There are two ways in which impulses can be generated: one is by the spontaneous discharge of a pacemaker cell in either the normal site or a subsidiary site, and the other is by reentry. The concept of reentry is a very old one postulated by clinical cardiologists many years before its demonstration.

In summary, we have found a close association between the stress of A-V nodal conduction and the occurrence of atrial dispersion or aberration. We could not explain atrial dispersion on the basis of incomplete recovery of atrial activity, slowing of atrial conduction, or the occurrence of atrial fusion. We postulate that the phenomenon of dispersion of atrial activation is related to the functional dissociation within the A-V node seen during situations which stress A-V conduction. This phenomenon may represent a mechanism whereby a second area of "turnaround" other than the A-V node is involved in reentry atrial tachyarrhythmias. Indeed, such …

This title implies that one has already decided that one's patient needs to be artificially ventilated, that is, that he is in respiratory failure. How does one diagnose respiratory failure? Much of what I have to say in this regard is in terms of arbitrary limits, values, and guidelines. Since these guidelines are arbitrary, there may exist legitimate grounds for differences of opinion about some of them. However, we have found these guidelines to be quite helpful, and experience would indicate that they are reasonable.

The advances in our knowledge of renal hypertension over the last 75 years represents outstanding accomplishments in experimental and clinical medicine. A review of the highlights of this important investigation in hypertension should begin with Tigerstedt and Bergman's work in Scandinavia which demonstrated in 1898, that extracts of kidney possessed a hypertensive action. They gave the name "renin" to the impure substance which produced this effect. For the next 30 years much work was done on the vasoconstrictor action of extracts of the adrenal medulla, the nervous system and the kidney.

The scope of toxicology, therefore, may extend from the study of a single element such as lead to a complex, chemically uncharacterized substance such as snake venom; from an overdose of heroin to the effects of long-term exposure to low levels of pesticides on anticonvulsant therapy. For the purpose of this discussion, we shall narrow the limits of the broad field of toxicology considerably, but it is important to be aware of the breadth and depth to which toxicology may at times extend.

Prepackaged laboratory kits for performing diagnostic procedures are frequently the most suitable alternative in the selection of laboratory methods, especially in the physicians' offices and small laboratories. Because of the previous lack of governmental regulations covering the manufacture of kits, many kits now on the market do not perform adequately and may produce misleading results. Each laboratory must evaluate each type of kit before it is put into routine use. For many years selenium was known to the livestock industries only because of its toxic properties. Some soils, particularly in the United States of America, were so high in selenium that many animals died after grazing plants on these soils.

In 1957 it was discovered that selenium supplements could prevent a number of diseases and, from that time, selenium has been regarded as an essential trace element.

White muscle disease (WMD) was first recognised in sheep in Western Australia in 1960 and has occurred each year since.
