รากฟันด้านเพดานปากสองรากในฟันกรามบนซี่ที่สอง : รายงานผู้ป่วย 1 ราย, แอนนา เทพวนังกูร

รายงานกล่าวถึงฟันกรามบนซี่ที่สองของผู้ป่วยที่เข้ามารับการรักษารากฟัน เป็นฟันกรามบนสี่ราก โดยเป็นรากฟันด้านเพดานปากสองราก และรากฟันด้านแก้มสองราก ซึ่งการมีจํานวนรากฟันด้านเพดานปากมากกว่าปกติเป็นลักษณะที่พบได้น้อยที่สุดของฟันกรามบน พื้นของโพรงในตัวฟันมีลักษณะเป็นรูปสี่เหลี่ยม มีรูเปิดเข้าสู่คลองรากฟันอยู่ที่มุมทั้งสี่ การพิจารณาภาพรังสีก่อนการรักษาเป็นสิ่งสําคัญ เนื่องจากสามารถเห็นลักษณะผิดปกติของรากฟันได้ก่อนการรักษา การค้นหาและรักษารากฟันได้ครบทุกคลองรากฟันเป็นสิ่งหนึ่งที่ทําให้การรักษารากฟัน ประสบความสําเร็จ จากการติดตามผลการรักษาเป็นเวลา 2 ปี 6 เดือน พบว่าฟันอยู่ในสภาพใช้งานได้ดี ไม่มี พยาธิสภาพเกิดขึ้น

ชายไทยอายุ 16 ปี ป่วยเป็นโรคท่อน้ำดีตีบตันและมีอาการตับแข็งในระยะสุดท้ายของโรคตับ และจําเป็นต้องรับการผ่าตัดเปลี่ยนตับ ส่งมาที่โรงพยาบาลคณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัยเพื่อรับการตรวจพิเคราะห์สภาพช่องปาก และรับบริการทันตกรรม โดยเน้นควบคุมการเกิดแผ่นคราบจุลินทรีย์และป้องกันฟันผุด้วย ก่อนที่จะผ่าตัดเปลี่ยนตับ ทั้งนี้เนื่องจากผู้ป่วยต้องได้รับยากดภูมิต้านทานเป็นเวลานานทําให้อัตราเสี่ยงต่อการติดเชื้อ เพิ่มขึ้น การรักษาเสร็จสมบูรณ์ใช้เวลาทั้งสิ้นประมาณ 2 เดือน ผลการรักษาเป็นที่น่าพอใจ ผู้ป่วยรายนี้มีสภาพช่องปากที่เปลี่ยนแปลงสัมพันธ์กับโรคนี้ ปัญหาทางการแพทย์ของผู้ป่วยเกี่ยวกับการแข็งตัวของเลือด ข้อบ่งชี้การใช้ยาปฏิชีวนะ เพื่อป้องกันตลอดจนยาที่มีผลทําลายตับ ซึ่งทั้งหมดนี้อาจเกิดปัญหากับการรักษาทางทันตกรรมได้หากไม่ได้ป้องกัน และทราบวิธีแก้ไขปัญหาล่วงหน้าโดยปรึกษากับแพทย์ผู้ทําการรักษาอย่างใกล้ชิด การดูแลรักษาทางทันตกรรมใน ผู้ป่วยที่รอการเปลี่ยนตับมีความสําคัญอย่างยิ่ง เพื่อให้ผลการรักษาและคุณภาพชีวิตหลังการเปลี่ยนตับดียิ่งขึ้น และป้องกันโรคแทรกซ้อนที่อาจเกิดขึ้นหลังการเปลี่ยนตับ

Verruciform Xanthoma, Kittipong Dhanuthai

Verruciform xanthoma is a rare, benign entity which preferentially affects the oral mucosa than any other tissue. The most frequent site of involvement is the gingiva. It affects mostly the middleaged person with a predilection for male gender. Clinically, verruciform xanthoma appears as papillary or verrucous, flat or slightly raised lesions. Aggregates of lipid-laden foam cells confined to the connective tissue papillae between epithelial rete ridges in association with papillary or verrucous epithelial hyperplasia are the histolopathologic hallmark of this lesion. These foam cells are actually of macrophage/monocyte lineage. The histopathological distinction of verruciform xanthoma from other lesions, which it …

Objective The purpose of this study was to investigate the phenotype of the mononuclear and the multinucleated cells in the oral giant cell fibroma. Materials and methods The study was performed in thirty-one cases of the oral giant cell fibroma. The surgical specimens of the lesions were stained with a panel of antibodies using indirect immunoperoxidase technique. Results The mononuclear and multinucleated giant cells of the giant cell fibroma showed positive staining by antibodies against vimentin and proliferating cell nuclear antigen (PCNA) while demonstrating negative staining by antibodies against S-100 antigen, leukocyte common antigen (LCA), macrophage marker (LNS), smooth muscle …

Carotid cavernous sinus fistula is an uncommon complication of craniocervical trauma which is usually associated with a skull base, frontal or midface fractures; besides it may also be a spontaneous phenomenon of a congenital, degenerative or iatrogenic origin. The blood shunts from either internal or external carotid arteries to the cavernous sinus resulting the blurred vision, ophthalmoparesis, pulsating exophthalmos, glaucoma, ocular ischemic syndrome, orbital headache and periorbital pain. We have shown previously that partial inhibition of the cardiac myocyte Na⁺/K⁺-ATPase activates signal pathways that regulate myocyte growth and growth-related genes and that increases in intracellular Ca²⁺ concentration ([Ca²⁺]i) and reactive oxygen species (ROS) are two essential second messengers within these pathways. Serological screening accuracy rate may be dependent on clinical and pathological determinants. The aim of this study was to evaluate the accuracy of Hp serology test (Roche Biomedical Lab., Labcorp), In the diagnosis of Hp infection in 121 children who were seen in the Pediatric Gastoenterology Clinic at the Marshall University Joan C. Edwards School of Medicine In Huntington. Positive serology detected children with Hpassociated gastritis with a sensitivity of 51.6%. Positive serology significantly correlated with the degree of gastric inflammation and density of Hp organisms in the gastric mucosa (ANOVA p < 0.001). The Labcorp. Hp-ELISA test had a poor accuracy rate for the detection of Hp-gastritis in children. Gastric biopsies should always be performed to establish the diagnosis of Hp infection in children.

Hyperbaric oxygen (HBO) effectively decreases the morbidity and mortality of carbon monoxide poisoning . Immediate HBO treatment is indicated as it yields a more rapid and greater neuropsychological recovery. HBO significantly decreases the half life of carboxy hemoglobin (COHb) and increases the tissue clearance of residual carbon monoxide, thus decreasing the neuropsychological deficit. HBO enhances the outcome even in the subacute phase of CO poisoning as in this case. Posttraumatic spinal cord cyst is a late sequelae of spinal cord injury. The cyst has no or little inf-luence on the patient’s neurological functions. We report three patients of spinal cord injury complica-ted with posttraumatic spinal cord cyst whose motor functions were compromized due to increased spasticity. The incidence of the cyst may be up to 50%. The cyst may come from liquefaction of the cord tissue after local edema, necrosis and hematoma, or a myelomalacia due to local ischemia. A case of delayed diagnosis of spinal dural arteriovenous fistula is presented. A 65-year-old male presented to our clinic for progressive stiffness and numbness over bilateral lower limbs for 6 months. Neurological examination was suggestive of thoracic spinal cord lesion with signs of both upper and lower motor neuron disorders. Electromyographic examination showed evidence of denervation and re-innervation in the bilateral paraspinal muscles of middle thoracic level. Magnetic resonance imaging (MRI) of the spine revealed trivial changes, but they were neglected initially. Scheuermann's disease is one of the common causes of structural abnormality in the thoracic or thoracolumbar spines. Many theories have been proposed for the etiologies of Scheuermann's disease, but the true etiology remains unclear. The disease has a strong familial tendency which may be inh-erited in an autosomal dominant mode. This article describes a 21-year-old male patient who was suff-ering from low back pain for 2 years. The patient was initially diagnosed as having a scoliosis of the thoracolumbar spine. There was no symptomatic alleviation after initial rehabilitation therapy.
