Background

The purpose of this study was to evaluate the bias and precision of 46 methods published from 1953 to 2000 for estimating resting energy expenditure (REE) of thermally injured patients.

Methods

Twenty-four adult patients with ≥20% body surface area burn admitted to a burn center who required specialized nutrition support and who had their REE measured via indirect calorimetry (IC) were evaluated. Patients with morbid obesity, human immunovirus, malignancy, pregnancy, hepatic or renal failure, neuromuscular paralysis, or those requiring a FiO2 >50% or positive end expiratory pressure (PEEP) ≥10 cm H2O were excluded. Warfarin interacts with many conventional drug products and herbal products,^1-3 Given the incompleteness of information on drug-herbal product interactions and the potential for health care practitioners to be unaware of their patients' utilization of herbal products, we surveyed the use of such products among patients receiving warfarin through an anticoagulation clinic. Patients at the University of Maryland Medical System Anticoagulation Clinic were asked to complete an anonymous written questionnaire during their regularly scheduled appointments over one month.

Iron complex chemistry that opens a new door to the medicinal and pharmaceutical worlds is the aim of this research. Specifically, ortho-quinone methide moieties are intermediates in several antitumor drugs and have been identified as bioreductive alkylators of DNA. In our research, a class of iron compounds has been targeted to resemble these quinone methides. It is hoped that these new compounds could be modified to provide a window of opportunity toward the discovery of a selective mode of drug delivery. The Contextual Approach In Health Research: Two Empirical Studies, Anita Kothari

Researchers are being encouraged to consider contextual influences on health-related outcomes. To support this perspective, two context-sensitive studies were conducted. The first study explored the utilization of a research report by Ontario public health units, and examined whether utilization differed by involvement in the research process. Research utilization was conceptualized as a three stage process (reading, information processing and application). Using a case study design, results from three "involved" public health units and three "uninvolved" units demonstrated that inclusion in the research process led to a greater understanding of the analysis and increased the value associated with the report. This paper reports findings of a study of mental health among urban Aboriginal peoples. Both qualitative and quantitative techniques were used to explore the relationship between substance abuse and physical/sexual abuse in a sample of Aboriginal peoples living in an urban community. Open-ended interviews were conducted in a sample of 30 Aboriginals that included Aboriginal social service providers (from health, legal and family services). Forty percent of the sample had been abused and many experienced social, psychological and emotional prob- lems associated with abusive experiences. Content analysis of the interviews revealed common themes in violence discourse including loss of identity …

There is a discrepancy in the literature regarding the effect of repeated experience on children’s suggestibility. Some researchers have concluded that repeated experience increases children’s suggestibility for variable details whereas others have reported no detrimental effect. This study demonstrated that the type of question used to test memory (cued-recall vs. yes/no questions) could account for the different reported conclusions. Children aged 5-6 years took part in an event either once or four times. Three or 21 days later, they were given a suggestive interview about the single/final occurrence of the event during which half of the event details were inaccurately …

Identifying the sources of memories (e.g., who carried out an action, whether an event happened or was suggested, when an instance of a repeated event occurred) is an important skill in providing accurate accounts of events in forensic investigations. Sensitivity to the nature and development of children’s source-monitoring skills can inform interviewing practices. Five perspectives addressing alternate aspects of the development of children’s source monitoring are outlined (source-monitoring theory, fuzzy-trace theory, schema theory, the person-based perspective, and the mental-state reasoning model). A substantial portion of the population will suffer from headache at some point in their lives. This study examines the effect of a simple behavioral treatment for one of the two most common forms of headache, tension headache. Participants selected for the study were drawn from introductory psychology and exercise courses at Marshall University, reported having at least one tension headache per week, and exercised two or fewer times per week. Participants enrolled in an eight-week physical education aerobic exercise course through the university, and monitored their headache activity both during the class and through an eight-week baseline period. Since the birth of psychotherapy, the need for therapists to attend to their own personal healing and growth has been recognized. This tradition of personal therapy has been incorporated into many current programs that train therapists/helpers in this country. Freud and a large number of subsequent practitioners have also advocated periodic or ongoing personal therapy throughout the therapist's career span. Self-exploration groups can be an excellent means to keep helpers in touch with their humanness, to further their personal and professional growth, and to reduce toxic consequences of practicing therapy.

Control of scrapie, an ovine transmissible spongiform encephalopathy or prion disorder, has been hampered by the lack of conventional antemortem diagnostic tests. Currently, scrapie is diagnosed by postmortem examination of the brain and lymphoid tissues for PrP^Sc, the protein marker for this group of disorders. For live, asymptomatic sheep, diagnosis using tonsil or third-eyelid lymphoid tissue biopsy and PrP^Sc assay has been described. To evaluate the feasibility and efficacy of third-eyelid testing for identification of infected flocks and individual infected sheep, 690 sheep from 22 flocks were sampled by third-eyelid lymphoid tissue biopsy and immunohistochemistry. In this investigation, the nature and distribution of histologic lesions and immunohistochemical staining (IHC) of a proteinase-resistant prion protein were compared in free-ranging mule deer (Odocoileus hemionus) dying of a naturally occurring spongiform encephalopathy (SE) and captive mule deer dying of chronic wasting disease (CWD). Sixteen free-ranging deer with SE, 12 free-ranging deer without SE, and 10 captive deer with CWD were examined at necropsy. Tissue sections were stained with hematoxylin and eosin, and duplicate sections were stained with a monoclonal antibody (F89/160.1.5). Histological lesions in the freeranging deer with SE and captive deer with CWD were found …

Sphingolipids and their phosphorylated metabolites play crucial roles in intracellular signaling in animals, and evidence is emerging for analogous situations in fungi and plants. Central to this signaling pathway is the phosphorylation of the sphingoid long chain base, sphingosine, which yields sphingosine-1-phosphate. Until recently, the enzyme responsible for the biosynthesis of sphingosine was unknown, but the Δ4-long chain base desaturase that carries out this reaction has now been identified. Orthologues are present in animals, plants and fungi, raising the possibility of using reverse genetics to determine the contribution of sphingosine-1-phosphate to signaling networks.

A number of Saccharomyces cerevisiae membranebound oxidoreductases were examined for potential roles in microsomal fatty acid elongation, by assaying heterologous elongating activities in individual deletion mutants. One yeast gene, YBR159w, was identified as being required for activity of both the Caenorhabditis elegans elongase PEA₁ (F₅₆H_11.4) and the Arabidopsis thaliana elongase FAE₁. Ybr159p shows some limited homology to human steroid dehydrogenases and is a member of the short-chain alcohol dehydrogenase superfamily. Disruption of YBR159w is not lethal, in contrast to previous reports, although the mutants are slow growing and display high temperature sensitivity. Both …

The YBR159w gene encodes the major 3-ketoreductase activity of the elongase system of enzymes required for very long-chain fatty acid (VLCFA) synthesis. Mutants lacking the YBR159w gene display many of the phenotypes that have previously been described for mutants with defects in fatty acid elongation. These phenotypes include reduced VLCFA synthesis, accumulation of high levels of dihydrosphingosine and phytosphingosine, and accumulation of medium-chain ceramides. In vitro elongation assays confirm that the ybr159Δ mutant is deficient in the reduction of the 3-ketoacyl intermediates of fatty acid elongation. The ybr159Δ mutant also displays reduced dehydration of the 3-OH acyl intermediates of fatty …

Intimin-γ is an outer membrane protein of enterohemorrhagic Escherichia coli (EHEC) O157:H7 that is required for the organism to adhere tightly to HEp-2 cells and to colonize experimental animals. Another EHEC O157:H7 protein, the Transferred intimin receptor (Tir), is considered the primary receptor for intimin-γ. Nevertheless, Tir-independent binding of intimin-γ to HEp-2 cells has been reported. This observation suggests the existence of a eukaryotic receptor(s) for intimin-γ. Disturbing new patterns have marked the occurrence of recent lethal school violence where young offenders have targeted large groups of fellow students as their victims. The current study is an effort to understand this growing trend of youth violence. Based on analysis of several cases that occurred between 1996 and 1999, a profile of the offenders is suggested built on similarities and differences between cases that were researched.
