To measure functional status, determine risk of functional decline and assess consistency between responses and standardized instruments. Design: A mailed survey which measured functional impairment, recent hospitalization and bereavement. A positive response on at least one of these factors indicated that the individual was “at risk” for functional decline. A random sample (n=73) of “at risk” subjects (specifically, family practice patients aged 70 and older) were assessed by a nurse. Results: The response rate was 89% (369/415), 59% of seniors were female and the mean age was 77.1 (SD=5.5) years. Self-reported risk, based on activities of daily living (ADLs), was …

The continuing debate over the issue of Medicare coverage for outpatient prescription drugs gave rise to this background paper, which discussed several key questions: Whose responsibility is it to provide coverage? Who should be covered? What should be covered? Who should pay for prescription drug coverage? How should costs be controlled? In addition, the paper explored options for structuring a Medicare outpatient prescription drug benefit. It also contained a glossary.

A Two-Year Prospective Study Of Infant Postural Torticollis, Baii-Jia Yang, Hwei-Chu Lin

Torticollis is a common physical finding in infants, and congenital muscular torticollis is the most commonly reported type. If the only findings are head tilting and facial asymmetry without limitation of ROM of the neck and lack of pathological change of the sternocleidomastoid muscle, infant postural torticollis is the most likely diagnosis. From June, 1995 to May, 1997 we prospectively studied 211 cases of postural and muscular torticollis and excluded other causes of head tilting. The effect of vibration on the median nerve excitability was studied in 18 healthy subjects by applying the vibrator on the skin overlying either the median nerve or the tendon of flexor digitorum superficialis. The excitability index (EI) was used to assess the median nerve excitability. This index was expressed as the ratio of the amplitude of submaximal evoked compound action potentials (SECAP) before vibration (baseline level) to that during or after the vibration. Comparison between the experimental and the control side showed that vibration over the median nerve did not induce any significant change of EI. However, vibration over …

The aim of this investigation is to define the normal values and the boundaries of compound muscle action potential (CMAP) dispersion of a specific age group in Chinese population. Measurements were made from median and ulnar motor nerves of 30 normal volunteers, age between 25-35 years. Supramaximal stimulations were applied to wrist, elbow and axilla of median and ulnar nerves individually. Negative peak areas, amplitudes and durations were recorded and the percentage of dispersion were calculated for each parameter. Using the non-invasive method to monitor the pulmonary artery pressure in the exercise training program of the chronic obstructive pulmonary disease patients might reduce the incidence of cardiac complication from the pulmonary hypertension and hypoxia. The purpose of this study was to detect the femoral artery pulse pressure and its harmonic changes of the rabbits in the hypoxia status.Ten rabbits underwent the femoral artery catherization for detecting their femoral artery pressure and their harmonics. Ten percent carbon dioxide mixed gas was inhaled to reduce the oxygen saturation of the rabbits from 95 % to 90% or lower. The mean femoral …

Acute leukemia is the most common malignancy involving the central nervous system (CNS) among childhood. Neurological manifestations of leukemia frequently occur during the course of acute leukemia. However, it is rarely an isolated feature as the initial presentation of leukemia. The spinal cord and spinal roots are less frequently involved than other CNS regions. The changing roles and challenges for higher education and the increased productivity required of faculty are driving forces for the development of more diverse and efficient teaching methods. Educational trends are toward more learner-centered materials. In response to these trends, colleges and universities are now offering courses at a distance and in forms other than traditional delivery.

Online courseware materials may be a viable means of fulfilling these numerous requirements but are very resource-intensive to develop. Multiple approaches to developing online learning have been tried, with limited success. The primary approach has been for faculty to enter their own course …

This study evaluated marginal fit discrepancy of the abutment-implant body interface employing various components commonly utilized for implant framework fabrication. Four types of components ( castable plastic patterns, premachined gold abutments, premachined titanium abutments and CAD-CAM custom premachined titanium abutments) were evaluated. Five castable plastic patterns and five premachined gold abutme~ts from each of two manufacturers, five premachined titanium abutments and five CAD-CAM custom premachined titanium abutments were used. Components were affixed to a 3.75 x 10 mm dental implant and standardized measurements were obtained of the abutment-implant interface using a computerassisted microscope at various experimental time intervals. Objective:
The objective of present study was to observe the histopathological pattern of intracranial tumors in children (< 15 yrs) and to correlate the site of lesion along with the histological diagnosis.
Setting:
The study included consecutive cases of intracranial tumors diagnosed in children (< 15 yrs.) in the section of histopathology at the Aga Khan University Hospital, Karachi during the period of three years.
Methods:
The initial histological evaluation of these lesions was performed on H and E stained section of paraffin embedded tissue. Special stains and immunohistochemical analysis was done whenever indicated.
Results:
During the study period, fifty-four cases of intracranial tumors were diagnosed in children. The age ranged from 1-1/2 years to 4 years with male to female ratio of 1.1:1. Astrocytoma comprised 39% of all intracranial tumors of childhood. Medulloblastoma …

Array technologies have made it straightforward to monitor simultaneously the expression pattern of thousands of genes. The challenge now is to interpret such massive data sets. The first step is to extract the fundamental patterns of gene expression inherent in the data. This paper describes the application of self-organizing maps, a type of mathematical cluster analysis that is particularly well suited for recognizing and classifying features in complex, multidimensional data. The method has been implemented in a publicly available computer package, GENECLUSTER, that performs the analytical calculations and provides easy data visualization.
