Walking Pneumonia - Not Always So Timid A Case Of Severe Ards Due To Fulminant Mycoplasma Pneumonia, Tharun Shyam, Momna Noor, Yan Naing Tun, Saumya Nanda, Omair Khan, Ravikaran Patti

Journal of Community Hospital Internal Medicine Perspectives

Mycoplasma pneumoniae pneumonia (MPP) most commonly presents as bronchitis or as a self-limited ‘walking’ community acquired pneumonia. A fulminant form of MPP may occur in 0.5% to 2% of patients and is characterized by rapidly progressive respiratory failure and Acute respiratory distress syndrome (ARDS). Our case highlights the importance of considering fulminant MPP as a cause of ARDS, especially in the young adult and middle-aged population. We review the current literature and management principles of this rare disease with an emphasis on antibiotic stewardship and corticosteroid therapy. A Rare Case Of Familial Hypocalciuric Hypercalcemia In Patient With Pancreatitis, Muhammad Ijaz Khan, Tanvi Jain, Muhammad Mohsin Abid, Samra Israr, Muhammad Haris Khan

Journal of Community Hospital Internal Medicine Perspectives

Familial hypocalciuric hypercalcemia (FHH) is a rare autosomal dominant condition caused by mutations in the calcium-sensing receptor gene (CASR), leading to asymptomatic hypercalcemia. Here, we report a case of hypercalcemia in a patient with acute pancreatitis, subsequently diagnosed with FHH. A 41-year-old male presented with abdominal pain and elevated pancreatic enzymes. Imaging revealed changes consistent with acute pancreatitis. Surprisingly, serum calcium was elevated, which is uncommon in acute pancreatitis. Further work-up demonstrated normal parathyroid hormone (PTH), PTH-related peptide (PTHrp), and vitamin D levels. A 24- hour urinary calcium excretion of 24 mg/24 hr and a calcium-to-creatinine clearance ratio (CCCR) of …

Immunoglobulin A (IgA) nephropathy is the most common glomerulonephritis worldwide. While IgA nephropathy mainly presents with renal manifestations, systemic symptoms associated with vasculitis are uncommon. Oftentimes, creatinine is used as a serum marker for renal dysfunction; however, it may not always be sensitive, especially in patients with low muscle mass. In such cases, cystatin C may prove to be a better indicator of renal function. Autoimmune hepatitis (AIH) is a chronic inflammatory liver disease characterized by the presence of circulating autoantibodies. The spectrum of disease manifestations extends from asymptomatic cases to mild symptoms and, in rare instances, acute liver failure. AIH is a diagnosis of exclusion, supported by the detection of autoantibodies such as anti-smooth-muscle antibody (ASMA). This case describes a 43-year-old female with myasthenia gravis, receiving monthly intravenous immunoglobulin (IVIG) infusions, who presented with persistently elevated liver enzymes and mildly elevated ASMA titers across multiple clinical encounters. Objective: The right-sided positioning of heart during embryonic development is termed dextrocardia, and it may be isolated or associated with visceral dextroposition or malformation. However, there are few studies on the condition. This study aimed to determine the incidence of cardiac and non-cardiac malformations, long-term follow-up, and survival analysis of patients with dextrocardia. Methods: This was a retrospective chart review of dextrocardia cases at King Faisal Specialist Hospital & Research Centre in Riyadh, Saudi Arabia from April 22, 1975, to December 31, 2016. Journal of Community Engagement and Higher Education

This paper reflects on a music therapy community engagement project that incorporates clinical training, service learning, and community music therapy. Two faculty members and a practicum student in a Midwestern university engaged community members connected to Parkinson’s disease to create a music-based program. We offer a conceptual framework that connects approaches situated within community music therapy (CoMT) qualities and community-based participatory research (CBPR) principles. We share project processes, findings, and recommendations, emphasizing all as equal stakeholders.

In this paper, we consider an extended SEIR compartmental model that incorporates young and old interacting subpopulations, allowing for cross-group transmission dynamics. Implicit behavioral changes are included to determine the influence of social behavior on coronavirus transmission dynamics. The basic reproduction number, the average number of secondary cases of infection produced by a single primary case, is derived for both the explicit and implicit model using the next-generation matrix method. We solve the associated differential equation systems and estimate useful parameters in the explicit model using physics-informed neural networks (PINNs). This paper presents the development of heritage-focused outreach materials with the objectives of positively impacting traffic safety culture for Indigenous youth to reduce the number and severity of traffic crashes impacting these communities. The approach presented in this paper includes involving local community leaders in traffic safety education and incorporating culturally based material in outreach education. This could empower Tribal communities to preserve their cultural identities while acquiring the expertise needed to promote traffic safety.

This article presents an interdisciplinary collaboration to address the needs of older adults at risk of social isolation exacerbated by the COVID-19 pandemic. An intergenerational service-learning program developed by the Pennsylvania Department of Aging was offered to several universities across the state of Pennsylvania. The authors share their experience and lessons learned from pairing older adults with graduate students studying in the fields of Gerontology, Public Health, and Social Work.

A midwestern Jesuit university worked to align an existing IPE program with a new core curriculum and its corresponding student learning outcomes (SLO’s). This paper describes the collaborative curriculum development process used within an institution to develop its first University Core Curriculum. The University-core curriculum development process resulted in a framework, outcomes, and essential criteria associated with nine student learning outcomes (SLO’s). A student-centered coaching program grounded in Jesuit and Mercy traditions, called “Coaching for Care,” was implemented at a midwestern Jesuit and Mercy institution. The purpose of the project was to evaluate the development of future nurse educators’ pedagogy, foster holistic licensure examination preparation among pre-licensure nursing students, and examine students’ perceptions towards the program’s guiding Jesuit and Mercy values. Pre-licensure students who received coaching felt that their uniqueness/holism, accountability, and reflection were important for their licensure exam preparation at significantly higher rates than pre-licensure students who did not receive coaching. Background: The aim of the study was to assess the effectiveness of the myofascial self-massage technique of the gastrocnemius muscle in determining the possibility of increasing pain tolerance and individual pain threshold, which could increase exercise efficiency, allow for greater training loads and accelerate post-exercise regeneration, as well as to determine the subjective assessment of pain intensity using the VAS scale. Polycystic Ovary Syndrome (PCOS) is a leading cause of infertility, primarily due to anovulation caused by hormonal imbalances, insulin resistance, and disrupted ovarian function. This paper describes a 28-year-old woman with PCOS who presented with subfertility after failing to conceive for 10 months. Initial treatment involved Metformin for insulin resistance, vitamin D, folic acid, and Dydrogesterone for cycle regulation. Ovulation induction with Letrozole was initiated but showed a suboptimal response in three cycles. Success was achieved in the fourth cycle by combining Letrozole with injectable Menotropins (HMG), leading to improved follicular growth, endometrial thickness, and successful conception. Synthesis Of A New Composite Nanomaterial Consisting Of Bentonite-Polyethylene Glycolphthalate, Tariq Yassin Mahmoud

In this study, a new composite material was characterized and prepared by polymerizing ethylene glycol located among bentonite layers with phthalic anhydride. The results showed that the polymer bonds with the clay structure through hydrogen bonding. The polymerization process led to the destruction of the three-dimensional crystal structure of the clay and the isolation of its layers in the form of two-dimensional nanoscale shells. The growth of the polymer around the isolated clay layers increased the total particle size at the microscopic scale.
