In the year 2000, there were approximately 35 million people in the United States who were 65 years of age or older, a ten-fold increase since 1900. The growth of this population has presented opportunities and challenges to American society. Health care needs, physical changes of aging, and the allocation of health care resources are several of the issues that need to be addressed (Federal Interagency Forum on Aging-Related Statistics, 2000). Improved Continuation Rate Of Depot-Medroxyprogesterone Acetate In Adolescent Mothers, Hatim A. Omar, Amy Fowler, Sandy D'Angelo

Pediatrics Faculty Publications

Poor compliance and high discontinuation rates of Depot-Medroxyprogesterone Acetate (DMPA) and other contraceptive methods are major factors in the continuing problem of adolescent pregnancy. In this study we attempted to determine if providing comprehensive health care for teen mothers and their babies would improve continuation rates of DMPA. Patients who started DMPA between 1/1/96 and 1/1/99 were included. Teen mothers and their babies received all their health care in this clinic, supported by State funding. Key elements regarding DMPA in this clinic were continuity of care, phone and mail reminders of appointments, free DMPA for patients without insurance, counseling at …

Prostate cancer (PCA) is the most common cancer and the second leading cause of death among American men. The high mortality is greatly attributed to the lack of early detection tools and effective treatment for metastasis and relapses. Biomarkers that can discriminate benign from malignant tumor and signal the development of androgen independent and metastatic tumor are needed. A biomarker designated prostate specific membrane antigen (PSMA) has the potential to fulfill this need. Objective: To test the effect of dietary nutrients on oral precancerous lesions in a reverse-smoking (i.e. smoking with the glowing end inside the mouth) population in South India. Design: Case–control. Cases with precancerous lesions were matched to an equal number of lesion-free controls matched on age (^5 years), sex and village. All subjects used tobacco in some form. Dietary data were obtained using an intervieweradministered food-frequency questionnaire, designed for use in this population. All interviews were conducted blinded to the disease status of the subject. Data were analysed using logistic regression. Setting: Nineteen rural villages in Srikakulam District, Andhra Pradesh. …

Cellular senescence forms a barrier that inhibits the acquisition of an immortal phenotype, a critical feature in tumorigenesis. The inactivation of multiple pathways that positively regulate senescence are required for immortalization. To identify these pathways in an unbiased manner, we performed DNA microarray analyses to assess the expression of 20,000 genes in human prostate epithelial cells (HPECs) passaged to senescence. These gene expression patterns were then compared with those of HPECs immortalized with the humanPapillomavirus 16 E7 oncoprotein. Senescent cells display gene expression patterns that reflect their nonproliferative, differentiated phenotype and express secretory proteases and extracellular matrix components. A …

This retrospective study analyzed the three muscles involved in knee extension: vastus medialis, rectus femoris, and vastus lateralis. lt was conducted to determine the reliability of normalization procedures when evaluating the EMG response detected from a muscle. During a maximal voluntary isometric contraction {MVIC), the EMG response of the muscle was recorded and the iEMG moment per unit area was determined. The iEMG response ofthe quadriceps muscles during dynamic exercise was normalized using the MVIC. Subjects (N = 44) performed two consecutive MVICs with a 2-min rest between trials at 110°, 90°, 70°, 50°, and 30° of knee flexion, …

Acknowledging the relationship that exists among health outcomes, socioeconomic factors, and access to health care is key to understanding the health status of Latinas in Massachusetts. For example, poverty disproportionately affects Latinos in Massachusetts, and they have the lowest rate of health insurance in the Commonwealth despite the fact that Massachusetts has one of the most inclusive and comprehensive public health insurance programs in the nation. This situation impacts access to regular health care for Latinos and affects their overall health status. In Vibrio cholerae, elaboration of toxin-coregulated pilus and protein secretion by the extracellular protein secretion apparatus occurred in the absence of both TonB systems. In contrast, the cognate putative ATPases were required for each process and could not substitute for each other.

The research on recovery from alcoholism and drug addiction has generally focused on the causes of relapse. Although the study of the causes of relapse is an important endeavor, the results of this focus remain inconclusive. Understanding the process and themes of recovery is also an important question that remains a significant challenge.

This study attempts to explain, in part, why it is that one person can have many relapse indicators and still thrive in recovery, while another who has multiple layers of support and opportunity for recovery, succumbs to addiction. This study also attempts to determine if and how …

Genetic Testing: A Cautionary Tale Of Foster And Pre-Adoptive Children, Janet Farrell Smith

New England Journal of Public Policy

Genetic testing of children in the foster care and pre-adoptive stage may be thought to facilitate child placement and satisfy prospective parents’ need to know. But, the policy analysis in this paper recommends great caution, especially given eugenic attitudes in the history of adoption and the risk of creating a second tier of un-adoptable children. Testing should be done only when two conditions are satisfied: test information is medically useful for childhood onset diseases; test information supports and does not diminish the child’s access to present and future healthcare (or the child’s future insurability). Preventing environmental exposures that threaten human health remains among the best but least attended to opportunities to improve everyone’s health. For more than a decade, medical care concerns, exacerbated by voracious competition among medical empires and the implacably growing number of uninsured, have often been misconstrued as constituting a complete agenda for health system reform.
