Following primary ocular infection, herpes simplex virus type 1 (HSV-1) establishes a lifelong latent infection in sensory neurons of the trigeminal ganglia. Latency-associated transcript (LAT), the only known viral gene abundantly transcribed during HSV-1 neuronal latency, is required for high levels of reactivation. Recently we showed that three different mutants that do not alter the LAT promoter but contain deletions within the 5′ end of the primary LAT transcript affect viral virulence (G. C. Perng et al., J. Bridgeport Hospital and Healthcare Services (BHHS) in Bridgeport, Conn., a part of the Yale New Haven Health System (YNHHS), embraced the balanced scorecard because it had been experiencing a loss in revenue and income due to managed care penetration in the local marketplace. This hospital is not alone in facing financial pressures. The balanced scorecard provides the framework for measuring performance in a complex and changing medical environment. Still retaining financial measures, the following drivers of financial success are incorporated into Bridgeport's scorecard: quality clinical outcomes; expert clinical care providers; satisfied patients, doctors, and staff; and volume and market-share growth. …

Treatment of early childhood caries (ECC) has challenged pediatric dentists. Finding esthetically pleasing yet durable restorations of anterior teeth can be difficult. Veneered stainless steel crowns are an option but clinically, the veneers can fracture. The purpose of this study was to identify the most fracture resistant veneered stainless steel crown and to determine the best method of repair if fractured. The clinical significance is an evaluation of the best method of repairing these fractured crowns. Plagiocephaly, a condition that is observed by deformation of the head, comes as the result of different etiologies. The most serious involving premature fusion of one or more of the cranial sutures, a condition referred to as craniosynostosis. While children with craniosynostosis may be considered for developmental screening to determine whether they fall at risk for developmental delay, children with plagiocephaly are typically not seen as being at risk, their condition typically treated as a cosmetic problem. The technique of bonding orthodontic brackets utilizing a transfer tray, otherwise known as indirect bonding, is a dichotomous subject. It allows for more accurate bracket placement and reduced chair time for the patient and doctor, yet its technique sensitivity can yield bracket failures at the time of tray removal. With the recent development of 3M's Sondhi Rapid Set, an adhesive specifically designed for orthodontic bonding, the purpose of this study was to compare this fairly new indirect adhesive with 3M's Transbond XT and Reliance's Rely-A-Bond.

Investigations into the effects of airway patency on craniofacial development are essential to ensure that interceptive orthodontic treatment provides the maximum benefit to the patient. The aims of this study were to (1) compare the size of the nasopharyngeal airway space on lateral cephalometric and 3D volumetric radiographs to determine if volumetric scans can be of greater diagnostic value than a lateral cephalometric film in determining the degree of airway blockage or patency and (2) to examine the correlation between functional airway resistance, as evaluated by rhinomanometry, and radiographic constriction due to nasal turbinate and adenoid size.

The purpose of this study was to compare the two-dimensional cephalometric and the three-dimensional tomographic radiographic methods in their ability to assess anatomic measurements while using the Onplant anchoring device in anterior retraction cases.

The Onplant was surgically placed beneath the periosteum of the hard palate. After osseointegration the Onplant was then uncovered and attached to the first molars by means of a transpalatal bar. Conventional orthodontic treatment was then used to retract the anterior teeth after 1^st premolars were extracted.

Interleukin-10 (IL-10) is a cytokine with anti-inflammatory activities. IL-10 knockout (KO) mice exhibit severe intestinal inflammation in the presence of normal gut flora. We hypothesized that IL-10 KO mice would also be susceptible to inflammatory periodontal disease resulting in alveolar bone loss (ABL). The purpose of this study was to compare naturally occurring ABL in IL-10 KO and wild type (WT) mice. Twelve IL-10 KO and 12 WT mice, age-(7 months) and sex-(male) matched, were used. The pulps of eighty-five maxillary first, second and third molar teeth from twenty male rats were experimentally exposed. Machine perfusion of livers may provide a mechanism for extended preservation of marginal donor organs before transplantation, as well as a method for viability assessment. It has proved possible in a series of experimental porcine liver perfusions to maintain liver viability for up to 72 h. However, a reduction in bile production with associated histological evidence of cholestasis was seen after 10 h of perfusion, damaging the biliary canaliculi during the preservation period and leaving these organs in an unacceptable condition for transplantation. This pilot study examined the relationship between perceived self-efficacy and regimen adherence on glycemic control among adolescents, aged 11 to 18 years, with Type 1 Diabetes Mellitus. Thirty-six adolescent-parent dyads participated in this study. Bivariate analysis using Pearson product moment-correlations identified no significant relationships among the variables. Regarding treatment regimen, adolescents on insulin pumps had a mean glycosylated hemoglobin level of 8.09% whereas adolescents on daily insulin injections had a mean glycosylated hemoglobin level of 9.26%. Bandura's theory of self-efficacy provided the theoretical framework for this study. The Self-Efficacy for Diabetes scale and Questionnaire of Self-Care Behaviors scale were used …

To study the significance of oligoclonal bands in neurological disorders, specifically in Multiple Sclerosis (MS).
Methods:
The study was designed to assess the sensitivity and specificity of the test methodology of CSF electrophoresis by performing the retrospective analysis of CSF samples sent for oligoclonal bands (OCB). A total of 603 samples were received by the Clinical Laboratories, Department of Pathology of The Aga Khan University, during a period of 54 months (January 1993-June 1997). This case study describes the process of examination, re-examination, and intervention for a collegiate runner with mechanical thoracic pain preventing athletic participation and limiting daily function. Unimpaired function fully returned in less than 3 weeks with biweekly sessions to re-establish normal and painfree thoracic mechanics via postural hygiene, exercise, mobilization, and manipulation. The outcome of this case study supports the original hypothesis that the pattern of impairments was in fact responsible for the functional limitations and disability in this athlete. Background: Albuminuria is long recognised as a sign of renal disease in diabetes. In type 1 diabetes, renal disease occurs after a longer duration of diabetic state. In type 2 diabetes, it is more variable.

Objective: To determine the prevalence and any risk factors of albuminuria in short­ term (≤2 yrs) type 2 diabetes.

Design: Cross sectional, descriptive study. Microalbuminuria was assessed using micro II strips.

Setting: Outpatient diabetic clinic at Kenyatta National Hospital, Nairobi.

Subjects: Patients who were newly diagnosed or had had type 2 diabetes for two years or less.
