My remarks today focus on four legal aspects of SIDS; the first three are problems of long standing and the fourth is less well recognized, an immediate problem to some but more of a cloud on the horizon to others. At the outset, I want to emphasize that I bring you no certain solutions. Rather my more modest objective is to provide a focus and framework for further discussions.

For most parents the death of their infant is the first serious crisis in their lives. This event is regarded by all young parents as more stressful than previous deaths in the family, divorce, separation, alcoholism, or suicide. The infant death may be the first death in the family. The grief reaction is not stereotyped; it is as varied as the individuals experiencing it and the families of which they are a part. Several patterns of grief will be treated here.

A variety of pulmonary complications occur in patients during and following nonthoracic surgery. These may include hypoxemia, atelectasis, bronchitis, pneumonia, and respiratory failure. The incidence of these complications is high and remains a major cause of morbidity and mortality in the postoperative state. Even so, this morbidity may be underestimated since hypoxemia, the most common pulmonary complication, exerts its influence throughout many organ systems through inadequate tissue oxygenation.

From 1959 to 1966, Dr. Richard Naeye, a pathologist at Hershey Medical School, participated in the Collaborative Perinatal Project which studied 53,518 pregnancies. Of the infants subsequently born, 125 died of the Sudden Infant Death Syndrome (SIDS). Through the mass of information gleaned from looking at the pregnancies, labor, delivery, examination of the placenta and subsequent events in the infants’ lives, Dr. Naeye was able to determine nine historical factors that increase an infant’s risk of dying of SIDS. According to his statistical analyses, these are additive factors so that the presence of a number of them increase the risk …

Pulmonary rehabilitation programs seem to have come into vogue along with the national craze for exercise. This paper will discuss the feasibility of a rehabilitation program in a community hospital. In order to do that, we will first address several questions: “Does pulmonary rehabilitation really work?”; “If so, how?”; and “Is it safe?” Having answered these questions, we will discuss some of the details of setting up a rehabilitation program.

Surgical management of symptomatic aortic valve disease has been successful in improving both survival and function. Data reported from the literature suggest an operative mortality of from 5% to 8%, with a lower risk for elective replacement, and a 65% to 75% five-year survival rate as opposed to 50% to 60% in non-surgical patients with symptomatic aortic valve disease.

Recent Techniques In Echocardiography: Two-Dimensional Echocardiography, Kiran B. Sagar

Conventional M-mode echocardiography is a widely used noninvasive diagnostic technique. It allows bedside assessment of cardiac chamber dimensions, valve motion and left ventricular function. The limitations of this technique are that it (1) provides only a one-dimensional (icepick) view of the heart, (2) displays cardiac structures in an unfamiliar form that bears no resemblance to cardiac anatomy, and (3) does not provide information regarding spatial orientation of cardiac structures. These limitations led to the development of two-dimensional (2-D) real-time echocardiography in the last decade. The 2-D echo allows simultaneous visualization of cardiac structures in real time through multiple planes.

Chronic obstructive pulmonary disease has become one of the more common problems which face physicians both in inpatient and outpatient settings. The incidence of the disease and increase in mortality have been documented in a number of studies as have other lung diseases, indicating a general rise in pulmonary disorders over the past several decades. Two types of disease are classified under the heading of chronic pulmonary disease: chronic bronchitis and pulmonary emphysema. In recent years technological advancements in nuclear medicine have resulted in increasing interest in the use of radioisotope techniques in the evaluation of cardiac disease, and cardiovascular nuclear medicine has developed into a useful noninvasive tool in clinical cardiology. Myocardial infarct imaging with technetium-99m pyrophosphate has been demonstrated to be a reliable method in the diagnosis of suspected myocardial infarction. Radioisotope cardiac flow studies are useful in the diagnosis and follow-up of congenital heart disease, and gated cardiac blood pool imaging is emerging as an important technique in the evaluation of left ventricular function and ejection fraction. Results Of Aortocoronary Bypass Surgery For Angina Pectoris, Richard R. Lower, Szabolcs Szentpetery, Timothy Wolfgang, V. Eric Kemp

To determine the relative risks and benefits of coronary bypass surgery for angina pectoris, we examined the results in our first consecutive 360 patients operated upon between May 1970 and December 1975. The age range was 27 to 75 years; there were 309 males and 51 females. The patients were classified clinically as having stable angina in 119 cases (33%), unstable angina in 205 cases (57%) and preinfarction angina in 36 cases (10%). Unstable angina was defined as a definite recent increase in severity or frequency of chest pain, angina at rest or nocturnal angina. Preinfarction angina was defined as …

The 32nd Stoneburner Lecture Series, “New Concepts in the Outpatient Management of Chronic Obstructive Pulmonary Disease and Asthma,” was presented on April 6 and 7, 1979, to an audience consisting principally of family practitioners and internists. The conference explored new concepts about COPD and asthma which have emerged from basic and clinical research during the last decade. In particular, practical management related to drug treatment, pulmonary rehabilitation, pulmonary function testing, pre-and postoperative care, and the use of oxygen were emphasized.

Temporary Plate เป็นฟันปลอมชนิดถอดได้สําหรับใส่ในระยะเวลาสั้น ๆ เท่านั้น แต่มีผู้ใช้เป็นฟันถาวรกันมาก ทําให้เหงือกอักเสบเกิดโรคปริทันต์ และโรคฟันผุได้ง่าย ในกรณีที่คนไข้เป็นโรคปริทันต์อยู่แล้ว หรือฟันธรรมชาติที่ เหลืออยู่เกไม่เรียงตัวอย่างปกติ จะเกิดปัญหาในการใส่และถอดฟันปลอม ทําให้ทันตแพทย์ต้องเสียเวลาในคลินิคเพิ่มขึ้น จําเป็นต้องกรอขอบด้านข้างของฐานฟันปลอม ออก เป็นผลเสียทําให้เศษอาหารติด จึงได้ทดลองทํา "Spoon denture" ในคน ไข้ 16 ราย พบว่าสามารถแก้ปัญหาต่าง ๆ เหล่านี้ได้ และยะงใช้เป็นฟันปลอมชนิดถาวรได้ด้วย

ศัลยแพทย์ช่องปากส่วนมากจะประสบปัญหา ในเรื่องการตรึงชิ้นของกระดูกขากรรไกรล่างที่หักโดยการถูกยิงด้วยกระสุนปืนจนกระดูกแตกละเอียดเป็นชิ้นเล็กชิ้นน้อย ให้ได้ผลดียาก เนื่องจากการตรึงชิ้นกระดูกที่หักนี้เราจําเป็นจะต้องตรึง ไว้เป็นเวลานานหลายเดือน จึงได้ทดลองใช้วิธีตรึงชิ้นกระดูกที่หักในลักษณะนี้ด้วย Alumino-acrylic Splint" กับผู้ป่วยที่ถูกยิงด้วยกระสุนปืนตรงตําแหน่งของ Anterior region ของ mandible พร้อมกับรายงานผู้ป่วยไว้ 2 ราย ซึ่งได้ผล อย่างที่คาดคิดไม่ถึง

“Asthma is a disease characterized by an increased responsiveness of the trachea and bronchi to various stimuli and manifested by a widespread narrowing of the airways that changes in severity either spontaneously or in response to therapy.” The airway narrowing is the end result of some combination of bronchial muscle contraction, tissue inflammation, mucosal edema, and luminal occlusion by cellular debris and thickened secretions.
