A Classification Scheme For Medical Expenditures, James Veney, Arnold Kaluzny
The Journal of Sociology & Social Welfare
Medical care represents a significant portion of society's response to problems affecting social welfare. While the problems surrounding medical care are complex, a major part of these problems can be attributed to the fact that medical care expenditures are viewed indiscriminately without regard for the nature of the expenditures themselves. This paper presents a framework to differentiate various types of medical care expenditures. The paper argues that medical care expenditures can be classified as either instrumental-consummatory or as external-internal relative to the medical care system. The consequences of this classification and some conclusions which may be drawn from it are …

พยาธิใบไม้หลอดลมในเป็ด : รายงานสัตว์ป่วย, มานพ ม่วงใหญ่, ประจักษ์ พุ่มวิเศษ, สมชัย เสถียรเนตร

พยาธิใบไม้หลอดลมในเป็ด (Tracheophilus sisowi) เกิดขึ้นที่ฟาร์มเป็ด เนื้อแห่งหนึ่ง ที่ตำบลบางภาษี อำเภอบางเลน จังหวัดนครปฐม อาการของโรคเริ่มแสดงใน เป็ดเมื่ออายุประมาณ 1 เดือน เกิดหลังจากที่ปล่อยให้หากินในนาข้าวที่เพิ่งจะเก็บเกี่ยวเสร็จ ประมาณ 10-14 วัน อาการที่แสดงมีไอและจาม จะไอมากเมื่อเวลากินอาหารผสม ต่อมาเมื่อ อาการมากขึ้นเสียงร้องจะค่อยลง (เสียงแหบ) ปากอ้าและตายในที่สุด ผ่าซากพบพยาธิจํานวน มากในทางเดินหายใจตั้งแต่ลิ้นปิดเปิดหลอดลม, หลอดลมใหญ่, หลอดลมเล็ก,หลอดลมฝอยและใน เนื้อปอด ในรายที่เป็นน้อยเป็ดจะกินอาหารได้น้อย โตช้า ทำให้เกิดการสูญเสียทางเศรษฐกิจ เป็นอย่างมาก

โรคบางวัวเป็นโรคระบาดในเป็ดที่เกิดขึ้นครั้งแรกที่จังหวัดฉะเชิงเทรา คุณสมบัติของไวรัส ที่ทำให้เกิดโรคบางวัวแตกต่างกับคุณสมบัติของไวรัสที่ทําให้เกิดโรค Duck Plague หลายประการ จึงทําให้เชื่อว่า ไวรัสของโรคบางตัวเป็นคนละชนิดกับไวรัสของโรค Duck Plague ถ้าไวรัสของโรคบางวัวเป็น Strain หนึ่งของไวรัส Duck Plague แล้ว คุณสมบัติของไวรัสทั้งสองควรจะเหมือนกัน ส่วนอาการและวิการอาจจะแตกต่างกันออกไปได้

Serum Calcium, Phosphorus And Alkaline Phosphatase In, Healthy And Osteomalacia Horses, Supis Chindavanig, Peerasak Chantraprateep
The Thai Journal of Veterinary Medicine
Normal values of serum Ca, P and ALP in horses in relation to age, breed and environment were studied. Mean serum Ca, Р and ALP in mixed breed horses, age 1 to 6 years were 11.80 ± 1.25, 3.29 ± 0.65 and 7.76 ±2.31, age 7 to 12 years were 12.10 ±0.99, 2.84 ± 0.66 and 6.06± 1.46 respectively. Serum ALP activity of thoroughbreds and ponies were not significant different from that of mixed breed. The differences in P concentration among breeds were significant. Effects of intrarenal artery perfusion on kidney function in the dog. Either the hypertonic solution (537 mOsm/kg) or the hypotonic solution (27 mOsm/kg) were infused directly into left or right renal artery at the rate of 2.5 ml/min instead of the control isotonic saline solution (280 mOsm/kg). During experimental period the contralateral kidney received isotonic saline solution continuously as during the control period. Infusion of hypertonic saline solution, raising sodium concentration in that renal artery by 4.59 ± 1.18 (SD) μEq/ml, caused a significant increase in V, UNa .V and VCl.V. Some Aspects Of Steroids Radioimmunoassay (Ria) Involved In The Study Of Weak Estrous Phenomenon Of Swamp Buffalo, Maneewan Kamonpatana
The Thai Journal of Veterinary Medicine
The paper illustrates some aspects and problems of steroids RIA findings, obtained during the studies on the problem of weak estrous in swamp buffaloes. The work described herein which stem from three aspects of steroids RIA; 1) chromatographic technique of steroids separation 2) immunoreactivity of labelled I steroids and displacement efficiency; 3) technique of the separation of free and bound fractions. The nonaffinity and low displacement efficiency are also discussed.

Much of the emphasis in the pathogenesis of diabetes mellitus has justifiably been placed on the endocrine gland, the pancreas. Extensive studies on the biosynthesis and release of insulin from the beta cell, bihormonal control of metabolism by insulin and glucagon, and more recently the role of somatostatin have attracted the attention of students of the subject; but considerable evidence exists to suggest at least some role of tissue resistance to insulin in the pathogenesis of this disorder. In summary, I believe in aiming for the best possible control for each patient through an empirical approach which seeks to arrive at the best schedule for the individual patient. We want especially tight control for the pregnant diabetic, and for this patient it is likely that multiple insulin injections are needed. If further experience confirms the benefits of multiple injections for juvenile patients, this type of treatment may also be indicated for these patients.

In conclusion, comment is made on the findings of the University Group Diabetes Program (UGDP). The purpose of the UGDP was to determine whether or not control of blood glucose levels would help to prevent or delay vascular disease in non-insulin-requiring diabetics. After 8 1/2 years of follow-up at 12 university-affiliated treatment centers, "The findings of the study indicate that the combination of diet and tolbutamide therapy is no more effective than diet alone in prolonging life. Moreover, the findings suggest that tolbutamide and diet may be less effective than diet alone or diet and insulin, at least insofar as …

Not many years ago the main purpose of “complement” seemed to be to drill holes in sheep erythrocytes. In the classic experiment which was part of every medical student’s microbiology laboratory experience, a magic stuff called complement, somewhat mysteriously obtained from guinea pigs, was either “fixed” or not “fixed” and the sheep cells either not lysed or lysed accordingly. That was about all there was to know about complement, and all one needed to know. Since immune responses play a major role in the development of connective tissue diseases, it is not surprising that a number of laboratory studies reflect these responses. Prior to the 1940s when rheumatoid and LE factors became widely known, one relied mainly on erythrocyte sedimentation rate and serum electrophoresis to identify protein abnormality. Elevated sedimentation rate depends on rouleaux formation, and rouleaux formation is dependent upon large asymmetric molecules of fibrinogen and gamma globulin in plasma. The demonstration of gamma globulin has become the cornerstone of the immunologist’s edifice. It is amazing to see how the subspecialty of immunology has …

Problems such as seizures and headache are regular features of many types of practice. Others such as acute ophthalmoplegias are infrequently seen outside the field of neurology. Bilateral ophthalmoplegia is that condition of weakness or paralysis involving one or more ocular muscles in each eye. Its sudden appearance due to an acute ocular myopathy is indeed unusual. Swash reported a single patient with acute necrotizing orbital myositis and carcinomatosis neuromyopathy. The more common bilateral ocular muscle diseases, such as dysthyroid exophthalmopathy, orbital myositis, and progressive external ophthalmoplegia, develop insidiously and are restricted to the orbit.

The future of antenatal genetic diagnosis is an exciting one. Recent advances include the application of chromosal banding techniques to identify subtle abnormalities and rearrangements; ultrasonography, using high resolution gray scale equipment to permit delineation of fetal soft tissue and skeletal anomalies; and fetoscopy, using a small fiberoptic instrument to view the fetus directly and observe its development. With these advances in technology, it should be possible to enable even more women at high risk for genetically diseased offspring to bear healthy children.

The examination of human chromosomes has been a part of the physician’s laboratory armamentarium since the correct diploid number of human chromosomes was established and a method was developed for the in vitro growth of peripheral blood leukocytes to yield metaphase chromosomes. The discovery that on ultraviolet microscopy (UV), metaphase chromosomes stained with fluorochrome dyes displayed a characteristic pattern of bright and dull bands unique for a given pair of homologous chromosomes, was a major technological breakthrough in human cytogenetics; for the first time, every chromosome in the karyotype could be unequivocally identified. The Genetic Counseling Clinic at the Medical College of Virginia, established by Drs. Peter Mamunes and R.B. Young in 1973, has been supported since its inception by a clinical service grant from the National Foundation – March of Dimes; it is one of 83 genetic counseling programs in the United States and one of three in Virginia that receive support from the Foundation. The Clinic provides counseling and diagnostic services for a variety of genetic disease and is the focus of clinical teaching and research activities of the Department of Human Genetics. In 1968, a program of screening for sickle trait carriers was begun as part of the work of the Hematology Division, Department of Medicine, at the Medical College of Virginia. It was felt that sickle cell anemia was more of a public health problem than was generally recognized, and in addition to instituting screening and education programs, data were collected to document the relative neglect of the problem.
