The Girl Who Cried Pain: A Bias Against Women In The Treatment Of Pain, Diane E. Hoffmann, Anita J. Tarzian

In general, women report more severe levels of pain, more frequent incidences of pain, and pain of longer duration than men, but are nonetheless treated for pain less aggressively. The authors investigate this paradox from two perspectives: Do men and women in fact experience pain differently - whether biologically, cognitively, and/or emotionally? And regardless of the answer, what accounts for the differences in the pain treatment they receive, and what can we do to correct this situation?

Asian American Mental Health Clients: Effects Of Ethnic Match And Age On Global Assessment And Visitation, Richard H. Dana, Glenn Gamst, Aghop Der-Karabetian, Terry Kramer

Effects of client-counselor ethnic match (i.e., match, no match) and client age group (child, adult) on counselor-evaluated Global Assessment of Function (GAF) and visitation were investigated. The sample consisted of 253 Asian-American outpatient clients (24.9% children 75.1% adults) of a community mental health center. Unadjusted results indicated that ethnically matched clients had more positive GAF evaluations and more clinic visits than nonmatched clients. When adjusted for eight covariates, results showed ethnically matched clients continued to show higher levels of visitation. Analysis of separate diagnostic categories showed that ethnically matched mood-disorder clients had higher levels of visitation. Conversely, nonethnically matched anxiety …

Vocational Rehabilitation Outcomes For People With Mental Retardation, Cerebral Palsy, And Epilepsy: An Analysis Of Trends From 1985 To 1998, Dana Scott Gilmore, Jennifer Schuster, John Butterworth

This monograph presents the results of secondary analysis of the RSA-911 database from the Rehabilitation Services Administration. All successful VR closures for individuals with mental retardation, cerebral palsy, and epilepsy for six data points between 1985 and 1998 were investigated. Trends in competitive labor market and extended employment (sheltered workshops) closures were examined. The use of supported employment in the VR system and its outcomes were also discussed. Findings include increased incidence of competitive labor market closures and supported employment services, with a decrease in extended employment closures.

Background

Gaps in disease surveillance capacity, particularly for emerging infections and bioterrorist attack, highlight a need for efficient, real time identification of diseases.

Methods

We studied automated records from 1996 through 1999 of approximately 250,000 health plan members in greater Boston.

Results

We identified 152,435 lower respiratory infection illness visits, comprising 106,670 episodes during 1,143,208 person-years. Three diagnoses, cough (ICD9CM 786.2), pneumonia not otherwise specified (ICD9CM 486) and acute bronchitis (ICD9CM 466.0) accounted for 91% of these visits, with expected age and sex distributions. Variation of weekly occurrences corresponded closely to national pneumonia and influenza mortality data. There was substantial …

External stimulus/loading initiates adaptations within skeletal muscle. Whilst performing flying manoeuvres under +Gz it has been previously found that the cervical area has the highest loading. The purpose of this study was to examine cervical muscle response to moderate +Gz force (+4-6Gz) loading generated during RAAF pilot training. Cervical muscle strength was monitored in nine RAAF pilots completing an eight-month flight training course and ten controls matched for gender, age, height and weight. The present study examined whether cognitive (beliefs), affective (emotions) and behavioural processes predicted young men's attitudes toward sexual health practices. For pragmatic reasons, most sexual health interventions are based on the provision of factual information, meaning most evaluations of these interventions have focused on the cognitive sources which may lead to behavioural change. However, the importance of emotional and behavioural influences has also been indicated, though rarely considered. Dealing With Conflicts Of Interest In Biomedical Research: Irb Oversight As The Next Best Solution To The Abolitionist Approach, Jesse Goldner

The author details the conflicts of interest facing individual investigators and research institutions and describes the current mechanisms, primarily focused at the relationship between the investigator and the research institution, to regulate these conflicts. The author finds these mechanisms insufficient and believes that the best approach is not to regulate conflicts, but to abolish them. The author acknowledges, however, that there is a lack of political will in an abolitionist approach. He proposes, therefore, institutional review board oversight at the level of the relationship between researcher and individual subjects as the next best solution.

Hemoglobin Concentration In Children In A Malaria Holoendemic Area Is Determined By Cumulated Plasmodium Falciparum Parasite Densities, H. Ekvall, Zul Premji, S. Bennett, A. Bjorkman

In malaria holoendemic areas children are anemic, but the exact influence of falciparum malaria on hemoglobin (Hb) concentration remains largely unsettled. Prospective data were therefore collected in children < 24 months of age during five months in a Tanzanian village. Children with mean asymptomatic parasitemia ≥ 400/μl had lower median Hb levels during the study than those with mean density < 400/μl. The difference was 9.7 g/L (95% confidence interval [CI] 2.8-17). In children with one or more clinical malaria episodes, the median Hb was 8.3 g/L (95% CI 0.9-16) lower than those without episode. If early treatment failure was recorded, the immediate effect on Hb was particularly important with a mean drop of 17 g/L. Interestingly, at study-end the Hb concentration represented a function of the area under the parasitemia curve (AUPC) during the previous five months, adjusting for age. In conclusion, stepwise deterioration in median Hb levels was found by asymptomatic parasitemia, clinical malaria episode, and most significantly, treatment failure.

Acute haemolysis associated with clinical episodes of high-level Plasmodium falciparum parasitaemia was studied in 20 children from an holoendemic area (coastal Tanzania). The change in blood haemoglobin (Hb) concentration ranged from -46 to +5 g/L during the 72-h observation period and was linearly related to maximum parasitaemia. Balance studies between loss of blood Hb, increase in plasma Hb and appearance of Hb in the urine indicated that extravascular clearance of red cells was the predominant mode of erythrocyte clearance. Most subjects, however, showed minor signs of intravascular haemolysis. The plasma Hb was ≪1% of blood Hb and haemoglobinuria was detected …

A number of empirical studies have demonstrated that one’s self-concept is multidimensional in nature, varies according to social context, and shows increased differentiation throughout adolescence. There has been relatively less work, however, examining the integration of multi-dimensional social selves. Rosenberg and Gara’s (1985) model of the multidimensional self (a model that utilizes a statistical procedure called “hierarchical classification” or HICLAS) was employed to investigate the integration of social selves during late adolescence. Magnesium is quantitatively the most important intracellular divalent cation (4), and has been found to serve in a wide range of enzymatic reactions (2). Studies have shown that up to ten percent of hospital patients may have hypomagnesaemia (6).

Atomic absorption spectrophotometry (AAS) under highly specified conditions is the de facto reference method for magnesium measurement but only about ten per cent of laboratories routinely use this method (4). Venous thrombosis occurs in about 1 per 1,000 individuals per year. It is a serious disorder and accounts for a significant number of hospital admissions and deaths annually. Thrombophilia describes the familial or acquired disorders of the haemostatic mechanism that predispose to thrombosis. Inherited thrombophilia can be defined as a genetically determined tendency to venous thromboembolism Mutations in genes that code for proteins involved directly (or indirectly) In blood coagulation have been associated with prothrombotic status.
