Controlled Software Evaluation Methodology: Case Simulation And Cognitive User Analysis, Kirt Walker, Sridhar Rajappan, Todd R. Johnson

Software for use with patient records is challenging to design and difficult to evaluate because of the tremendous variability of patient circumstances. A method was devised by the authors to overcome a number of difficulties. The method evaluates and compares objectively various software products for use in emergency departments and compares software to conventional methods like dictation and templated chart forms. The technique utilizes oral case simulation and video recording for analysis. The methodology and experiences of executing a study using this case simulation are discussed in this presentation.

Is the solution for medical errors medical or cognitive? In this AMIA2001 panel on medical error, we argued that medical error is primarily an issue for cognitive science and engineering, not for medicine, although the knowledge of the practice of medicine is essential for the research and prevention of medical errors. The three panelists presented studies that demonstrate that cognitive research is the foundation for theories of medical errors and interventions of error reductions.

Healthcare has been slow in using human factors principles to reduce medical errors. The Center for Devices and Radiological Health (CDRH) recognizes that a lack of attention to human factors during product development may lead to errors that have the potential for patient injury, or even death. In response to the need for reducing medication errors, the National Coordinating Council for Medication Errors Reporting and Prevention (NCC MERP) released the NCC MERP taxonomy that provides a standard language for reporting medication errors. Eating disorders have dramatically increased over the last 20 years, resulting in prevalence rates exceeding other high-incidence medical conditions treated in the educational setting. Chapter 2 presents a review of literature covering eating disorder identification, history, prevalence and epidemiological data. Theoretical perspectives, risk and protective factors, symptoms, and medical complications are also reviewed. Current assessment measures used for screening and diagnosing anorexia and bulimia is additionally presented along with a summary of school-based primary prevention programs. The second chapter concludes with a brief overview of eating disorder treatment.

The purpose of this study was to determine if there were any relationships between the demographics and board experience of board members at Centers for Independent Living (CILs) in Iowa and their perceptions of their roles and responsibilities. A survey was administered to board members of five of the seven CILs in Iowa (N = 62, n = 36).

Overall, various avenues of research state that ADHD is a complex disorder that requires treatment by multiple modalities. Most research states that a combination of medication and behavior therapy is necessary to resolve the symptoms. It should be noted that the dose of stimulant medication prescribed to a child with ADHD should be adjusted based on the child's response to a behavioral intervention, as well as the combination of the behavioral and pharmacological treatments. Balance is critical for optimal function in completion of everyday tasks. Physical therapists frequently work with people that have balance disturbances in order to help return them to optimal function. One tool used in the assessment and treatment of balance disorders is the NeuroCom® Balance Master 7.06 (NBM), a computerized forceplate system that provides objective measurements of balance performance. Purpose: The purpose of this study was to determine the intra- and inter-tester reliability of the DualerTM Digital Inclinometer when measuring lumbar flexion and extension. Past studies examining reliability of inclinometers have reported variable results. Background and Purpose

The purpose of this study is to describe lower extremity muscle activity and joint range of motion while moving both forward and backward on an elliptical trainer at minimum and maximum inclines.

Subjects

Ten healthy subjects (7 female and 3 male) gave informed consent to voluntarily participate in this study.

Methods

Inhibitory interneurons play crucial roles in the regulation of patterns of activity in the hippocampus, and some types are thought to be vulnerable in epilepsy. The connections between excitatory and inhibitory synapses are important for generation of bursting activity in pyramidal neurons. The present study investigates the influences of changes in the connectivity of interneurons on the patterns of bursting in several excitatory connections using a multicompartmental pyramidal cell model. Simulations show that bursting activity depends upon changes in the connectivity of the inhibitory interneuron, and the location of the inhibitory synapses on excitatory neurons.

Infection with an obligate intracellular bacterium, the Chlamydia trachomatis lymphogranuloma venereum (LGV/L2) strain or the guinea pig inclusion conjunctivitis serovar of Chlamydia psittaci, leads to apoptosis of host cells. The apoptosis is not affected by a broad-spectrum caspase inhibitor, and caspase-3 is not activated in infected cells, suggesting that apoptosis mediated by these two strains of Chlamydia is independent of known caspases. Overexpression of the proapoptotic Bcl-2 family member, Bax, was previously shown to induce caspase-independent apoptosis, and we find that Bax is activated and translocates from the cytosol to the mitochondria in C. psittaci-infected cells. This study determined if the cleaning efficiency of NiTi rotary files in an endodontic electric handpiece using a no torque control setting was superior to that obtained when using the torque control feature. Fifty extracted human anterior teeth with straight canals were divided into two groups of 20 and two control groups of 5. Canals were instrumented with GT and .04 Profile NiTi files until a size 35 advanced to working length. Samples were sectioned, the apical 6 mm of the canal was photographed (x20) and projected onto a 3 x 4 foot grid with squares measuring 0.5 inches each. …

The perception of temporal speech cues, lexical knowledge, and their interactions were examined in children (6;0-9;6) with specific language impairment (SLI). An identification task was used to test four 12-step speech continua: word-word (FEET—FEED), nonword-nonword (ZEST—ZEED), word-nonword (CHEAT—CHEED) and nonword-word (REAT—READ). The stimuli were naturally recorded and digitally edited. The vowel steady state, which varied in duration from 110 to 350 milliseconds in 20-millisecond steps, was the acoustic cue to the voicing characteristic of the final consonant in each stimulus. The analyses revealed that both the TLD and SLI groups used vowel duration as a perceptual cue. For the word-word …

Previous work has suggested that chronic pain patients report psychological distress through higher endorsement of somatic rather than cognitive signs of anxiety and depression. The present study compared female Non-Pain (11=52), Orofacial Pain (n=3 17) and Fibromyalgia (n=50) groups. on SCL-90-R Somatization, Anxiety and Depression raw scores and cognitive-somatic symptom patterning of the Anxiety and Depression scales. Comparisons were also made amongst orofacial pain diagnostic subgroups and subgroups based on Multidimensional Pain Inventory (MPI) classification groups. The Somatization. Spatio-temporal variation in activation rates during ventricular fibrillation (VF)provides insight into mechanisms of sustained re-entry during VF. This study had three objectives related to spatio-temporal dynamics in activation rates during VF.

The first objective was to quantify spatio-temporal variability in activation rates,that is, in dominant frequencies, computed from epicardial electrograms recorded during VF in swine. Results showed that temporally and spatially, dominant frequencies variedas much as 20% of the mean dominant frequency, and the mean dominant frequencies increased during first 30 sec of VF. These results suggest that activation rates are nonstationary during VF.

Molecular chaperone complexes containing heat shock protein (Hsp) 70 and Hsp90 are regulated by cochaperones, including a subclass of regulators, such as Hsp70 interacting protein (Hip), C-terminus of Hsp70 interacting protein (CHIP), and Hsp70-Hsp90 organizing factor (Hop), that contain tetratricopeptide repeats (TPRs), where Hsp70 refers to Hsp70 and its nearly identical constitutive counterpart, Hsc70, together. These proteins interact with the Hsp70 to regulate adenosine triphosphatase (ATPase) and folding activities or to generate the chaperone complex. Here we provide evidence that small glutamine-rich protein/viral protein U–binding protein (SGT/UBP) is a cochaperone that negatively regulates Hsp70. Taurine efflux occurs in association with cell swelling in both hyposmotic and isosmotic conditions and during cell shrinkage in apoptotic death. Release occurs through a leak pathway, is largely Ca²⁺-independent and is sensitive to Cl^– channel blockers. Taurine efflux elicited by hyposmolarity is reduced or suppressed by tyrosine kinase blockers and increased by tyrosine phosphatase inhibitors. The specific kinases involved are still unknown and may be different in the various cell types. The present invention relates to methods and compositions for eliciting an immune response against bovine viral epitopes. The methods comprise combining at least one heat shock protein with at least one bovine viral epitope to form a purified epitope/heat shock protein complex and administration of an immune system stimulating amount of the purified epitope/heat shock protein complex. The compositions comprise, a purified epitope/heat shock protein complex comprising at least one bovine viral epitope complexed with at least one heat shock protein, and a pharmaceutically acceptable carrier, diluent or excipient.

Lesions in four captive pronghorn antelope (Antilocapra americana) naturally infected with Parelaphostrongylus tenuis in eastern Nebraska (USA) are described in this report. Animals were bright and alert with hind limb ataxia that progressed to sternal or lateral recumbency between July 28 and October 17, 1998. Animals were euthanized due to disease progression despite therapy. Multifocal decubital ulcers over bony prominences occurred in two animals and chronic unilateral otitis media was present in one animal. Histopathologic examination revealed severe Wallerian degeneration randomly scattered throughout the spinal cords of all four animals. Spinal cord sections from two animals contained adult …
