฟัน ซึ่งมีลักษณะเป็นปุ่มนูนยื่นออกมาจาก ผิวเคลือบฟัน มักพบว่าเนื้อเยื่อในฟันตายเพราะปุ่มฟันหักทําให้ แบคทีเรียแทรกซึมเข้าไปได้ ก่อให้เกิดพยาธิสภาพ ที่ปลายรากฟันและอาจทําให้มีรอยโรคในช่องปากร่วมด้วย โดยทั่วไปมักจะเกิดในฟันที่ปลายรากฟันยังไม่ปิด ใน รายงานนี้แสดงผลการรักษาคลองรากฟันเดนซ์อีแวกจีเนทัสในฟันกรามน้อยที่ 2 ล่างขวาซึ่งมีรอยโรคในช่องปาก ร่วมด้วย โดยใช้ แคลเซียมไฮดรอกไซด์ เพื่อกระตุ้นให้มีการเสริมสร้างการปิดที่ปลายรากฟัน และอุดคลองรากฟัน ด้วยวิธี Lateral condensation จากการติดตามผลการรักษาเป็นเวลา 1 ปี 6 เดือน พบว่ามีการสร้างกระดูกที่บริเวณ ปลายรากฟัน ฟันใช้งานได้ดี ไม่มีรอยโรค หรือพยาธิสภาพเกิดขึ้นใหม่

ความพึงพอใจของผู้ป่วยต่อประสิทธิภาพของฟันปลอมบางส่วนถอดได้ข้างเดียวชนิดโครงโลหะ, ร่าโพ โรจนกิจ

วัตถุประสงค์ เพื่อให้ทราบถึงความพึงพอใจของผู้ป่วยต่อประสิทธิภาพของฟันปลอมบางส่วนถอดได้ข้างเดียวชนิด โครงโลหะ ที่ใช้ทดแทนฟันกรามที่ขาดหายไป 1-2 ซี่ ว่าช่วยในการบดเคี้ยวได้เพียงใด และเป็นข้อมูลประกอบการ ตัดสินใจสําหรับทันตแพทย์ที่จะเลือกทําฟันปลอมชนิดนี้ให้แก่ผู้ป่วย วัสดุและวิธีการ จัดส่งแบบสอบถามทางไปรษณีย์ จํานวน 150 ชุด ให้กับผู้ป่วยที่ได้รับการใส่ฟันปลอมบางส่วน ถอดได้ข้างเดียวชนิดโครงโลหะ ที่ผู้วิจัยเป็นผู้ทําให้ แบบสอบถามแบ่งออกเป็น 4 ส่วน คือ ข้อมูลทั่วไปของผู้ตอบ แบบสอบถาม ข้อมูลทางทันตกรรมของผู้ตอบแบบสอบถาม ข้อมูลเกี่ยวกับความพึงพอใจต่อประสิทธิภาพของ ฟันปลอมที่ใช้อยู่ และสรุปความเห็นในการเลือกชนิดของฟันปลอมในอนาคตของผู้ตอบแบบสอบถาม โดยนํามา วิเคราะห์เป็นอัตราร้อยละ และหาความสัมพันธ์ของความพึงพอใจรวมทั้งหมดกับความพึงพอใจในด้านต่าง ๆ ของ ชิ้นฟันปลอมทางสถิติด้วยเปียร์สัน ไค สแควร์ (Pearson Chi-square test) ที่ระดับความเชื่อมั่นร้อยละ 95 (α = 0.05) ผลการศึกษา แบบสอบถามส่งกลับมาและสามารถนํามาวิเคราะห์ได้ 121 ชุด โดยระดับความรู้ของผู้ตอบแบบ สอบถามเฉลี่ยอยู่ในระดับปริญญาตรี มีอายุเฉลี่ย 50.5 ปี และรายได้เฉลี่ย 30,966 บาทต่อเดือน จํานวนชิ้นงาน ฟันปลอมบางส่วนถอดได้ข้างเดียวที่ผู้ตอบแบบสอบถามใส่อยู่มีทั้งหมด 166 ชิ้น โดยผู้ป่วยมีความพอใจประสิทธิภาพ ของฟันปลอมในภาพรวมอยู่ในเกณฑ์ดี และความพึงพอใจในภาพรวมทั้งหมดต่อชิ้นฟันปลอม มีความสัมพันธ์ กันกับความพอใจในด้านต่าง ๆ ของชิ้นฟันปลอมอย่างมีนัยสําคัญทางสถิติ (α = 0.05) เมื่อทดสอบด้วยเปียร์สัน ได สแควร์ และผู้ป่วยถึงร้อยละ 62 เลือกที่จะใส่ฟันปลอมแบบเดิมในกรณีที่เลือกทําฟันปลอมใหม่ในอนาคต สรุป ความพึงพอใจของผู้ป่วยต่อประสิทธิภาพของฟันปลอมบางส่วนถอดได้ข้างเดียวชนิดโครงโลหะอยู่ในเกณฑ์ดี และถึงแม้จะให้เลือกชนิดของฟันปลอมในอนาคต ผู้ป่วยส่วนมากยังตัดสินใจที่จะใช้ฟันปลอมชนิดนี้ต่อไป ดังนั้นจึงน่า จะเป็นทางเลือกหนึ่งของทันตแพทย์ที่จะทําฟันปลอมชนิดนี้ให้แก่ผู้ป่วยได้ในกรณีที่เหมาะสม
