Methodological Issues In Studying Treatment Effects In Patients With Cerebrovascular Disease, Mark W. Shatz

Evaluation of the neuropsychological effects of surgical treatment on cerebrovascular disease is beset by numerous methodological difficulties. These include problems specific to this patient population as well as others inherent in all retrospective studies. Five such problems are described: 1) nonrandomized subject selection; 2) dropout from follow-up; 3) natural history of cerebrovascular disease; 4) effects of hospitalization; and 5) the role of practice effects. This paper examines these methodological problems for their impact on our knowledge and proposes alternative research directions to address their shortcomings.

Carotid Artery Disease, Carotid Endarterectomy, And Behavior: A Critical Appraisal, Mark P. Kelly

We reviewed the recent literature on the relationship between carotid artery disease, carotid endarterectomy, and behavior. The methodological adequacy of each study and the complexities involved in interpreting behavioral changes in patients with carotid disease are emphasized. Experimental design issues qualify conclusions in all studies. The weight of the evidence suggests that carotid artery disease may result in cognitive impairment, and that carotid endarterectomy is followed by psychometrically determined cognitive improvement in some patients. The Effects Of Cerebrovascular Surgery On Behavior: What Has Been Demonstrated?, Laurence M. Binder

We studied the effect of the superficial temporal artery to middle cerebral artery bypass (STA-MCA) procedure on neuropsychological function. Nineteen patients with cerebrovascular disease of the anterior carotid circulation (12 on surgical and seven on medical therapy) received an extensive battery of neuropsychological tests; the average test-retest interval was eight weeks. Both groups improved on some measures, and no patients deteriorated. There was no evidence of greater improvement by the surgical group. The results are compatible with a spontaneous remission/practice effect interpretation. A review of the carotid endarterectomy and STA-MCA bypass literature suggests that the null hypothesis of no effect …

A Radiologic Method Of Assessment Of Bone And Joint Destruction In Rheumatoid Arthritis, Gilbert B. Bluhm, David W. Smith, Walter M. Mikulashek

We employed radiographic analysis and a numerical scoring system to evaluate bone and joint destruction in 39 patients who received aspirin and nonsteroidal, antiinflammatory drugs or benoxaprofen therapy over a continuous study period of 36 months (mean). Our purpose was to compare the radiological changes in these patients over two observation intervals and to test the reproducibility of the scoring system. Our modified rating system has sufficient intra-rater reliability to provide a statistically adequate, reproducible method for evaluating bone and joint destruction in rheumatoid arthritis.

Fractures of the pelvis occur commonly in patients who are involved in high speed vehicle accidents or who fall from a substantial height. Fortunately, with improved surgical management, these patients usually recover not only from the pelvic fractures but from associated visceral injuries as well. Some survivors of pelvic fractures subsequently develop unusual hernias. We report the case of a man who developed hernia of the urinary bladder following pelvic fracture. Two previous operations had failed to eliminate his hernia. We describe the technique of repair used successfully in this case and review similar reported cases. Surgeons who care for …

Retrospective Comparison Of Nonsurgical Management To Surgical Treatment For Correction Of Bilateral Carpal Valgus In Foals, Marissak Kalpravidh

The Thai Journal of Veterinary Medicine

Nonsurgical management and surgical treatment for correction of bilateral carpal valgus in 22 foals were retrospectively compared. Eleven foals were nonsurgically treated by box stall rest, corrective hoof trimming, and nutritional alteration. Cast and splint were additionally applied in 3 and 2 foals respectively. Ten of these foals were successfully corrected. Nine foals were surgically corrected by stapling technique while other 2 cases were treated by tension band wiring application. The success of surgical method was evident in 6 cases. By analysis of variance, no significant difference between the success of the nonsurgical management and that of the surgical method. …

The Potency Test Of Pasteurella Multocida Vaccine And Immunoglobulins, Vimolmas Lipipun, Kriengsag Saitanu, Santi Thoongsuwan

Two types vaccine, formalinized whole cell vaccine (W.C. - vaccine) and polysaccharide vaccine (P.S. vaccine), were prepared from a duck strain of Pasteurella multocida. The immunological responses of rabbits to the vaccines was examined. The passive protection studies of the immunoglobulins were tested in white mice. Two groups of three rabbits each were immunized with each type of vaccines. The W.C. - vaccine was given intravenously 5 times at 3-days intervals. The P.S. - vaccine was given intramuscularly 5 times at 1 - week intervals. The W.C. - vaccine gave highest titer, about 1 1,600 30 days after the first …

พยาธิ์ภายนอกในลูกปลาดุกด้าน (Clarias Batrachus) จากฟาร์มเพาะเลี้ยง, เชิดชัย กำวิจิตรรัตนโยธา, บุญชัย วิวัฒน์สัตยา, จิรศักดิ์ ตั้งตรงไพโรจน์

จากการตรวจหาพยาธิภายนอกจากลูกปลาดุกด้านสามขนาด คือ ปลาลูกไร (1 ซม.) ปลาคว่ำบ่อ (2 ซม.) และปลาเซนต์ (4 ซม.) เป็นจำนวน 3 ครั้ง ครั้งแรกตรวจ ตัวอย่างปลา 10 ตัว จากแต่ละขนาดครั้งที่ 2 ตรวจตัวอย่างปลา 20 ตัว จากแต่ละขนาด ครั้งที่ 3 ตรวจเฉพาะในลูกปลาขนาด ลูกไรและคว่ำบ่อ ขนาดละ 20 ตัวอย่างทําการหาชนิด และจำนวนพยาธิ โดยใช้แว่นขยาย 20 เท่าก่อนแล้วจึงนำมาตรวจนับจำนวนบนกล้องจุลทรรศน์ ด้วยกำลังขยาย 100 และ 400 เท่าตามลำดับ ผลการศึกษาพบพยาธิภายนอก 2 ชนิด คือ Gyrodactylus spp. และ Trichodina spp. โดยพบ Gyrodactylus spp. มากที่สุด ในปลาลูกไรโดยเฉพาะในเดือนกันยายน รองลงมาในปลาคว่ำบ่อ และพบน้อยที่สุดในปลาเซนต์ ส่วน Trichodina spp. พบมากที่สุดในปลาเซนต์ในช่วงเดือนสิงหาคม รองลงมาในปลาคว่ำบ่อ และพบน้อยที่สุดในปลาลูกไร

รายงานสัตว์ป่วย : โรค Infectious Bovine Rhinotracheitis ในลูกโค, ธงชัย เฉลิมชัยกิจ, เทอด ประทีป, ประจักษ์ พุ่มวิเศษ

ลูกโคอายุ 3 เดือน พันธุ์พื้นเมืองใน อ.บัวใหญ่ จ.นครราชสีมา มีอาการไข้สูง (103.2 - 106.4°F) ปอดบวม เดี๋ยวฟัน ขบกรามแน่น น้ําลายไหลยืด บางครั้งล้มลงซักแบบ Opisthotonus ได้ให้การรักษาตั้งแต่วันแรกที่แสดงอาการจนถึงวันที่ 4 ลูกโคมีอาการทรุดลงเรื่อย ๆ จึงยุติการรักษาและทําการผ่าซากชัณสูตร ผลทางพยาธิวิทยาพบว่าเป็น non-purulent encephalitis, pharyngitis, diphtheritic tracheitis และ intersitial pneumonia มี intranuclear inclusion bodies ใน glial cells, neurones ของสมองโดยเฉพาะส่วน cerebrum และ eosinophilic intranuclear inclusion bodies ใน epithelial cells ของ trachea จากอาการและผลทางพยาธิวิทยาสรุปได้ว่าเป็นโรค infectious bovine rhinotracheitis (IBR) ซึ่งเป็นรายงานครั้งแรกในประเทศไทย

The Non-Orthodox Cancer Therapy Movement: Emergent Organization In Health Care Crisis, Joseph Behar

The Journal of Sociology & Social Welfare

The ideology and organization of the non-orthodox cancer therapy movement are analyzed as social constructions in an area of professional ambiguity and failure. The movement articulates, integrates, and orders the personally and socially disabling consequences of health care failure in cancer. The protest activities of the movement are characterized by political opposition to medical "orthodoxy" and "monopoly." The challenges of the non-orthodox movement are generally ineffective, non-legitimated, or coopted. Yet, in providing conceptual and organizational frames for the disordering consequences of medical failure and in establishing a politically polarized deviant position in relation to conventional practice, this movement socially organizes …

วิธี Single radial gel diffusion test สามารถใช้ตรวจพิสูจน์ชนิด ของเนื้อสัตว์ได้ การเตรียมแอนติซีรั่มจำเพาะที่ใช้สำหรับทดสอบใช้วิธี absorption ด้วย ซีรั่มของสัตว์ที่เกิดปฏิกริยาข้ามต่อกัน จากการตรวจพิสูจน์ตัวอย่างเนื้อสัตว์สดที่ได้จากตลาด ในเขตกรุงเทพมหานคร พบว่ามีเนื้อกระบือถูกแอบอ้างขายเป็นเนื้อโคทุกตัวอย่าง และจากการตรวจทดสอบตัวอย่างผลิตภัณฑ์ไม่พบการปลอมปนเนื้อสัตว์ วิธีนี้ไม่สามารถตรวจเนื้อสัตว์ที่ทําให้สุกแล้ว

Porcine Parvorirus Antibodies In Human And Swine Sera On A Farm In Thailand, Wattana Wattanvijarn, Donald S. Burke, Ted Tesprateep, Sumittra Wattanodorn

The Thai Journal of Veterinary Medicine

One hundred and twenty five swine sera and 59 human sera were tested for hemagglutination inhibiting antibodies of porcine parvovirus. Sow and boar sera showed 90% positive (94/105), and gilt sera showed 50% positive (10/20). Human sera showed no antibody to PPV. This evidence suggests that humans are resistant to infection with the PPV.
