Classroom Instructional Assistants are being used more frequently to provide care for medically fragile children who are mainstreamed into the regular education setting. The purpose of this evaluative study was to develop a beginning health curriculum that could be provided as an inservice program for classroom Instructional Assistants. Content for the inservice consisted of handwashing, medication administration, asthma, and seizures. This study used a pretest and posttest design to look at the effectiveness of the educational inservice. There were 17 participants, 16 of whom completed the pretest and posttest. รายงานการเกิดภาวะ Cushing's syndrome ในสุนัขพันธุ์พุดเดิลขนาดเล็ก เพศเมีย อายุ 7 ปี ซึ่งมา รักษาที่โรงพยาบาลสัตว์เล็ก คณะสัตวแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย จากการตรวจพบว่าสุนัขมีช่อง ท้องขยายใหญ่และมีขนร่วง 2 ข้างของลำตัว สุนัขแสดงอาการปัสสาวะและดื่มน้ำมาก จากการตรวจนับเม็ด เลือดพบว่า เม็ดเลือดขาวชนิด lymphocyte และ eosinophil ลดลง การตรวจทางชีวเคมีในเลือดพบเอนไซม์ alanine aminotransferase และ alkaline phosphatase เพิ่มขึ้นร่วมกับการเพิ่มขึ้นของกลูโคส คลอเรสเตอรอล และปริมาณโซเดียม จากการตรวจโดยการเอกซเรย์ช่องท้องพบมีการขยายใหญ่ของตับ การตรวจด้วยอัลตราซาวด์ พบมีการขยายใหญ่ของต่อมหมวกไตทั้ง 2 ข้าง สุนัขได้รับการทดสอบโดยการฉีด dexamethazone ขนาดต่ำและสูงตามลำดับ เพื่อเป็นการทดสอบและแยกภาวะ Cushing syndrome ว่ามีสาเหตุมาจากต่อมใต้ สมองหรือจากต่อมหมวกไตเอง จากผลการทดลองชี้แนะว่าสุนัขเป็น Cushing's syndrome ชนิดที่มีความผิด ปกติของต่อมใต้สมอง (pituitary dependent adrenocorticism) สุนัขเสียชีวิต 1 เดือน ภายหลังจากที่ได้รับการ รักษาที่โรงพยาบาล และได้ทําการชันสูตร ผลจากการตรวจทางจุลพยาธิวิทยา พบว่าต่อมหมวกไตทั้ง 2 ข้าง มีการเจริญขยายใหญ่ขึ้น นอกจากนี้ยังพบพยาธิสภาพที่อวัยวะอื่น ๆ รวมทั้งตับและตับอ่อน ซึ่งพยาธิสภาพ ดังกล่าวพบในสุนัขที่เป็น canine Cushing's syndrome

Although viral gene expression occurs in the peripheral nervous system during acute infection, bovine herpesvirus 1 (BHV-1) gene expression is extinguished, many neurons survive, and latency ensues. The only abundant viral transcript expressed during latency is the latency-related (LR) RNA, which is alternatively spliced in trigeminal ganglia during acute infection (L. Devireddy and C. Jones, J. Virol. 72:7294–7301, 1998). Electrical burns dramatically affect the nervous system, muscles, bones, and other tissues within the human body. Myelopathy after an electrical burn has seldom been mentioned.This article describes two cases of myelopathy admitted to our burn center after a high-voltage electrical burn incurred in the workplace, with symptoms onset around 16th and 4th day, respectively. The patients both experienced a course of deterioration, followed by a slow and partial recovery. The possible mechanisms and pathophysiology associated with electric burns are discussed.

Traumatic carotid artery dissection is often not recognized at the time of admission, possibly leading to cerebral infarction. This article reports on a 77-year-old man suffering from headache and neck pain after a traffic accident. Although the initial head computed tomography ( CT ) scan revealed no intracranial lesion, progressive left limbs weakness, slurred speech and blurred vision of his right eye developed on the next day after the accident. This study attempts to define the reliability of ice-water test (IWT) in detecting the detrusor reflex in patients with chronic spinal cord lesion (SCL) and further explore its clinical application. 220 patients with SCL over a period of four years were included in this study. 105 of the patients were rated as the acute group with the injury less than six months. 115 patients with lesions over six months were grouped as the chronic one. Uroflowmetry and ultrasonic residual urine measurements had been done to 85 urologically asymptomatic diabetic patients and 20 control subjects. The prevalence of voiding dysfunction was 60% (51/85), and increased residual urine was found in 27% (23/85) of diabetic patients. The abnormal findings included increased residual urine (23/85), interrupted urine stream (9/85), voiding with abdominal strain (31/85), low flow rate (28/85), and enlarged bladder capacity (6/85). The group with voiding dysfunction had lower maximal and average flow rate, longer voiding time, and larger residual urine amount than the diabetic patients with normal voiding function and the healthy controls. A total of 827 Functional Independence Measure (FIM) scores, which from three separate cohorts research from 1995 to 1998 in Taiwan, were conducted the structural analysis of FIM. The 872 subjects included residents of nursery center, outpatients of neurology and neuro-surgery clinics as well as inpatients and outpatients of rehabilitation department. Both principle factor analysis and Rasch model analysis were able to identified two major factors of FIM in Taiwan. We utilized the mouse mammary tumor virus (MMTV) long terminal repeat (LTR) in vivo to understand how the interaction of the glucocorticoid receptor (GR) with a nucleosome-assembled promoter allows access of factors required for the transition from a repressed promoter to a derepressed, transcriptionally competent promoter. A mutation (C644G) in the ligand binding domain (LBD) of the mouse GR has provided information regarding the steps required in the derepression/activation process and in the functional significance of the two major transcriptional activation domains, AF1 and AF2. จุดประสงค์ของงานวิจัยนี้เพื่อนำวิธีการปฏิสนธินอกร่างกายมาทดสอบความสมบูรณ์พันธุ์ของพ่อ สุกรนำน้ำเชื้อที่รีดจากพ่อสุกรพันธุ์ดูร็อค ลาร์จไวท์ และแลนด์เรซ ที่ใช้อยู่ในฟาร์มสุกรพ่อแม่พันธุ์ จำนวน สายพันธุ์ละ 3 ตัว มาปฏิสนธิกับโอโอไซต์ที่เก็บมาจากรังไข่ของสุกรสาวและนำมาเลี้ยงไว้เพื่อให้พร้อมปฏิสนธิ โดยเลี้ยงรวมกันนาน 18 ชม. ที่อุณหภูมิ 39 °C ในบรรยากาศ 5% คาร์บอนไดออกไซด์ แล้วนำตัวอ่อนที่ได้ไป เลี้ยงในน้ำยาเลี้ยงตัวอ่อนชนิดที่ซีเอ็ม 199 ผสมกับ 20% ฟิตัล คาล์ฟ ซีรั่ม นาน 5 วัน เพื่อตรวจดูการแบ่ง ตัวของตัวอ่อน จากการศึกษาพบว่าพ่อพันธุ์สุกรแต่ละตัวในแต่ละพันธุ์ มีอัตราความสำเร็จในการปฏิสนธิ คือ อัตราการแบ่งตัวและอัตราการพัฒนาของตัวอ่อนเป็นระยะมอรูล่าแตกต่างกันอย่างมีนัยสำคัญ (P<0.001 และ P<0.05 ตามลำดับ) ในกรณีที่ไม่พบความแตกต่างของอัตราการแบ่งตัวในระยะแรกระหว่างพ่อสุกรแต่ละตัว อัตราการพัฒนาเป็นตัวอ่อนระยะมอรูล่าใช้แยกความแตกต่างของคุณภาพน้ำเชื้อได้ ผลการทดลองนี้สรุปได้ว่าสามารถประยุกต์เทคนิคการปฏิสนธินอกร่างกายมาใช้เพื่อทดสอบความสามารถในการปฏิสนธิของพ่อสุกรพันธุ์ได้

ทำการศึกษาความชุกของการเกิดภาวะโลหิตจางที่มีผลมาจากภูมิคุ้มกันในสุนัขในประเทศไทยโดยทำการศึกษาย้อนหลังจากค่าโลหิตวิทยาของสุนัขซึ่งมีประวัติแสดงอาการโลหิตจาง และมีค่าเม็ดเลือดแดงอัดแน่น ต่ำกว่าร้อยละ 29 ที่เข้ามารับการรักษา ณ โรงพยาบาลสัตว์เล็ก คณะสัตวแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย คลินิกและโรงพยาบาล สัตว์เอกชน ในระหว่างเดือนมิถุนายน ถึงกันยายน 2541 จำนวน 55 ตัวและทําการศึกษาไปข้างหน้า ในสุนัขที่สงสัย ภาวะโลหิตจางเนื่องจากภูมิคุ้มกันในระหว่างเดือนตุลาคม ถึงธันวาคม 2541 จำนวน 13 ตัว ผลการศึกษาพบว่าความ ชุกของการเกิดภาวะโลหิตจางที่มีผลมาจากภูมิคุ้มกันในสุนัขกลุ่มที่ 1 เท่ากับ 5.45% เนื่องจากตรวจพบเซลล์เม็ดเลือด Spherocyteและพบการจับกลุ่มของเม็ดเลือดแดงในน้ำเกลือ พันธุ์สุนัขที่ตรวจพบว่ามีภาวะโลหิตจางมากที่สุด คือพันธุ์ผสม 70.90% (40/55) โดยมีอัตราส่วนเพศผู้ต่อเพศเมียเท่ากับ 1.6:1 ช่วงอายุที่พบมากในสุนัขกลุ่มนี้คือ ระหว่าง 2-8 ปี พบ 30.19% (17/55) ส่วนในกลุ่มที่ 2 ทำการศึกษาไปข้างหน้า พบว่าความชุกของการเกิดภาวะโลหิต จางที่มีผลมาจากภูมิคุ้มกันในสุนัขที่ให้ผลบวกต่อการทดสอบด้วยวิธีปาเป็น 76.92% (10/13) พันธุ์ที่พบมากที่สุดคือ สุนัขพันธุ์ผสม 50% (5/10) โดยมีอัตราส่วนเพศผู้ต่อเพศเมียเท่ากับ 1.5.1 สำหรับช่วงอายุที่พบมากในสุนัขกลุ่มนี้คือ 2-8 ปี พบ 80% (8/10) อาการทางคลินิกพบว่า สุนัขทั้งหมด ซึม เบื่ออาหาร เยื่อเมือกซีด อาการที่พบบ่อยที่สุด คือ อ่อนแรง 80% (8/10) ภาวะเกล็ดเลือดต่ำ 80% (6/10) อาเจียน 50% (5/10) มีไข้ 40% (4/10) และหายใจเร็ว 40% (4/10) เมื่อนำสุนัขในกลุ่มที่ 2 นี้จำนวน 5 ตัวมาทำการทดสอบด้วยวิธีคูม ได้ผลบวกจำนวน 2 ตัวบ่งชี้ถึงการเกิดภาวะโลหิตจางเนื่องจากภูมิคุ้มกันในสุนัข

Abstract: Hospital closures occurred nationally with increasing frequency between 1970 and 1990. In particular, large urban areas experienced a dramatic number of closures. Of the 61 general hospitals operating in Chicago in 1970, 22 (36%) had closed by 1991. While a growing body of literature has examined the etiology and determinants of hospital closure over the last two decades, few empirical studies have focused on the neighborhood correlates of closure, and none have examined specific health outcomes associated with hospital failure. Using Professionally Trained Actors In Social Work Role-Play Simulations, Helen E. Petracchi

The literature available to social work educators contains limited discussion of the use of role play simulation in the classroom. This paper presents the experiences of first-year MSW students in a foundation generalist practice class who were evaluated using role play for their final examination. In a unique arrangement, clients in these simulations were role-played by firstyear acting students from the university's theater arts department. The reaction of the social work students to role play with professionally trained actors is described and discussed from voluntarily submitted descriptions of the experience.

เต้านมอักเสบในโคเป็นปัญหาที่ก่อให้เกิดความสูญเสียทางเศรษฐกิจ โดยสาเหตุส่วนใหญ่เกิดจากการติดเชื้อแบคทีเรีย การตอบสนองของภูมิคุ้มกันเป็นขบวนการในการป้องกันตนเองที่สำคัญต่อการรุกรานของจุลินทรีย์ที่เข้ามา ในเต้านม โดยทั่วไปกลไกการป้องกันตนเองของเต้านม ประกอบด้วย ลักษณะทางกายวิภาคของเต้านม ระบบภูมิคุ้ม กันชนิดเซลล์ และระบบภูมิคุ้มกันที่ไม่เกี่ยวข้องกับเซลล์โดยตรง ลักษณะรูหัวนม และการรีดนมออกเป็นขบวนการ ป้องกันที่สามารถลดการติดเชื้อที่เต้านม เซลล์แมคโครเฟจ และ นิวโทรฟิลล์สามารถเก็บกินเชื้อที่ก่อโรคในเต้านม นอกจากนั้น ในระบบภูมิคุ้มกันเซลล์แมคโครเฟจยังสามารถหลั่งสารสื่ออักเสบ และ ถ่ายทอดสัญญาณที่เกี่ยวข้องกับ แอนติเจนให้กับเซลล์อื่นๆ ลิมโฟไซด์ ทั้ง บีเซลล์ และ ทีเซลล์ มีบทบาทสำคัญในระบบภูมิคุ้มกันในเต้านมเช่นเดียว กัน ระบบภูมิคุ้มกันที่ไม่เกี่ยวข้องกับเซลล์โดยตรง ได้แก่ แอนติบอดี แลคโตเฟอริน ไลโซไซม์ ระบบคอมพลิเมนท์ และระบบเอนไซม์แลคโตเปอร์ออกซิเดส/ไธโอไซยาเนท/ไฮโดรเจนเปอร์ออกไซด์ เป็นต้น ระบบป้องกันตนเองของเต้า นมมีการเปลี่ยนแปลงอย่างมาก ในช่วงแรกหลังระยะพักนม และในช่วงคลอดที่มีการกดของระบบภูมิคุ้มกัน ในช่วงเหล่า นี้จึงเป็นช่วงที่มีความไวต่อการติดเชื้อเป็นอย่างมาก ในปัจจุบันการป้องกันเต้านมอักเสบยังไม่มีวิธีใดที่สามารถลดปัญหา นี้ได้ การกระตุ้นระบบภูมิคุ้มกันในเต้านม เช่น โดยการใช้วัคซีน ไซโตคีนส์ โสม หรือ กลูแคน อาจลดปัญหาเหล่านี้

ผลของความถี่ในการรีดเก็บน้ำเชื้อต่อคุณภาพน้ำเชื้อสุนัขไทยหลังอาน, เตือนตา ชาญศิลป์, สมชาย โอฬารกนก

สุนัขพ่อพันธุ์ไทยหลังอานอายุ 18- 24 เดือน จำนวน 5 ตัว ถูกรีดเก็บน้ำเชื้อด้วยการกระตุ้นด้วยนิ้วมือรวม 80 ครั้ง โดยการรีดเก็บน้ำเชื้อแบ่งเป็น 4 ช่วงเวลา ๆ ละ 20 ครั้ง คือ รีดเก็บน้ำเชื้อทุกวัน ทุก 2 วัน ทุก 3 วัน และทุก 4 วัน พบว่าช่วงเวลาของความถี่ในการรีดเก็บน้ำเชื้อมีอิทธิพลต่อคุณภาพน้ำเชื้อเกือบทุกลักษณะ (P<0.05) ยกเว้น ปริมาตร การรีดเก็บน้ำเชื้อทุก 2 วัน มีผลทำให้ความเข้มข้นของตัวอสุจิและจำนวนตัวอสุจิ / การหลั่งน้ำเชื้อมากที่สุด โดยมีค่าเฉลี่ย 0.153 ± 0.221 x 10 ตัว / มล. และ 1884.45 ± 1994.50 x 10 ตัว ตามลำดับ ส่วนความผิดปกติ ของตัวอสุจิและเปอร์เซ็นต์ตัวตายของตัวอสุจิพบน้อยที่สุดที่ช่วงเวลาการรีดเก็บน้ำเชื้อทุก 3 วัน โดยมีค่าเฉลี่ยความผิด ปกติของตัวอสุจิ และเปอร์เซ็นต์ตัวตายของตัวอสุจิเท่ากับ 6.18 ± 2.68 และ 5.20 ± 3.03% ตามลำดับ สำหรับอัตรา การเคลื่อนไหวเฉพาะตัวของตัวอสุจิ ไม่มีความแตกต่างกันทางสถิติ (P<0.05) ระหว่างการรีดเก็บทุก 2 วัน และทุก 3 วัน โดยมีค่าเฉลี่ยเท่ากับ 87.75 ± 4.72 และ 87.75 ± 4.13 % ตามลำดับ การศึกษาครั้งนี้แสดงให้เห็นว่า ช่วงเวลา ของความถี่ในการรีดเก็บน้ำเชื้อมีผลต่อคุณภาพน้ำเชื้อโดยช่วงเวลาที่เหมาะสมของความถี่ในการรีดเก็บน้ำเชื้อสุนัขไทยหลังอานคือ รีดเก็บน้ำเชื้อทุก 2-3 วัน

A9-year-old female Shih Tzu dog was presented with a 3-day history of anorexia, depression, vomiting, and watery diarrhea. The clinical symptoms progressed continuously until the dog died, 3 days after admission. At necropsy numerous white nodules of various sizes were found in the small intestine, particularly the ileum. Microscopically, the mass revealed diffused, infiltrative growth of a well-differentiated adenocarcinoma invading the submucosa and muscular layers of the small intestinal wall. To identify an effective medium for communicating with adolescents in a large-scale, cost-effective violence prevention program.

A set of youth violence prevention programs was established at The Stamford Hospital, a level II trauma center. The traveling version of the program was presented to middle school students in four parts: 1) a rap music video created by our violence prevention staff, 2) a facilitated discussion about dealing with anger, 3) a video of a trauma resuscitation in our emergency department, and 4) a commercial video of a teenage boy paralyzed after a gunshot wound.
